Hot snare polypectomy vs endoscopic mucosal resection using bipolar snare for intermediate size colorectal lesions:Propensity score matching

BACKGROUND Endoscopic resection(ER)with bipolar snare,in which the electric current only passes through the tissue between the device’s two electrodes,is a prominent method used to prevent perforation due to electricity *** using bipolar snare with or without submucosal injection enabled safe resection of colorectal lesions measuring 10-15 mm in an ex vivo porcine *** with bipolar snare is expected to have good treatment outcomes in 10-15 mm colorectal lesions,with high safety even without submucosal ***,no clinical reports have compared treatment outcomes with and without submucosal *** To compare the treatment outcomes of bipolar polypectomy with hot snare polypectomy(HSP)to those with endoscopic mucosal resection(EMR).METHODS In this single-centre retrospective study,we enrolled 10-15 mm nonpedunculated colorectal lesions(565 Lesions in 463 patients)diagnosed as type 2A based on the Japan Narrow-band Imaging Expert Team classification,resected by either HSP or EMR between January 2018 and June 2021 at the National Cancer Center Hospital *** were divided into HSP and EMR groups,and propensity score matching was *** the matched cohort,en bloc and R0 resection rates and adverse events were compared between the two *** Of the 565 lesions in 463 patients,117 lesions each in the HSP and EMR groups were selected after propensity score *** the original cohort,there was a significant difference in antithrombotic drug use(P<0.05),lesion size(P<0.01),location(P<0.01),and macroscopic type(P<0.05)between the HSP and EMR *** the matched cohort,the en bloc resection rates were comparable between both groups[93.2%(109/117)vs 92.3%(108/117),P=0.81],and there was no significant difference in the R0 resection rate[77.8%(91/117)vs 80.3%(94/117),P=0.64].The incidence of delayed bleeding was similar in both groups[1.7%(2/117)].Perforation occurred in the EMR group[0.9%(1/117)]but not in the HSP ***

Let C be a nonempty closed convex subset of a 2-uniformly convex and uniformly smooth Banach space E and {An}n∈N be a family of monotone and Lipschitz continuos mappings of C into E＊. In this article, we consider the improved gradient method by the hybrid method in mathematical programming [i0] for solving the variational inequality problem for {AN} and prove strong convergence theorems. And we get several results which improve the well-known results in a real 2-uniformly convex and uniformly smooth Banach space and a real Hilbert space.

Gd-EOB-DTPA-enhanced magnetic resonance imaging features of hepatic hemangioma compared with enhanced computed tomography

AIM:To clarify features of hepatic hemangiomas on gadolinium-ethoxybenzyl-diethylenetriaminpentaacetic acid (Gd-EOB-DTPA)-enhanced magnetic resonance imaging (MRI) compared with enhanced computed tomography (CT). METHODS:Twenty-six patients with 61 hepatic hem- angiomas who underwent both Gd-EOB-DTPA-enhanced MRI and enhanced CT were retrospectively reviewed. Hemangioma appearances (presence of peripheral nodular enhancement, central nodular enhancement, diffuse homogenous enhancement, and arterioportal shunt during the arterial phase, fill-in enhancement during the portal venous phase, and prolonged enhancement during the equilibrium phase) on Gd-EOB-DTPA-enhanced MRI and enhanced CT were *** degree of contrast enhancement at the enhancing portion within the hemangioma was visually assessed using a five-point scale during each phase. For quantitative analysis, the tumor-muscle signal intensity ratio (SIR), the liver-muscle SIR, and the attenuation value of the tumor and liver parenchyma were calculated. The McNemar test and the Wilcoxon's signed rank test were used to assess the significance of differences in the appearances of hemangiomas and in the visual grade of tumor contrast enhancement between Gd-EOB-DTPA-enhanced MRI and enhanced CT. RESULTS:There was no significant difference between Gd-EOB-DTPA-enhanced MRI and enhanced CT in the presence of peripheral nodular enhancement (85% vs 82%), central nodular enhancement (3% vs 3%), diffuse enhancement (11% vs 16%), or arterioportal shunt (23% vs 34%) during arterial phase, or fill-in enhancement (79% vs 80%) during portal venous phase. Prolonged enhancement during equilibrium phase was observed less frequently on Gd-EOB-DTPA-enhanced MRI than on enhanced CT (52% vs 100%, P < 0.001). Relationship of Abundance of Oceanic Sea Skaters, Halobates in the Tropical Pacific Ocean to Surface Biomass and Chlorophyll/Oxygen Concentrations

Relationship of population density of oceanic sea skaters collected from tropical and subtropical zones in the Pacific Ocean was examined to chlorophyll concentration/Dissolved Oxygen concentration and biomass in surface sea water. The four parameters shown above were measured at the site of 12oN 135oN during the cruise, MR13-03 cruise, and at the site of 25oN 160oE during another cruise KH-14-02. Significant and positive correlation between all biomass (especially invertebrates) and population density of oceanic sea skaters collected with a Neuston-Net trailing during 15 min was shown in overall analysis on the data of the both samplings (p oN 160oE than that at 12oN 135oN with similar value of chlorophyll value (p oN 160oE than that at 12oN 135oN for keeping high density population of oceanic sea skaters.

Endovascular Treatment of Brachiocephalic and Subclavian Arterial Disease

Objectives: To review our experience of stentgraft deployment for vascular aneurysm or pseudoaneurysm of the brachiocephalic or subclavian artery. Methods: Participants comprised 7 patients (4 men, 3 women;mean age, 61 years;range, 47 - 76 years) who underwent endovascular repair of brachiocephalic or subclavian arterial vascular lesions between July 2001 and November 2008. Causes of vascular lesions were: traffic accident, n = 4;infection, n = 2;and post-irradiation state of esophageal cancer, n = 1. Safety, technical success, and clinical follow-up were evaluated. Results: Stentgraft deployment was successful in all cases. No complications related to stent fracture were encountered during follow-up (up to 2308 days). One male patient with esophageal cancer died of rebleeding from the tracheostomy hole 13 days after treatment with size mismatch between the stentgraft and brachiocephalic artery. Conclusion: Stentgraft deployment represents acceptable treatment for the injured brachiocephalic artery or proximal side of the subclavian artery.

Visualization of utero-ovarian anastomoses: Comparison between patients with and without uterine fibroid

Purpose: To retrospectively review uteroovarian anastomosis (UOA) visualization during uterine artery embolization (UAE) in patients with or without uterine fibroids (UFs). Material and Methods: 43 patients underwent UAE for uterine fibroids (n = 23) and gynecological hemorrhage (n = 20). The frequency of angiographic visualization of UOAs was compared between the two groups and analyzed by the Fisher’s exact test. Results: Except for one patient with no right uterine artery, 85 uterine arteries were evaluated. Of these 85 uterine arteries, 23 UOA were recognized: 14 UOAs (58%) (right: 5 [21.7%], left: 9 [37.5%]) were visualized in 23 patients with UFs, and 9 UOAs (47.4%) (right: 4 [21%], left: 5 [26.3%]) were visualized in 20 patients with gynecological hemorrhage. In both groups, UOA was observed after UAE in one patient. Statistically significant difference was not observed for UOAs visualized between patients with or without UFs except a group of left UAE in patients with UFs (P = 0.036). Conclusion: The frequency of UOAs visualized during angiography was similar between patients with or without UFs. Therefore, UOA should be visualized carefully during UAE in patients with gynecological hemorrhage.

Modeling of a Lithium-Ion Capacitor and Its Charging and Discharging Circuit in a Model-Based Design

For several years now, electric vehicles (EVs) have been expected to become widely available in the micro-mobility field. However, the insufficiency of such vehicles’ battery-charging and discharging performance has limited their practical use. A hybrid energy storage system, which comprises a capacitor and battery, is a promising solution to this problem;however, to apply model-based designs, which are indispensable to embedded systems, such as the electronic control units used in EVs, a simple and accurate capacitor model is required. Within this framework, a lithium-ion capacitor (LIC) model is proposed, and its charging and discharging performances are evaluated against an actual LIC. The model corresponds accurately to the actual LIC, and the results indicate that the proposed LIC model will work well when used with Model-Based Design (MBD).

Relationship between Cool Coma and Heat Coma Temperatures in the Oceanic Sea Skaters Halobates Collected near the Sumatra Island in the Indian Ocean

In the cruise, MR15-04 by R/V MIRAI, the samplings by the neuston net were performed in 23rd November to 14th December 2016 and three species of Halobates (H. germanus, H. micans, H. sp) were used for the temperature tolerance experiments after the collection. The neuston net was towed three times (3 × 15 min) on the starboard side of R/V MIRAI on the water surface with ship speed of 2 knot to water every 3 nights (19:00 - 20:00) at the fixed point in the south-western direction which was located at 50 km from the Sumatra island (4o03'S - 4o05'S, 101o53'E) in the Indonesia. Experiments on cool coma and heat coma were performed on the three species. Seconds for recovery from cool coma and heat coma were also examined on the Halobates in this study. Cool coma temperatures, gap temperature needed (temperature from the adapted temperature) for the cool coma and seconds for the recover from cool coma ranged 13.0oC to 25.0oC, 3.1oC to 16.1oC, 1 second to 4370 seconds, respectively. Heat coma temperature, gap temperature needed for the heat coma, seconds for the recover from heat coma ranged 29.4oC to 43.1oC, 1.9oC to 15.5oC, 2 seconds to 6420 seconds, respectively. The higher temperature of cool coma temperature during the last five days was shown when Madden-Julian Oscillation has passed over the ship, R/V MIRAI than the previous 10 days in the adults of H. germanus collected at the fixed place neat to Sumatra island (One way ANOVA: F-value = 2.314, df = 7, p = 0.028). Adults of un-described species, H. sp collected near to the Sumatra island, showed lower cool coma temperature [Mean ± SD: 15.51 ± 3.76 (9)] than those of H. germanus collected in the same place [16.96 ± 2.57 (191)]. This lower cool coma temperatures shown by this un-described species might be related to that this species should be a “shore” species inhabiting shore water in which many precipitation could cause the decreased surface temperature from 30oC - 31oC into about 25oC. Efficacy of Uterine Artery Embolization for Patients with Postpartum Hemorrhage

Purpose: To investigate the efficacy of uterine arterial embolization (UAE) in patients with post-partum hemorrhage (PPH). Materials and Methods: The subjects were 40 women (mean age, 33 years;age range, 21 - 42 years) who underwent UAE for PPH at two institutes from June 2001 to May 2011. The rates of clinical success (avoidance of hysterectomy) and complications were calculated. Differences in related factors between primary PPH and secondary PPH and between caesarean section and vaginal delivery were examined. The risk factors associated with hysterectomy were also examined. Results: The overall clinical success rate was 90% (93% of primary PPH, 77% of secondary PPH, and 87.5% of PPH with cesarean section), and the overall complication rate was 10%. There were significant differences in time to PPH (P 1000 mL bleeding, P = 0.03), and embolization of non-uterine arteries (P = 0.02).

The Distribution of the Oceanic Sea Skaters, <i>Halobates</i>inside and outside the <i>Kuroshio</i>

The purpose of this study was to clarify the distribution of oceanic Halobates in the area of the Kuroshio flowing near the southern shore in the direction of 100° - 120°, and also to compare the population density of Halobates between the area within or outside the area of the Kuroshio and also among seasons. This study was carried out during 8 cruises by R/V TANSEIMARU. The Kuroshio area south of the southern Japan coast (30°00'N - 35°00'N, 130°25'E - 141°04'E) was dominated by H. sericeus, and the averaged population-density of this species was significantly higher inside the Kuroshio than outside this current. On the Kuroshio, H. sericeus was dominant with the population density of 16,396.4 km-2 ± 66,138.4 [26] (Mean ± SD [n]), whereas the density of H. germanus was 8,581.9 km-2 ± 24,443.2 [26]. The two oceanic sea skaters, H. sericeusand H. germanus showed significant seasonal variation in the population density, with significantly higher density in October than other months, whereas there was no such significant October peak in the cosmopolitan oceanic sea skater, H. micans. The results of this study may suggest that H. sericeus could use the Kuroshio as a transportation tool to distribute a wide latitude area of from 10°N to 40°N in the western tropical, subtropical and temperate area in the Pacific Ocean.
