Efficacy of premedication with activated Dimethicone or N-acetylcysteine in improving visibility during upper endoscopy

AIM:To assess the efficacy of N-acetylcysteine(NAC) and activated Dimethicone in improving endoscopic mucosal ***:A total of 148 patients were randomly allocated into four groups to receive one of the following premedications:group A:100 mL water alone;group B:activated Dimethicone plus water(up to 100 mL);group C:NAC plus water(up to 100 mL);and group D:activated Dimethicone and NAC plus water(up to 100 mL).A single endoscopist blinded to the patients group assessed the gastric mucosal visibility scores(range 1-4) at four *** sum of the scores from the four sites was considered as the total mucosal visibility score(TMVS).RESULTS:The patients in group B showed a significantly lower TMVS than those of groups A and C(P 0.05).The difference between TMVS of groups C and A was not significant(P > 0.05).CONCLUSION:Premedication with activated Dimethicone 20 min prior to the upper endoscopy leads to the best *** does not improve visualization by itself.

BACKGROUND：Rheum ribes *** a plant native to China,Iran,Turkey,India,and a few other *** activity is considered to be one of its important properties according to various systems of traditional *** increasing rate of bacterial resistance to antibiotics has led to treatment failure in some cases of shigellosis in children,and underlines a need for safe,efficient and valid ***：The purpose of this study is to evaluate the efficacy of *** syrup as a complementary medicine for treatment of shigellosis in ***,SETTING,PARTICIPANTS AND INTERVENTIONS：This randomized,doubleblind,placebocontrolled trial started with a group of 150 children aged between 12–72 months with suspected Shigella *** syrup or placebo syrup was administered to the intervention and control groups,respectively for 5 *** addition,the standard antibiotic treatment（ceftriaxone for the first 3 days and cefixime syrup for 2 further days）was administered to both *** OUTCOME MEASURES：Body temperature,abdominal pain,need for antipyretics,defecation frequency,stool volume and consistency and microscopic stool examination were recorded as outcome *** observed adverse effects were also ***：Mean duration of fever and diarrhea in the *** group was significantly lower than that in the placebo group（P=0.016 and 0.001,respectively）.In addition,patients in the *** group showed shorter duration of need for antipyretics and shorter duration of abdominal pain（P=0.012 and 0.001,respectively）.However,there were no significant differences between the two groups regarding the microscopic stool ***,no adverse effect was ***：*** syrup can be recommended as a complementary treatment for children with Shigella dysentery.

Some of the chemical and physical water qualities of the Graff River in the city of Kut were studied, and for two sites of the river, One was at the Crimea site, the other in the Jihad district, and for the period from October/2018 to March/2019, Seven variables of river water have been analyzed: temperature PH, electrical conductivity, TDS, turbidity, alkaline, and chloride. The results showed that most of the chemical and physical water properties of the river were in normal proportions and did not rise, except for the turbidity, which was at a very high level, and that the pH values were close to the basal side. The results of the statistical analysis revealed positive significant relationships between the pH and (chloride and TDS). On the other hand, between electrical conductivity and both previous variables as well. And a negative significant connection between temperature and alkaline.

Nowadays, there is a growing emphasis on Inter-basin water transfer projects as costly activities with ambiguous effects on environment, society and economy. Since the concept of climate change was in its embryonic phase before 1990’s, the majority of these projects planned before that period have not considered the effect of long term variation of water resources. In all of these numerous operational and under-construction projects, an intelligent selection of the best water transmission protocol, can help the governments to optimize their expenditures on these projects ,and also can help water resources managers to face climate change effects wisely. In this paper as a case study, Dez to Qomrood inter-basin water transfer project is considered to evaluate the efficiency of three different protocols in long term. The effect of climate change has been forecasted via a wide range of GCMs (Global Circulation Model) in order to calculate the change of flow in the basin's area with different climate scenarios. After these calculation, a water allocation model has been used to evaluate which of these three water transmission protocols (Proportional Allocation (PA), Fix Upstream allocation (FU), and Fix Downstream allocation (FD)) is the most efficient logic switch economically in a framework including both upstream and downstream stakeholders. As the final result, it can be inferred that Fix Downstream allocation (FD) protocol can supply more population especially with urban water for a fix expense and also is the most adapted protocol with future global change, at least in the first round of sustainability assessment.

Liquid sloshing is a common phenomenon in the transportation of liquid-cargo *** waves lead to fluctuating forces on the tank *** these fluctuations are not predicted or controlled,for example,by using baffles,they can lead to large forces and *** volume of fluid(VOF)two-phase numerical model in Open FOAM open-source software has been widely used to model the liquid ***,a big challenge for modeling the sloshing phenomenon is selecting a suitable turbulence ***,in the present study,different turbulence models were studied to determine their sloshing phenomenon prediction *** predictions of these models were validated using experimental *** turbulence models were ranked by their mean error in predicting the free surface *** renormalization group(RNG)k-ε and the standard k–ω models were found to be the best and worst turbulence models for modeling the sloshing phenomena,respectively;moreover,the SST k-ω model and v2-f k-ε results were very close to the RNG k-εmodel result.

In this paper a bi-directional system based on linear channel estimation and data detection using turbo detection algorithm is proposed. By combining channel estimation with an iterative chip-by-chip detection process (inner loop) in an iterative way (outer loop), communications performance can be further increased. We present results on blind reception in case of Interleave Division Multiple Access (IDMA) system when the channel coefficients are unknown. We develop a low-complexity iterative joint channel/code estimation method. The philosophy of the turbo processing is the iterative exchange of related information which yields a substantial improvement of the overall system performance. We analyze the achievable performance of the iterative system proposed. Simulation results demonstrate the efficiency of the proposed algorithms.

近年来,人工智能的方法广泛地应用在包括医学应用的不同领域。在医学领域,已经存在有许多专家系统（ESs）。诊断呼吸系统疾病非常重要,需要得到重视,与呼吸系统相关的人体所有疾病的新技术需要得到发展。首先我们的工作是根据医生的意见使用模糊集,设计出专家系统诊断疾病。这里使用的模糊集诊断方法是基于20名医生的意见,该方法的输出能诊断出三种呼吸系统疾病（呼吸系统疾病的主要种类）：肺炎（PEN）,肺结核（TB）,普通流感（INF）。四种症状：X射线,呼吸频率（RR）,咳嗽（CO）和发烧（F）以数值数据的形式作为计算机的输入。这就是模糊逻辑的输入,输出是风险的范围和呼吸系统疾病的类型。在这项工作中,我们讨论的是如何使用人工智能研究（模糊集）去诊断疾病,而不是使用传统的诊断方法。同样地,考虑问题——解决过程的方法也是非常重要的。本论文是基于传统的诊断方法,研究医疗诊断问题。同时也认识到注意复杂问题领域的一些细节的必要性。本论文尝试在医疗诊断问题中融入模糊集模板和范例,特别是纯粹的概念探讨本研究分三个部分,第一阶段医生检查疾病然后收集他或她的症状如体温等等。第二阶段把数值数据输入到程序中,最后我们实施方案获得结果。这就是所有步骤。■

Assessment of the Effect of Neyshabur Green Spatial Configuration on the Temperature of Land Surface and Heat Islands

Urban heat island is a global issue and a consequence of rapid urbanization that leads to higher land surface temperature in urban areas. The range is 0.6°C - 1.3°C compared to rural and suburban areas. LST (Land surface temperature) is an important parameter in determining the heat island. Understanding the relationship between green space configuration and LST is essential to the effective design of the mechanisms, which reduce the effect of urbanization on UHI (urban heat island). This study examines the correlation between LST and spatial configuration of green space in the urban landscape of Neyshabur city, Iran. Satellite images are obtained from Landsat ETM+ satellite sensor with a spatial resolution of 60 meters in August 2010 and used for the estimation of LST. In order to identify the configuration of green space, five configuration metrics LSI (landscape Shape Index), MPFD (Mean Patch Fractal Dimension), ED (Edge Density), MPS (Mean Patch Size) and MSI (Mean Shape Index) are used. In addition, configuration of the green space and temperature is compared by Pearson’s correlation-coefficient. Negative values represent a suppressive/negative effect on each other;the fact that other indicators of spatial configuration are inversely related to temperature means that they weaken the effect of UHI. Results of the study showed that the spatial configuration of the green space notably affects increased LST and UHI. On the other hand, the configuration indicator with the greatest impact on LST was ED, because with increase in margin density further decreases LST.

Male sexual response is controlled by a series of neurally mediated phenomena regulating libido, motivation, arousal and genital responses such as penile erection and ejaculation. These neural events that occur in a hormonally defined milieu involve different neurophysiological, neurochemical, and neuropsychological parameters controlled by central mechanisms, spinal reflexes and peripheral nervous system. Epidemiologic studies have suggested the high prevalence of male sexual dysfunction worldwide with significant impact on the quality of life of patients suffering from this problem. The incidence of sexual dysfunction is particularly high among men with neurologic disorders. Sexual dysfunction in men, such as loss of sexual desire, erectile dysfunction(ED), changes in arousal, and disturbances in orgasm and ejaculation may involve organic causes, psychological problems, or both. Organic male sexual disorders include a wide variety of neurologic, vasculogenic, neurovascular or hormonal factors that interfere with libido,erection, ejaculation and orgasm. Neurogenic sexual dysfunction may result from a specific neurologic problem or it could be the presenting symptom of a developing neurologic disease. Neurologic ED could result from complications of chronic neurologic disorders, trauma, surgical injury or iatrogenic causes. These etiologic factors and the underlying pathophysiologic conditions could overlap, which should be considered when making a diagnosis and selecting a treatment. A detailed history of physical examination, neurologic disorders, as well as any past history of psychological and psychiatric disturbances, and a thorough neurological examination will provide better understanding of the underlying causes of neurogenic sexual dysfunction. In patients with spinal cord injury, the location of the lesion and the time of onset of injury should be determined. Therapeutic strategies against erectile dysfunction are initiated with the least invasive options
