Eubacterium limosum ameliorates experimental colitis and metabolite of microbe attenuates colonic inflammatory action with increase of mucosal integrity

AIM： To examine the effect of Eubacteriuro Iiroosuro （***） on colonic epithelial cell line in vitro, and to evaluate the effect of *** on experimental colitis.METHODS： *** was inoculated anaerobically and its metabolites were obtained. The growth stimulatory effect of the *** metabolites on T84 cells was evaluated by SUDH activity, and the anti-inflammatory effect by IL-6 production. The change in mRNA of toll like receptor 4 （TLR4） was evaluated by real time PCR. Colitis was induced by feeding BALB/C mice with 2.0% dextran sodium sulfate. These mice received either 5% lyophilized *** （n=7） or control diet （n=7）. Seven days after colitis induction, clinical and histological scores, colon length, and cecal organic acid levels were determined.RESULTS： The *** produced butyrate, acetate, propionate, and lactate at 0.25, 1.0, 0.025 and 0.07 retool/L, respectively in medium. At this concentration, each acid had no growth stimulating activity on T84 cells; however, when these acids were mixed together at the above levels, it showed significantly high activity than control. Except for lactate, these acids significantly attenuated IL-6 production at just 0.1 mmol/L. In addition, under TNF-α stimulation, butyrate attenuated the production of TLR4 mRNA. The treatment with *** significantly attenuated clinical and histological scores of colitis with an increase of cecal butyrate levels, compared with the control group.CONCLUSION： *** can ameliorate experimental colonic inflammation. In part, the metabolite of ***, butyrate, increases mucosal integrity and shows anti-inflammatory action modulation of mucosal defense system via TLR4.

Nanomedicine and drug delivery strategies for treatment of inflammatory bowel disease

Crohn's disease and ulcerative colitis are two important categories of human inflammatory bowel disease(IBD). Because the precise mechanisms of the inflammation and immune responses in IBD have not been fully elucidated,the treatment of IBD primarily aims to inhibit the pathogenic factors of the inflammatory cascade. Inconsistencies exist regarding the response and side effects of the drugs that are currently used to treat IBD. Recent studies have suggested that the use of nanomedicine might be advantageous for the treatment of intestinal inflammation because nanosized molecules can effectively penetrate epithelial and inflammatory cells. We reviewed nanomedicine treatments,such as the use of small interfering RNAs,antisense oligonucleotides,and anti-inflammatory molecules with delivery systems in experimental colitis models and clinical trials for IBD based on a systematic search. The efficacy and usefulness of the treatments reviewed in this manuscript have been demonstrated in experimental colitis models and clinical trials using various types of nanomedicine. Nanomedicine is expected to become a new therapeutic approach to the treatment of IBD.

To investigated characteristics, diagnosis, bowel-cleansing preparation, sedation, and colonoscope length and diameter in Japanese pediatric patients receiving total colonoscopy. METHODSThe present study evaluated consecutive patients aged ≤ 15 years who had undergone their first colonoscopy in Kurume University between January 2007 and February 2015. Data were retrospectively analyzed. We identified 110 pediatric patients who had undergone colonoscopy that had reached the cecum, allowing the observation of the total colon. RESULTSHematochezia, abdominal pain, and diarrhea were the most common symptoms. For bowel-cleansing preparation, pediatric patients aged ≤ 12 years were treated with magnesium citrate, and patients aged 13-15 years were treated with polyethylene glycol 4000. For sedation, thiamylal with pentazocine, which has an analgesic effect, was used in patients aged ≤ 6 years, and midazolam with pentazocine was used in patients aged ≥ 7 years. Regarding the choice of endoscope, short and thin endoscopes were selected for younger patients, particularly patients aged ≤ 3 years. Positive diagnoses were made in 78 patients (70.9%). Inflammatory bowel disease (n = 49, 44.5%), including ulcerative colitis (n = 37, 33.6%) and Crohn’s disease (n = 12, 10.9%), was the most common diagnosis. CONCLUSIONColonoscopy offers a high diagnostic capability for pediatric patients with gastrointestinal symptoms. The selection of appropriate management the performance of colonoscopy is important in pediatric patients.

Prediction of colorectal tumor grade and invasion depth through narrow-band imaging scoring

AIM To determine the usefulness of assigning narrow-band imaging(NBI) scores for predicting tumor grade and invasion depth in colorectal *** A total of 161 colorectal lesions were analyzed from138 patients who underwent endoscopic or surgical resection after conventional colonoscopy and magnifying endoscopy with NBI. The relationships between the surface and vascular patterns of the lesions, as visualized with NBI, and the tumor grade and depth of submucosa(SM) invasion were determined *** were assigned to distinct features of the surface microstructures of tubular and papillary-type *** a multivariate analysis, a model was developed for predicting the tumor grade and depth of invasion based on NBI-finding *** NBI findings that correlated with a high tumor grade were associated with the "regular/irregular"(P < 0.0001)surface patterns and the "avascular area" pattern(P =0.0600). The vascular patterns of "disrupted vessels"(P = 0.0714) and "thick vessels"(P = 0.0133) but none of the surface patterns were associated with a depth of invasion of ≥ 1000 μm. In our model, a total NBIfinding score ≥ 1 was indicative of a high tumor grade(sensitivity: 0.97; specificity: 0.24), and a total NBIfinding score ≥ 9(sensitivity: 0.56; specificity: 1.0) was predictive of a SM invasion depth ≥ 1000 μm. Scores less than these cutoff values signified adenomas and a SM invasion depth < 1000 μm, respectively. Associations were also noted between selected NBI findings and tumor tissue architecture and *** Our multivariate statistical model for predicting tumor grades and invasion depths from NBI-finding scores may help standardize the diagnosis of colorectal lesions and inform therapeutic strategies.

Inflammatory bowel disease(IBD), including Crohn's disease and ulcerative colitis, is a chronic intestinal inflammation of unknown etiology. The diagnosis of IBD is based on endoscopic, radiologic and histopathologic criteria. Recently, the search for a noninvasive marker that could augment or replace part of this diagnostic process has become a focus of IBD research. In this review, antibody markers, including microbial antibodies, autoantibodies and peptide antibodies, will be described, focusing on their common features. At present, no single marker with qualities that are satisfactory for the diagnosis and treatment of IBD has been identified, although panels of some antibodies are being evaluated with keen interest. The discovery of novel IBD-specific and sensitive markers is anticipated. Such markers could minimize the use of endoscopic and radiologic examinations and could enable clinicians to implement individualized treatment plans designed to improve the long-term prognosis of patients with IBD.

A Case Report of Kissing Molars Class II

Kissing molars (KMs) is a condition of occlusal molar surfaces in a single follicular space with roots extending in opposite directions. Here, we have reported a case of KMs in a 58-year-old woman. The patient complained of pain in the right mandibular molar region and was diagnosed with KMs consisting of the right mandibular second and third molars. Because of the patient’s pain having subsided at the time of the visit and her unwillingness to undergo tooth extraction, the patient was followed up. KMs is classified into three classes (I-III) and is either true-KMs or pseudo-KMs and presents with or without cystic variants of dental follicles. The presents as true-KMs class II without a cystic variant. With reference to the literature and based on our analysis, the mean age of patients affected by this specific case of KMs is 31.7 years and unilateral KMs is relatively more common (85.7%). Histopathological findings of dentigerous cysts are more often indicated (42.9%). The treatment policy for KMs should therefore be based on the classification of KMs. Importantly, the focus should be on preserving the first and second molars as much as possible. The treatment approach, such as the employment of surgical removal or orthodontics, should be determined by considering the associated factors such as the crowns, tissues, and age of the patient.

Recently, many channelized rivers tend to be heavily vegetated due to regime shifts in hydrological, fluvial and ecological processes. Dense vegetation in a river frequently obstructs a flood flow and reduces conveyance capacity of channels. On the other hand, river vegetation provides various ecological services such as habitats for various species and life, natural cycle of organic and inorganic substances, etc.. It is of engineering importance to understand vegetation hydrodynamics in order to preserve vegetation nature and keep a certain level of flow conveyance capacity. In view that willows tend to be densely vegetated along the shoreline of floodplains or sandbars, a field measurement, a physical model experiment and a numerical analysis were carried out for investigating hydrodynamics in an open channel with riparian vegetation. Discussion was made focusing on flow and shear layer structures developed around the vegetation canopy.

Formation and Annealing of BaTiO_3 and SrTiO_3 Nanoparticles in KOH Solution

Barium titanate （BaTiO3） and strontium titanate （SrTiO3） nanoparticles were synthesized separately through hydrothermal reaction of crystalline TiO2 particles and corresponding alkaline earth hydroxides, Ba（OH）2 and St（OH）2 respectively, in 50 mol·dm^-3 KOH solution at 150℃. Each structural evolution of BaTiO3 and SrTiO3 during the hydrothermal treatment was investigated by X-ray diffraction （XRD）, field emission scanning electron microscopy （FE-SEM）, field emission transmission electron microscopy （FE-TEM） and thermogravimetry- differential analysis （TG-DTA）. In the BaTiO3 system, round particles with cubic perovskite-type structure were obtained within 1 h. However, these particles were gradually dissolved and then were re-precipitated in the form of cube-shape BaTiO3 particles with a smaller lattice constant ac than that of the former phase. After the BaTiO3 particles formed firstly have disappeared completely, or the two phases coexistence stage with different lattice constant ac passed, lattice constant ac of BaTiO3 phase re-precipitated continuously decreased with annealing time. In contrast, once SrTiO3 particles are formed, the lattice constant ac decreased continuously throughout the reaction. The result indicates that SrTiO3 particles were annealed without dissolution and precipitation process under the present condition.
