Effectiveness of Traditional Chinese Medicine on coronavirus disease 2019 in 92 patients:a retrospective study

OBJECTIVE:To investigate the antipyretic effect of early treatment with Traditional Chinese Medicine(TCM) on coronavirus disease 2019(COVID-19) ***:We retrospectively analyzed 369 patients from January 26th,2020 to April 15th,2020,who had been diagnosed with *** 92 eligible cases,45 cases were identified as treatment group Ⅰ(n = 45) and 47 cases were identified as treatment group Ⅱ.Patients in the treatment group Ⅰ were treated with TCM herbal decoction within 5 d after *** in the treatment group Ⅱ were treated with TCM herbal decoction after the 6th admission *** onset time of antipyretic effect,the antipyretic time,the time of negative oropharyngeal swab nucleic acid conversion,and the changes of cell count in blood routine test were ***:The treatment group Ⅰ showed shorter average antipyretic duration(4 vs 7 d;P 38 ℃,patients in the treatment group I had shorter median onset time of antipyretic effect than those in the treatment group Ⅱ(3 vs 4 d;P < 0.05).The absolute lymphocyte(LYMPH) count and absolute eosinophil(EOS) count on the 3rd day after admission and the neutrophil/lymphocyte ratio on the 6th day after admission of patients in the treatment group I were notably different from those in the treatment group Ⅱ at the same time point(P < 0.05).Based on Spearman’s rank correlation analysis,the change of body temperature on the 3rd day after admission was positively correlated with the increase of EOS count and the increase of EOS count and LYMPH counts on the 6th day after admission(P < 0.01).CONCLUSIONS:Early TCM intervention within 5 d after hospital admission shortened the onset time of antipyretic effect and fever duration of COVID-19 patients,reduced the time required for PCR test results to turn ne

Effect of "Yang-warming and kidney essence-replenishing" herbal paste on cold-related asthma exacerbation

OBJECTIVE: To observe the effect of "Yang-warming and kidney essence-replenishing" herbal paste on cold-related asthma exacerbation. METHODS: One hundred and fifty one patients with moderate to severe persistent asthma were randomly divided into a treatment group (n=74) and a control group (n=69). Both groups were given basic treatment according to the Global Initiative for Asthma scheme. The treatment group was also treated with herbal paste in the winter. The frequency of asthma exacerbation, cold and cold-re-lated asthma exacerbation, scores of the asthma control test (ACT), and kidney-deficiency syndrome during the one-year follow-up in each group were recorded. RESULTS: Compared with the control group, the onset frequencies of catching cold, asthma exacerbation, cold-related asthma exacerbation, and kidney-deficiency syndrome score were significantly reduced. There was no significant difference in scores of ACT between the two groups. CONCLUSION: "Yang-warming and kidney essence-replenishing" herbal paste could reduce cold-related asthma exacerbation.

目的观察复方佛耳草合剂对慢性阻塞性肺疾病(chronic obstructive pulmonary disease,COPD)大鼠气道炎症的调控作用。方法 60只大鼠分为正常组、模型组、丝裂原活化蛋白激酶p38(p38 mitogen-activated protein kinase,p38MAPK)抑制剂组、复方佛耳草合剂低、中、高剂量组。除正常组外,其余大鼠均采用香烟烟雾暴露联合气管注射脂多糖(lipopolysaccharide,LPS)建立COPD模型大鼠。模型建立成功后给予24周药物治疗,采用动物肺功能仪测定大鼠肺功能相关参数的变化,观察支气管-肺组织病理与超微结构改变,实时荧光定量聚合酶链式反应(Real time quantitative polymerase chain reaction, RT-qPCR)法测定肿瘤坏死因子(tumor necrosis factor,TNF)-α、白细胞介素(interleukin,IL)-8、p38MAPK及重组人激活转录因子-2(activating transcription factor-2,ATF-2)m RNA表达,Western blot法测定p38MAPK、ATF-2蛋白表达。结果不同剂量复方佛耳草合剂均能改善大鼠通气功能障碍,修复受损肺组织,显著下调肺组织中TNF-α、IL-8、p38MAPK及ATF-2 m RNA水平(P<0.05,P<0.01)与p38MAPK、ATF-2蛋白表达(P<0.05,P<0.01),其中复方佛耳草合剂高剂量组疗效最佳。结论复方佛耳草合剂具有改善COPD肺部炎症的作用,其具体机制可能与抑制p38MAPK通路相关。

清肺颗粒对慢阻肺急性加重期患者的血清炎症因子及外周血单核细胞TOLL样受体表达的影响

目的:观察清肺颗粒对慢性阻塞性肺疾病急性加重痰热郁肺证患者的血清炎症因子(IL-6、IL-8、TNF-α)以及外周血单核细胞Toll受体(TLR2、TLR4)表达等的影响。方法:将符合AECOPD(痰热郁肺证)的120例患者分为两组,每组各60例。治疗组60例予基础西医加服清肺颗粒治疗,对照组60例予基础西医加服清肺颗粒模拟剂治疗,两组治疗时间均为10d。观察两组治疗前后的中医证侯疗效、中医主症积分、血清炎症因子(IL-6、IL-8、TNF-α)水平、外周血单核细胞Toll样受体(TLR-2、TLR-4)表达水平的差异。结果:最终共有108例患者完成研究,其中治疗组55例、对照组53例。治疗组的中医证侯疗效为90.91%,显著高于对照组的疗效(73.58%),P<0.05;两组患者治疗后咳、痰、喘症状均较治疗前改善(P<0.05),且治疗组在咳、痰、喘三项主症的改善程度方面优于对照组(P<0.05);两组患者治疗后血清IL-6、IL-8、TNF-α水平均下降(P<0.05),治疗组血清IL-6、IL-8、TNF-α水平的下调明显优于对照组(P<0.05);两组患者治疗后TLR2、TLR4水平均较治疗前上升(P<0.05),且治疗组上升更明显(P<0.05)。结论:血清炎症因子与外周血单核细胞TOLL样受体表达具有负相关性,表明清肺颗粒在减轻气道炎症反应的同时又可增强机体免疫功能;两组患者至治疗结束后心电图、肝肾功能、尿常规均未见明显异常变化,说明清肺颗粒能较好地减轻患者咳、痰、喘症状,在降低气道炎症反应的同时有效调节机体固有免疫,且临床应用安全。

目的:探讨复方佛耳草合剂对慢性阻塞性肺疾病(chronic obstructive pulmonary disease,COPD)大鼠气道重塑的作用机制。方法:60只大鼠随机分为正常对照组、模型对照组、地塞米松0.5 mg/kg组、复方佛耳草合剂6.8、13.6、27.2 g/kg组。除正常对照组外,其余大鼠均采用香烟烟雾暴露联合气管注射脂多糖(LPS)建立COPD模型大鼠,复方佛耳草合剂和地塞米松于造模后第12 w灌胃给药,正常对照组和模型对照组灌胃给予等体积生理盐水,连续24 w。采用动物肺功能仪测定大鼠肺功能相关参数的变化;观察各组大鼠肺部病理变化;测定气管壁面积(WAt)、平滑肌面积(WAm)、管腔内周长(Pi)及平滑肌细胞核数(N);ELISA法测定血清及肺泡灌洗液中基质金属蛋白酶-2(MMP-2)、基质金属蛋白酶-9(MMP-9)、转化生长因子β1(TGF-β1)及基质金属蛋白酶抑制剂-1(TIMP-1)含量变化;实时荧光定量PCR、Western blot法检测肺组织Mmp2、Mmp9、Tgfb1及Timp1 mRNA和蛋白表达情况。结果:与正常对照组比较,模型对照组大鼠存在肺通气功能障碍,肺组织和支气管病理改变明显,全身及肺组织MMP-2、MMP-9、TGF-β1以及TIMP-1含量升高,肺组织Mmp2、Mmp9、Tgfb1及Timp1 mRNA和蛋白表达显著上调(P<0.01);与模型对照组比较,复方佛耳草合剂13.6、27.2 g/kg组和地塞米松0.5 mg/kg组均能够改善肺通气功能障碍,减少支气管管壁平均厚度和平滑肌面积,降低血清与肺泡灌洗液MMP-2、MMP-9、TGF-β1及TIMP-1含量,下调肺组织Mmp2、Mmp9、Tgfb1及Timp1 mRNA和蛋白表达(P<0.05或P<0.01)。结论:复方佛耳草合剂能够改善COPD大鼠肺组织损伤和气道重塑,其机制可能与其能够下调MMP-2、MMP-9、TGF-β1以及TIMP-1蛋白和mRNA表达有关。
