Metastatic lung tumours rarely lead to development of pneumothorax, and no case of bilateral secondary pneumothorax due to lung metastases arising from tongue cancer has been reported. Here, we report a case of a patient with tongue cancer with lung metastases complicated by bilateral secondary pneumothorax soon after the completion of concurrent chemoradiotherapy. A 39-year-old man with cervical lymph node metastases originating from pT2N0M0 tongue cancer underwent neck dissection and postoperative concurrent chemoradiotherapy. Shortly after the completion of chemoradiotherapy, he developed bilateral secondary pneumothorax. Subsequently, he underwent partial lung resection for the pulmonary fistulae for diagnostic and therapeutic purposes;nodular lesions found in both the lungs. The diagnosis of secondary pneumothorax was based on histopathological findings. Although all pulmonary fistulae disappeared after partial lung resection, he died of the primary disease despite our best efforts to control the metastatic pulmonary lesions.

A Study of 100 Cases of Cervical Schwannoma Treated with Inter-Capsular Resection

Background: Cervical schwannoma is a relatively rare disease, and it is difficult to experience many surgical cases because it may be followed up without surgery. We examined 100 patients who underwent inter-capsular resection for cervical schwannomas at our center and classified the patients according to the nerve of origin. Methods: We retrospectively reviewed 100 patients who underwent inter-capsular resection for cervical schwannoma at our center from April 2005 to September 2019. We examined the patient’s characteristics including age, sex, tumor size (maximum diameter), origin nerve, preoperative symptoms, and postoperative neurological deficits for all cases. We classified the cases according to the nerve of origin and the occurrence of postoperative neurological deficits. Results: The occurrence of postoperative neurological deficit for all cases was as follows: “none” was 73%, “temporary paralysis” was 21%, and “permanent paralysis” was 6%. In the case of vagus nerve: “none” was 65.4%, “temporary paralysis” was 23.1%, “permanent paralysis” was 11.5%. In the case of sympathetic nerve: “none” was 64.7%, “temporary paralysis” was 29.4%, “permanent paralysis” was 5.9%. In the case of brachial plexus: “none” was 87.0%, “temporary paralysis” was 13.0%, “permanent paralysis” was 0%. In the case of cervical and accessory nerves: “none” was 86.4%, “temporary paralysis” was 13.6%, “permanent paralysis” was 0%. In the case of facial nerve: “none” was 0%, “temporary paralysis” was 80.0%, “permanent paralysis” was 20%. In the case of lingual nerve: “none” was 80.0%, “temporary paralysis” was 20.0%, “permanent paralysis” was 0%. Conclusions: Inter-capsular resection is useful for the treatment of cervical schwannoma and a simple comparison is difficult, but probably with good results. This study provides information that will be useful for the treatment of cervical schwannoma.

Case Report of a Clear-Cell Variant of Follicular Thyroid Carcinoma

Clear-cell variants of follicular carcinoma are rare subtypes of thyroid cancer. There is no unified view of the histopathological features of clear cell variants, but follicular carcinomas composed predominantly of clear cells are distinguished from clear cell variants. In clinical practice, it is important to determine whether clear cell variants arise primarily from the thyroid gland or are thyroid metastases of other clear cell carcinomas, such as renal cell carcinoma. We present a case in which a patient with initially suspected anaplastic thyroid carcinoma due to a rapidly progressive anterior neck mass was diagnosed with a clear cell variant of follicular carcinoma after a tissue biopsy. The patient was treated with lenvatinib, then his performance status improved, and he was discharged from the hospital. On day 188 after discharge, a contrast-enhanced computed tomography (CECT) scan of the neck showed further shrinkage of the tumor. However, a CECT scan of the chest revealed multiple lung metastases. On day 233 after discharge, the patient developed severe pneumonia resulting from tracheal rupture due to intratumoral necrosis. It was difficult to decide whether lenvatinib should have been discontinued or reduced when lung metastasis appeared. It is necessary to accumulate additional cases to make informed decisions about continuing lenvatinib therapy.

Minimally invasive surgeries for insertional Achilles tendinopathy:A commentary review

Studies of minimally invasive surgery for insertional Achilles tendinopathy are limited.To establish this surgery,the following techniques must be minimally invasive:Exostosis resection at the Achilles tendon insertion,debridement of degenerated Achilles tendon,reattachment using anchors or augmentation using flexor hallucis longus(FHL)tendon transfer,and excision of the posterosuperior calcaneal prominence.Studies on these four perspectives were reviewed to establish minimally invasive surgery for insertional Achilles tendinopathy.Techniques for exostosis resection were demonstrated in one case study,where blunt dissection around the exostosis was performed,and the exostosis was resected using an abrasion burr under fluoroscopic guidance.Techniques for debridement of degenerated Achilles tendon were demonstrated in the same case study,where the space left after resection of the exostosis was used as an endoscopic working space,and the degenerated Achilles tendon and intra-tendinous calcification were debrided endoscopically.Achilles tendon reattachment techniques using suture anchors have been demonstrated in several studies.However,there are no studies on FHL tendon transfer techniques for Achilles tendon reattachment.In contrast,endoscopic posterosuperior calcaneal prominence resection is already established.Additionally,studies on ultrasound-guided surgeries and percutaneous dorsal wedge calcaneal osteotomy as minimally invasive surgery were reviewed.

Stochastic model predictive braking control for heavy-duty commercial vehicles during uncertain brake pressure and road profile conditions

When heavy-duty commercial vehicles(HDCVs)must engage in emergency braking,uncertain conditions such as the brake pressure and road profile variations will inevitably affect braking control.To minimize these uncertainties,we propose a combined longitudinal and lateral controller method based on stochastic model predictive control(SMPC)that is achieved via Chebyshev–Cantelli inequality.In our method,SMPC calculates braking control inputs based on a finite time prediction that is achieved by solving stochastic programming elements,including chance constraints.To accomplish this,SMPC explicitly describes the probabilistic uncertainties to be used when designing a robust control strategy.The main contribution of this paper is the proposal of a braking control formulation that is robust against probabilistic friction circle uncertainty effects.More specifically,the use of Chebyshev–Cantelli inequality suppresses road profile influences,which have characteristics that are different from the Gaussian distribution,thereby improving both braking robustness and control performance against statistical disturbances.Additionally,since the Kalman filtering(KF)algorithm is used to obtain the expectation and covariance used for calculating deterministic transformed chance constraints,the SMPC is reformulated as a KF embedded deterministic MPC.Herein,the effectiveness of our proposed method is verified via a MATLAB/Simulink and TruckSim co-simulation.

Massive gastrointestinal bleeding from gastrointestinal varices is one of the most serious complications in patients with portal hypertension. However, if no bleeding point can be detected by endoscopy in the predilection sites of gastrointestinal varices, such as the esophagus and stomach, ectopic gastrointestinal variceal bleeding should be considered as a differential diagnosis. Herein, we report a case of ectopic ileal variceal bleeding in a 57-year-old woman, which was successfully diagnosed by multi-detector row CT(MDCT) and angiography and treated by segmental ileum resection. To date, there have been no consensus for the treatment of ectopic ileal variceal bleeding. This review was designed to clarify the clinical characteristics of patients with ectopic ileal variceal and discuss possible treatment strategies. From the PubMed database and our own database, we reviewed 21 consecutive cases of ileal variceal bleeding diagnosed from 1982 to 2017. MDCT and angiography is useful for the rapid examination and surgical resection of an affected lesion and is a safe and effective treatment strategy to avoid further bleeding.

Carbon nanotubes(CNTs)are an emerging nanomaterial because of their outstanding performance in various applications.In recent years,the segment molecules of CNTs,referred to as carbon nanorings(CNRs)or carbon nanobelts(CNBs),have gained attention for their unique structures and properties,as well as their potential as seed molecules for the precise synthesis of CNTs.CNBs are rigid and thick segments of CNTs whose synthesis has been addressed by scientists fascinated by the uniqueness of CNBs long before the discovery of CNTs.After 60 years of efforts by synthetic chemists all over the world,the synthesis of the first CNB,(6,6)CNB,was achieved by our group in 2017.Since this milestone,diverse types of nanobelts have been synthesized through various synthetic routes,thereby demonstrating their photophysical,magnetic,and redox properties derived from rigid belt structures.The applications of CNBs have also been introduced recently.The formation of the host−guest complex,transformation to three-dimensional molecules,and measurement of conductivity have been reported for CNBs.This paper summarizes the brief history and perspective of CNBs.Further synthetic campaigns and aggressive application of CNBs would create novel and groundbreaking scenes in materials science.

The Ujaragssuit Nunat layered(UNL)unit in the Itsaq Gneiss Complex,west Greenland,has been consid-ered to contain one of the oldest chromitites on Earth based on~ca.4.1 Ga Hadean whole rock Pt-Os model ages and ca.3.81 Ga zircon U-Pb age of the surrounding orthogneiss.This study obtained zircon from the chromitite within this unit as well as granitoid sheets that intruded into the UNL unit.In-situ U-Pb-Hf-O isotope measurements were made on the zircons.Zircons from both the chromitite and the intrusive granitoids show concordant U-Pb ages of ca.2.97-2.95 Ga.In contrast,Hf and 0 isotopic anal-yses indicate that zircons in the chromitites have a different origin from those in the intrusive granitoids.Zircons from granitoids yielded Th/U ratios higher than 0.2,initial Hf isotope ratios of 0.2805-0.2807(i.e,initial:Hf value of-11 to-5),andδ^(18)O values of mostly 6.0‰-7.0‰,which are typical for felsic igneous rocks in Archean continental crust.The least altered zircons from a chromitite exhibited initial Hf isotope ratios of 0.28078-0.28084(i.e.,initial:Hf value of-1.1 to-0.4),close the chondritic value at ca.3.0 Ga and the depleted mantle at ca.3.2 Ga.These zircons also haveδ^(18)O values of 4.2‰6.1‰which correspond to typical mantle values.The other chromitite zircons yielded Th/U ratios lower than 0.1,and Hf and 0 isotopic compositions ranging between the least altered zircons and the intrusive granitoid zir-cons.These results indicate that the zircons in the chromitites crystallized before or during the 2.97-2.95 Ga granitoid intrusion and most of the zircons were altered by subsequent metasomatism.Furthermore,the present results suggest that zircons in the chromitites originally had depleted Hf iso-topic compositions at ca.3.2-3.0 Ga.This can be explained by two different models of the evolution of the UNL unit.One is that if the UNL unit was formed at>3.81 Ga as previously thought,with the zircons in the chromitites subsequently being precipitated by ca.3.2-2.95 Ga during metamorphism or metaso-matism.The other

Assessment of disease progression in patients with transfusion-associated chronic hepatitis C using transient elastography

AIM:To evaluate the relationship between liver stiffness and duration of infection in blood transfusion-associated hepatitis C virus (HCV) patients with or without hepatocellular carcinoma (HCC).METHODS:Between December 2006 and June 2008,a total of 524 transfusion-associated HCV-RNA positive patients with or without HCC were enrolled.Liver stiffness was obtained noninvasively by using Fibroscan (Echosens,Paris,France).The date of blood transfusion was obtained by interview.Duration of infection was derived from the interval between the date of bloodtransfusion and the date of liver stiffness measurement (LSM).Patients were stratified into four groups based on the duration of infection (17-29 years;30-39 years;40-49 years;and 50-70 years).The difference in liver stiffness between patients with and without HCC was assessed in each group.Multiple linear regression analysis was used to determine the factors associated with liver stiffness.RESULTS:A total of 524 patients underwent LSM.Eight patients were excluded because of unsuccessful measurements.Thus 516 patients were included in the current analysis (225 with HCC and 291 without).The patients were 244 men and 272 women,with a mean age of 67.8 ± 9.5 years.The median liver stiffness was 14.3 kPa (25.8 in HCC group and 7.6 in nonHCC group).The patients who developed HCC in short duration of infection were male dominant,having lower platelet count,with a history of heavier alcohol consumption,showing higher liver stiffness,and receiving blood transfusion at an old age.Liver stiffness was positively correlated with duration of infection in patients without HCC (r=0.132,P=0.024) but not in patients with HCC (r=-0.103,P=0.123).Liver stiffness was significantly higher in patients with HCC than in those without in each duration group (P < 0.0001).The factors significantly associated with high liver stiffness in multiple regression were age at blood transfusion (P < 0.0001),duration of infection (P=0.0015),and heavy alcohol consumption (P=0.043) CONCLUSION:Although liver
