Utility of “loco-check,” self-checklist for “locomotive syndrome” as a tool for estimating the physical dysfunction of elderly people

Aim: A new concept of locomotive syndrome has been proposed by the Japanese Orthopaedic Association. The aim of this study is to clarify the utility of its self-checklist, “loco-check,” as a tool for estimating the physical dysfunction of elderly people. Methods: Subjects were 1124 community-dwelling Japanese people, 557 men and 567 women, aged 40-89 years. Information about the seven “loco-check” items was obtained from present inquiry sheets. Physical functions were examined by grip strength, knee extension strength, walking speed and one-leg standing time with open eyes. The averages of these test values, controlled for age and BMI, were compared between the “loco-check” (+) group and the “loco-check” (-) group. Also we examined about the trend of decline of physical function, together with SF36 physical function subscale score, as the number of the items chosen increased. Results: Adjusted average values of all four physical function examinations in the “lococheck” (+) group were significantly lower than those of the “loco-check” (-) group (all, p . Also the adjusted average values of the majority of four tests were significantly lower in those who checked each of the “loco-check” items than those who did not, for most of the items. It was also revealed that the more items subjects checked, the lower the adjusted average values were, except for one-leg standing time. It was also the case with SF36 physical function subscale score. Conclusion: We showed the utility of “loco-check” as a simple tool not only for noticing the physical dysfunction of elderly people, but also for estimating the extent of it, except for balancing ability, particularly by counting the number of checked items.

Subjective symptoms related to depression and suicidal risk in a Japanese community: a cross-sectional study

BACKGROUND: This cross-sectional study aimed to assess the association between self-reported somatic and mental symptoms and the presence of major depressive disorder (MDD) and suicidal risk among community dwellers in Japan. METHODS: From two locations in Japan, we recruited 734 community dwellers who underwent an annual health screening. Basic symptoms of MDD, dysthymia, and the presence of associated suicidal risk were determined using a brief structured diagnostic psychiatric interview, Mini International Neuropsychiatric Interview (MINI). Information regarding self-reported somatic and mental symptoms was obtained from a self-administered questionnaire used in the annual health check-up. Suicidal risk was evaluated on the basis of six relevant questions asked in MINI. Logistic regression model was used to calculate age- and gender-adjusted odds ratios (ORs) and 95% confidence intervals (CIs) for MDD. Further adjustment for basic symptoms of MDD was performed to calculate ORs and CIs for suicidal risk. RESULTS: A myriad of somatic symptoms, including headache, heavy headedness, eye strain, and shoulder stiffness [adjusted OR (95% CI), 11.4 (1.22 - 107) at location 1;5.17 (1.23 - 21.7) at location 2], were associated with the presence of MDD. Dysmenorrhea [6.07 (1.14 - 32.3) at location 1] and dysesthesia, arthralgia, and swelling in the extremities [2.72 (1.14 - 6.47) at location 2] were significantly associated with an increase in suicidal risk, independent of the presence of basic symptoms of MDD. CONCLUSION: Several somatic symptoms, especially pain-related ones, may serve as possible signs of depression and suicidal risk among community dwellers.

The current research conducted a longitudinal study of new graduate nurses at 3 times-prior to their employment as nurses and 3 months and 1 year afterward. This study yielded 3 findings. 1) The mental health of new graduate nurses significantly worsens 3 months after employment as a nurse and mental health 1 year after employment as a nurse was significantly worse than that before employment. 2) A sense of coherence (SOC) is the ability to cope with stressors, and the 3 components of SOC decreased significantly in the year after employment as a nurse. 3) The mental health of new graduate nurses was significantly affected by a low SOC and by previous diminishing of SOC. Findings suggested the importance of support to sustain and improve the mental health of new graduate nurses and the importance of approaches in the workplace to encourage the development of SOC.

In this study, we compared the motion accuracy of six degrees of freedom (6D) couch for precision radiotherapy with or without weights attached to the couch. Two digital cameras were focused on the iso-center of a linear accelerator. Images of a needle which had been fixed to the 6D couch were obtained using the cameras when the couch moved in translation and rotation around each axis. The three-dimensional (3D) coordinates of the needle were calculated from coordinate values in the images. A coordinate error of the needle position relative to the theoretical position was calculated. The errors were obtained with or without a 60 kg weight attached to the 6D couch, and these errors were compared with each other. The methanol extracts of 168 plant species from 68 families were evaluated for their inhibitory activity against lettuce seedling elongation. Among the plant species tested, 12 species had EC50 values for radicle growth inhibition ranging from 0.01 to 5.00 mg fresh weight equivalent mL-1. Enterolobium contortisiliquum, a traditionally used herbal medicine, exhibited the strongest inhibitory activity (estimated EC50: 0.28 fresh weight equivalent mL-1). Among the 12 species, Pachysandra terminalis, Tamarindus indica, and Albizia guachapele required investigation, because only little has been reported about their chemical constituents to date. The data in the present study would be useful in finding new lead compounds for natural herbicides.

To investigate the surgical therapies for gastric cancer (GC) patients of age 85 or older in a multicenter *** opportunities for elderly GC patients have expanded in conjunction with extended life expectancy. However, the number of cases encountered in a single institution is usually very small and surgical therapies for elderly GC patients have not yet been standardized completely. In the present study, a total of 134 GC patients of age 85 or older who underwent surgery in 9 related facilities were retrospectively investigated. The relationships between surgical therapies and clinicopathological or prognostic features were ***-nine of the patients (66%) presented with a comorbidity, and 26 (19% overall) presented with more than two comorbidities. Radical lymphadenectomy was performed in 59 patients (44%), and no patient received pre- or post-operative chemotherapy. Forty of the patients (30%) experienced perioperative complications, but no surgical or perioperative mortality occurred. Laparoscopic surgery was performed in only 12 of the patients (9.0%). Univariate and multivariate analyses of the 113 patients who underwent R0 or R1 resection identified the factors of pT3/4 and limited lymphadenectomy as predictive of worse prognosis (HR = 4.68, P = 0.02 and HR =2.19, P = 0.05, respectively). Non-cancer-specific death was more common in cStage I patients than in cStage II or III patients. Limited lymphadenectomy correlated with worse cancer-specific survival (P = 0.01), particularly in cStage II patients (P < 0.01). There were no relationships between limited lymphadenectomy and any comorbidities, except for cerebrovascular disease (P = 0.07).CONCLUSIONNon-cancer-specific death was not negligible, particularly in cStage I, and gastrectomy with radical lymphadenectomy appears to be an effective treatment for cStage II elderly GC patients.

As among persons with normal anatomy, occasional patients with situs inversus develop malignant tumors. Recently, several laparoscopic operations have been reported in patients with situs inversus. We describe laparoscopic hemicolectomy with radical lymphadenectomy in such a patient. Careful consideration of the mirror-image anatomy permitted safe operation using techniques not otherwise differing from those in ordinary cases. Thus, curative laparoscopic surgery for colon cancer in the presence of situs inversus is feasible and safe.

BACKGROUND Few studies compared the oncological and biological characteristics between ampullary carcinoma(AC) and cancer of the second portion of the duodenum(DC-Ⅱ), although both tumors arise from anatomically close *** To elucidate differences in clinicopathological characteristics, especially the patterns of lymph node metastasis(LNM), between AC and DC-Ⅱ.METHODS This was a retrospective cohort study of 80 patients with AC and 27 patients with DC-Ⅱ who underwent pancreaticoduodenectomy between January 1998 and December 2018 in two institutions. Clinicopathological factors, LNM patterns, and prognosis were compared between the two *** The patients with AC and DC-Ⅱ did not exhibit significant differences in 5-year overall survival(66.0% and 67.1%, respectively) and 5-year relapse-free survival(63.5% and 62.2%, respectively). Compared to the patients with DC-Ⅱ, the rate of preoperative biliary drainage was higher(P = 0.042) and the rates of digestive symptoms(P = 0.0158), ulcerative-type cancer(P < 0.0001), large tumor diameter(P < 0.0001), and advanced tumor stage(P = 0.0019) were lower in the patients with AC. The LNM rates were 27.5% and 40.7% in patients with AC and DC-Ⅱ,respectively, without significant difference(P = 0.23). The rates of LNM to hepatic nodes(N-He)and pyloric nodes(N-Py) were significantly higher in patients with DC-Ⅱ than in those with AC(metastasis to N-HE: 18.5% and 5% in patients with DC-Ⅱ and AC, respectively;P = 0.0432;metastasis to N-Py: 11.1% and 0% in patients with DC-Ⅱ and AC, respectively;P = 0.0186)CONCLUSION Although there were no significant differences in the prognosis and recurrence rates between the two groups, metastases to N-He and N-Py were more frequent in patients with DC-Ⅱ than in those with AC.
