近年来,经自然腔道取标本手术(NOSES)治疗结直肠癌已引起了广泛关注。NOSES应用于结直肠手术的潜在优势包括减轻术后疼痛,减少伤口并发症,减少对术后镇痛的需求,加快肠道功能的恢复,缩短住院时间,更好的美容效果,以及给患者带来了更佳的心理状态。尽管NOSES的手术创伤显著减轻,然而该技术也存在一些潜在问题。尤其随着这项新技术的兴起,人们对其无菌的把控、肿瘤学结果和病例选择等问题表示出关切。因此,迫切需要一部操作指南来规范NOSES手术在结直肠癌中的开展。经过国际NOSES联盟全体成员的三轮讨论,最终形成了该部共识。这一共识有助于NOSES在全球范围内的长远发展。

At present,natural orifice specimen extraction surgery(NOSES)has attracted more and more attention worldwide,because of its great advantages including minimal cutaneous trauma and post-operative pain,fast post-operative recovery,short hospital stay,and positive psychological impact.However,NOSES for the treatment of gastric cancer(GC)is still in its infancy,and there is great potential to improve its theoretical system and clinical practice.Especially,several key points including oncological outcomes,bacteriological concerns,indication selection,and standardized surgical procedures are raised with this innovative technique.Therefore,it is necessary to achieve an international consensus to regulate the implementation of GC-NOSES,which is of great significance for healthy and orderly development of NOSES worldwide.

Development of donor specific antibodies after SARS-CoV-2 vaccination:What do we know so far?

Vaccination against Coronavirus disease-19(COVID-19)was pivotal to limit spread,morbidity and mortality.Our aim is to find out whether vaccines against COVID-19 lead to an immunological response stimulating the production of de novo donor specific antibodies(DSAs)or increase in mean fluorescence intensity(MFI)of pre-existing DSAs in kidney transplant recipients(KTRs).This study involved a detailed literature search through December 2nd,2023 using PubMed as the primary database.The search strategy incorporated a combination of relevant Medical Subject Headings terms and keywords:"COVID-19","SARS-CoV-2 Vaccination","Kidney,Renal Transplant",and"Donor specific antibodies".The results from related studies were collated and analyzed.A total of 6 studies were identified,encompassing 460 KTRs vaccinated against COVID-19.Immunological responses were detected in 8 KTRs of which 5 had increased MFIs,1 had de novo DSA,and 2 were categorized as either having de novo DSA or increased MFI.There were 48 KTRs with pre-existing DSAs prior to vaccination,but one study(Massa et al)did not report whether pre-existing DSAs were associated with post vaccination outcomes.Of the remaining 5 studies,35 KTRs with pre-existing DSAs were identified of which 7 KTRs(20%)developed de novo DSAs or increased MFIs.Overall,no immunological response was detected in 452(98.3%)KTRs.Our study affirms prior reports that COVID-19 vaccination is safe for KTRs,especially if there are no pre-existing DSAs.However,if KTRs have pre-existing DSAs,then an increased immunological risk may be present.These findings need to be taken cautiously as they are based on a limited number of patients so further studies are still needed for confirmation.

Santa Catarina is a small,developed,and relatively safe state in South Brazil.Despite having positive social economic indicators,it still faces multiple challenges regarding forensic practices for personal identification.The objective of this paper is to discuss the recent advances and current challenges in the region,from the perspectives of anthropological and dental postmortem human identification,missing persons,and disaster victim identification(DVI)from 2019 to 2021.The recent creation of a Forensic Anthropology Sector(SAF)in the state’s official forensic institution(Polícia Científica-PCI)has significantly improved identification of unidentified remains and optimised resources available for DNA analysis.However,SAF is still quite understaffed,which negatively affects the recovery of skeletal material,its preparation,and the time needed for filing reports.Santa Catarina has passed legislation for missing persons in 2015,4 years prior to the sanction of federal laws implementing the national policy for the disappeared.Nonetheless,a lack of integration between stakeholders remains a problem that PCI has tried to circumvent with the Conecta Programme,a multidisciplinary and integrated initiative between families of the missing persons,police agencies,and the Public Ministry.The programme aims to collect not only reference DNA samples,but also relevant anthropological and dental data.It also offers facial progression services in cases of disappearances that occurred many years ago.Despite a history of disasters in the state,PCI still needs to implement international DVI standards at an institutional level.Recent training on Phase 1 DVI procedures,integrated with other responding institutions,indicates better preparation for future disasters.There are many challenges ahead for Santa Catarina’s forensic institution and professionals that have yet to be addressed,but the overall situation on routine personal identification,missing persons initiatives,and DVI has improved over the last 2 years.

Background Primary eosinophilic gastrointestinal disorders(EGIDs)constitute chronic allergic inflammation.The number of eosinophils is one of the diagnostic criteria;more than 20 eosinophils per high-power field(HPF)in the gastrointestinal(GI)tract are considered abnormal in Japan.However,the quantity of eosinophils considered normal varies according to anatomical location and geographical region;such values have not been reported in Japanese pediatric patients,nor have the numbers of lymphocytes in the normal pediatric stomach.To establish a reference for defining diagnostic criteria for EGIDs,we evaluated the number of eosinophils in the normal Japanese pediatric GI tract.Methods We examined 131 biopsy cases without significant clinical history,endoscopic abnormality,or histological abnor-mality.Immunohistochemical analysis of CD3 and CD20 was performed.Results The mean eosinophil density was highest in the cecum(49.5±22.4 per HPF).Counts of more than 20 eosinophils per HPF were observed in the duodenum[bulb(20.0±9.6)and second portion(30.0±15.8)],terminal ileum(38.3±22.7),cecum(49.5±22.4),ascending colon(42.3±25.3),transverse colon(29.4±17.0),and descending colon(32.2±17.9).Counts of fewer than 10 eosinophils per HPF were observed in the stomach and rectum;a count of fewer than one eosinophil per HPF was observed in the esophagus.More than 100 CD3-positive T cells per HPF were observed in the stomach.Conclusions The mean numbers of eosinophils in the bowel were greater than 20 per HPF.For Japanese pediatrics,the cur-rent threshold eosinophil count should be revised.

BK viral infection remains to be a challenging post-transplant infection,which can result in kidney dysfunction.The mainstay approach to BK infection is reduction of immunosuppression.Alterations in immunosuppressive regimen with minimization of calcineurin inhibitors,use of mechanistic target of rapamycin inhibitors,and leflunomide have been attempted with variable outcomes.Over the past few years,investigators have explored potential therapeutic options for BK infection.Fluoroquinolone prophylaxis and treatment was found to have no benefit in kidney transplant recipients.The utility of cidofovir is limited by its nephrotoxicity.Intravenous immunoglobulin is becoming a popular option for treatment and prophylaxis for BK infection,as it increases the neutralizing antibody titers against the most common BK virus serotypes.Virus-specific T cell therapy is an emerging treatment option for BK viremia.In this review,we will explore management and therapeutic options for BK infection and recent evidence available in literature.

Simulation of Crack Pattern Formation Due to Shrinkage in a Drying Material

Crack patterns observed in nature have attracted the interest of researchers in various fields, and the mechanism of the pattern formation has been investigated. However, the phenomenon is very complicated, and many factors affect the process. Therefore, we are motivated to construct a general simulation code with a simple algorithm. In this study, crack pattern formation due to shrinkage caused by the drying of a wet material was simulated. The process was simplified as follows: tensile force is generated in the model, and a crack is generated when the tension exceeds a critical value. The tensile forces in the x and y directions are independently evaluated. A crack propagates perpendicular to the tension until it reaches another crack or a boundary. Based on this modeling, simulations with a two-dimensional square domain were performed. Consequently, a cross-divided pattern was generated. Assuming zigzag crack propagation, more realistic patterns were obtained. The effects of the boundary and domain size were also considered, and various characteristic patterns were obtained. Furthermore, the orientation dependency was simulated, and 45˚ declined patterns and rectangularly divided patterns were generated. The model presented in this study is very simplified and is expected to be applicable to various objects.

We have measured the cross section for π^＋π^- production in two-photon collisions using a data sample with an integrated luminosity of 85.9 fb^-1 collected with the Belle detector. The f0（980） resonance is observed as a peak in the energy spectrum of the cross section. We also report preliminary results for γγ→ π^0π^0 with two-photon center-of-mass energies ranging from 0.6 to 4.0 GeV, based on a 95 fb-1 data sample. We find at least four resonant structures including a peak from f0（980）. In addition, there is evidence for Xc0 production. We also make a preliminary discussion of the angular dependence and cross section ratio of γγ →π^＋π^- and γγ→π^oπ^o.

Barley(Hordeum vulgare ssp.vulgare)is one of the first crops to be domesticated and is adapted to a wide range of environments.Worldwide barley germplasm collections possess valuable allelic variations that could further improve barley productivity.Although barley genomics has offered a global picture of allelic variation among varieties and its association with various agronomic traits,polymorphisms from East Asian varieties remain scarce.In this study,we analyze exome polymorphisms in a panel of 274 barley varieties collected worldwide,including 137 varieties from East Asian countries and Ethiopia.We reveal the underlying population structure and conduct genome-wide association studies for 10 agronomic traits.Moreover,we examin genome-wide associations for traits related to grain size such as awn length and glume length.Our results demonstrate the value of diverse barley germplasm panels containing Eastern varieties,highlighting their distinct genomic signatures relative to Western subpopulations.
