Sewerage Force Adjustment Technology for Energy Conservation in Vacuum Sanitation Systems

The vacuum sanitation is the safe and sound disposal approach of human excreta under the specific environments like flights, high speed trains and submarines. However, the propulsive force of current systems is not adjustable and the energy consumption does not adapt to the real time sewerage requirement. Therefore, it is important to study the sewerage force adjustment to improve the energy efficiency. This paper proposes an energy conservation design in vacuum sanitation systems with pneumatic ejector circuits. The sewerage force is controlled by changing the systematic vacuum degree according to the amount of the excreta. In particular, the amount of the excreta is tested by liquid level sensor and mass sensor. According to the amount of the excreta, the relationship between the excreta amount and the sewerage force is studied to provide proper propulsive force. In the other aspect, to provide variable vacuum degrees for different sanitation requirements, the suction and discharge system is designed with pneumatic vacuum ejector. On the basis of the static flow-rate characteristics and the vacuum generation model, the pressure response in the ejector circuit is studied by using the static flow rate characteristics of the ejector and air status equation. The relationship is obtained between supplied compressed air and systematic vacuum degree. When the compressed air is supplied to the ejector continuously, the systematic vacuum degree increases until the vacuum degree reaches the extreme value. Therefore, the variable systematic vacuum degree is obtained by controlling the compressed air supply of the ejector. To verify the effect of energy conservation, experiments are carried out in the artificial excreta collection, and the variable vacuum-degree design saves more than 30% of the energy supply. Background: Surveys of pediatric endotracheal tube (ETT) management previously reported that specialists in pediatric anesthesia and intensive care medicine preferred to use uncuffed ETTs for children younger than 8 to 10 years of age. The aim of this study was to reveal the most recent attitudes and clinical practices of pediatric ETT management in Japan. Methods: The attitudes and clinical practices of pediatric ETT management were investigated using the data sheets of each institution and each patient. The data sheets contained information on patient characteristics and type of hospital, surgical procedures, devices used for intubation, and ETT information including types, size, depth, intracuff pressure (ICP), interval of ICP measurement, laryngeal packing, ETT exchange, airway complications, and reintubations. Results: The response rate of this survey was 66.7%. More than half of children older than 2 years of age were intubated with cuffed ETTs;83.5% of cuffed ETTs were used with the cuffs inflated, and ICP was measured in 80.7% of cuffed ETTs. More than half of ICP measurements were only taken at the time of intubation. Post-extubation stridor was rarely observed in cuffed (0.4%) or uncuffed ETTs (1.2%). The pediatric ETT management questionnaire revealed age-based size selection, differences in pressure of air leakage between cuffed (15 - 20 cmH2O) and uncuffed ETTs (20 - 30 cmH2O) of different sizes, the depthmarking method of insertion length. Continuous measurement of ICP was not common. Conclusion: This study revealed widespread use of cuffed ETTs in children older than 2 years of age, rarely occurrence of post-extubation stridor, inflation of cuffs, and practice of ICP measurement.

Is endoscopic papillary balloon dilatation really a risk factor for post-ERCP pancreatitis?

Endoscopic papillary balloon dilatation(EPBD) is useful for decreasing early complications of endoscopic retrograde cholangio-pancreatography(ERCP), including bleeding, biliary infection, and perforation, but it is generally avoided in Western countries because of a relatively high reported incidence of post-ERCP pancreatitis(PEP). However, as the efficacy of endoscopic papillary largeballoon dilatation(EPLBD) becomes widely recognized, EPBD is attracting attention. Here we investigate whether EPBD is truly a risk factor for PEP, and seek safer and more effective EPBD procedures by reviewing past studies. We reviewed thirteen randomised control trials comparing EPBD and endoscopic sphincterotomy(EST) and ten studies comparing direct EPLBD and EST. Three randomized controlled trials of EPBD showed significantly higher incidence of PEP than EST, but no study of EPLBD did. Careful analysis of these studies suggested that longer and higher-pressure inflation of balloons might decrease PEP incidence. The paradoxical result that EPBD with small-calibre balloons increases PEP incidence while EPLBD does not may be due to insufficient papillary dilatation in the former. Insufficient dilatation could cause the high incidence of PEP through the use of mechanical lithotripsy and stress on the papilla at the time of stone removal. Sufficient dilation of the papilla may be useful in preventing PEP.

AIM: To investigate the effects of taurolithocholate (TLC)on the canalicular motility in isolated rat hepatocyte cou-plets (IRHC).METHODS: TLC was added to IRHC at concentrationsof 10 and 50 μmol/L, respectively. In each group, fi vetime-lapse movies containing 3 representative bile cana-liculi were taken under phase-contrast microscopy for12 h. The number of bile canalicular contractions andthe intervals between consecutive canalicular contrac-tions were calculated. Furthermore, the effects of TLC onIRHC were examined by transmission electron ***: The bile canalicular contractions were spon-taneous and forceful in the controls. Active vesicularmovement was observed in the pericanalicular *** after the addition of TLC, the bile canaliculiwere deformed, and canalicular bile was incorporatedinto the vacuoles. The canaliculi were gradually dilated,and canalicular contractions were markedly inhibited byTLC. The vesicular movements became extremely slowin the pericanalicular region. The number of canalicularcontractions significantly decreased in the TLC-treatedgroups, as compared with that in the controls. The timeintervals were prolonged, as the TLC dosage increased,indicating that bile secretion into the canaliculi wasimpaired with TLC. Transmission electron microscopyrevealed the lamellar transformation of the canalicularmembranes in IRHC treated with ***: TLC impairs both the bile canalicularcontractions and the canalicular bile secretion, possiblyby acting directly on the canalicular membranes in TLC-induced cholestasis.

Objective To assess whether the low-density lipoprotein cholesterol （LDL-C） target value and preventive effect of statins are different between elderly and younger patients. Methods We investigated 304 patients with previous percutaneous coronary intervention who underwent coronary angiography from January 2007 to December 2016 for examination of recurrent ischemia beyond the early restenosis. Patients were classified into two groups： age ≥ 75 years （elderly group： n = 140） and 〈 75 years （younger group： n = 164）. Relationships between the achieved LDL-C level, incidence of late coronary events, and the effectiveness of statins were evaluated. Results During follow-up, 179 patients underwent late coronary revascularization. Recurrent ischemia presenting as acute coronary syndrome （ACS） occurred in 83 cases. Kaplan-Meier curve analysis revealed that in the younger group, recurrent ACS was significantly lower in patients with LDL-C 〈 70 mg/dL than in those with LDL-C ranging from 70 to 〈 100 mg/dL （P = 0.035）; however, there was no difference between these in the elderly group （P = 0.863）. Instead, recurrent ACS was less frequent in patients with LDL-C ranging from 70 mg/dL to 〈 100 mg/dL than in those with LDL-C ≥ 100 mg/dL in the elderly group （v = 0.033）. Statin use was associated with decreased recurrent ACS （P = 0.005）; moreover, only using statins was an independent predictor in the elderly group （HR： 0.375; P = 0.007）. Conclusions Strict control of LDL-C to 〈 70 mg/dL was effective for reducing the incidence of recurrent ACS in younger patients. However, LDL-C 〈 100 mg/dL might be sufficient as the target value of LDL-C-lowering therapy for secondary prevention of ischemic events in Japanese elderly patients.
