BACKGROUND The oral nucleos(t)ide analogue,entecavir(ETV)was demonstrated to reduce the rate of hepatocellular carcinoma(HCC)in patients with hepatitis B virus(HBV)-associated liver ***,the reduction of HCC differs in various regions of the *** To investigate the reduction of HCC development due to ETV therapy by *** We surveyed the differences in HCC development following ETV treatment based on published articles using PubMed(2004-2019).RESULTS The regions with the most marked reduction in HCC development due to ETV therapy were Spain(1.0%/year)and Canada(Southern part,1.3%/year),and the most ineffective areas were South Korea(3.6%-3.8%/year),China(3.3%/year),Taiwan(2.4%-3.1%/year),and Hong Kong(2.8%/year).Following ETV administration,the incidence of HCC in genotype D regions(1.89%±0.28%/year,mean±SE)was significantly lower than that in genotype C regions(2.91%±0.24%/year,P<0.01).With regard to the initial HBV-DNA level,in genotype C patients(average:5.61 Log10IU/mL)this was almost the same as that in genotype D patients(average:5.46 Log10IU/mL).Moreover,there was no association between the prevalence ratio of HBV and the incidence of HCC on ETV *** The effectiveness of ETV in preventing HCC development in HBV-associated liver cirrhosis is genotype-dependent.

A lipoplex （i.e., pDNA#1/lipid complex and transfection reagent for pDNA delivery） containing galactosylceramide （GalCer） and an amidine-bearing lipid （TRX） was examined whether the bound pDNA was specifically ingested by hepatocyte via asialoglycoprotein receptor （ASGPR） and then expressed protein. Gel electrophoresis and small-angle X-ray scattering （SAXS） confirmed that the TRX-GalCer liposome#2 complexed with pDNA and the resultant lipoplex took a hexagonally packed inverted cylinder structure when the GalCer composition was less than 20 wt.% of the total lipid. When the lipoplex carrying pGL3 （luciferase-cording pDNA） was administrated to HepG2, the luciferase activity was increased with increasing the GalCer composition until it reached 3 wt.% and then decreased upon further addition of GalCer. When we added galactose itself as a competitor, the luciferase activity was decreased, while glucose did not show such decrease, suggesting that HepG2 ingested the lipoplex via ASGPR-mediated endocytosis. This paper indicated that the hexagonally packed inverted cylinder structures of lipoplex may not always provide excellent transfection and presented a possibility that the TRX lipoplex#3 can obtain a cellulartargeting ability through the receptors for oligosaccharide.

Genetic mechanisms are implicated as a cause of some male infertility, yet are poorly understood. Meiosis is unique to germ cells and essential for reproduction. The synaptonemal complex is a critical component for chromosome pairing, segregation and recombination. Hormadl is essential for mammalian gametogenesis as knockout male mice are infertile. Hormadl-deficient testes exhibit meiotic arrest in the early pachytene stage and synaptonema! complexes cannot be visualized. To analyze the hypothesis that the human HORMAD1 gene defects are associated with human azoospermia caused by meiotic arrest, mutational analysis was performed in all coding regions by direct sequence analysis of 30 Japanese men diagnosed with azoospermia resulting from meiotic arrest. By the sequence analysis, three polymorphism sites, Single Nucleotide Polymorphism 1 （c. 163A〉G）, SNP2 （c. 501T〉G） and SNP3 （c. 918C〉T）, were found in exons 3, 8 and 10. The 30 patients with azoospermia and 80 normal pregnancy-proven, fertile men were analyzed for HORMAD1 polymorphisms. Both SNP1 and SNP2 were associated with human azoospermia caused by complete early meiotic arrest （P〈0.0S）. We suggest that the HORMAD1 has an essential meiotic function in human spermatogenesis.

The English language has a history of about one thousand and five hundred years since it began to be spoken from around the middle of the fi fth century in England. During that time many internal and external events which are associated with the history, the culture, and society of England infl uenced the language at each stage. These events have brought changes to the system of the language and qualitative improvement directly or indirectly in every stage of the history of the English language. This paper focuses on the period stretching from Old English to Early Modern English, and its purpose is to discuss what sort of events were involved in the forming of Present-day English.

目的初步探讨复发缓解型多发性硬化(RRMS)缓解期肾虚湿热证患者的血清学特征。方法回顾性选取RRMS缓解期患者27例作为RRMS组,以同期招募的26例健康志愿者作为健康组,并将RRMS组患者辨证分为肾虚湿热证和肾虚非湿热证两个亚组。采集所有受试者临床信息,采用扩展残疾量表(EDSS)评估患者神经功能损伤情况,利用单分子阵列分析技术检测受试者血清神经丝轻链蛋白(NfL)、可溶型程序性死亡受体1(sPD-1)、可溶型程序性死亡配体1(sPD-L1)水平,并采用Spearman法对RRMS患者血清NfL、sPD-1、sPD-L1水平之间的相关性及其与病程、发作次数、EDSS评分的相关性进行分析。结果RRMS组患者血清NfL、sPD-1、sPD-L1水平显著高于健康组(P<0.05或P<0.01);肾虚湿热证亚组患者血清NfL、sPD-1、sPD-L1水平均高于肾虚非湿热证亚组患者(P<0.05或P<0.01)。RRMS组患者血清NfL水平与sPD-1、sPD-L1水平均呈正相关(P<0.01),sPD-1水平与sPD-L1水平呈正相关(P<0.001);RRMS组患者血清sPD-1水平与发作次数呈正相关(P=0.030)。结论RRMS缓解期患者血清NfL、sPD-1、sPD-L1显著升高,且肾虚湿热证患者较肾虚非湿热证患者升高程度更为明显,提示RRMS肾虚湿热证患者可能存在更为严重的免疫炎症和神经损伤。

Motor symptoms are cardinal clinical features of Parkinson’s disease (PD). Progress in drug therapy and rehabilitation has been presenting beneficial effect for motor symptoms. However, non-motor symptoms and signs in PD have been accumulated growing attentions and its amelioration may also give beneficial effect for PD patients’ and their care givers’ quality of life. In this mini-review, I overviewed non-motor symptoms and signs in PD.

Aims: To prevent fetal damage by excess oxytocin administration of manually controlled infusion, by automatic and safe increasing infusion setting with monitoring uterine contraction and fetal heart rate. Methods: Starting infusion level was 2 milliU/ml, as oxytocin sensitivity of a pregnant uterus is unknown, to avoid hyper contraction and fetal bradycardia caused by unexpected excess oxytocin sensitivity. The infusion automatically increased with automatic monitoring of uterine contraction curve, then the increasing stopped when contraction reached to normal labor level, where the infusion level maintained, which continued until delivery, if there is no trouble. However, the infusion continued until expiring all fluid in case of insensitive uterus, where the induction was performed in another day. The infusion stopped automatically when contraction was too strong, or fetal heart rate is abnormal. Thus, oxytocin sensitive case is protected from excess contraction and fetal asphyxia. Results: Normal vaginal delivery was achieved in 28/33 cases (85%), which was more than manually controlled infusion. No case was abnormal in successful oxytocin infusion. Conclusion: The automated technique will be applied to oxytocin labor induction.
