Can Traditional Breathing Methods Reduce Stress?

Purpose: Traditional medicines have unique breathing methods and they are widely believed that they are good for health promotion and good for relax. However, there is little evidence. The purpose of this study is to discuss if such specific breathings could effectively reduce stress and be able to help to reach a relaxed state in the body as well as in the mind. To objectively understand the state of stress/relax, we assessed the condition of the autonomic nervous system (ANS) as well as heart rate (HR) and body temperature (BT), which instantly reflect ANS. Methods: Fourteen healthy male volunteers participated in this study and we investigated four kinds of breathing with them: Natural breathing (Control), Abdominal Pressure breathing (Pressure), Abdominal breathing (Abdominal) and Reverse Abdominal breathing (Reverse). Results: Only Control group showed increase of HF/decrease of HR, showing increase parasympathetic nervous system, which indicates the condition of relax. On the hand, all experimental groups of unique breathings showed increase of sympathetic nervous system, meaning stress. Discussion: Our hypothesis was that traditional breathing methods would decrease stress, however, our results were the total opposite to it. This reason might be found: 1) our subjects might be too young and naive, 2) they did not have enough training or experience, and 3) enough and appropriate duration of breathings was needed. Therefore, we consider that appropriate program and enough training time to use to specific breathing are needed to obtain the effects of traditional breathing.

Let C be a nonempty closed convex subset of a 2-uniformly convex and uniformly smooth Banach space E and {An}n∈N be a family of monotone and Lipschitz continuos mappings of C into E＊. In this article, we consider the improved gradient method by the hybrid method in mathematical programming [i0] for solving the variational inequality problem for {AN} and prove strong convergence theorems. And we get several results which improve the well-known results in a real 2-uniformly convex and uniformly smooth Banach space and a real Hilbert space.

Impact of indoor ventilation efficiency on acetone inhalation exposure concentration and tissue dose in respiratory tract

A significant feature of the indoor environment is the heterogeneity of airflow and pollutant distributions,which are primarily dependent on ventilation *** the case of short-and high-concentration exposures to hazardous chemical pollutants,it may be necessary to precisely determine the concentration in the breathing zone or,more directly,the inhalation exposure concentration in the respiratory tract,rather than the representative room average concentration in an indoor environment,because of the non-uniformity of pollutant concentration *** this study,we developed a computer-simulated person with a detailed respiratory system to predict inhalation exposure concentration and inhalation dose via transient breathing and reported a demonstrative numerical simulation for analyzing acetone concentration distributions in a simplified model *** numerical analysis revealed that the ventilation efficiency distribution in a room could change significantly by changing the design of the ventilation system,and that the inhalation exposure concentration estimated by a computer-simulated person could differ from the representative concentration,such as perfect-mixing or volume-averaged acetone concentration,by a factor of two or more.

Technical tips of endoscopic ultrasound-guided choledochoduodenostomy

Endoscopic ultrasound(EUS) is clinically useful not only as a diagnostic tool during EUS-guided fine needle aspiration,but also during interventional ***-guided biliary drainage has been developed and performed by experienced ***-guided choledocoduodenostomy(EUS-CDS) is relatively well established as an alternative biliary drainage method for biliary decompression in patients with biliary *** reported technical success rate of EUS-CDS ranges from 50% to 100%,and the clinical success rate ranges from 92% to 100%.Further,the over-all technicalsuccess rate was 93%,and clinical success rate was 98%.Based on the currently available literature,the overall adverse event rate for EUS-CDS is 16%.The data on the cumulative technical and clinical success rate for EUS-CDS is ***,EUS-CDS can still lead to several problems,so techniques or devices that are more feasible and safe need to be ***-CDS has the potential to become a first-line biliary drainage procedure,although standardizing the technique in multicenter clinical trials and comparisons with endoscopic biliary drainage by randomized clinical trials are still needed.

Technical tips for endoscopic ultrasound-guided hepaticogastrostomy

Interventional procedures using endoscopic ultrasound(EUS) have recently been developed. For biliary drainage, EUS-guided trans-luminal drainage has been reported. In this procedure, the transduodenal approach for extrahepatic bile ducts is called EUSguided choledochoduodenostomy, and the transgastric approach for intrahepatic bile ducts is called EUSguided hepaticogastrostomy(EUS-HGS). These procedures have several effects, such as internal drainage and avoiding post-endoscopic retrograde cholangiopancreatography(ERCP) pancreatitis, and they are indicated for an inaccessible ampulla of Vater due to duodenal obstruction or surgical anatomy. EUS-HGS has particularly wide indications and clinical impact as an alternative biliary drainage method. In this procedure, it is necessary to dilate the fistula, and several devices and approaches have been reported. Stent selection is also important. In previous reports, the overall technical success rate was 82%(221/270), the clinical success rate was 97%(218/225), and the overall adverse event rate for EUS-HGS was 23%(62/270). Adverse events of EUS-biliary drainage are still high compared with ERCP or PTCD. EUSHGS should continue to be performed by experienced endoscopists who can use various strategies when adverse events occur.

Retinoic acid-inducible gene I(RIG-l)-like receptors(RLRs)are RNA sensor molecules that play essential roles in innate antiviral *** the three RLRs encoded by the human genome,RIG-1 and melanoma differentiation-associated gene 5,which contain N-terminal caspase recruitment domains,are activated upon the detection of viral RNAs in the cytoplasm of virus-infected *** RLRs induce downstream signaling via their interactions with mitochondrial antiviral signaling proteins and activate the production of type Ⅰ and Ⅲ interferons and inflammatory *** studies have shown that RLR-mediated signaling is regulated by interactions with endogenous RNAs and host proteins,such as those involved in stress responses and posttranslational *** RLR-mediated cytokine production is also involved in the regulation of acquired immunity,the deregulation of RLR-mediated signaling is associated with autoimmune and autoinflammatory ***,RLRmediated signaling might be involved in the aberrant cytokine production observed in coronavirus disease *** the discovery of RLRs in 2004,significant progress has been made in understanding the mechanisms underlying the activation and regulation of RLR-mediated signaling ***,we review the recent advances in the understanding of regulated RNA recognition and signal activation by RLRs,focusing on the interactions between various host and viral factors.

Does endoscopic ultrasound-guided biliary drainage really have clinical impact?

The well established, gold standard method for treatment of obstructive jaundice involves biliary drainage under endoscopic retrograde cholangiopancreatography(ERCP) performed by pancreatobiliary endoscopists. Recently, interventions using endoscopic ultrasound(EUS) have been developed not only for obtaining cytological and histological diagnosis, but also for biliary drainage as alternative method. EUS-guided biliary drainage(EUSBD) was first reported by Giovannini et al. EUS-BD broadly includes EUS-guided rendezvous technique, EUS-guided choledochoduodenostomy, and EUS-guided hepaticogastrostomy. More recently, EUS-guided antegrade stenting and EUS-guided gallbladder drainage have also been reported. many case reports, series, and retrospective studies on EUS-BD have been reported. However, because prospective studies and comparisons between the different biliary drainage methods have not been reported, the technical success, functional success, adverse events, and stent patency with long-term follow up of EUS-BD are still unclear. Therefore, prospective, randomized controlled studies addressing these issues are needed. Despite this, EUSBD undoubtedly is clinically useful as an alternative biliary drainage method. EUS-BD has the potential to be a first-line biliary drainage method instead of ERCP if results of clinical trials are favorable and the technique is simplified.

The kinase inhibitor sorafenib is the only systemic therapy proven to have a positive effect on survival of patients with advanced hepatocellular carcinoma(HCC).After development of sorafenib and its introduction as a therapeutic agent used in the clinic,several critical questions have been *** parameters and biomarkers predicting sorafenib efficacy are the most important issues that need to be *** it is difficult to know the responders in advance using conventional characteristics of patients,there are specific serum cytokines and/or gene amplification in tumor tissues that have been reported to predict efficacy of *** and benefits of continuation of sorafenib beyond radiological progression is another issue to consider because no other standard therapy for advanced HCC as yet *** addition,effectiveness of the expanded application of sorafenib is still controversial,although a few studies have shed some light on combinational treatment with sorafenib for intermediate-stage ***,over 50 relevant drugs have been developed and are currently under *** efficacy of some of these drugs has been extensively examined,but none have demonstrated any superiority over sorafenib,so ***,there are several drugs that have shown efficacy for treatment after sorafenib failure,and these are proceeding to further *** address these issues and questions,we have done extensive literature review and summarize the most current status of therapeutic application of sorafenib.

Lumen-apposing metal stent deployment for walled-off necrosis using semi-free hand technique from the intestine

Lumen-apposing metal stent (LAMS) deployment under en- doscopic ultrasonography (EUS) guidance has been used for walled-off necrosis (WON) [1] . Recently, LAMS with electrocautery enhanced delivery system has been developed. One of the advan- tages of this stent is that the stent can be delivered without any dilation devices. However, to prevent stent migration into the in- testinal lumen, it is important that stent delivery system should be inserted within the WON. When the diameter of WON is not large enough to insert stent delivery system, contralateral wall of WON may be injured. To prevent this adverse event, guidewire insertion may be useful after puncturing. In addition, according to the re- cent reports [2–4] , EUS-guided LAMS placement for WON drainage has been performed. We herein described a case of LAMS deploy- ment from the second part of the duodenum using semi-free hand technique.
