A 66-year-old female presented with the main complaint of defecation trouble and abdominal distention. With diagnosis of rectal cancer, c SS, c N0, c H0, c P0, c M0 c Stage Ⅱ, Hartmann's operation with D3 lymph node dissection was performed and a para-aortic lymph node and a disseminated node near the primary tumor were resected. Histological examination showed moderately differentiated adenocarcinoma, p SS, p N3, p H0, p P1, p M1(para-aortic lymph node, dissemination) f Stage Ⅳ. After the operation, the patient received chemotherapy with FOLFIRI regimen. After 12 cycles of FOLFIRI regimen, computed tomography(CT) detected an 11 mm of liver metastasis in the posteroinferior segment of right hepatic lobe. With diagnosis of liver metastatic recurrence, we performed partial hepatectomy. Histological examination revealed moderately differentiated adenocarcinoma as a metastatic rectal cancer with cut end microscopically positive. After the second operation, the patient received chemotherapy with TS1 alone for 2 years. Ten months after the break, CT detected a 20 mm of paraaortic lymph node metastasis and a 10 mm of lymph node metastasis at the hepato-duodenal ligament. With diagnosis of lymph node metastatic recurrences, we performed lymph node dissection. Histological examination revealed moderately differentiated adenocarcinoma as metastatic rectal cancer in paraaortic and hepato-duodenal ligament areas. After the third operation, we started chemotherapy with modified FOLFOX6 regimen. After 2 cycles of modified FOLFOX6 regimen, due to the onset of neutropenia and liver dysfunction, we switched to capecitabine aloneand continued it for 6 mo and then stopped. Eleven months after the break, CT detected two swelling 12 mm of lymph nodes at the left supraclavicular region. With diagnosis of Virchow lymph node metastatic recurrence, we started chemotherapy with capecitabine plus bevacizumab regimen. In this paper, the improved Background Oriented Schlieren technique called CBOS (Colored Background Oriented Schlieren) is described and used to reconstruct the density fields of three-dimensional flows. The Background Oriented Schlieren technique (BOS) allows the measurement of the light deflection caused by density gradients in a compressible flow. For this purpose the distortion of the image of a background pattern observed through the flow is used. In order to increase the performance of the conventional Background Oriented Schlieren technique, the monochromatic background is replaced by a colored dot pattern. The different colors are treated separately using suitable correlation algorithms. Therefore, the precision and the spatial resolution can be highly increased. Furthermore a special arrangement of the different colored dot patterns in the background allows astigmatism in the region with high density gradients to be overcome. For the first time an algebraic reconstruction technique (ART) is then used to reconstruct the density field of unsteady flows around a spike-tipped model from CBOS measurements. The obtained images reveal the interaction between the free-stream flow and the high-pressure region in front of the model, which leads to large-scale instabilities in the flow.

Purpose: To improve the image resolution of magnetic resonance imaging (MRI), conventional interpolation methods are commonly used to magnify images via various image processing approaches;however, these methods tend to produce artifacts. While super-resolution (SR) schemes have been introduced as an alternative approach to apply medical imaging, previous studies applied SR only to medical images in 8-bit image format. This study aimed to evaluate the effectiveness of sparse-coding super-resolution (ScSR) for improving the image quality of reconstructed high-resolution MR images in 16-bit digital imaging and communications in medicine (DICOM) image format. Materials and Methods: Fifty-nine T1-weighted images (T1), 84 T2-weighted images (T2), 85 fluid attenuated inversion recovery (FLAIR) images, and 30 diffusion-weighted images (DWI) were sampled from The Repository of Molecular Brain Neoplasia Data as testing datasets, and 1307 non-medical images were sampled from the McGill Calibrated Color Image Database as a training dataset. We first trained the ScSR to prepare dictionaries, in which the relationship between low- and high-resolution images was learned. Using these dictionaries, a high-resolution image was reconstructed from a 16-bit DICOM low-resolution image downscaled from the original test image. We compared the image quality of ScSR and 4 interpolation methods (nearest neighbor, bilinear, bicubic, and Lanczos interpolations). For quantitative evaluation, we measured the peak signal-to-noise ratio (PSNR) and structural similarity (SSIM). Results: The PSNRs and SSIMs for the ScSR were significantly higher than those of the interpolation methods for all 4 MRI sequences (PSNR: p p Conclusion: ScSR provides significantly higher image quality in terms of enhancing the resolution of MR images (T1, T2, FLAIR, and DWI) in 16-bit DICOM format compared to the interpolation methods.

A clinical trial of radiotherapy with modified simultaneous integrated boost(SIB)technique against huge tumors was conducted.A 58-year-old male patient who had a huge pelvic tumor diagnosed as a rectal adenocarcinoma due to familial adenomatous polyposis was enrolled in this *** total dose of 77 Gy(equivalent dose in 2Gy/fraction)and 64.5 Gy was delivered to the center of the tumor and the surrounding area respectively,andapproximately 20%dose escalation was achieved with the modified SIB *** tumor with an initial maximum size of 15 cm disappeared 120 d after the start of the *** status of the patient improved from 4 to *** with modified SIB may be effective for patients with a huge tumor in terms of tumor shrinkage/disappearance,improvement of QOL,and prolongation of survival.

BACKGROUND Emergency pancreaticoduodenectomy(EPD)is a rare event for complex periam-pullary *** intraoperative blood loss is correlated with poor post-operative *** SUMMARY Two patients underwent EPD using a no-touch isolation technique,in which all arteries supplying the pancreatic head region were ligated and divided before manipulation of the pancreatic head and *** operative times were 220 and 239 min,and the blood loss was 70 and 270 g,*** patients were discharged on the 14^(th) and 10^(th) postoperative day,***-two patients underwent EPD for the treatment of neoplastic *** mean operative time was 361.6 min,and the mean blood loss was 747.3 *** comp-lication rate was 37.5%.The in-hospital mortality rate was 9.38%.CONCLUSION The no-touch isolation technique is feasible,safe,and effective for reducing intraoperative blood loss in EPD.

Genetic polymorphisms of MAFK, encoding a small Maf protein, are associated with susceptibility to ulcerative colitis in Japan

To investigate whether single nucleotide polymorphisms in maf protein K (MAFK), which encodes the MAFK, lead to increased susceptibility to ulcerative colitis in the Japanese population. METHODSThis case control study examined the associations between MAFK single nucleotide polymorphisms (rs4268033 G>A, rs3735656 T>C and rs10226620 C>T) and ulcerative colitis susceptibility in 174 patients with ulcerative colitis (UC) cases, and 748 subjects without no lower abdominal symptoms, diarrhea or hematochezia (controls). In addition, as the second controls, we set 360 subjects, who have an irregular bowel movement without abnormal lower endoscopic findings (IBM controls). RESULTSThe genotype frequency of rs4268033 AA and allelic frequency of the rs4268033A allele were significantly higher in the UC cases than in both controls (P = 0.0005 and < 0.0001, P = 0.015 and 0.0027 vs controls and IBM controls, respectively). Logistic regression analysis after adjustment for age and gender showed that the rs4268033 AA and rs3735656 CC genotypes were significantly associated with susceptibility to UC development (OR = 2.63, 95%CI: 1.61-4.30, P = 0.0001 and OR = 1.81; 95%CI: 1.12-2.94, P = 0.015, respectively). Similar findings were observed by the comparison with IBM controls. In addition, the rs4268033 AA genotype was significantly associated with all phenotypes of UC except early onset. There was no significant association between rs10226620 and ulcerative colitis. CONCLUSIONOur results provide the first evidence that MAFK genetic polymorphisms are significantly associated with susceptibility to UC development. In particular, rs4268033 is closely associated with an increased risk for the development of UC.

Hemophagocytic Lymphohistiocytosis Caused by Pyogenic Liver Abscess during Pregnancy: A Case Report and Literature Review

Introduction: Hemophagocytic lymphohistiocytosis during pregnancy is a rare and severe condition, and timely diagnosis is quite difficult. We present the first case of hemophagocytic lymphohistiocytosis caused by pyogenic liver abscess during pregnancy and discuss the clinical presentation. Case Presentation: A 26-year-old Japanese primigravida at 23 weeks of gestation complained of extremely high fever with a fast heart rate (140 beats per minute). She presented with systemic inflammatory response syndrome (SIRS). Only 2 days later, she died. Autopsy findings proved that this was the first case of hemophagocytic lymphohistiocytosis caused by pyogenic liver abscess during pregnancy. Conclusion: Hemophagocytic lymphohistiocytosis should be considered when patients meet the SIRS criteria, especially whose clinical presentation includes extremely high fever (39°C) and a fast heart rate (greater than 110 bpm) during pregnancy, despite relatively normal laboratory data, because such vital signs may be associated with the onset of hemophagocytic lymphohistiocytosis.
