Objective： The mucus production is an indicator for the histological grade of mucinous epithelial ovarian cancer （mEOC）. In our previous study, Crk expression was targeted in the human ovarian mucinous adenocarcinoma cell line MCAS through RNA interference, resulting in the establishment of Crk knock down cells. These cells exhibited decreased tumorigenic potential both in vitro and in vivo. The purpose of this study was to investigate if there is any change in the capability of forming mucus in these Crk knock down cells. Methods： Cytoplasmic periodic acid Schiff （PAS） staining and particle excluding assay were conducted to assess the mucus formation within and around cells, respectively. Additionally, the amount of mucus formed in tumor lumps from nude mice model was measured following HE and PAS staining. Results： The increased mucus production in Crk knockdown mEOC cells （MCAS） was manifested by increased number of enlarged cells filled with vacuoles-like mucus observed by phase-contrast microscope and cytoplasmic PAS staining; and enhanced mucus secretion was represented by the assembly of pericellular matrix in particle excluding assay and increased mucus area in tumor lumps from nude mice models. Conclusion： The course of carcinogenesis in mEOC is associated with the altered pattern of mucus production and secretion. The adaptor protein Crk is implicated in both pathways.

Pancreatic Carcinoma with Prominent Mucin Production: A Clinicopathologic and Immunohistochemical Study of 9 Cases

Pancreatic carcinoma occasionally associated with prominent mucin production and this type of tumor designated as PCM (pancreatic carcinoma with prominent mucin production) was diagnosed depends on subjective estimation of the amount of mucous area, and there has been no report on a quantitative evaluation of the amount of mucinous area in the tumor. To examine the feature of PCM, we analyzed 9 cases of PCM among 243 cases of pancreas carcinoma and evaluated the amount of mucin by imaging analysis. Morphologically, 5 cases were classified as intradactal papillary mucinous neoplasms (IPMN)-derived PCM and 4 cases were as ductal adenocarcinoma (DA)-derived PCM. Mucous composition was found to be more than 50% in all IPMN-derived PCM cases, and that was 40% - 50% in DA-derived PCM cases with one exception. IPMN-derived PCM cases showed expansive growth with pancreatic duct dilatation filled with mucin, while DA-derived PCM cases possessed mucin infiltration into interstitial tissue. Immunohisto-chemically, three of 4 DA-derived PCM cases were MUC1(–)/MUC2(+), and the results of expressions for p16 and Dpc4 suggesting that DA-derived PCM was similar to IPMN-derived PCM rather than ordinary DA. Survival rate of DA-derived PCM cases was lower than that of IPMN-derived PCM cases. We advocate that DA-derived PCM may constitute a borderline group between IPMN and ordinary DA.

目的　探讨JC病毒 (JCV)病毒样颗粒 (VLP)是否可以直接转运进入细胞核。方法　应用JCV主要外壳蛋白VP1体外表达、重组VLP ,在其表面标记异硫氰基荧光素 (FITC) ,同时在其内部包裹荧光染料Cy3,感染培养的HeLa细胞和SVG细胞 ,荧光显微镜观察VLP入核转运。结果　HeLa和SVG细胞感染包裹Cy3的FITC VLP时 ,FITC与Cy3同时出现于细胞核内相同部位 ;而感染FITC VP1与Cy3混合物时 ,FITC虽可在细胞核内检测到 ,但Cy3信号几乎消失。包裹Cy3的VLP用SDS PAGE展开 ,荧光显像后行考马斯亮蓝 (CBB)染色 ,发现Cy3和VLP移行至不同部位 ,证明Cy3不能与VP1结合 ,提示VLP以完整的颗粒形式转运进入细胞核。应用包裹外源性DNA的VLP感染培养的HeLa和SVG细胞 ,发现包裹的DNA在细胞浆和细胞核内均可检测到 ,提示JCV入核过程与VLP相同。结论　VLP可以不经裂解直接转运进入细胞核 ,JCV入核转运可能与VLP相同。

目的:探讨氯化甲基汞(MMC)中毒大鼠小脑变性的病理改变及机制。方法:本研究在大鼠服用MMC4mg/(kg·d)所致的亚急性汞中毒模型上,分别在服用MMC后第11、15、18和21天断头,采用组织和免疫病理技术动态观察小脑的病理演变,并采用TUNEL染色观察细胞凋亡改变,同时在电镜下观察了超微结构改变。结果:在服用MMC第18天,TUNEL染色显示小脑颗粒细胞有散在的凋亡细胞,主要位于脑回深部近白质区的颗粒层。第21天,凋亡细胞明显增加,颗粒细胞减少。MRF-1染色可见大量的小胶质细胞反应,GFAP示胶质细胞增生明显,而Purkinje细胞则保留完整。服用MMC第18天的超微结构观察表明,细胞核皱缩,体积缩小,形态不规则,电子密度增高,可见密集深染的染色质,部分细胞核破碎,呈现为均一深染的泪珠状染色质,符合凋亡改变。结论:本研究结果表明汞中毒的小脑变性的病理机制为细胞凋亡,其病理变化符合人类汞中毒表现。

Rhinoplasty for Saddle Nose with Sarcoidosis

Background: Cases of acquired saddle nose have been reported;however, reports of sarcoidosis with acquired saddle nose are very rare. The main cause of sarcoidosis is estimated to be an abnormal immune reaction rather than infection. Aim: Rhinoplasty using an autologous rib was planned and performed without plate fixation. Case Presentation: Our patient was diagnosed with sarcoidosis at the age of 25 years. She developed nasal congestion and rhinitis at the age of 42 years. Her nose began to show the saddle nose deformity at the age of 43 years. A rhinoplasty using an autologous rib was performed without plate fixation. Conclusion: After the operation, her nose maintained a favorable shape without sarcoidosis worsening. This use of rhinoplasty for treating saddle nose possibly prevents the worsening of sarcoidosis.

A Case Report: Puerperal Ovarian Cancer in a Patient with a Prior History of Fertility Treatment and Ovarian Endometrial Cyst

Introduction: It has been suggested the relationship between infertility treatment and ovarian cancer. Furthermore, endometriosis that is one of the causes of the infertility tends to develop into endometrioid adenocarcinoma and clear cell carcinoma in many studies. Case: A 38-year-old woman underwent laparoscopic enucleation of bilateral ovarian endometrial cysts at 29 and 33 years old. She then received 11 sessions of artificial insemination and 7 cycles of ovarian stimulation, egg collection,?in vitro?fertilization, and embryo transfer at multiple medical institutions before conceiving and giving birth to her first child. After naturally conceiving her second child, she presented at our hospital for pregnancy/delivery management with a right ovarian tumor. She underwent abdominal right adnexectomy for a mucinous cystic tumor of borderline malignancy at 36 years old (17 weeks of pregnancy with the second child). After vaginal delivery at 38 weeks, a 1-month postpartum checkup revealed a left adnexal mass and ascites. Further workup led to a diagnosis of left ovarian cancer. The cancer was diagnosed by ovarian biopsy as unresectable stage-IIIc endometrioid carcinoma. Despite chemotherapy, the patient developed liver metastasis during treatment and died due to worsened general condition 9 months after childbirth. In addition to ovarian endometrial cyst, repeated cycles of injury and repair of the ovarian surface epithelium are also suggested to mediate the development of epithelial ovarian cancer. Conclusion: Evaluation of long-term outcome and follow-up of patients who have undergone fertility treatment should be addressed.
