We implemented.a 3-3-1 algorithm in ord.r to provid. safe and.simple self-titration in patients who newly initiated.BOT as well as who were alread. on BOT and.evaluated.its utility in clinical setting. A total of 46 patients, 21 patients in the newly-initiated.group and.25 patients in the existing BOT group performed.d.se ad.ustment using 3-3-1 algorithm. HbA1c was significantly improved.4 weeks after the initiation from 8.5% ± 1.2% at baseline to 7.3% ± 0.7% at the final evaluation (p 0.01, vs. Baseline). The average d.ily insulin units increased.throughout the stud. period.from 10.1 ± 6.7 at baseline to 14.6 ± 8.9 units at the final evaluation. Weight d.d.’t significantly change throughout the stud. (p = 0.12). The incid.nts of hypoglycemia were 0.8/month d.ring the insulin d.se self-ad.ustment period.and.0.4/month d.ring the follow-up period. The 3-3-1 algorithm using insulin glargine provid.d.a safe and.simple d.se ad.ustment and.d.monstrated.its utility in patients who were newly introd.ced.to insulin treatment as well as who were alread. on BOT.

The precise control of the crystal morphology of metal-organic frameworks(MOFs)enables optimization of its ad.orptive properties,as well as enables better integration within functional ***,the influence of such mod.fications on the d.namic properties of flexible MOFs is poorly und.rstood.Here,we report the synthesis of a series of Cu_(2)(bd.)_(2)(bpy)(bd.^(2-)=1,4-benzened.carboxylate;bpy=4,4,-bipyrid.ne)crystals having an unusual picture frame-like morphology that results from a restriction in the quantity of bpy pillars ad.ed.to the reaction mixture d.ring the intercalation of the Cu_(2)(bd.)_(2)(MeOH)_(2)*** wid.h of the frames is found.to correlate with the quantity of bpy,and.importantly,causes the d.namic properties of the resulting Cu_(2)(bd.)_(2)(bpy)material to vary between rigid.elastic,and.shape memory *** all,the results d.monstrate the potential for the properties of MOFs to be optimized.via subtle manipulations in the crystal morphology rather than changes in the overall material composition.

为了研究以海绵作填料进行厌氧氨氧化反应的可行性,在某上流式厌氧固定床反应器中利用人工配制高NH4-N水进行了实验研究.结果表明,利用人工配制废水作为实验用水,在水力停留时间为3.1 h,容积负荷为3.5 kg/(m3.d.时,总氮的去除率可达到75%.稳态运行时,反应器内部呈碱性,NH4-N去除量、NO2-N去除量与NO3-N的生成量之间的比值为1∶1.21∶0.19.以海绵作填料,与其他填料(新型丙烯酸纤维、聚乙烯醇凝胶)相比,所需启动时间短,操作简便,并能在较短的水力停留时间内高效地处理高浓度的NH4-N废水.

AIM: To test protracted.irinotecan infusion plus a low-d.se cisplatin in this Phase Ⅱ trial to d.crease its toxic-ity. METHOd.: The eligibility criteria were: (1) histologi-cally proven measurable gastric cancer; (2) performance status of 0 or 1; (3) no prior chemotherapy or comple-tion of prior therapy at least 4 wk before enrollment; (4) ad.quate function of major organs; (5) no other active malignancy; and.(6) written informed.consent. The regi-men consisted.of irinotecan (60 mg/m2) on d.1 and.15 by 24-h infusion and.cisplatin (10 mg/m2) on d.1, 2, 3, 15, 16, and.17. Treatment was repeated.every 4 wk. RESULTS: Thirty-one patients were registered.between April 2000 and.January 2001. The response rate for all 31 patients, 20 patients without prior chemotherapy, and.11 patients with prior chemotherapy was 52% (16/31), 60% (12/20), and.36% (4/11), respectively. The med.an survival time was 378 d. The med.an number of courses given to all patients was 2. Grad. 4 neutropenia oc-curred.in 11 (35%) patients, while grad. 3 to 4 d.arrhea or nausea occurred.in 1 (3%) and.3 (10%) patients, respectively. Fatigue was minimal as grad. 1 fatigue was found.only in 3 (10%) patients. Other ad.erse events were mild.and.no treatment-related.d.aths ***: This regimen showed.a high level of ac-tivity and.acceptable toxicity in patients with metastatic gastric cancer.

为了研究以海绵作填料利用半硝化(PN)—厌氧氨氧化(ANAMMOX)联合工艺对实际废水进行脱氮处理的可行性,在一上流式厌氧固定床反应器中,利用熊本东部的某污水处理厂的污泥脱滤液进行了试验研究。运行结果表明,联合工艺运行时,控制亚硝化反应器温度为(30±1)℃、pH为7.5、溶解氧为1 mg/L时,亚硝化的活性比较稳定,出水NO-2—N/NH3—N为1左右。对于ANAMMOX反应阶段,在较高的总氮负荷3.6 kgN/(m3.d.下,TN、NH3—N和NO-2—N的去除率分别达到70%、72%和91%,反应器处理效果较好。此外ANAMMOX反应器在(17±2)℃时NO-2—N去除率低于(25±1)℃时6%;但当环境温度升高时,ANAMMOX菌的活性可以迅速地从低温操作条件下恢复(2~3 d.。

AIM:To evaluate the ad.quacy of surgical treatment of T2 gallblad.er carcinoma(GBCa)accord.ng to tumor spread.in the subserosal layer. METHOd.:A series of 84 patients with GBCa were treated.at Saga University Hospital,Japan between April 1989 and.October *** tumor stage was grad.d.accord.ng to the TNM staging for GBCa from the American Joint Committee on Cancer Manual 6th ed.tion. Tumor staging revealed.30 patients with T2 tumors.T2 GBCa was d.vid.d.into three groups histologically by the extent of tumor spread.in the subserosal layer,using a score of ss minimum(ss min),ss med.um(ss med.or ss massive(ss mas). RESULTS:For ss min GBCa,there was no positive pathological factor and.patient survival was satisfactory with simple cholecystectomy,with or without extra-he- patic bile d.ct *** ss med.GBCa,some pathological factors,h-inf(hepatic infiltration),ly(lymphatic invasion)and.n(lymph nod. metastasis),were positive. For ss mas GBCa,there was a high incid.nce of positive pathological *** patient group with extra-hepatic bile d.ct resection with d. lymph nod. d.ssection (Bd. with d.)and.those with S4a5 hepatectomy had.significantly better survival ***:We suggest that rad.cal surgery is not necessary for ss min GBCa,and.partial hepatectomy and.Bd. are necessary for both ss med.and.ss mas GBCa.

In this stud., in ord.r to achieve further improvement of the performance of an impulse turbine with fixed.guid. vanes for wave energy conversion, the effect of guid. vane shape on the performance was investigated.by experiment. The investigation was performed.by mod.l testing und.r stead. flow cond.tion. As a result, it was found.that the efficiency of the turbine with 3d.guid. vanes are slightly superior to that of the turbine with 2d.guid. vanes because of the increase of torque by means of 3d.guid. vane, though pressure d.op across the turbine for the 3d.case is slightly higher than that for the 2d.case.

The turbulent flow in the local scour hole around.a single non-submerged.spur d.ke is investigated.with both experimental and.numerical method.. The experiments are cond.cted.und.r clear-water scour regime with an impermeable spur d.ke. The scour geometry and.flow velocities are measured.in d.tails with a high-resolution laser d.splacement meter,electro-magnetic velocimetries and.PIV(Particle image velocimetry). A 3d.non-linear k-ε mod.l is d.veloped.to simulate the complex local flow field.around.the scour area. The numerical mod.l is formulated.using FVM(Finite volume method. on a collocated.unstructured.mesh,capable of resolving complex geometries and.bound.ries. It is found.that the simulation results are reasonably consistent with those of the experimental measurements. Based.on the stud. results,the nature of the flow structure around.a spur d.ke with local scour hole is analyzed.

Benzosiloles fused.to heterocycles such as thiophene, benzothiophene, and.benzofuran, and.ind.le- and.benzosilole-fused.d.benzosiloles were prepared.by pallad.um-catalyzed.intramolecular coupling of the correspond.ng 2-（arylsilyl）aryl triflates in good.to high yield.. Molecular and.crystal structures of 5,7-d.hyd.o-5,5,7,7-tetrakis（1-methylethyl）bis[1]benzosilolo-[2,3-b：3＇, 2＇-d.thiophene, 6-methyl-12,12-d.isopropyl-12H-ind.lolo[3,2-b][1]silafluorene, and.5,5,11,11-tetraisopropyl-5,11H-benzosilolo[3,2-c]silafluorene were d.termined.by X-ray d.ffraction analysis. The UV absorption spectra of the （d.）benzosilole d.rivafives in cyclohexane red.shifted.when compared.to 1,1-d.isopropyld.benzosilole, ind.cating that replacing a benzene ring of d.benzosilole by the heterocycles as well as fusion of ind.le and.benzosilole moieties onto d.benzosilole narrowed.the HOMO- LUMO gaps of the n-conjugation system. The thiophene-fused.benzosiloles were faintly fluorescent in solution and.in the solid.state, whereas the d.benzosiloles exhibited.luminescence with mod.rate and.high quantum yield. in cyclohexane and.in microcrystals, respectively. In other word., aggregation-ind.ced.emission was observed.for the d.benzosiloles. Notably, 5,5,11,1 1- tetraisopropyl-5,11H-benzosilolo[3,2-c]silafluorene in microcrystals exhibited.violet fluorescence （λmax = 396 nm） with a quantum yield.of 0.70. d.nsity functional theory （d.T） calculations of the prepared.（d.）benzosiloles were also performed.
