A 19-year-old female was diagnosed with ulcerative colitis when she presented with persistent melena, and has been treated with 5-aminosalicylic acid for 4 years, with additional azathioprine for 2 years at our hospital. The patient experienced high-grade fevers, chills, and cough fve d prior to presenting to the outpatient unit. At frst, the patient was suspected to have developed neutropenic fever; however, she was diagnosed with Epstein-Barr virus-associated hemophagocytic syndr-ome （EB-VAHS） upon fulfilling the diagnostic criteria after bone marrow aspiration. When patients withinflammatory bowel disease treated with immunomo-dulators, such as thiopurine preparations, develop fever, EB-VAHS should be considered in the differential diagnosis.

Recently, various control methods represented by proportional-integral-derivative （PID） control are used for robotic control. To cope with the requirements for high response and precision, advanced feedforward controllers such as gravity compensator, Coriolis/centrifugal force compensator and friction compensators have been built in the controller. Generally, it causes heavy computational load when calculating the compensating value within a short sampling period. In this paper, integrated recurrent neural networks are applied as a feedforward controller for PUMA560 manipulator. The feedforward controller works instead of gravity and Coriolis/centrifugal force compensators. In the learning process of the neural network by using back propagation algorithm, the learning coefficient and gain of sigmoid function are tuned intuitively and empirically according to teaching signals. The tuning is complicated because it is being conducted by trial and error. Especially, when the scale of teaching signal is large, the problem becomes crucial. To cope with the problem which concerns the learning performance, a simple and adaptive learning technique for large scale teaching signals is proposed. The learning techniques and control effectiveness are evaluated through simulations using the dynamic model of PUMA560 manipulator.

In this paper, a vibration motion control is proposed and implemented on a foamed polystyrene machining robot to suppress the generation of undesirable cusp marks, and the basic performance of the controller is verified through machining experiments of foamed polystyrene. Then, a 3 dimensional （3D） printer-like data interface is proposed for the machining robot. The 3D data inter- face enables to control the machining robot directly using stereolithography （STL） data without conducting any computer-aided man- ufacturing （CAM） process. This is done by developing a robotic preprocessor that helps to remove the need for the conventional CAM process by directly converting the STL data into cutter location source data called cutter location （CL） or cutter location source （CLS） data. The STL is a file format proposed by 3D systems, and recently is supported by many computer aided design （CAD）/CAM soft- waxes. The STL is widely used for rapid prototyping with a 3D printer which is a typical additive manufacturing system. The STL deals with a triangular representation of a curved surface geometry. The developed 3D printer-like data interface allows to directly control the machining robot through a zigzag path, rectangular spiral path and circular spiral path generated according to the information included in STL data. The effectiveness and usefulness of the developed system are demonstrated through actual machining experiments.

Although aligned arrays of semiconducting single-walled carbon nanotubes (s-SWNTs) are promising for use in next-generation electronics owing to their ultrathin bodies and ideal electrical properties, even a small portion of metallic (m-) counterparts causes excessive leakage in field-effect transistors (FETs). To fully exploit the benefits of s-SWNTs for use in large-scale systems, it is necessary to completely eliminate m-SWNTs from as-grown SWNT arrays and thereby obtain purely semiconducting large-area arrays, wherein numerous FETs can be flexibly built. In this study, we performed electrical burning of m-SWNTs assisted by water vapor and polymer coating to eliminate m-SWNTs over a long length for the scalable fabrication of transistors from the remaining s-SWNT arrays. During the electrical-breakdown process, the combination of water vapor and the polymer coating significantly enhanced the burning of the SWNTs, resulting in a self-sustained reaction along the nanotube axis. We found that m-SWNT segments partially remaining on the anode side resulted from one-way burning from the initial breakdown position, where Joule-heating-induced oxidation first occurred. The s-SWNT-enriched arrays obtained were used to fabricate multiple FETs with a high on-off current ratio. The results indicate the advantages of this approach over conventional electrical breakdown for the large-scale purification of s-SWNTs.

Associations of body mass index and hospital-acquired disability with post-discharge mortality in older patients with acute heart failure

OBJECTIVES To investigate the effect of hospital-acquired disability(HAD)on all-cause mortality after discharge according to the body mass index(BMI)in older patients with acute decompensated heart *** We included 408 patients aged≥65 years who were hospitalized for acute decompensated heart failure and had undergone an acute phase of cardiac rehabilitation at the Sakakibara Heart Institute between April 2013 and September 2015(me-dian age:82 years,interquartile range(IQR):76-86;52%male).Patients were divided into three groups based on BMI at hospital admission:underweight(<18.5 kg/m^(2)),normal weight(18.5 to 25 kg/m^(2)),and overweight(≥25 kg/m^(2)).HAD was defined as a decrease of at least five points at discharge compared to before hospitalization according to the Barthel *** The median follow-up period was 475(IQR:292-730)days,and all-cause mortality during the follow-up period was 84 deaths(21%).According to multivariate Cox regression analysis,being underweight(HR:1.941,95%CI:1.134−3.321,P=0.016)or overweight(HR:0.371,95%CI:0.171−0.803,P=0.012),with normal BMI as the reference,and HAD(HR:1.857,95%CI:1.062−3.250,P=0.030)were independently associated with all-cause *** with HAD exhibited a significantly lower cumulative survival rate in the underweight group(P=0.001)and tended to have a lower cumulative survival rate in the normal weight group(P=0.072).HAD was not significantly associated with cumulative survival in the overweight group(P=0.392).CONCLUSIONS BMI and HAD independently predicted all-cause mortality after discharge in older patients with acute de-compensated heart ***,HAD was significantly associated with higher all-cause mortality after discharge,espe-cially in the underweight group.

Hepatocellular carcinoma(HCC) is the fifth most common cancer and the second leading cause of cancer-related deaths worldwide. Although the prognosis of patients with HCC is generally poor, the5-year survival rate is > 70% if patients are diagnosed at an early stage. However, early diagnosis of HCC is complicated by the coexistence of inflammation and cirrhosis. Thus, novel biomarkers for the early diagnosis of HCC are required. Currently, the diagnosis of HCC without pathological correlation is achieved by analyzing serum α.fetoprotein levels combined with imaging techniques. Advances in genomics and proteomics platforms and biomarker assay techniques over the last decade have resulted in the identification of numerous novel biomarkers and have improved the diagnosis of HCC. The most promising biomarkers,such as glypican-3, osteopontin, Golgi protein-73 and nucleic acids including microRNAs, are most likely to become clinically validated in the near future. These biomarkers are not only useful for early diagnosis of HCC, but also provide insight into the mechanisms driving oncogenesis. In addition, such molecular insight creates the basis for the development of potentially more effective treatment strategies. In this article,we provide an overview of the biomarkers that are currently used for the early diagnosis of HCC.
