Usefulness of bispectral monitoring of conscious sedation during endoscopic mucosal dissection

AIM: To assess the usefulness of bispectral index (BIS) monitoring in order to carry out endoscopic submucosal dissection (ESD) safely and with patients' satisfaction. METHODS: Three hundred sixty-six patients with an early-stage neoplasm of the digestive tract were enrolled. The BIS monitor (A-1050: Aspect medical systems/NIHON KOHDEN, Tokyo, Japan) was used. The appropriate sedative condition was set at 55 to 75 BIS levels (BIS value) during the endoscopic procedures. RESULTS: Among 366 cases, 13 were accompanied by adverse events during and/or after ESD. All episodes occurred in cases with BIS value between 56 and 65. Hypotension was observed in four cases, and bradycardia in six. Respiratory distress was observed in two cases with chronic pulmonary obstructive disease. All patients with adverse events were able to leave the hospital without extension of the hospitalization. CONCLUSION: BIS monitoring is useful to safely perform ESD. A BIS value of 70 to 75 is suitable for ESD.

The relationship between the wear behaviour of an ultra-high-molecular-weight polyethylene(GUR1050)pin and surface profile of a Co-28Cr-6Mo alloy(ASTM F-75)disc was investigated.Tribological tests have been performed by pin-on-disc wear testing machine with multi-directional pathways,obtaining the influence of micro slurry-jet erosion(MSE)processed Co–Cr–Mo alloy.The specific wear and the wear particles have been investigated.It was elucidated that the Co–Cr–Mo alloy surface processed by MSE influenced the specific wear rate of polyethylene;however,the morphological aspect of polyethylene wear particles was not drastically changed.The wear particles isolated from the lubricating liquid were added to a culture medium,and human monocyte-derived macrophages were incubated by using an upright/inverted method.The production of TNF-α and IL-6 seemed to have a correlation with the amount of wear particles added,however,the influence of particle size on the production of TNF-α and IL-6 was not obvious.This means that the improvements for the incubation method,i.***.upright/inverted method,need further investigation for the accurate analysis.

Usefulness of biodegradable stents constructed of poly-l-lactic acid monofilaments in patients with benign esophageal stenosis

AIM: To report 13 patients with benign esophageal stenosis treated with the biodegradable stent. METHODS: We developed a Ultraflex-type stent by knitting poly-l-lactic acid monofilaments. RESULTS: Two cases were esophageal stenosis caused by drinking of caustic liquid, 4 cases were due to surgical resection of esophageal cancers, and 7 cases were patients with esophageal cancer who received the preventive placement of biodegradable stents for post- endoscopic mucosal dissection (ESD) stenosis. The preventive placement was performed within 2 to 3 d after ESD. In 10 of the 13 cases, spontaneous migration of the stents occurred between 10 to 21 d after placement. In these cases, the migrated stents were excreted with the feces, and no obstructive complications were experienced. In 3 cases, the stents remained at the proper location on d 21 after placement. No symptoms of re-stenosis were observed within the follow-up period of 7 mo to 2 years. Further treatment with balloon dilatation or replacement of the biodegradable stent was not required. CONCLUSION: Biodegradable stents were useful for the treatment of benign esophageal stenosis, particularly for the prevention of post-ESD stenosis.

Comparison of High and Medium Iodine Concentrations of Contrast Materials for Preoperative CT Studies in Breast Cancer Patients

We compared the contrast effect of high and medium iodine-concentration contrast materials for preoperative CT eva- luation of breast cancer patients. Female breast cancer patients who underwent enhanced CT of the chest and upper abdomen before surgery were analyzed retrospectively. High (370 mg I/mL, 100 mL) or medium (300 mg I/mL, 125 mL) concentrations of contrast material were injected for 60 sec, followed by saline flush, and postcontrast CT images were obtained 90 sec after contrast injection. CT values were assessed for the breast tumor, normal breast parenchyma, liver, aorta, and muscle. For the high and medium concentration agents, 45 and 49 patients were analyzed, respectively. No significant differences between the two contrast agents were found in terms of CT values of the breast tumor, normal breast parenchyma, liver, aorta, or muscle. The tumor-background differences in CT values did not differ significantly between the two agents. A comparable contrast effect was indicated in single-phase CT of breast cancer patients between high and medium iodine-concentration contrast material when the total iodine dose and injection duration were identical.
