Background: Malnutrition is a strong predictor of prognosis in maintenance hemodialysis patients (MHD). We previously proposed a new and simple protein-energy wasting (PEW) score that appears to be useful for assessing the risk of mortality in patients on MHD. Objectives: In the present study, we evaluated the reliability of this PEW score as a predictor of hospitalization in Japanese patients on MHD. Methods: In this single-center, prospective cohort study conducted in Japan, PEW score was calculated for 180 MHD patients. PEW score ranged from 0 (best: S1) to 4 (worst: S4) and was calculated based on nutritional indicators including serum albumin, body mass index, serum creatinine level, and protein intake. The outcome was the number of hospitalizations during the 2-year study period. Results: Thirty-six patients were hospitalized during the study period. Kaplan-Meier curves showed there were fewer hospitalizations in the group with a PEW score of 0/1 than in the group with a score of 3/4. Multivariate analysis revealed a hazard ratio for hospitalization of 3.109 for S3/4 versus S0, 2.777 for S3/4 versus S1, and 2.048 for S3/4 versus S2. Conclusion: The new and simple PEW score is a useful predictor of hospitalization in MHD patients and is also useful for identifying subgroups of MHD patients with a high risk of mortality.

Background: Malnutrition is associated with higher risk of mortality in maintenance hemodialysis (MHD) patients. The geriatric nutritional risk index (GNRI) has been developed as a tool to assess the nutritional risk. Objectives: The purpose of the present study was to examine the significance of the GNRI as a mortality predictor in MHD patients. Methods: We retrospectively examined the GNRI of 259 MHD patients aged 59.2 ± 12.8 years, and followed up for 36 months. The patients were divided into two groups according to GNRI values of 91.0. Predictors for all-cause mortality were examined using Kaplan-Meier and Cox proportional-hazards analyses. Results: During the follow-up period of 36 months, a total of 76 patients died. Background: New erythropoiesis-stimulating agents (ESAs) with a longer half-life have been developed for the treatment of anemia as a complication of patients with end-stage renal disease. Objectives: The objective of the present study was to assess the hemoglobin (Hb) stability of a Japanese cohort of hemodialysis (HD) patients who were simultaneously switched from darbepoetin alfa (DA) to epoetin beta pegol (CERA). Methods: This was an observational, prospective study of HD patients 20 years of age or more who were switched from intravenous (IV) DA to IV CERA and continued on HD for at least 3 months. The dose was adjusted to maintain the Hb level to within 1.0 g/dl of the baseline value. Results: A total of 68 HD patients (75.0% male, median age 63.0 years) were enrolled. The patients’ mean Hb levels were 10.8 ± (0.6) g/dl at Month 0, 10.9 ± 0.7 at Month 1, 10.8 ± 0.7 at Month 2, and 10.9 ± 0.8 at Month 3, and the differences from the level at Month 0 were not significant. After the switch, the ESA dose decreased significantly (P P < 0.0001). Conclusion: Switching from DA to CERA was associated with approximate 89% reduction of the required dose in Japanese HD patients being treated with an ESA and showed a favorable impact on the treatment of renal anemia, including the need for less frequent injections and a reduction of the ESA dose.

Particle selectivity of filtering by C. elegans

A nematode Caenorhabditis elegans(C. elegans) is a filter feeder, which draws a suspension of bacteria and separates bacteria from the solvent by using pharyngeal pumping motions and specific mouth parts. This mechanism has not been fully understood. We investigated the mechanism of filtering of bacteria in the pharynx of C. elegans by visualization by fluorescent particles and dyed E. coli. We succeeded in quantifying the selectivity of bacteria-sized particles by C. elegans. The most accumulated particles were those of 0.5 μm in diameter. The quantity of accumulated particles of 0.2 μm or 1.0 μm in diameter was about one third of that of particles of 0.5μm in diameter. The least accumulated particles were those of 0.05 μm in diameter. These results suggest that the pharyngeal structures of C. elegans would be suitable for eating bacteria because the size of bacteria ingested by C. elegans worms is about 0.5 μm in diameter. We also succeeded in visualizing pharyngeal structures and pumping motions and flow in the pharynx. We found that there were phase differences in the motions among procorpus, metacorpus and isthmus. This result suggests filtering would occur at the two tips of procorpus and isthmus by the phase differences. We found that bacteria-sized particles and bacteria were flowed and trapped in the channels, which existed along the central lumen from tip of procorpus to isthmus. From our results, we proposed the novel mechanism of filtering of bacteria through the channels for flowing and trapping. In future, this selective filtering mechanism of C. elegans would be applied to development of microfluidic filtration devices for medical and biological equipment.

Guide vanes are installed in the Wells turbine in order to improve its efficiency, self-rotating characteristics and off design performance with stall. This work attempts to explain the role of these guide vanes on the basis of momentum theory. It is shown that the upstream vanes are more effective in enhancing efficiency than the downstream ones. A design method for guide vanes is suggested based on experimental data and potential theory. Experimental studies carried out by the author confirm the theory proposed.

Specific Surface Free Energy of As-Grown and Polished Faces of Synthetic Quartz

Contact angles of water droplet on as-grown Z, +X, –X, and m faces of synthetic quartz crystals with growth term of 20 and 48 days, and polished Z, +X, –X, Y, and 45° cut faces of synthetic quartz crystal were observed. The average of the contact angles on as-grown Z, +X, and –X faces increased with the growth term, and they were larger than that on polished Z, +X, and –X faces. On the other hand, the average of the contact angles of water on m face decreased with the growth term, and they were smaller than that on polished Y cut face. Growth rate of the faces of synthetic crystals was measured and the order of growth rate was, m < –X < +X < Z. Specific surface free energy (SSFE) was calculated using Neumann’s equation. The SSFE of polished face was in the order of, m < –X < +X < Z, which corresponds to the order of the growth rate. The SSFE was larger for the face with larger growth rate.

AIM The single nucleotide polymorphism(SNP) c.415C>T in exon 3 of NUDT15 affects thiopurine-induced leukopenia in Asian patients with Crohn's disease. Meanwhile, three additional genetic variants of NUDT15 were reported in patients with acute lymphoblastic leukemia. We evaluated the effects of these additional genetic variants of NUDT15 in patients with inflammatory bowel disease(IBD) treated with *** Ninety-six Japanese patients with IBD were enrolled. Genotyping for the NUDT15 and TPMT genes was performed using Custom Taq Man SNP genotyping assays or Sanger sequencing. The changes in white blood cell(WBC) count, mean corpuscular volume(MCV), platelet count, hemoglobin, CRP, amylase, albumin, AST, ALT, and ESR were *** Genetic variants of exon 1 and exon 3 of NUDT15 were identified in 24 of 96 patients(25.0%). C.52G > A and c.36_37 insG GAGTC in exon 1 were found in three patients each. All three patients with c.36_37 insG GAGTC in exon 1 were heterozygotes of ***139 Cys in exon 3. Eighteen patients had ***139 Cys in exon 3 alone. The WBC count gradually decreased after initiation of thiopurine treatment in the mutated cases(n = 24), and was significantly lower at 6, 8, 10, and 16 wk(P = 0.0271, 0.0037, 0.0051, and 0.0185, respectively). The WBC counts were also evaluated in patients with and without prednisolone treatment. In the patients with prednisolone treatment, the WBC count tended to show a greater decrease in the mutated cases, with significant differences at 8 and 10 wk(P = 0.012 and 0.029, respectively). In the patients without prednisolone treatment, the WBC count was significantly lower at 2, 4, 8, and 14 wk in mutated cases(P = 0.0196, 0.0182, 0.0237 and 0.0241, respectively). MCV increased after starting thiopurine treatment in the mutated cases, and was significantly higher at 10 wk(P = 0.0085). Survey of Annual and Seasonal Fungal Communities in Japanese <i>Prunus mume</i>Orchard Soil by Next-Generation Sequencing

Fungi play a vital role in the management of soil environment. Although various fungal communities are found in soil, it is difficult to determine the fungal community structure in soil. In this study, we conducted a comprehensive survey of fungal communities in Japanese Prunus mume orchard soil from 2010 to 2012 growing seasons using next-generation sequencing technology. Fungal DNA was directly extracted from the soil samples and the internal transcribed spacer 1 region was amplified by PCR and sequenced. We identified 34,826 fungal clone sequences from the soil samples. The fungal clones were sorted into 2132 operational taxonomic units and a majority of the discriminated clone sequences were classified as Ascomycota and Basidiomycota. The number of fungal species belonging to Ascomycota showed increases in June in the three growing seasons. That belonging to Glomeromycota showed increases in August in the three growing seasons. As Ascomycota fungi are wood decomposers and saprotrophs, the results suggested that the number of plant pathogenic fungi increased in Japanese P. mume orchard soil in June. These findings show for the first time the annual and seasonal fungal community structures in Japanese P. mume orchard soil, and are expected to provide valuable clues for improvement when planting new P. mume trees in Japanese orchards.
