Fasting biochemical hypoglycemia and related-factors in nondiabetic population: Kanagawa Investigation of Total Check-up Data from National Database-8

Current role of fenofibrate in the prevention and management of non-alcoholic fatty liver disease

Non-alcoholic fatty liver disease(NAFLD) is a common health problem with a high mortality burden due to its liver- and vascular-specific complications. It is associated with obesity, high-fat diet as well as with type 2 diabetes mellitus(T2DM) and metabolic syndrome(MetS).Impaired hepatic fatty acid(FA) turnover together with insulin resistance are key players in NAFLD pathogenesis. Peroxisome proliferator-activated receptors(PPARs)are involved in lipid and glucose metabolic *** novel concept is that the activation of the PPARαsubunit may protect from liver steatosis. Fenofibrate, by activating PPARα, effectively improves the atherogenic lipid profile associated with T2DM and MetS. Experimental evidence suggested various protective effects of the drug against liver steatosis. Namely, fenofibraterelated PPARα activation may enhance the expression of genes promoting hepatic FA β-oxidation. Furthermore, fenofibrate reduces hepatic insulin resistance. It also inhibits the expression of inflammatory mediators involved in non-alcoholic steatohepatitis *** include tumor necrosis factor-α, intercellular cell adhesion molecule-1, vascular cell adhesion molecule-1and monocyte chemoattractant protein-1. Consequently, fenofibrate can limit hepatic macrophage infiltration. Other liver-protective effects include decreased oxidative stress and improved liver microvasculature function. Experimental studies showed that fenofibrate can limit liver steatosis associated with high-fat diet,T2DM and obesity-related insulin resistance. Few studies showed that these benefits are also relevant even in the clinical setting. However, these have certain limitations. High-speed railway tunnels in various countries have continuously reported accidents of vault falling concrete *** the concrete block falling occurs,serious consequences follow,and traffic safety may be *** aerodynamic shockwave evolves from the initial compression wave may be an important inducement causing the tunnel lining cracks to grow and form falling concrete blocks.A joint calculation framework is established based on ANSYS Fluent,ABAQUS,and FRANC3D for calculating the crack tip field under the aerodynamic *** intensification effect of aerodynamic shockwaves in the crack is revealed,and the evolution characteristics of the crack tip field and the influence factors of stress intensity factor(SIF)are *** show that(1)the aerodynamic shockwave intensifies after entering the crack,resulting in more significant pressure in the crack than the input *** maximum pressure of the inclined and longitudinal cracks is higher than the corresponding values of the circumferential crack,respectively.(2)The maximum SIF of the circumferential,inclined,and longitudinal crack appears at 0.5,0.68,and 0.78 times the crack front *** maximum SIF of the circumferential crack is higher than that of the inclined and longitudinal *** possibility of crack growth of the circumferential crack is the highest under aerodynamic shockwaves.(3)The influence of train speed on the SIF of the circumferential crack is more than 40%.When the train speed,crack depth,and crack length change,the change of pressure in the crack is the direct cause of the change of SIF.

Enhancement of the thermoelectric performance of half-metallic full-Heusler Mn_(2)VAl alloys via antisite defect engineering and Si partial substitution

A half-metallic full-Heusler Mn_(2)VAl alloy is a potential p-type thermoelectric material that can directly generate electricity from waste heat via the Seebeck *** practical use,the Seebeck coefficient S of Mn_(2)VAl should be increased while maintaining a high electrical conductivity s from its half-metallic ***,we achieved this objective through antisite defect ***,it was predicted that the S was maximized by regulating partial density of states of majority-spin sp-electrons through the control of the fraction of antisite defect,f_(AD),between V and Al atoms in Mn_(2)***-mentally,a significant increase in S and a slight decrease in s were observed for an Mn_(2)VAl sample with an optimal fAD=33%,enhancing the thermoelectric power factor PF by 2.7 times from an Mn_(2)VAl sample with fAD=14%.Furthermore,we combined the antisite defect engineering with a partial substitution *** Mn_(2)V(Al_(0.96)Si_(0.04))sample with fAD=33%exhibited the highest PF=4.5×10^(-4)W·m^(-1)·K^(-2)at 767 K among the *** maximum dimensionless figure-of-merit zT of the Mn_(2)V(Al_(0.96)Si_(0.04))sample with f_(AD)=33%was measured to be 3.4×10^(-2)at 767 K,which is the highest among the p-type half-metallic full-Heusler alloys.

Salvage brachytherapy for locally recurrent prostate cancer after external beam radiotherapy

External beam radiotherapy （EBRT） is a standard treatment for prostate cancer. Despite the development of novel radiotherapy techniques such as intensity-modulated conformal radiotherapy, the risk of local recurrence after EBRT has not been obviated. Various local treatment options （including salvage prostatectomy, brachytherapy, cryotherapy, and high-intensity focused ultrasound [HIFU]） have been employed in cases of local recurrence after primary EBRT. Brachytherapy is the first-line treatment for low-risk and selected intermediate-risk prostate tumors. However, few studies have examined the use of brachytherapy to treat post-EBRT recurrent prostate cancer. The purpose of this paper is to analyze the current state of our knowledge about the effects of salvage brachytherapy in patients who develop locally recurrent prostate cancer after primary EBRT. This article also introduces our novel permanent brachytherapy salvage method.

Resistivity of reservoir sandstones and organic rich shales on the Barents Shelf: Implications for interpreting CSEM data

Marine controlled source electromagnetic(CSEM) data have been utilized in the past decade during petroleum exploration of the Barents Shelf, particularly for de-risking the highly porous sandstone reservoirs of the Upper Triassic to Middle Jurassic Realgrunnen Subgroup. In this contribution we compare the resistivity response from CSEM data to resistivity from wireline logs in both water-and hydrocarbon-bearing wells. We show that there is a very good match between these types of data, particularly when reservoirs are shallow. CSEM data, however, only provide information on the subsurface resistivity. Careful, geology-driven interpretation of CSEM data is required to maximize the impact on exploration success. This is particularly important when quantifying the relative resistivity contribution of high-saturation hydrocarbon-bearing sandstone and that of the overlying cap rock. In the presented case the cap rock comprises predominantly organic rich Upper Jurassic–Early Cretaceous shales of the Hekkingen Formation(i.e. a regional source rock). The resistivity response of the reservoir and its cap rock become merged in CSEM data due to the transverse resistance equivalence principle. As a result of this, it is imperative to understand both the relative contributions from reservoir and cap rock, and the geological significance of any lateral resistivity variation in each of the units. In this contribution, we quantify the resistivity of organic rich mudstone, i.e. source rock, and reservoir sandstones, using 131 exploration boreholes from the Barents Shelf. The highest resistivity(>10,000 Ωm) is evident in the hydrocarbon-bearing Realgrunnen Subgroup which is reported from 48 boreholes, 43 of which are used for this study.
