Phase Ⅱ study of protracted irinotecan infusion and a low-dose cisplatin for metastatic gastric cancer

AIM: To test protracted irinotecan infusion plus a low-dose cisplatin in this Phase Ⅱ trial to decrease its toxic-ity. METHODS: The eligibility criteria were: (1) histologi-cally proven measurable gastric cancer; (2) performance status of 0 or 1; (3) no prior chemotherapy or comple-tion of prior therapy at least 4 wk before enrollment; (4) adequate function of major organs; (5) no other active malignancy; and (6) written informed consent. The regi-men consisted of irinotecan (60 mg/m2) on d 1 and 15 by 24-h infusion and cisplatin (10 mg/m2) on d 1, 2, 3, 15, 16, and 17. Treatment was repeated every 4 wk. RESULTS: Thirty-one patients were registered between April 2000 and January 2001. The response rate for all 31 patients, 20 patients without prior chemotherapy, and 11 patients with prior chemotherapy was 52% (16/31), 60% (12/20), and 36% (4/11), respectively. The median survival time was 378 d. The median number of courses given to all patients was 2. Grade 4 neutropenia oc-curred in 11 (35%) patients, while grade 3 to 4 diarrhea or nausea occurred in 1 (3%) and 3 (10%) patients, respectively. Fatigue was minimal as grade 1 fatigue was found only in 3 (10%) patients. Other adverse events were mild and no treatment-related deaths ***: This regimen showed a high level of ac-tivity and acceptable toxicity in patients with metastatic gastric cancer.

Risk and Protective Factors Associated with Depression and Anxiety among Pregnant Women during the COVID-19 Pandemic

Background: The coronavirus disease (COVID-19) pandemic has impacted perinatal women’s mental health. However, protective factors associated with depression among pregnant Japanese women during the pandemic have not been reported. Purpose: The present study investigated the risk and protective factors associated with depression and anxiety among pregnant women during the COVID-19 pandemic. Methods: An online questionnaire was administered to 157 pregnant women between October 2022 and May 2023 at two general hospitals in Japan. The Japanese versions of the Patient Health Questionnaire-9 (PHQ-9) and General Anxiety Disorder-7 (GAD-7) were used to assess symptoms of depression and anxiety. The χ2 test or Fisher’s exact test and multivariate logistic regression model were used to examine factors associated with depression and anxiety among pregnant women. Results: Overall, 47.1% and 35.7% of the pregnant women reported depressive and anxiety symptoms, respectively. A “history of mental illness” was a risk factor for depression and anxiety among pregnant women during the COVID-19 pandemic. Additionally, unmarried status was a risk factor for anxiety among pregnant women, whereas outdoor and indoor exercises were protective factors against depression and anxiety, respectively. Conclusions: Exercise may have protected pregnant women from depression and anxiety during the COVID-19 pandemic. Encouraging exercise may help maintain the mental health of pregnant women who do not have exercise restrictions.

目的评价naldemedine单、多次给药在中国健康成人受试者中的安全性、耐受性和药代动力学特征。方法10例中国健康受试者单、多次口服naldemedine后,用液相色谱质谱联用仪检测血浆中naldemedine以及代谢物(nor-naldemedine)的浓度,经Phoenix WinNonlin 8.3.1软件计算药代动力学参数。结果相比于单次给药,多次给药后naldemedine的tmax(单次为1.06 h,多次为1.08 h)、C_(max)(单次为4.16 ng·mL^(-1),多次为4.11 ng·mL^(-1))均无明显变化,AUC0-τ高于单次给药(单次为23.88 ng·h·mL^(-1),多次为28.93 ng·h·mL^(-1)),t_(1/2)稍有延长(单次为9.26 h,多次为12.50 h),多次给药后有轻微蓄积(C_(max)的蓄积比为0.99,AUC的蓄积比为1.22);nor-naldemedine的t_(1/2)(单次为18.10 h,多次为29.30 h)、C_(max)(单次为0.19 ng·mL^(-1),多次为0.38 ng·mL^(-1))和AUC0-τ(单次为3.13 ng·h·mL^(-1),多次为6.02 ng·h·mL^(-1))均明显升高,tmax(单次为3.49 h,多次为3.56 h)无明显变化。代谢物与naldemedine的C_(max)比值和AUC比值明显升高,多次给药后存在蓄积(C_(max)的蓄积比为2.01,AUC的蓄积比为1.95)。所有治疗后出现的不良事件均为轻度。结论中国健康受试者连续多次(第1天单次、第4~13天每天1次)口服naldemedine片0.2 mg后,在体内消除较快,有轻度蓄积,在多次给药阶段第2次给药前达到稳态,且有良好的安全性和耐受性。

[Purpose] Capacitive and resistive electric transfer (CRET) is becoming prevalent in sports settings. CRET is effective for improving pain and healing injured tissue;however, its influence on muscle function and morphology is still unclear. This study confirmed the immediate effects of CRET on the duration of muscular stiffness and range of motion (ROM). [Method] This study describes the protocol for a single-arm trial with the non-blinding of participants and researchers. Twenty-four healthy men participated in the study. They received CRET therapy for their low back areas. The muscular stiffness of the multifidus muscle (superficial and deep) and the ROM using the active straight leg raise (ASLR) test were measured pre-intervention, post-intervention (immediately), and 15 and 30 min post-intervention. We compared these parameters using a one-way analysis of variance and Dunnett’s test (multiple comparison tests for subtests). [Results] The muscular stiffness of the superficial and deep multifidus muscles was significantly decreased, and the ASLR test showed a significant increase compared with the test performed pre-intervention. In addition, these effects persisted for 30 min. [Conclusion] Warm-up is vital for improving muscular stiffness and increasing the ROM. CRET is a useful device for achieving these aims, particularly as a passive warm-up method in sports settings.

Exploring Passive Exoskeleton-Induced Changes in Lumbar Muscle Activity

Purpose: The purpose of this study was to evaluate the effect of using a passive exoskeleton on lumbar muscle activity during lifting movements, and to determine whether muscle activity remains altered after exoskeleton removal. This study sought to identify the potential risks and benefits associated with the use of passive exoskeletons for the prevention and treatment of low back pain. Methods: Eighteen healthy adult participants lifted a 10 kg suitcase while wearing a passive exoskeleton. Muscle activity and postures were measured during lifting and before, during, and after exoskeleton use. This study examined whether the reduced muscle activity observed during exoskeleton use persisted after exoskeleton removal. Muscle activity was assessed using electromyography and postures were recorded using reflective markers and a camera. Results: The study found that Lumbar muscle activity decreased significantly (approximately 40%) during exoskeleton use compared to that without exoskeleton use. Importantly, lumbar muscle activity remained low after exoskeleton removal, at levels similar to those observed during exoskeleton use. This suggests that individuals adapted to the exoskeleton support and maintained altered muscle control, even without the exoskeleton. Conclusion: This study demonstrates that passive exoskeletons significantly reduce lumbar muscle activity during lifting tasks, and that this altered muscle control persists after exoskeleton removal. These findings contribute to the understanding of the risks and benefits of passive exoskeletons, potentially aiding their development and informing their use in the prevention and treatment of low back pain.

Relationship among Eating Behavior, Effortful Control, and Working Memory in Female Young Adults

The management of eating behavior plays an important role in health maintenance. In this study, we investigated the relationship between eating behavior and effortful control in female young adults. Participants completed the questionnaire measures of effortful control and eating behaviors and Stroop cognitive interference task. The results showed that restrained eating was positively correlated with activation control;emotional eating was negatively correlated with inhibitory control and attentional control;external eating was negatively correlated with inhibitory, activation, and attentional control. The scores for activation control and restrained eating were higher for participants with a low Stroop error rate than for those with a high Stroop error rate. These results indicate that restrained eating has a different association with effortful control than doing emotional and external eating.
