Laser-assisted direct roller imprinting of large-area microstructured optical surfaces

In this study,a high-throughput fabrication method called laser-assisted direct roller imprinting(LADRI)was developed to lower the cost of nanoimprinting large-area polymer films and to address problems associated with nanoimprinting,namely,microstructural damage and precision in flatness of entire film.With LADRI,the laser directly heats the microstructured surface of the roller mold,which heats and melts the surface of a polymethyl methacrylate(PMMA)film to replicate the microstructures on the mold rapidly.In this study,the effects of laser power density,scanning speed,size of the microstructures,and contact pressure on the replication speed were investigated experimentally.The replication speed increased as the power and scanning speed increased.However,because the film required heating until it filled the entire depth of the microstructure,an appropriate replication speed was necessary.This result was supported by simulation of the temperature distribution inside the mold and the PMMA using transient heat conduction analyses.To demonstrate the applications of LADRI,two different optical surfaces were replicated:an antireflection(AR)structure with conical structures sized several hundred nanometers and a lightextraction structure with a microlens array(MLA)comprising 10μm lenses,for display and illumination,respectively.The replication degree of the MLA was governed by the contact pressure.Polymer flow simulation indicated that the heat conduction and flow speeds of the melted PMMA surface were comparable within several tens of micrometers.In addition,the reflectivity of the AR structure decreased from 4 to 0.5%,and the light intensity of the light-extraction structure increased by a factor of 1.47.

Long-term treatment with proton pump inhibitor is associated with undesired weight gain

AIM:To examine the effects of long-term proton pump inhibitor (PPI) therapy on body weight (BW) and body mass index (BMI) in patients with gastroesophageal reflux disease (GERD).METHODS: The subjects were 52 patients with GERD and 58 sex-and age-matched healthy controls. GERD patients were treated with PPI for a mean of 2.2 years (range, 0.8-5.7 years), and also advised on lifestyle modifications (e.g. selective diet, weight management). BW, BMI and other parameters were measured at baseline and end of study.RESULTS: Twenty-four GERD patients were treated daily with 10 mg omeprazole, 12 with 20 mg omeprazole, 8 with 10 mg rabeprazole, 5 with 15 mg lansoprazole, and 3 patients with 30 mg lansoprazole. At baseline, there were no differences in BW and BMI between reflux patients and controls. Patients with GERD showed increases in BW (baseline: 56.4±10.4 kg, end: 58.6±10.8 kg, mean±SD, P<0.0001) and BMI (baseline: 23.1±3.1 kg/m2, end: 24.0±3.1 kg/m2, P<0.001), but no such changes were noted in the control group. Mean BW increased by 3.5 kg (6.2% of baseline) in 37 (71%) reflux patients but decreased in only 6 (12%) patients during treatment.CONCLUSION: Long-term PPI treatment was associated with BW gain in patients with GERD. Reflux patients receiving PPI should be encouraged to manage BW through lifestyle modifi cations.

Preventing pancreatic fistula after distal pancreatectomy: An invagination method

Following an increase in the use of the GIA stapler for treating a pancreatic stump, more techniques to prevent postoperative pancreatic juice leakage have been required. We describe one successful case using our new technique of invaginating the cut end of the pancreas into the stomach to prevent a pancrea-tic fistula(PF) from occurring. A 50-year-old woman with pancreatic cancer in the tail of the pancreas underwent distal pancreatectomy, causing a grade A PF. We resected the distal pancreas without additional reinforcement to invaginate the stump into the gastric posterior wall with single layer anastomosis using a 3-0 absorbable suture. The drain tubes were removed on the third postoperative day. Although a grade A PF was noted, the patient was discharged on foot on the eleventh postoperative day. Our technique may be a suitable method for patients with a pancreatic body and tail tumor.

Clinical differentiation of acute appendicitis and right colonic diverticulitis: A case-control study

BACKGROUND Acute right colonic diverticulitis (ARCD) is an important differential diagnosis of acute appendicitis (AA) in Asian countries because of the unusually high prevalence of right colonic diverticula. Due to qualitative improvement and the high penetration rate of computed tomography (CT) scanning in Japan, differentiation of ARCD and AA mainly depends on this modality. But cost, limited availability, and concern for radiation exposure make CT scanning problematic. Differential findings of ARCD from AA are based on several small studies that used univariate comparisons from Korea and Taiwan. Previous studies on clinical and laboratory differences between AA and ARCD are limited. AIM To determine clinical differences between AA and ARCD for differentiation of these two diagnoses by creating a logistic regression model. METHODS We performed an exploratory single-center retrospective case-control study evaluating 369 Japanese patients (age ≥ 16 years), 236 (64.0%) with AA and 133 (36.0%) with ARCD, who were hospitalized between 2012 and 2016. Diagnoses were confirmed by CT images. We compared age, sex, onset-to-visit interval, epigastric/periumbilical pain, right lower quadrant (RLQ) pain, nausea/vomiting, diarrhea, anorexia, medical history, body temperature, blood pressure, heart rate, RLQ tenderness, peritoneal signs, leukocyte count, and levels of serum creatinine, serum C-reactive protein (CRP), and serum alanine aminotrans-ferase. We subsequently performed logistic regression analysis for differentiating AA from ARCD based on the results of the univariate analyses.RESULTS In the AA and ARCD groups, median ages were 35.5 and 41.0 years, respectively (p=0.011);median onset-to-visit intervals were 1 [interquartile range (IQR): 0-1] and 2 (IQR: 1-3) days, respectively (P < 0.001);median leukocyte counts were 12600 and 11500/mm3, respectively (P = 0.002);and median CRP levels were 1.1 (IQR: 0.2-4.1) and 4.9 (IQR: 2.9-8.5) mg/dL, respectively (P < 0.001). Clinical prediction of complicated appendicitis:A case-control study utilizing logistic regression

BACKGROUND Since high-quality evidence on conservative treatment of acute appendicitis using antibiotics has increased,differentiation of patients with complicated appendicitis(CA)from those with simple appendicitis(SA)has become increasingly important.Previous studies have revealed that male gender,advanced age,comorbid conditions,prehospital delay,fever,and anorexia are risk factors of perforated appendicitis.Elevated serum C-reactive protein(CRP)level and hyponatremia have also been reported as predictive biomarkers of CA.However,confounding between various factors is problematic because most previous studies were limited to univariate analysis.AIM To evaluate non-laboratory and laboratory predictive factors of CA using logistic regression analyses.METHODS We performed an exploratory,single-center,retrospective case-control study that evaluated 198 patients(83.9%)with SA and 38 patients(16.1%)with CA.Diagnoses were confirmed by computed tomography images for all cases.We compared age,sex,onset-to-visit interval,epigastric/periumbilical pain,right lower quadrant pain,nausea/vomiting,diarrhea,anorexia,medical history(of previous non-surgically treated appendicitis,diabetes,hypertension,dyslipidemia,liver cirrhosis,hemodialysis,chronic lung diseases,malignant tumors,immunosuppressant use,and antiplatelet use),vital signs,physical findings,and laboratory data to select the explanatory variates for logistic regression.Based on the univariate comparisons,we performed logistic regression for clinical differentiation between CA and SA using only nonlaboratory factors and also including both non-laboratory and laboratory factors.RESULTS The 236 eligible patients consisted of 198 patients(83.9%)with SA and 38 patients(16.1%)with CA.The median ages were 34 years old[interquartile ranges(IR),24-45 years]in the SA group and 49 years old(IR,35-63 years)in the CA group(P<0.001).The median onset-to-visit interval was 1 d(IR,0-1)and 1 d(IR,1-2)in the SA and CA groups,respectively(P<0.001).Heart rate,body temperature,and serum

Critical Current and Fracture Behavior of Nb3Al Superconducting Strands under Cyclic Electromagnetic Force

To obtain basic information on fracture behavior and its critical current properties in superconducting strands with long-term periodic excitation, a unique experiment of cyclic strain loading was carried out. The strain was applied to a superconducting filament directly at 4.2 K using an electromagnetic (Lorenz) force instead of the usual mechanical load. The sample strands were Jelly-roll process Nb3Al with a copper ratio of 2.1. A new magnet system to evaluate long-term cyclic strain loading was set up. It was able to generate external fields up to 14 T with 58 mm bore. Cyclic strain was applied by the sample current control: 500 A, 0.1 Hz. In the experiment, critical current measurements and applied cyclic strain were carried out in alternation and the variation of critical current was obtained during the cyclic test. In this paper, the experimental results of cyclic strain loading on the critical current of Nb3Al strand and mirostructural observation of fracture surface of the filaments is described. Our results show that degradation of critical current properties was not observed at cycle number n = 1500 under the applied axial strain of 0.17%. A noxious sample, from which the outside copper stabilizer was deliberately removed, broke after n = 10, and crack propagation was observed at the fracture surface of the Nb3Al filaments. Under a microstructural observation of the break sample, the fracture points of the filaments were seen to be different in a longitudinal direction. A striped pattern like a beach mark due to crack propagation was also observed at the Nb3Al fracture surface. It was found that the cross-sectional homogeneity of the strand is important to avoid the damage and breaking from cyclic loading.

在高等植物形态发生中RNA和蛋白质起着重要作用,可是到目前为止对于胚形成初期RNA和蛋白质合成的知识仍然很少。有关RNA合成的研究主要集中在萌发的胚上。前已报道,在球形胚中出现许多激烈的发育变化,指数生长和双重节律生长开始于球形胚100—200细胞阶段,这些发育的变化反映了RNA和蛋白质代谢方面质和量的变化。本试验测定了水稻和大麦胚形成初期3H-尿苷和3H-亮氨酸的掺入情况。
