Patients with familial hypercholesterolemia(FH)have elevated low-density lipoprotein cholesterol(LDL-C)levels and are at high risk of premature cardiovascular disease.1 Heterozygous FH(HeFH)is one of the commonest genetic disorders,and is more frequent among those with ischemic heart disease(IHD),atherosclerotic cardiovascular disease(ASCVD)and premature IHD.

目的:评价心房颤动及心房扑动智能评分系统在门诊的应用情况及来诊患者特点。方法:收集2020年8月至2022年6月于中国医学科学院阜外医院门诊就诊的25 223例心房颤动及心房扑动患者的所有就诊记录。比较系统更新(2021年9月)前后来诊患者的特点。另从25 223例患者中筛选出系统更新前后1年内相邻2次就诊的患者404例,比较2次评分的变化趋势及抗凝药物使用情况。结果:与系统更新前比,系统更新后CHA_(2)DS_(2)-VASc评分中男性≥2分或女性≥3分的患者比例差异无统计学意义(P>0.05),这些患者中抗凝治疗比例升高(25.2%vs. 35.9%,P0.05)。结论:本院门诊所应用的智能评分系统可有效帮助医生掌握来诊心房颤动及心房扑动患者的血栓栓塞和出血风险及相应的变化趋势,并帮助改善抗凝药物的使用。但还需进一步优化评分流程,规范患者评估及治疗。

他汀不耐受是指患者服用他汀类药物后出现一种或多种他汀类药物相关不良反应,减量或停用他汀类药物可改善,重启后再次出现的临床现象。他汀不耐受的相关报道并非少见,但真实世界中他汀不耐受的临床诊断与处理存在一定的困惑或误区,包括缺乏统一的定义和诊断标准、诊断的把握度偏宽、处理策略欠科学等问题,这在一定程度上阻碍了他汀类药物的科学、合理地使用。据此,本共识全面复习全球他汀不耐受相关研究数据,参考国外他汀不耐受相关指南与共识,提出适合我国人群的他汀不耐受的基本定义,并推荐相应的诊断标准及处理策略,以期改善我国人群他汀类药物的临床应用现状,进而提高动脉粥样硬化性心血管疾病的防治水平。

Background：Despite its severity,coronary artery ectasia （CAE） is still poorly ***-sensitivity C-reactive protein （hs-CRP） has been recognized as a prognostic factor in some cardiovascular diseases but not assessed in *** aim of this observational study was to investigate the prognostic value of hs-CRP in ***：Our analysis evaluated the effect of the baseline hs-CRP on cardiovascular events （CVs）（cardiac death and nonfetal myocardial infarction） in consecutively enrolled stable CAE *** used the Cox proportional hazards regression models to examine the association between baseline hs-CRP level and follow-up CVs in *** net reclassification improvement and integrated discrimination improvement （IDI） of hs-CRP were also ***：We obtained the follow-up results of 540 patients over a median follow-up period of 36 （37.41 ± 15.88） *** multivariable Cox analysis showed that the hs-CRP was a significant predictor of adverse outcomes in CAE （hazard ratio [HR]：2.99,95% confidence interval [CI]：1.31-6.81,P =0.0091）.In Kaplan-Meier analysis,the group with hs-CRP 〉3 mg/L had a lower cumulative 66-month event-free survival rate （log-rank test for trend,P =0.0235） and a higher risk ofCVs （HR =2.66,95% CI：1.22-5.77,P =0.0140） than the group with hs-CRP ≤3 mg/***-CRP added predictive information beyond that given by the baseline model comprising the classical risk factors （P value for IDI =0.0330）.Conclusions：A higher level of hs-CRP was independently associated with cardiac death and nonfatal myocardial infarction in CAE *** hs-CRP level may therefore provide prognostic information for the risk stratification of CAE patients.

Objective Dilated cardiomyopathy(DCM) is generally considered to be accompanied by both left and right ventricular dysfunction,but most studies only analyze the left ventricular function. In this study,we evaluated the effect of arotinolol on right ventricular function in patients with DCM. Methods Right ventricular ejection fraction(RVEF) and right ventricular diameter(RVD) were measured by two-dimensional echocardiography(2-DE) in 33 DCM patients;RVEF measured by first-pass radionuclide angiography(FPRA) was compared with that by 2-DE. Results The treatment with arotinolol for one year resulted in a reduction in the right ventricular diameter(baseline,23.0 ± 8.3 mm vs after one-year treatment,20.7 ± 5.4 mm;P=0.004 ) and an associated increase in ejection fraction(baseline,36.9 ± 10.3% vs after one-year treatment,45.8 ± 9.6%;P < 0.001 ) ;there is a high correlation between the 2-DE method and radionuclide ventriculographic method. The correlation coefficient is 0.933(P<0.001) . Conclusion Arotinolol therapy could not only improve left ventricular function,but also improve right ventricular function in DCM patients.

Objective Periprocedural myocardial infarction (PMI) after percutaneous coronary intervention (PCI) is the most common procedural complication and may lead to serious clinical outcome. Clinical studies of PMI have primarily attempted to identify predictors of PMI. Also, randomised trials have been performed to investigate if certain drugs could reduce the rate of PMI after PCI. However, there are no established risk models for risk prediction of PMI. Clinical prediction models involving clinical, angiographic and PCI procedural indices are needed.
