Metabolic surgery and intestinal gene expression:Digestive tract and diabetes evolution considerations

AIM: To investigate the effects of bariatric surgery on metabolic parameters, incretin hormone secretion, and duodenal and ileal mucosal gene ***: nine patients with type 2 diabetes mellitus(T2DM), chronic serum hyperglycemia for more than2 years, and a body mass index(BMI) of 30-35 kg/m2 underwent metabolic surgery sleeve gastrectomy with transit bipartition between May 2011 and December2011. Blood samples were collected pre and 3, 6 and12 mo postsurgery. Duodenal and ileal mucosa samples were collected pre- and 3 mo postsurgery. Pre- and postoperative blood samples were collected in the fasting state before ingestion of a standard meal(520kcal) and again 30, 60, 90, and 120 min after the mealto determine hemoglobin A1c(HbA1c) levels and the lipid profile, which consisted of triglyceride and total cholesterol levels. Intestinal gene expression of p53 and transforming growth factor(TGF)-β was analyzed using quantitative reverse-transcription PCR. Gastric inhibitory polypeptide(GIP) and glucagon-like peptide-1(GLP-1)were quantified using the enzyme-linked immunoassay method and analyzed pre- and ***'s t test or repeated measurements analysis of variance with Bonferroni corrections were performed as ***: BMI values decreased by 15.7% within the initial 3 mo after surgery(31.29 ± 0.73 vs 26.398± 0.68, P nd then stabilized at 22% at 6mo postoperative, resulting in similar values 12 mo postoperatively(20-25 kg/m2). All of the patients experienced improved T2 DM, with 7 patients(78%)achieving complete remission(HbA1c nd 2patients(22%) achieving improved diabetes(HbA1cnts). We report the discovery of a peculiar X-ray transient,EP240408a,by Einstein Probe （EP） and follow-up studies made with EP,Swift,nICER,GROnD,ATCA and other ground-based multiwavelength *** new transient was first detected with Wide-field X-ray Telescope （WXT） on board EP on April 8th,2024,manifested in an intense yet brief X-ray flare lasting for *** flare reached a peak flux of 3.9×10-9erg cm-2s-1in 0.5-4 keV,300 times brighter than the underlying X-ray emission detected throughout the *** and more precise follow-up observations by EP/FXT,Swift and nICER confirmed the finding of this new *** X-ray spectrum is non-thermal in 0.5-10 keV,with a power-law photon index varying *** X-ray light curve shows a plateau lasting for4d,followed by a steep decay till becoming undetectable10 d after the initial *** on its temporal property and constraints from previous EP observations,an unusual timescale in the range of 7-23 d is found for EP240408a,which is intermediate between the commonly found fast and long-term *** counterparts have been found in optical and near-infrared,with the earliest observation at 17 h after the initial X-ray detection,suggestive of intrinsically weak emission in these *** demonstrate that the remarkable properties of EP240408a are inconsistent with any of the transient types known so far,by comparison with,in particular,jetted tidal disruption events,gamma-ray bursts,X-ray binaries and fast blue optical *** nature of EP240408a thus remains an *** suggest that EP240408a may represent a new type of transients with intermediate timescales of the order of10 *** detection and follow-ups of more of such objects are essential for revealing their origin.

Background: Acute respiratory infections (ARI) are recognized as an important cause of morbidity, mortality, and hospitalization among children in developing countries. Objectives: To identify the respiratory viruses circulating in Central African children before the SARS-COV2 pandemic and to assess the clinical manifestations. Methodology: This is a cross-sectional, descriptive, multicenter study, run from March 1, 2019, to March 31, 2020. Children aged 28 days to 15 year-old, with respiratory symptoms ≤10 days had been included. nasopharyngeal swabs were taken and sent to the Institute Pasteur in Bangui (WHO national Referral Center for influenza). Virus research was done by cell and molecular culture techniques. Data were recorded and processed with Access 2019 software, then analyzed with STATA version 14 software. Chi-square test and AnOVA test were used to compare proportions at the p 0.05 threshold. Results: Out of 659 children included during the study period, viruses were identified in 231 children, for an overall positivity rate of 35.05% (231/659). Rhinoviruses (RV) and influenza viruses were found in 66.23% and 16.88% respectively. Virus-virus co-infections were found in 10 (10/231) children (4.32%). Children under 5 years of age were more represented (78.60%). The main reasons for consultation were: fever (96.20%), cough (95.45%), runny nose (78.5%), and breathing difficulty (30.50%). ILI (Influenza-Like Illness) was found in 71.02% versus 28.98% of SARI (Severe Acute Respiratory Infection). There was a statistically significant association between age 5 years and severity of acute respiratory infection (p = 0.001). The outcome was known for the 122 children at the CHUPB site with a mortality rate of 17.21% (n = 21). Conclusion: Viral ARI is common in children in Central African Republic. Care givers should think about it in order to reduce the inappropriate prescription of antibiotics.

Background:The use of laparoscopic(LLR)and robotic liver resections(RLR)has been safely performed in many institutions for liver tumours.A large scale international multicenter study would provide stronger evidence and insight into application of these techniques for huge liver tumours≥10 ***:This was a retrospective review of 971 patients who underwent LLR and RLR for huge(≥10 cm)tumors at 42 international centers between ***:One hundred RLR and 699 LLR which met study criteria were *** comparison between the 2 approaches for patients with huge tumors were performed using 1:3 propensity-score matching(PSM)(73 vs.219).Before PSM,LLR was associated with significantly increased frequency of previous abdominal surgery,malignant pathology,liver cirrhosis and increased median *** PSM,RLR and LLR was associated with no significant difference in key perioperative outcomes including media operation time(242 vs.290 min,P=0.286),transfusion rate rate(19.2%vs.16.9%,P=0.652),median blood loss(200 vs.300 mL,P=0.694),open conversion rate(8.2%vs.11.0%,P=0.519),morbidity(28.8%vs.21.9%,P=0.221),major morbidity(4.1%vs.9.6%,P=0.152),mortality and postoperative length of stay(6 vs.6 days,P=0.435).Conclusions:RLR and LLR can be performed safely for selected patients with huge liver tumours with excellent *** was no significant difference in perioperative outcomes after RLR or LLR.

Objective: To assess structural and metabolic brain changes in subjects affected by Fabry disease (FD) or carrying the disease mutation. Background: FD is an X- linked metabolic disorder due to α - galactosidase A deficiency, which leads to storage of glycosphingolipids in many tissues and organs. Previous MR studies have shown structural and metabolic brain abnormalities in FD patients. It is not clear, however, whether tissue damage can be seen in both the brains of hemizygous and heterozygous and whether quantitative MR metrics are useful to monitor disease evolution. Design/Methods: We studied 4 males and 4 females with FD. Each subject underwent brain proton MRI/MR spectroscopic imaging (MRSI) examinations to obtain measures of total brain volumes, total brain lesion volumes, magnetization transfer ratios (MTr) in WM and central brain levels of n- acetylaspartate (nAA) to creatine (Cr). A second MR examination was performed in five subjects after 2 years. Results: Focal WM lesions were found in 2 males and 1 female. The MTr values were always low in the WM lesions of FD subjects (p nd also were low in the normal- appearing WM of 2 affected males. Total brain volumes were never decreased in FD subjects. Brain nAA/Cr values were significantly (p = 0.005) lower in FD subjects than in normal controls and correlated closely with Rankin scale measures (r = - 0.79). On follow- up examinations, no significant MR changes were found. However, the small changes in nAA/Cr correlated closely with changes in Rankin scores (r = - 0.86). Conclusions: Subtle structural and metabolic tissue damage can extend beyond WM lesions in FD subjects. Diffuse brain nAA/Cr decrease can be found in FD subjects in relation to the degree of their CnS involvement and its evolution over time.

试验旨在评估饲料中蛋白质和亚麻籽含量对牛奶产量和品质的影响,特别是对牛奶中脂肪酸含量的影响。试验采取2×3方阵设计,包括2种粗蛋白质(CP)含量和3种亚麻籽(FS)添加水平。各组饲粮分别为:低CP日粮(12%DM),无亚麻籽(LP),低CP日粮(12%DM)、亚麻籽含量低(500 g·d^(-1),LPFS500),低CP日粮(12%CP)、亚麻籽含量适中(1 000 g·d^(-1),LPFS 1 000),中CP日粮(15%DM)、无亚麻籽(MP),中CP日粮(15%DM)、亚麻籽含量低(500 g·d^(-1))(MPFS 500),中CP日粮(15%DM)、亚麻籽含量适中(1 000 g·d^(-1),MPFS 1 000)。补充亚麻籽量适中的MP组可提高乳蛋白和酪蛋白含量,而不添加亚麻籽或添加量较少的LP组则含量较低。结果表明,当饲料中补充亚麻籽时,可缓解低蛋白饲粮对产奶量、乳蛋白含量和酪蛋白含量的影响。饲料中添加亚麻籽较多时,牛奶中的短链脂肪酸含量较低,特别是C8:0、C10:0含量最低。FS1 000组中链脂肪酸含量最低,FS500组含量适中,而HP组和LP组含量最高。FS1 000组中长链脂肪酸含量最高,FS500组含量适中,不添加亚麻籽组最低。饲料中蛋白质水平影响C18:0的百分比,MP组高于LP组。共轭亚油酸含量变化与长链脂肪酸的变化趋势相同,与对照组相比,在FL500组中增加约7%,在FL1 000组中增加约22%。除了饲料中蛋白质水平不同外,FS1 000组粥样硬化生成指数、凝血指标和n-6/n-3最低;FS500组的凝血指标和n-6/n-3适中。亚麻籽可提高牛奶的营养价值,且改善情况在一定范围内随添加量的增加而提高。
