Background:To compare the arithmetic mean(M-SIA)and the summated vector mean of surgically induced astigmatism(SVM-SIA)according to the incision site after phakic intraocular lens(Visian implantable collamer lens(ICL),STAAR Surgical)***:This study comprised 121 eyes of 121 consecutive patients undergoing ICL surgery through a 3.0-mm temporal or superior clear corneal *** magnitude and the axis of corneal astigmatism preoperatively and 3 months postoperatively were measured using an automated *** M-SIA and the SVM-SIA were determined according to the incision ***:The magnitude of corneal astigmatism significantly increased from 1.23±0.59 D preoperatively to 1.46±0.72 D postoperatively in the temporal incision group(Wilcoxon signed-rank test,P<0.001),but it significantly decreased from 1.09±0.36 D preoperatively to 0.86±0.41 D postoperatively in the superior incision group(P<0.001).The M-SIA was 0.48±0.30 D,and the SVM-SIA was 0.23±0.52 D at a meridian of 82°in the temporal incision *** M-SIA was 0.57±0.30 D,and the SVM-SIA was 0.47±0.45 D at a meridian of 1°in the superior incision ***:ICL implantation induces the M-SIA by approximately 0.5 D,but the SVM-SIA decreased to 50%and 80%of the M-SIA in magnitude through temporal and superior incisions,*** direction of the SVM-SIA showed a tendency toward corneal flattening to the incisional *** should be noted that the M-SIA is somewhat different from the SVM-SIA according to the incision site.

AIM: To examine the effects of Kampo medicine, Keishi-ka-Shakuyaku-to (TJ-60) on the signal transduction in diabetic gastrointestinal dysfunction. METHODS: Experimental diabetic models were prepared using streptozotocin (STZ)-treated Wistar rats. Randomly selected STZ rats were treated with insulin (12 U·kg-1·d-1) or TJ-60 (1% of food intake). Diacylglycerol (DG) and DG kinase activities were quantified in isolated aortic smooth muscle tissue. RESULTS: One of the key element of the Pi-turnover, DG kinase activity in resting state in gastric smooth muscle was significantly increased compared to the control value, and carbachol (CCh)-induced response was not detectable, but it was detected in the control rats. On the other hand resting activity in ileum did not differ from the control, but the CCh-induced responses were suppressed. Variation in Texture and Lankford Value of 1070 Aluminum Sheet Rolled by Cone-shaped Roll

A rolling with cone-shaped roll, the diameter of which continuously varies along the axial direction, has been proposed as a new shear rolling for controlling the texture of an aluminum alloy sheet. In this study, variations in the texture and Lankford value of a 1070 aluminum sheet rolled by the cone-shaped roll were investigated. Rolling with the cone-shaped roll was found to impose intense shear strain at the edges of the specimen, specifically near the surface. The shear directions in the left and right portions of the specimen were opposite to each other. The surface and middle layer of the specimen rolled by the cone-shaped roll and the reference specimen were characterized by a shear texture and typical recrystallization texture components, respectively. Notably, the specimen rolled by the cone-shaped roll exhibited smaller texture intensity than the reference specimen, especially at the surface, and the shear texture-components were observed at relatively deeper positions. As a result of Lankford value measurements, the specimen rolled with the cone-shape roll exhibited a smaller planar anisotropy than the reference specimen and an average Lankford value close to unity, which are likely due to the texture modifications introduced during rolling with the cone-shaped roll.

Anti-inflammatory effect of recombinant thrombomodulin for fulminant hepatic failure

Fulminant hepatic failure(FHF) is a critical illness that can be comorbid to primary liver *** shows a high mortality rate,and patients with FHF require intensive therapy,including plasma ***,intensive care at the present is not enough to restore the severe liver damage or promote hepatocellular reproduction,and a standard therapy for the treatment of FHF has not been *** 86-year-old female with FHF was admitted to our *** manifestation demonstrated a clinical situation of systemic inflammatory response syndrome(SIRS) and disseminated intravascular coagulation.A diagnosis of fulminant hepatitis was made according to the definition given in the position paper of the American Association for the Study of Liver *** serum hepatocyte growth factor(HGF) level had increased to 11.84 ng/m *** HGF level indicated massive liver damage as seen in *** thrombomodulin(r TM) was administered daily from the admission day for 1 wk at 380 U/*** patient's white blood cells and C-reactive protein responded to the r TM treatment within a few *** HGF level and PT recovered to the normal *** levels of proinflammatory cytokines(tumor necrosis factor-α and interleukin-1β) were suppressed by the administration of r *** patient's hepatic function(e.g.,PT and albumin) completely recovered without plasma exchange.r TM may modulate the over-response of SIRS with the improvement of proinflammatory *** underlying mechanism is thought to be the inhibitory effect of r TM on highmobility group box 1(HMBG1).The pathogenesis of HMBG1 protein in fulminant hepatic failure has beenalready known.A novel favorable effect of r TM for SIRS would be promising for FHF,and the wide application of r TM for SIRS should be considered.

AIM: To retrospectively investigate the association between dry eye symptoms and clinical or in vivo confocal microscopy parameters in patients with dry eye disease(DED), and to compare these parameters between eyes with DED and normal ***: This retrospective, cross-sectional, controlled study comprised 25 consecutive patients with non-Sj?gren dry eye disease and age-and sex-matched 25 healthy subjects. Each patient underwent a complete examination of the ocular surface in the following order: tear osmolarity measurements, InflammaDry test, tear break-up time, corneal fluorescein staining, Schirmer I test, subjective symptoms questionnaire using the dry eye-related qualityof-life score(DEQS), and in vivo confocal microscopy analysis of the central cornea. Beck depression inventory(BDI) as depressive scale and history of medications and smoking were also evaluated. Stepwise multiple regression analysis was used to assess the factors affecting the ***: In univariate analysis, DEQS was associated with tear break-up time(ρ=-0.48, P=0.01), oral medications, such as hypotensive drug(ρ=0.56, P=0.004) and antidepressant(ρ=0.57, P=0.003), and BDI(ρ=0.61, P=0.001) in patients with DED. In multiple regression analysis, explanatory variables relevant to the DEQS were the anti-depressant medications(P=0.04, partial regression coefficient B=21.04) and BDI(P=0.02, B=0.76, adjusted R2=0.54) in these patients. CONCLUSION: Our study shows a significant association between depression and dry eye symptoms. It suggests that dry eye symptoms associate with higher depressive symptoms and its medications, although our patients were not followed longitudinally.

Successful treatment of advanced hepatocellular carcinoma by combined administration of 5-fluorouracil and pegylated interferon-a

We report a case of hepatocellular carcinoma （HCC） treated successfully by transarterial chemoembolization （TACE） followed by combination therapy of 5-fluorouracil （5-FU） and pegylated interferon-α （PEG-IFN-α）. In the present case, the patient had massive and advanced HCC with a diameter of over 8 cm located in segment 7 （S7） of the liver. Furthermore, the tumor invaded into the major branch of the portal vein （Vp3）. After TACE, combined administration of 5-FU and PEG-IFN-α was performed for 5 too. HCC was totally eradicated and the serum levels of tumor markers were markedly decreased by the treatment. Although it has been reported that the combined use of conventional IFN-α and 5-FU showed striking effects on HCC in some cases, this case may suggest the more promising effect of PEG-IFN-α with a long-lasting effect, in the combined use with 5-FU for the treatment of massive advanced HCC.

Comparing mass screening techniques for gastric cancer in Japan

AIM： To discuss the efficacy of endoscopic mass screening for gastric cancer.METHODS： The data used in this study were the results of mass screening programs for gastric cancer in Niigata City from 2002 to 2004. The number of participants was 35089 in 2002, 34557 in 2003 and 36600 in 2004. The finding ratio referred to the final diagnosis of gastric cancer after a double check of endoscopic files and histological findings. The costs of identifying one case of gastric cancer were calculated based on the total expense for each screening program and additional close examinations. RESULTS： From the analysis of individual screening program with endoscopy, individual screening program with X-ray （ISX） and mass screening program with photofluorography （MSP） in reference to the finding ratio of gastric cancer, endoscopic examination was the best for detecting early gastric cancer, the finding ratio was 0.87% in 2004, approximately 2.7 and 4.6 times higher than those of the ISX and MSP groups. In addition, this novel method was the cheapest means regarding the cost of identifying one case of gastric cancer, which was estimated to be 1608000 Japanese yen in 2004. CONCLUSION： Endoscopic mass screening is a promising method and can be effectively applied if a sufficient number of skilled endoscopists become available to staff the system and if city offices support it.

High-solids anaerobic mono-digestion of riverbank grass under thermophilic conditions

The purpose of this study was to investigate the potential of high-solids anaerobic mono-digestion of riverbank grass under thermophilic conditions, focusing on the effects of the strength and the amount of inoculum. Ensiled grass was inoculated with three different inocula; inoculum from liquid anaerobic digester（LI）, inoculum from dry anaerobic digester（DI）, and mixture of LI and DI（MI）, at feedstock-to-inoculum ratio（FIR） of 1, 2 and 4. The ensiling process of riverbank grass reduced moisture content（p 〉 0.05）, while the hemicellulose content was significantly increased from 30.88% to 35.15%（p 〈 0.05）, on dry matter basis. The highest methane production was at an FIR of 2 with MI（167 L/kg VSadded）,which was significantly higher（p 〈 0.05） than with DI, but not significant compared to LI（p 〉 0.05）. At an FIR of 4, digesters inoculated with LI and DI failed to produce methane,whereas 135 LCH4/kg VSaddedwas obtained with MI. The kinetic studies showed that at an FIR of 1 with LI and MI, the inoculum had less of effects on the hydrolysis rate constant（0.269 day-1and 0.245 day-1） and methane production（135 versus 149 L/kg VSadded）; rather,it affected the lag phase. In a thermophilic HS-AD of riverbank grass, the mixture of inoculum with low and high total solids content（TS） helps increase the TS of inoculum and digestion process. An FIR of 2 was deducted to be the limit for a better startup time and higher volumetric productivity of methane.

Effects of 8-Hydroxyisocapnolactone-2-3-diol and friedelin on mast cell degranulation

ObjectiveTo investigate the effects of friedelin (terpenoid) and 8-hydroxyisocapnolactone-2-3-diol (coumarin) with concentration 10 μM, 30 μM, and 100 μM on inhibiting mast cells (MCs) *** investigation was performed in vitro by administering each compound into rat peritoneal MCs and rat basophilic leukemia-2H3 cells followed by activation with 50 μg/mL of compound 48/80 or 1 μM of ionomycin. The concentration of histamine released from each group was measured by a high-performance liquid chromatography-fluorometry system with post-column derivatization using ***8-Hydroxyisocapnolactone-2-3-diol inhibited degranulation of compound 48/80 activated-rat peritoneal MCs with the histamine release percentages of 74.57%, 72.21% and 51.79% when the 10 μM, 30 μM and 100 μM concentrations were used, respectively. Where as about 81% histamine was released by the control group. Degranulation inhibition ability was also observed in ionomycin-activated rat basophilic leukemia-2H3 cells. In contrast, friedelin failed to inhibit degranulation in either cell type. The inhibition of 8-hydroxyisocapnolactone-2-3-diol was not related to the depletion of histamine synthesis as implied by the total histamine *** results exhibit the promising of 8-hydroxyisocapnolactone-2-3-diol is a potential parent structure for developing a MCs stabilizer.
