Important clinical features of atypical antipsychotics in acute bipolar depression that inform routine clinical care: a review of pivotal studies with number needed to treat

English-language literature cited in MEDLINE from January,1980 to October 30,2014 was searched by using terms of antipsychotic,generic and brand names of atypical antipsychotics, ＂bipolar depression/bipolar disorder＂, ＂placebo＂,and ＂trial＂.The parameters of response（≥50%improvement on MADRS,Montgomery-Asberg Depression Rating Scale total score）,remission（either ≤12 or 8 on MADRS total score at endpoint）,discontinuation due to adverse events（DAEs）,somnolence,≥7%weight gain,overall extrapyramidal side-effects（EPSs）,and akathisia,were extracted from originally published primary outcome papers.The number needed to treat to benefit（NNT） for response and remission or harm（NNH） for DAEs or other side effects relative to placebo were estimated and presented with the estimate and 95%confidence interval.Olanzapine monotherapy,olanzapine-fluoxetine combination（OFC）,quetiapine-IR monotherapy,quetiapine-XR monotherapy,lurasidone monotherapy,and lurasidone adjunctive therapy were superior to placebo with NNTs for responses of 11-12,4,7-8,4,4-5,and 7,and NNTs for remission of 11-12,4,5-11,7,6-7,and 6,respectively.There was no significant difference between OFC and lamotrigine,and between aripiprazole or ziprasidone and placebo in response and remission.Olanzapine monotherapy,quetiapine-IR,quetiapine-XR,aripiprazole,and ziprasidone 120-160 mg/day had significantly increased risk for DAEs with NNHs of 24,8-14,9,12,and 10,respectively.For somnolence,quetiapine-XR had the smallest NNH of 4.For ≥7%weight gain,olanzapine monotherapy and OFC had the smallest NNHs with both of 5.For akathisia,aripiprazole had the smallest NNH of 5.These findings suggest that among the FDA-approved agents including OFC,quetiapine-IR and-XR,lurasidone monotherapy and adjunctive therapy to a mood stabilizer,the differences in the NNTs for response and remission are small,but the differences in NNHs for DAEs and common side-effects are large.Therefore,the selection of an FDA-approved atypical antipsychotic for bipolar depression should be based upon safety

Based on the theory of the passive hydrogen maser, along with the technology of frequency modulation and modulation transfer spectroscopy, the theoretical expression of the single frequency modulation for the passive hydrogen maser and the function of the cavity and H line error signals separation are derived, which are basically coincident with the experiment. The absorption and dispersion spectrum curves with different resonance widths show that the cavity and hydrogen transition serve as discriminators, and the two error signals can be separated. Through the calculations of the two error signals in the passive hydrogen maser, it analyzes the traditional method of the two error signals separation, and then describes a new improved method for the passive hydrogen servo loops consisting in the use of a single modulation frequency and frequency discrimination. A null interaction of the two error signals for the new selection of the phase setting is deduced theoretically and validated by the simulation. The preliminary experimental result confirms the feasibility of this new approach, which can reduce the influence from the cavity frequency variety on the crystal oscillator and contribute significantly to the long term performance of the passive hydrogen maser.

Synthesis and characterization of BZSM-5 and its catalytic performance in the methanol to hydrocarbons reaction

Five kinds of BZSM-5 molecular sieve with different Si/B ratio and a SiZSM-5 molecular sieve were prepared by hydrothermal synthesis method followed by acid exchange and pelletization.The samples were characterized by XRD,SEM,FT-IR,ICP,low temperature N_(2) physical adsorption and desorption,NH3-TPD and Py-IR.The catalytic performance in the reaction of methanol to hydrocarbons was evaluated in the fixed bed reactor.Compared with SiZSM-5,the amount and strength of Bronsted(B)acid were enhanced by introducing skeleton boron and the activity of the catalyst was greatly improved.The characterization and evaluation results indicated that the BZSM-5 catalyst synthesized from the gel of SiO_(2)/B2 O320 with Si/B ratio 74.48 had modest acidity strength,acid amount of 0.18 mmol NH3·g^(-1) and large mesopore volume of 0.23 cm3·g^(-1).The B acid ratio was higher and the acid strength of BZSM-5 was weaker than that of AIZSM-5,which could inhibit the deep coke formation and increase the activity stability.B-2 had the best lifetime which could reach 672 h under the same evaluation reaction conditions,due to the best matching of moderate acidity and good diffusion properties.

Background Glaucoma secondary to systemic lupus erythematosus （SLE - an uncommon but serious complication that threatens vision and therefore cannot be neglected. A few cases of second, y glaucoma resulting from lupus-induced or iatrogenic ocular impairments have been reported in association with SLE. However, a systematic analysis of the diagnosis and treatment of glaucoma secondary to SLE has not been reported in the literature. The aim of this study is to further investigate the relationship between glaucoma and SLE. Methods In this study, we reviewed nine eyes of five patients diagnosed with secondary glaucoma associated with SLE, including one case of neovascular glaucoma and four cases of steroid-induced glaucoma. Results Neovascular glaucoma was successfully treated by Ahmed glaucoma valve implantation surgery with adjunctive ranibizumab intravitreal injection, followed by panretinal photocoagulation （PRP）. The steroid-induced glaucoma in eight eyes of four cases were controlled by trabeculectomy along with antiproliferative agents. Conclusion Regular follow-up ocular examinations should be conducted to ensure early diagnosis and treatment of secondary glaucoma in SLE patients to improve the prognosis of vision.

The study of methanol aromatization on transition metal modified ZSM-5 catalyst

In this article, transition metals of Cu, La and Zn were used as adjuvant to prepare modified HZSM-5 by impregnation method. The catalysts were characterized by XRD, BET, NH_3-TPD and Py-IR to reveal the microstructure and acid property. The catalysis performances of methanol aromatization of catalysts were investigated in a fixed-bed reactor. The results show that the strength and distribution of acid center of these catalysts are significantly influenced by the species of transition metal. There are more mediate strong Lewis acid center in Zn modified HZSM-5 catalyst and therefore exhibits higher selectivity to aromatic, benzene, toluene and xylenes in the MTA reaction..

Background Although 15 mA transcranial alternating current stimulation (tACS) has a therapeutic effect on depression, the activations of brain structures in humans accounting for this tACS configuration remain largely unknown.Aims To investigate which intracranial brain structures are engaged in the tACS at 77.5 Hz and 15 mA, delivered via the forehead and the mastoid electrodes in the human brain.Methods Actual human head models were built using the magnetic resonance imagings of eight outpatient volunteers with drug-naive, first-episode major depressive disorder and then used to perform the electric field distributions with SimNIBS software.Results The electric field distributions of the sagittal, coronal and axial planes showed that the bilateral frontal lobes, bilateral temporal lobes, hippocampus, cingulate, hypothalamus, thalamus, amygdala, cerebellum and brainstem were visibly stimulated by the 15 mA tACS procedure.Conclusions Brain-wide activation, including the cortex, subcortical structures, cerebellum and brainstem, is involved in the 15 mA tACS intervention for first-episode major depressive disorder. Our results indicate that the simultaneous involvement of multiple brain regions is a possible mechanism for its effectiveness in reducing depressive symptoms.

Technology and test results of space adaptability of passive hydrogen maser

Based on the operating principle and the electric property design of the passive hydrogen maser, the technology and test results of its space adaptability are carried out under the special launch conditions and space environment. The various perturbations affecting the output frequency of such a standard used for the navigation satellite system are specified, such as magnetic field change, vibration, thermal vacuum and radiation. Through the adaptability technology in the aspects above, the security and reliability of the space passive hydrogen maser sufficiently fulfill the requirements of space operation. At present, the space passive hydrogen maser is working normally on board, indicating that the space adaptability satisfies the design requirement.

Background Patients who suffer comorbidity of major depressive disorder(MDD)and chronic pain(CP)maintain a complex interplay between maladaptive prospective memory(PM)and retrospective memory(RM)with physical pain,and their complications are still unknown.Aims We aimed to focus on the full cognitive performance and memory complaints in patients with MDD and CP,patients with depression without CP,and control subjects,considering the possible influence of depressed affect and chronic pain severity.Methods According to the fifth edition of the Diagnostic and Statistical Manual of Mental Disorders and the criteria given by the International Association of Pain,a total of 124 participants were included in this cross-sectional cohort study.Among them,82 depressed inpatients and outpatients from Anhui Mental Health centre were divided into two groups:a comorbidity group(patients with MDD and CP)(n=40)and a depression group(patients with depression without CP)(n=42).Meanwhile,42 healthy controls were screened from the hospital’s physical examination centre from January 2019 to January 2022.The Hamilton Depression Rating Scale-24(HAMD-24)and Beck Depression Inventory-II(BDI-II)were used to evaluate the severity of depression.The Pain Intensity Numerical Rating Scale(PI-NRS),Short-Form McGill Pain Questionnaire-2 Chinese version(SF-MPQ-2-CN),Montreal Cognitive Assessment-Basic Section(MoCA-BC),and Prospective and Retrospective Memory Questionnaire(PRMQ)were used to assess pain-related features and the global cognitive functioning of study participants.Results The impairments in PM and RM differed remarkably among the three groups(F=7.221,p<0.001;F=7.408,p<0.001)and were severe in the comorbidity group.Spearman correlation analysis revealed the PM and RM were positively correlated with continuous pain and neuropathic pain(r=0.431,p<0.001;r=0.253,p=0.022 and r=0.415,p<0.001;r=0.247,p=0.025),respectively.Regression analysis indicated a significant positive relationship between affective descriptors and total BDI-II score(β=0.
