Heparin bridge therapy and post-polypectomy bleeding

AIM To identify risk factors for post-polypectomy bleeding(PPB), focusing on antithrombotic agents. METHODS This was a case-control study based on medical records at a single center. PPB was defined as bleeding that occurred 6 h to 10 d after colonoscopic polypectomy and required endoscopic hemostasis. As risk factors for PPB, patient-related factors including anticoagulants, antiplatelets and heparin bridge therapy as well as polyp- and procedure-related factors were evaluated. All colonoscopic hot polypectomies, endoscopic mucosal resections and endoscopic submucosal dissections performed between January 2011 and December 2014 were reviewed. RESULTS PPB occurred in 29(3.7%) of 788 polypectomies performed during the study period. Antiplatelet or anticoagulant agents were prescribed for 210(26.6%)patients and were ceased before polypectomy except for aspirin and cilostazol in 19 cases. Bridging therapy using intravenous unfractionated heparin was adopted for 73 patients. The univariate analysis revealed that anticoagulants, heparin bridge, and anticoagulants plus heparin bridge were significantly associated with PPB(P < 0.0001) whereas antiplatelets and antiplatelets plus heparin were not. None of the other factors including age, gender, location, size, shape, number of resected polyps, prophylactic clipping and resection method were correlated with PPB. The multivariate analysis demonstrated that anticoagulants and anticoagulants plus heparin bridge therapy were significant risk factors for PPB(P < 0.0001). Of the 29 PPB cases, 4 required transfusions and none required surgery. A thromboembolic event occurred in a patient who took anticoagulant. CONCLUSION Patients taking anticoagulants have an increased risk of PPB, even if the anticoagulants are interrupted before polypectomy. Heparin-bridge therapy might be responsible for the increased PPB in patients taking anticoagulants.

An overview of the contribution of acupuncture to thyroid disorders

Thyroid dysfunction, affecting people of all ages, not only damages human growth and energy metabo- lism but is also comorbid with other illnesses such as cardiovascular disease, kidney disease and gastroin- testinal disorders. With the increasing acceptance of alternative and complementary therapies, acupuncture, a traditional Chinese medical practice, has also been employed to address this problem. Analysing 29 clinical projects that were retrieved from 29 major digital databases and include 1757 patients aged 7-79 years from China, Italy, Korea, Macedonia and Russia, this narrative review offers an overview of the efficacy, and evaluated the safe and cost-effective use of acupuncture against hyper- thyroidism, hypothyroidism and thyroid-relevant illnesses. Findings indicated reductions in patient symptoms and improvements in biomarkers where acupuncture was used alone or in combination ther- apy. In addition to showing the role of acupuncture as an alternative and complementary medicine or as an adjunctive therapy for curative and rehabilitative purposes, more well-designed researches are needed to achieve reliable data.

The applications of ultrasonic vibrations for material removal processes exist predominantly in the area of vertical processing of hard and brittle materials. This is because the power generated by vertical vibrating oscillators generates the greatest direct penetration, in order to conduct material removal on workpieces by grains. However, for milling processes, vertical vibrating power has to be transformed into lateral (horizontal) vibration to produce the required horizontal cutting force. The objective of this study is to make use of ultrasonic lateral transformation theory to optimize processing efficiency, through the use of the finite element method for design and analysis of the milling tool. In addition, changes can be made to the existing vibrating system to generate best performance under consistent conditions, namely, using the same piezoelectric ceramics.

Many modern video encoders use the Lagrangian rate-distortion optimization （RDO） algorithm for mode deci- sions during the compression procedure. For each encoding stage, this approach involves minimizing a cost, which is a function of rate, distortion and a multiplier called Lambda. This paper proposes to improve the RDO process by applying two modifications. The first modification is to increase the ac- curacy of rate estimation, which is achieved by computing a non-integer number of bits for arithmetic coding of the syntax elements. This leads to a more accurate cost computation and therefore a better mode decision. The second modification is to search and adjust the value of Lambda based on the char- acteristics of each coding stage. For the encoder used, this paper proposes to search multiple values of Lambda for the intra-4x4 mode decision. Moreover, a simple shift in Lambda value is proposed for motion estimation. Each of these modi- fications offers a certain gain in RDO performance, and, when all are combined, an average bit-rate saving of up to 7.0% can be achieved for the H.264/AVC codec while the same concept is applicable to the H.265/HEVC codec as well. The extra added complexity is contained to a certain level, and is also adjustable according to the processing resources available.

Population management of herbivores is widely applied to decrease damage to forests in the northern hemisphere. Culling more antlerless deer, including females and fawns, is an effective means of reducing the population and helps to improve management efficiency. To increase the efficiency of antlerless-biased culling, we assessed the spatiotemporal distribution of the antlerless ratio in culled sika deer (Cervus nippon) in relation to population density. We hypothesized that the antlerless ratio is higher at the center of the distribution than at the margins of the deer’s range, because dispersal processes differ according to sex and age: Young male deer first disperse into an unoccupied area, and then males may become established in the new area for approximately 10–15 years before the number of females increases. A statistical model revealed spatiotemporal changes in the antlerless ratio of culled sika deer. The change in the ratio basically fit the density distribution of the deer, which was estimated independently on the basis of fecal pellet count surveys. The antlerless ratio estimated from the model increased asymptotically as deer density rose. The results support our hypothesis and suggest that antlerless sika deer will be more easily culled at the center rather than at the margins of the distribution range. These findings should help to increase the efficiency of managing the deer via antlerless-biased culling.
