Drug-induced lung injury caused by acetaminophen in a Japanese woman:A case report

BACKGROUND All drugs have the potential to cause drug-induced lung injury both during and after drug administration.Acetaminophen has been reported to cause druginduced lung injury,although this is extremely rare.Herein,we present an extremely rare case of acetaminophen-induced pneumonia.CASE SUMMARY A healthy 35-year-old Japanese woman visited a neighborhood clinic with complaints of fever and malaise following a tick bite.Her treatment included1,500 mg acetaminophen(Caronal^(?)) and subsequently minocycline(200 mg) and acetaminophen(2,000 mg;Caronal^(?)) daily when her condition did not improve;the patient was eventually hospitalized.The patient’s chest computed tomography(CT) revealed consolidation and ground-glass opacities in the right middle and lower lobes.Minocycline was shifted to sulbactam/ampicillin.However,her fever did not improve during follow-up,and her chest CT revealed extensive ground-glass opacities in the right middle and lower lobes and thick infiltrative shadows in the bilateral basal areas.Drug-induced lung injury was suspected;hence,acetaminophen was discontinued.The fever resolved immediately,and inflammatory response and respiratory imaging findings improved.A drug-induced lymphocyte stimulation test was performed against acetaminophen(Caronal^(?)),and significant proliferation of lymphocytes was noted only for acetaminophen(stimulation index,2.1).CONCLUSION Even common drugs such as over-the-counter drugs can cause drug-induced lung damage.

Integrating the effect of abutments in estimating the average vertical stress of elastic hard rock pillars by combining numerical modelling and artificial neural networks

Estimating average vertical pillar stresses is a critical step in designing room-and-pillar mines.Several analytical methods can be used to estimate the vertical stresses acting on the pillars.However,the present analytical methods fail to adequately account for the influence of abutments on the distribution of vertical stresses,especially when applied to narrow panel widths and pillar layouts comprising evenly spaced barriers.In this study,a multi-layer perceptron neural network(MLPNN)was applied to predict the vertical loads of regular pillars more accurately.Hundreds of room-and-pillar mine layouts were modeled using a displacement discontinuity method(DDM),and a database of 2355 sampled pillar cases was compiled.The MLPNN was trained based on this database,and its prediction capabilities were further validated using simulations by a finite difference code(i.e.,FLAC3D).The model predictions and the FLAC3D simulations reasonably agreed with a regression coefficient of 0.99.The model was also adapted for mine cases with evenly spaced barrier pillars,and its application to a real case study mine has shown to provide accurate pillar stress estimations;hence,this model is suitable for practical use at mines.Even though the MLPNN model cannot be applied universally to all mine situations,it seems as a significant improvement over existing analytical techniques in terms of accounting for the influence of abutments on pillar stresses.

Efficient Origin-Destination Estimation Using Microscopic Traffic Simulation with Restricted Rerouting

Traffic simulators are utilized to solve a variety of traffic-related problems.For such simulators,origin-destination(OD)traffic volumes as mobility demands are required to input,and we need to estimate them.The authors regard an OD estimation as a bi-level programming problem,and apply a microscopic traffic simulation model to it.However,the simulation trials can be computationally expensive if full dynamic rerouting is allowed,when employing multi-agent-based models in the estimation process.This paper proposes an efficient OD estimation method using a multi-agent-based simulator with restricted dynamic rerouting to reduce the computational load.Even though,in the case of large traffic demand,the restriction on dynamic rerouting can result in heavier congestion.The authors resolve this problem by introducing constraints of the bi-level programming problem depending on link congestion.Test results show that the accuracy of the link traffic volume reproduced with the proposed method is virtually identical to that of existing methods but that the proposed method is more computationally efficient in a wide-range or high-demand context.

Spurious-mode vibrations caused by alternating current poling and their solution process for Pb(Mg_(1/3)Nb_(2/3))O_(3)-PbTiO_(3) single crystals

The effects of alternating current poling(ACP)at 80℃on electrical properties of[001]-oriented 0.72 Pb(Mg_(1/3)Nb_(2/3))O_(3)-0.28PbTiO_(3)(PMN-28PT)single crystals(SCs)have been investigated.The square-wave ACP SCs poled at high voltage(HV,5 kV_(rms)/cm)occasionally showed large fluctuations and low opposite values of piezoelectric coefficient(d_(33)=±1370 pC/N)in one plate.This revealed spuriousmode vibrations(SMV)of impedance spectrum.However,after depolarizing and repolarizing the sample with a sine-wave ACP at low voltage(LV,3.5 kV_(rms)/cm),the d_(33) enhanced to be 1720 pC/N(+26%)and did not exhibit large fluctuation or opposite values in one plate any more.The impedance spectrum became clean and the abnormal SMV disappeared.We proposed four possible mechanisms of the SMV,and speculate that the main cause maybe by macro-scale sub-domain structure and/or phase change in the main domain structure and/or phase in the SC plate due to the specific poling conditions not eternal mechanical damage of PMN-PT SCs.This study will be useful to realize a high d_(33) and improve other properties of PMN-PT ACP SC ultrasonic transducers without any SMV for high-frequency medical imaging equipment.

BACKGROUND A 70-year-old man with hepatitis C virus-related recurrent hepatocellular carcinoma was admitted for further diagnosis of a 1 cm iso-hyperechoic nodule in segment(S)5.CASE SUMMARY Gadolinium ethoxybenzyl diethylenetriamine pentaacetic acid-enhanced magnetic resonance imaging(EOB-MRI)revealed the nodule in S5 with a defect at the hepatobiliary phase,hyperintensity on diffusion weighted imaging(DWI)and hypointensity on apparent diffusion coefficient(ADC)map.Contrast-enhanced computed tomography revealed hypervascularity at the early phase,and delayed contrast-enhancement was observed at the late phase.Contrast-enhanced ultrasound(US)revealed incomplete defect at the late vascular phase.Inflammatory liver tumor,lymphoproliferative disease,intrahepatic cholangiocarcinoma(small duct type)and bile duct adenoma were suspected through the imaging studies.US guided biopsy,however,showed a noncaseating hepatic sarcoid-like epithelioid granuloma(HSEG),and histopathological analysis disclosed spindle shaped epithelioid cells harboring Langhans-type multinucleated giant cells.One month after admission,EOB-MRI signaled the disappearance of the defect at the hepatobiliary phase,of hyperintensity on DWI,of hypointensity on ADC map,and no stain at the early phase.CONCLUSION That the patient had received BNT162b2 messenger RNA(mRNA)coronavirus disease 2019 vaccination 3 mo before the occurrence of HSEG,and that its disappearance was confirmed 4 mo after mRNA vaccination suggested that the drug-induced sarcoidosis-like reaction(DISR)might be induced by the mRNA vaccination.Fortunately,rechallenge of drug-induced DISR with the third mRNA vaccination was not confirmed.

Flat friction spot joining of aluminum alloy to carbon fiber reinforced polymer sheets:Experiment and simulation

The Al alloy and carbon fiber reinforced polymer(CFRP)hybrid structures,incorporating the performance advantages of the two materials,have been attracting more attention in high-end manufacturing fields.In the current investigation,the flat friction spot joining(FSJ)was employed in joining the AA6061-T6 alloy and CFRP sheets.The significance of temperature distribution in influencing joint quality was highlighted through analyzing interface microstructural features,weld defect formation as well as fractography.To understand the role of thermal energy generation and conduction in the process comprehensively,a 3D thermal-mechanical coupling finite element model was established.The interfacial temperature was characterized by an uneven distribution behavior due to the inhomogeneous heat distribution.The peak temperatures on the top surface and Al alloy to CFRP interface at 1500 rpm rotational speed with 0.1 mm/s plunging speed were 498℃and 489°C,respectively.The peak interface temperature was reduced to286℃at 250 rpm,which produced an extremely small melted area.Compared with the plunging speed,rotational speed was found to be the predominant parameter for determining the joint property,which could be optimized to simultaneously realize the avoidance of thermal decomposition of CFRP,the sufficient melting duration time,and the wide enough melted area.Simulated thermal histories and melted area profiles were in agreement with experimental ones.The findings could be utilized to provide some feasible guidance for process optimization of dissimilar FSJ of metals and composites.

Effect of the processing route on the microstructure and mechanical behavior of superlight Mg-9Li-1Zn alloy via friction stir processing

In this study, the effect of the processing route using a friction stir processing(FSP) method on the microstructure and mechanical behavior of a Mg-9Li-1Zn alloy was systematically investigated. In the FSP method, the odd-numbered(1st and 3rd) process directions and even-numbered(2nd and 4th) passes were alternated to distribute the strain throughout the whole processed zone uniformly. Consequently, the processed zone had a much more uniform microstructure and hardness distribution than the processed zone obtained using the conventional FSP method. Using this method, the grain size of a Mg-9Li-1Zn sheet alloy was refined from ~31 μm to ~0.21 μm with uniformly distributedα and β phases. The processed alloy exhibited a high strength-ductility synergy with an ultimate tensile strength(UTS) of 220.1 MPa and total elongation of 70.0% at a strain rate of 10^(-3)s^(-1), overwhelmingly higher than those of the base metal, 155.6 MPa in UTS and 36.0%in elongation. The in-situ SEM-DIC analysis and TEM observation demonstrated that such an outstanding ductility with moderate strength is caused by grain boundary sliding, the dominant deformation mechanism of the ultra-fine-grained sample after FSP. The processing route with reverse processing direction was proven to be efficient in producing the ultrafine grain size microstructure and improving the mechanical properties of superlight Mg-9Li-1Zn alloy.

Association of a Tobacco-specific Nitrosamine Carcinogen with Urinary Cotinine,Urinary Sodium Excretion,and Total Energy Intake in Adolescents and Children

This study investigated the association of a tobacco-specific nitrosamine carcinogen,4-(methylnitrosamino)-1-(3-pyridy1)-I-butanol(NNAL)with urinary cotinine(uCot),urinary sodium(uNa)excretion,systolic blood pressure(sBP),and total energy intake in adolescents and children in relation to the subjects'age.A total of 790 subjects aged 6-19 years were evaluated.NNAL,uCot,corrected NNAL(cNNAL),the NNAL/uCot ratio,uNa,sBP,and nutrient intake were measured.A strong association between uCot and cNNAL was observed in children who were 11 years of age(r=0.881,P<0.001);however,no significant association was noted in adolescents who were 19 years of age.The uNa level was significantly higher(133.9 mmol/L vs.107.8 mmol/L,P<0.001)and sBP was significantly lower(105.3 mmHg vs.110.6 mmHg,P=0.012)in adolescents with elevated NNAL than in those without elevated NNAL.NNAL was significantly higher in subjects with increased uNa excretion than in those without increased uNa excretion.NNAL was positively correlated with uNa(r=0.183,P<0.001)and negatively correlated with sBP(r=0.142,P<0.001).Non-smokers with elevated NNAL/uCot ratios had significantly lower total energy intake than those without elevated NNAL/uCot ratios(1729.0 kcal/day vs.1911.0 kcal/day,P=0.008).The relationship between NNAL and uCot varied according to the subjects'age.NNAL seems to play a role in decreasing sBP by enhancing uNa excretion.Insufficient nutrient intake may contribute to endogenous formation of NNAL in non-smoking adolescents and children.

Potential sex differences in activation of pain-related brain regions in nonhuman primates with a unilateral spinal nerve ligation

The lack of truly robust analgesics for chronic pain is owed,in part,to the lack of an animal model that reflects the clinical pain state and of a mechanismbased,objective neurological indicator of pain.The present study examined stimulus-evo ked brain activation with functional magnetic resonance imaging in male and female cynomolgus macaques following unilateral L7 spinal nerve ligation and the effects of clinical analgesics pregabalin,duloxetine,and morphine on brain activation in these macaques.A modified straight leg raise test was used to assess pain severity in awake animals and to evo ke regional brain activation in anesthetized animals.The potential effects of clinical analgesics on both awake pain behavior and regional brain activation were examined.Following spinal nerve ligation,both male and female macaques showed significantly decreased ipsilateral straight leg raise thresholds,suggesting the presence of radicula rlike pain.Morphine treatment increased straight leg raise thresholds in both males and females whereas duloxetine and pregabalin did not.In male macaques,the ipsilateral straight leg raise activated contralateral insular and somatosensory cortex(Ins/SII),and thalamus.In female macaques,the ipsilateral leg raise activated cingulate cortex and contralateral insular and somatosensory cortex.Straight leg raises of the contralateral,unligated leg did not evoke brain activation.Morphine reduced activation in all brain regions in both male and female macaques.In males,neither pregabalin nor duloxetine decreased brain activation compared with vehicle treatment.In females,however,pregabalin and duloxetine decreased the activation of cingulate cortex compared with vehicle treatment.The current findings suggest a diffe rential activation of brain areas depending on sex following a peripheral nerve injury.Diffe rential brain activation observed in this study could underlie qualitative sexual dimorphism in clinical chronic pain perception and responses to analgesics.Future pain management approaches fo
