Use of Human Embryonic Stem Cell-Derived Cardiomyocyte Clusters to Assess Potential for Chronic Treatment with Compounds to Cause QT Prolongation

Drug-induced QT prolongation is a serious clinical issue in developing novel drug candidates and marketing drugs. A major cause of QT prolongation is direct inhibition of human ether-à-go-go-related gene (hERG) channels. Reduction in repolarization-related channel expression levels on plasma membranes is another mechanism that induces QT prolongation. Recently, we established a system for assessing the risk of QT prolongation by using human embryonic stem cell-derived cardiomyocyte clusters (hES-CMCs) in which the field potential duration (FPD) or corrected FPD (FPDc) was measured as an indicator of drug-induced QT interval prolongation. Here, we examined whether this system was able to detect FPDc prolongation caused by pentamidine or probucol, both of which can induce QT prolongation after long-term treatment. hES-CMCs were treated with pentamidine or probucol, and the FPDc of the same clusters was measured 10 min, 4 h, and 24 h after the start of treatment. Concentration-dependent FPDc prolongation was observed at 24 h, but not at 10 min, with pentamidine or probucol treatment. These results suggest that the hES-CMC-based assessment system can be used to detect both acute (at 10 min) and delayed (at 24 h) QT prolongation risk on the same platform by simple alteration of the extended culture period.

E3024 (3-but-2-ynyl-5-methyl-2-piperazin-1-yl-3,5-dihydro-4H-imidazo[4,5-d]pyridazin-4-one tosylate) is a dipeptidyl peptidase-IV (DPP-IV) inhibitor that was expected to be an antidiabetic agent. Its safety, tolerability, pharmacokinetics (PK), and pharmacodynamics (PD) were investigated in a randomized, double-blind, placebo-controlled, ascending single-dose study in 48 healthy Japanese male subjects. Fasted subjects were orally administered E3024 (5, 10, 20, 40, or 80 mg) or placebo. E3024 was rapidly absorbed, with tmax values ranging between 0.33 and 3 h after dosing. The mean t1/2 ranged from 5.34 to 11.68 h. AUC0-inf and Cmax increased dose-proportionately. PK-PD relationship of E3024 was evaluated by using an Imax model, indicating that plasma E3024 concentrations and inhibitory effects of plasma DPP-IV activity were well correlated. The IC50 value was calculated as 33.7 ng/mL, which was consistent with in vitro data. Thus, E3024 showed a good PK profile and inhibited DPP-IV dose-dependently. Of 30 subjects administered E3024, 12 (40%) experienced adverse events (AEs). Dose escalation to 160 mg was abandoned owing to undesired subjective/objective findings in 4 of 6 subjects receiving 40 mg and 5 of 6 subjects receiving 80 mg. The most prominent AE was rash, but there were no serious AEs or deaths. The maximum tolerated dose was considered to be 20 mg. We hypothesized that histamine was a cause of the rash induction, and examined blood histamine levels of normal Fischer rats treated with E3024. Blood histamine levels were increased significantly by E3024 at 500 mg/kg (p < 0.001), but not by vildagliptin or valine-pyrrolidide (DPP-IV inhibitors) at the same dose. No blood histamine increases were observed in genetically mast cell-deficient Ws/Ws rats treated with E3024 at 500 mg/kg. We established a QT interval assessment system that uses human embryonic stem cell-derived cardiomyocyte clusters (hES-CMCs) in which the field potential duration (FPD) or corrected FPD (FPDc) was measured as an indicator of drug-induced QT interval prolongation. To investigate the applicability of the hES-CMC system to drug safety assessment, we investigated short-term variability in FPDc (STVFPDc) (beat rate rhythmicity) as a marker of torsadogenic risk. We investigated the FPDc and STVFPDc of hES-CMCs treated with hERG channel blockers (E-4031 or cisapride) or with our proprietary compounds X, Y, and Z. We also evaluated the electrocardiograms and hemodynamics of dogs treated with compound X, Y, or Z. The torsadogenic hERG channel blockers increased STVFPDc and prolonged FPDc. Compounds X, Y, and Z had hERG inhibitory activity. Compound X prolonged FPDc with increased STVFPDc, whereas compounds Y and Z tended to shorten FPDc in the hES-CMC system. In the in vivo canine study, compound X prolonged corrected QT (QTc), and compounds Y and Z tended to shorten QTc, showing a good correlation with the results in hES-CMCs. These findings suggest that combined assessment of FPDc and STVFPDc in the hES-CMC system increases the predictability of torsadogenic risk.

Threshold dynamics of reaction-diffusion partial differential equations model of Ebola virus disease

We study the reaction-diffusion Ebola PDE model that consists of equations that govern the evolution of susceptible, in fee ted, recovered and deceased human individuals, as well as Ebola virus pathogens in the environment, with diffusive terms in all except the equation of the deceased human individuals. Under the setting of a spatial domain that is bounded, we prove the global well-posedness of the system;in contrast to the previous work on similar models such as cholera, avian influenza, malaria and dengue fever, diffusion coefficients may be different. Moreover, we derive its basic reproduction number, and under the condition that the diffusion coefficients of the susceptible and infected hosts are same, we prove the global stability of the disease-free-equilibrium, and uniform persistence in cases when the basic reproduction number lies beneath and above one, respectively. Again, we do not require that the diffusion coefficients of the recovered hosts be the same as the diffusion coefficients of the susceptible and infected hosts, in contrast to previous work on other models of infectious diseases. Another technical difficulty in our model is that the solution semiflow is not compact due to the lack of diffusion in the equation of the deceased human individuals;we overcome this difficulty using functional analysis techniques concerning Kura to wski measure of non-compactness.

Surgical treatment of hepatocellular carcinoma: Evidence-based outcomes

Surgeons may be severely criticized from the perspective of evidence-based medicine because the majority of surgical publications appear not to be convincing. In the top nine surgical journals in 1996, half of the 175 publications refer to pilot studies lacking a control group, 18% to animal experiments, and only 5% to randomized controlled trials (RCT). There are five levels of clinical evidence:level 1 (randomized controlled trial), level 2 (prospective concurrent cohort study), level 3 (retrospective historical cohort study), level 4 (pre-post study), and level 5 (case report). Recently, a Japanese evidence-based guideline for the surgical treatment of hepatocellular carcinoma (HCC) was made by a committee (Chairman, Professor Makuuchi and five members). We searched the literature using the Medline Dialog System with four Keywords:HCC, surgery, English papers, in the last 20 years. A total of 915 publications were identified systematically reviewed. At the first selection (in which surgery-dominant papers were selected), 478 papers survived. In the second selection (clearly concluded papers), 181 papers survived. In the final selection (clinically significant papers), 100 papers survived. The evidence level of the 100 surviving papers is shown here:level-1 papers (13%), level-2 papers (11%), level-3 papers (52%), and level-4 papers (24%);therefore, there were 24% prospective papers and 76% retrospective papers. Here, we present a part of the guideline on the five main surgical issues:indication to operation, operative procedure, peri-operative care, prognostic factor, and post-operative adjuvant therapy.

Evaluation of interfacial strength by an instrumented indentation method and its application to an actual TBC vane

The thermal fatigue behaviour of an air plasma sprayed thermal barrier coating was investigated. And also the interfacial strengths of thermal barrier coated specimens subjected to thermal fatigue, as well as a retired TBC vane were also evaluated by means of an instrumented indentation machine. The results indicated that, （1） the TGO grew at the interface during thermal fatigue cycle as a function of the exposure time at elevated temperature; （2） the microcracks were initiated in the top coating and at the interface after thermal cycle tests; （3） the interfacial strength of TBC, which was evaluated by the indentation method, increased with the thermal cycles; （4） the interfacial strength of the retired TBC vane was almost equal with that of the as-sprayed TBC specimen.

Electron scattering by Friedel oscillations in carbon nanotubes

Project Scheduling Using Hybrid Genetic Algorithm with Fuzzy Logic Controller in SCM Environment

In supply chain management (SCM) environment, we consider a resource-constrained project scheduling problem (rcPSP) model as one of advanced scheduling problems considered by a constraint programming technique. We develop a hybrid genetic algorithm (hGA) with a fuzzy logic controller (FLC) to solve the rcPSP which is the well known NP-hard problem. This new approach is based on the design of genetic operators with FLC through initializing the serial method which is superior for a large rcPSP scale. For solving these rcPSP problems, we first demonstrate that our hGA with FLC (flc-hGA) yields better results than several heuristic procedures presented in the literature. We have revealed a fact that flc-hGA has the evolutionary behaviors of average fitness better than hGA without FLC.
