Background:Voltage-gated L-type Ca channel has been suggested to be involved in many essential processes by the Ca-dependent regulation,including muscle contraction,gene expression and hormone secretion. When Ca ions flow through the pore of an individual Ca channel,both positive and negative feedback mechanisms: Ca-dependent facilitation(CDF) that enhances calcium entry and Ca -dependent inactivation(CDI) that limits calcium entry,respectively,have been indicated in those ***(CaM) is an important regulator of ion channels,serving as a Ca sensor for Ca1.2 calcium *** binding of Ca to CaM, bound to the amino-terminal(N-terminus) and carboxy-terminal(C-terminus) tail of the pore-formingα1 subunit can enhance channel opening and accelerate channel closing on the Ca1.2 *** the other hand,CaM is a protein formed by two lobes,each comprising two Ca binding *** mutated at Ca binding sites 1 and 2 in the amino-terminal(N-lobe,CaM12) and CaM mutated at Ca binding sites 3 and 4 in the carboxy-terminal lobe (C-lobe,CaM34) have been produced to demonstrate Ca-dependent regulation of Ga1.2 *** previous findings have highlighted that both N-lobe and C-lobe of CaM are of critical importance in the modulation of Ca1.2 calcium ***,the detailed mechanisms of how Ca and CaM modulate Ca2+ channel are not yet *** In an attempt to further explore the detailed roles of CaM and its mutants(CaM12 and CaM34) in the concentration and calcium regulation of Ca1.2 channel,we set out to apply CaM and its mutants to investigate the facilitation and inactivation changes of Ca1.2 channel by inside-out patch clamp technique in guinea pig ventricular *** and Methods:The point mutations for CaM12 and CaM34 were introduced into the Ca binding sites in CaM by means of a Quickchange TM site-directed mutagenesis *** CaM12,Glu was changed to Ala at two conserved calcium ligands in the first(E31 A-CaM) and second(E67A-Ca

Practical guidelines for diagnosis and early management of drug-induced liver injury

The spectrum of drug-induced liver injury （DILI） is both diverse and complex. The first step in diagnosis is a suspicion of DILl based on careful consideration of recent comprehensive reports on the disease. There are some situations in which the suspicion of DILI is particularly strong. Exclusion of other possible etiologies according to the pattern of liver injury is essential for the diagnosis. In patients with suspected DILl, diagnostic scales, such as the Councils for International Organizations of Medical Sciences/ Roussel Uclaf Causality Assessment Method （CIOMS/RUCAM） scale, may be helpful for the final diagnosis. Early management of DILl involves prompt withdrawal of the drug suspected of being responsible, according to serum levels of alanine aminotransferase （ALT）, alkaline phosphatase （ALP）, and total bilirubin （T-Bil）. However, as DILI patients may show resolution of liver injury without discontinuation of the drug, it should be carefully evaluated whether the suspected drug should be discontinued immediately with adequate consideration of the importance of the medication.

Recent advances in the management of pruritus in chronic liver diseases

Cooling Curve's Effect on Hardening Behavior of Steel

Quenching Oils are comprised mostly of mineral oil as major component, but mineral oil alone result in lower cooling performance. Therefore, various additives are formulated as required for each particular case. Cooling curves of mineral oil as determined by JIS method can be divided into three stages, vapor film stage, boiling stage and convection stage. And there are many types of additives used for quenching oil, one effective to shorten vapor film stage and another to shift boiling stage toward lower temperature side. As rapid quenching required temperature range for particular steel varies pending on its shape of CCT curve"1, it is possible to provide higher quench hardening performance using quenching oil formulated of adequate additives.

Systemic abnormalities in liver disease

Systemic abnormalities often occur in patients with liver disease. In particular, cardiopulmonary or renal diseases accompanied by advanced liver disease can be serious and may determine the quality of life and prognosis of patients. Therefore, both hepatologists and non-hepatologists should pay attention to such abnormalities in the management of patients with liver diseases.

A case of hemangiopericytoma of the soft palate with articulate disorder and dysphagia

We report a case of hemangiopericytoma of the soft palate of 60-year-old patient, who noticed a mass of the soft palate and experienced difficulty in speaking. We found a pediculate, hard, elastic mass measuring 38 mm （cross-sectional diameter）. Computed tomography （CT） scans and dynamic magnetic resonance imaging （MRI） confirmed irregularly shaped mass and revealed a heterogeneous internal composition, consistent with vascular tumors. We excised the tumor under general anesthesia. Histopathological diagnosis was based on positive immunoreactivity of CD99 and vimentin and weak, positive staining of CD34. Three and half years following tumor excision, there is no recurrence or metastasis.

Influence of oxygen flow rate and compost addition on reduction of organic matter in aerated waste layer containing mainly incineration residue

Landfilling municipal solid waste incineration （MSWI） residue alkalizes the waste layer, causing a subsequent decrease in microbial activity and a delay in the decomposition of organic matter. In this study, efficiencies of neutralization of the leachate and organic matter decomposition in the waste layer in a column filled with MSWI residue using aeration and compost addition were evaluated. Total organic carbon （TOC） reduction in the waste layer is large at high oxygen flow rate （OFR）. To effectively accelerate TOC reduction in the waste layer to which compost was added, a high OFR exceeding that by natural ventilation was required. At day 65, the pH of the leachate when OFR was above 102 mol-O2/（day.m3） was lower than that when OFR was below 101 mol-Oz/（day.m3）. At the same OFR, the pH of waste sample was lower than that of waste sample with compost. Although leachate neutralization could be affected by compost addition, TOC reduction in the waste layer became rather small. It is possible that humic substances in compost prevent the decomposition of TOC in MSWI residue.

Efficacy and Safety of the Atrial Septal Defect Closure for Patients with Absent or Malaligned Aortic Rim Using a Figulla Flex II Device Flared and Straddling Behind the Aorta

Background:Although transcatheter closure of atrial septal defect(ASD)is safe and effective for patients with sufficient rim,ASD patients with absent and/or malaligned aortic and/or superior rim have higher risks of device embolization and cardiac *** have treated such high-risk patients using a Figulla Flex II(FFII)device shaped flared and straddling behind the aorta because this method would avoid such serious ***,its long-term efficacy and safety remain ***,the midterm efficacy and safety of this method were ***:We retrospectively evaluated the outcome of 47 consecutive patients with such rim(age 6–73 years,weight 17–75 kg,31 females)treated with this method at our hospital between February 2016 and September *** make the flared and straddling shape,we selected a FFII 4–6 mm larger than the balloon sizing diameter by stop-flow *** also studied the device shape,the disc pressure to the Valsalva wall and their changes over 6 months by transesophageal ***:All procedures were successful,and leakage disappeared within 1 *** a mean follow up of 37±12 months,complications included a transient sinus node dysfunction and one I°atrioventricular block within 3 *** device shape changed from bulky to thin:the device waist and thickness significantly decreased by around 1.5 mm and 3.5 mm,respectively(p<0.05),but the two discs remained flared and straddling behind the aorta over 6 months;therefore,the disc edges seldom pressed the Valsalva wall perpendicularly,even though the inner plane of either disc often slightly pressed the ***:ASD closure using a FFII shaped flared and straddling behind the aorta is probably effective and safe for patients with absent and/or malaligned aortic and/or superior rim although requiring care for conduction disorders.

Secretory carcinoma——impact of translocation and gene fusions on salivary gland tumor

Secretory carcinoma（SC）, previously described as mammary analogue secretory carcinoma（MASC）, is a recently described salivary gland tumor which morphologically resembles mammary secretory carcinoma. The first description of SC/MASC, reported by Skálová et al. in 2010, was as a rare salivary carcinoma imitating secretory carcinoma of the breast. SC/MASC is a unique salivary gland tumor with morphological overlap with acinic cell carcinoma（Aci CC）, mucoepidermoid carcinoma（MEC）, and adenocarcinoma not otherwise specified（ADCNOS）. SC/MASC shares similar clinicopathological features with Aci CC. As a critical difference between SC/MASC and Aci CC, SC/MASC characteristically has the chromosomal translocation t（12;15）（p13;q25） which leads to a fusion gene between the ETV6 gene on chromosome 12 and the NTRK3 gene on chromosome 15. This genetic background is an important differential diagnostic finding for excluding other salivary gland tumors and may be a critical factor determining the prognosis for patients with SC/MASC. Research in recent years has provided a large body of new data on SC/MASC and suggests the possibility that the ETV6-NTRK3 translocation could be a therapeutic target. Here, we review the morphological and clinicopathological features of SC/MASC and discuss new directions for therapy.

Endoscopic and histopathological study on the duodenum of Strongyloides stercoralis hyperinfection

AIM:To investigate endoscopic and histopathological findings in the duodenum of patients with Strongyloides stercoralis(***)hyperinfection. METHODS:Over a period of 23 years(1984-2006),we investigated 25 patients *** hyperinfection who had had an esophagogastroduodenoscopy before undergoing treatment for *** clinical and endoscopic findings were analyzed retrospectively. RESULTS:Twenty-four(96%)of the patients investigated were under immunocompromised condition which was mainly due to a human T lymphotropic virus type 1(HTLV-1)*** abnormal endoscopic findings,mainly edematous mucosa,white villi and erythematous mucosa,were observed in 23(92%) *** degree of duodenitis including villous atrophy/destruction and inflammatory cell infiltration corresponded to the severity of the endoscopic findings. The histopathologic yield for identifying larvae was 71.4% by duodenal *** endoscopic findings of duodenitis were more severe in patients whose biopsies were positive for larvae than those whose biopsies were negative(Endoscopic severity score:4.86±2.47vs 2.71 ±1.38,P<0.05). CONCLUSION:Our study clearly demonstrates that,in addition to stool analysis,endoscopic observation and biopsies are very *** also emphasize that *** and HTLV-1 infections should be ruled out before immunosuppressive therapy is administered in endemic regions.
