Successful initial ablation therapy contributes to survival in patients with hepatocellular carcinoma

AIM： to evaluate the outcome predictors of percutaneous ablation therapy in patients with unresectable hepatocellular carcinoma （HCC）, especially to identify whether the initial treatment response contributes to the survival of the patients. MEtHODS： the study cohort included 153 patients with single （102） and two or three （51） HCC nodules 5 cm or less in maximum diameter. As an initial treatment, 110 patients received radiofrequency ablation and 43 patients received percutaneous ethanol injection. RESULtS： the Kaplan-Meier estimates of overall 3- and S-year survival rates were 75% and 59%, respectively. the log-rank test revealed statistically significant differences in the overall survivals according to ChildPugh class （P = 0.0275）, tumor size （P = 0.0130）, serum albumin level （P = 0.0060）, serum protein induced by vitamin K absence or antagonist Ⅱ level （P = 0.0486）, and initial treatment response （P = 0.0130）. the independent predictors of survival were serum albumin level （dsk ratio, 3.216; 950 CI, 1.407-7.353; P = 0.0056） and initial treatment response （risk ratio, 2.474; 95% CI, 1.076-5.692; P = 0.0330） based on the Cox proportional hazards regression models. the patients had a serum albumin level 3.5 g/dL and the 3- and 5-year survival rates of 86% and 82%. CONCLUSION： In HCC patients treated with percutaneous ablation therapy, serum albumin level and initial treatment response are the independent outcome predictors.

trong>Purpose:trong> traditional medicines have unique breathing methods and they are widely believed that they are good for health promotion and good for relax. However, there is little evidence. the purpose of this study is to discuss if such specific breathings could effectively reduce stress and be able to help to reach a relaxed state in the body as well as in the mind. to objectively understand the state of stress/relax, we assessed the condition of the autonomic nervous system (ANS) as well as heart rate (HR) and body temperature (Bt), which instantly reflect ANS. trong>Methods: trong>Fourteen healthy male volunteers participated in this study and we investigated four kinds of breathing with them: Natural breathing (Control), Abdominal Pressure breathing (Pressure), Abdominal breathing (Abdominal) and Reverse Abdominal breathing (Reverse).trong> Results:trong> Only Control group showed increase of HF/decrease of HR, showing increase parasympathetic nervous system, which indicates the condition of relax. On the hand, all experimental groups of unique breathings showed increase of sympathetic nervous system, meaning stress. trong>Discussion:trong> Our hypothesis was that traditional breathing methods would decrease stress, however, our results were the total opposite to it. this reason might be found: 1) our subjects might be too young and naive, 2) they did not have enough training or experience, and 3) enough and appropriate duration of breathings was needed. therefore, we consider that appropriate program and enough training time to use to specific breathing are needed to obtain the effects of traditional breathing.

Long Acting Injectable (LAI) medications for patients with schizophrenia is commonly administered to relieve their symptoms. through shared decision-making and clinical evidence-based, psychiatrists should systematically offer LAIs to all patients requiring long-term antipsychotic treatment as a first-line treatment. Gluteal intramuscular (IM) injection requires accurate insertion of needles into the specific muscle area, often the outer upper quadrant of the buttocks, in order to achieve the required blood concentration. the purposes of this study were to compare the “Distance from the Epidermis to the Under-Fascia (DEUF)” and “Distance from the Epidermis to the Iliac Bone (DEB)” of the buttocks IM injection sites at the dorsogluteal and ventrogluteal sites among healthy Japanese volunteer subjects, and to identify the optimal insertion injection needle length. the DEUF and DEB at the gluteal regions were measured by ultrasonography. Welch’s one-way analysis of variance was used to compare the DEUF and the DEB at the gluteal IM injection regions. there was no statistically significant difference observed between the right and left mean values of DEUF for Hochstetter and Clark’s point at the ventrogluteal sites, and the Four and three-way split or Double Cross point at the dorsogluteal sites. However in the DEB, the Hochstetter’s point (P t ventrogluteal site on the right side, and Clark’s point (P tly shorter than the Double Cross point at dorsogluteal sites (F = 4.38). the left buttocks Hochstetter’s point was significantly shorter than the Double Cross point (F = 4.38, P these results, however, did not establish a statistically significant difference in the DEUF among injection sites. It was considered that the difference in the DEB depended on muscle volume and thickness in the gluteal injection sites.

Primary biliary cirrhosis (PBC) is an autoimmune disease characterized by anti-mitochondrial antibodies and destruction of intra-hepatic bile ducts. though little is known about the etiology of PBC, some reports suggest that xenobiotics and viral infections may induce PBC. We describe a case of PBC after the aortoiliac reconstruction surgery using a Y-graft.

Efficacy of Intravenous Immunoglobulin in a Case of Emanuel Syndrome

We report a case of a male patient with Emanuel syndrome (ES) with multiple systemic malformations. He had frequent recurrent seizures and infectious diseases after six months. Because the level of immunoglobulin (IgG) was below the normal range for his age, he was administered intravenous immuno-globulin (IVIG) whenever infection occurred. As he grew up, the frequency of infections and seizures were reduced after IVIG therapy. Although the mechanism was not identified, IVIG might be effective for not only recurrent infections, but also for reducing recurrent seizures.

the importance of sleep has been described in proverbs such as “the early bird catches the worm”. However, there are few scientific reports on the effects of early rising. therefore, early risers (Group E) and late risers (Group L) were compared and the number and ratio of leukocytes, body temperature, glucose and its associated hormones were studied. Furthermore, each group was divided into two groups by the duration of sleep and the following four groups were compared: early risers with short sleep (Group E-S);early risers with long sleep (Group E-L);late risers with short sleep (Group L-S);and late risers with long sleep (Group L-L). then, compared with Group L, Group E showed lower numbers and ratio of granulocytes and a higher ratio of lymphocytes showing parasympathetic nerve dominance. Group E showed higher levels of glucose and its related hormones than Group L, indicating sympathetic nerve dominance. Compared with Groups E-S and L-S, Groups E-L and L-L showed lower glucose and cortisol levels, respectively. these results indicated that early rising might affect leukocyte subsets, and adequate duration of sleep could decrease levels of glucose via modification of the autonomic nervous system.

Recycled PEt (R-PEt) is well known to exhibit brittle behavior in the presence of notches and indicated the low heat distortion temperature. In addition, it is hard to prevent some impurities such as cap or label of the bottle that mixed into R-PEt during the recycling process. In this paper, the effect of the amounts and kinds of compatibilizers on the morphological characteristics and mechanical performance of recycled poly(ethylene terephthalate) (R-PEt) compounded with polyethylene (PE), polypropylene (PP), and polystyrene (PS) was investigated. From the results, with an increase in the glycidyl methacrylate modified PE (EGMA) additive contents, in the increment of the Izod impact strength of Composite-G was obtained. In addition, it was found that the miscibility of Composite-G was improved with increasing the amount of EGMA, which indicated from the result of SEM images.

Intramuscular Diffusion Status of Risperidone and Aripiprazole Long Acting Injectable (LAI) by Ultrasonography

the aim of this study was to consider the characteristics of intramuscular diffusion status of risperidone and aripiprazole long acting injectable (LAI) by ultrasonography. Subjects were 40 adult subjects diagnosed with schizophrenia and treated with LAI [32 patients were risperidone LAI (RLAI) and 8 patients were aripiprazole LAI (ALAI)]. However, in this paper, only three cases (one RLAI case and 2 ALAI cases) were selected to illustrate the diffusion effects of both LAI. Dorsogluteal intramuscular (IM) injection sites were measured at prone position using the “double cross” method. Before LAI injection, the distance from the epidermis to the under-fascia (DEUF), and distance from the epidermis to the iliac bone (DEI) at the IM injection site were assessed by using ultrasonography: 1) the injection needle was inserted to the gluteus medius, and 2) observed the diffusion status within the muscle injected RLAI and ALAI were confirmed using the B-mode ultrasonography. Both RLAI and ALAI were depicted as high echogenicity with acoustic shadowing. It was considered that the diffusion states of LAIs by ultrasonography were important time course evaluations providing objective evidence.

the effects of application of an ancient type of acupuncture needle on increase in urination of hospitalized oldest-old people

the di-zhen (DZ) is an ancient type of acupuncture needle with a history dating back to more than 2000 years. Unlike modern acupuncture needles, the DZ is not inserted subcutaneously and is considered to be safe for common use, even at the bedside of terminally ill patients. In this case study, DZ application was selected and applied 14 times on a 92-year-old patient who had suffered severe oliguria along with administration of diuretics. Urine volume increased 12 out of 14 times following DZ treatment;however, the other two times it was ineffective. to investigate the reason and the differences in effective/ineffective DZ application, two internal conditions (effective vs ineffective) were compared by analyzing daily nursing reports (vital signs such as blood pressure, urine volume and body temperature) and weekly clinical data (including renal, liver and immune function, urea nitrogen, creatinine, ALt, ASt, the ratio and number of leukocytes, erythrocytes). DZ application increased urine volume via a transient modification of the autonomic nervous system when internal homeostasis was maintained. Aging populations, who often have several diseases and medications, are rapidly increasing in developed countries. therefore, the study and application of DZ may contribute to a reduction in medical costs, which have been rapidly and heavily expanding recently.
