Helicobacter cinaedi bacteremia with cellulitis after ABO-incompatible living-donor liver transplantation: Case report

Helicobacter cinaedi(H. cinaedi), a Gram-negative spiral-shaped bacterium, is an enterohepatic nonHelicobacter pylori Helicobacter species. We report the first case of H. cinaedi bacteremia with cellulitis after liver transplantation. A 48-year-old male, who had been a dog breeder for 15 years, underwent ABO-incompatible living-donor liver transplantation for hepatitis C virus-induced decompensated cirrhosis using an anti-hepatitis B core antibody-positive graft. The patient was preoperatively administered rituximab and underwent plasma exchange twice to overcome blood type incompatibility. After discharge, he had been doing well with immunosuppression therapy comprising cyclosporine, mycophenolate mofetil, and steroid according to the ABO-incompatible protocol of our institution. However, 7 mo after transplantation, he was admitted to our hospital with a diagnosis of recurrent cellulitis on the left lower extremity, and H. cinaedi was detected by both blood culture and polymerase chain reaction analysis. Antibiotics improved his symptoms, and he was discharged at day 30 after admission. Clinicians should be more aware of H. cinaedi in immunocompromised patients, such as ABO-incompatible transplant recipients.

Numerical Simulation of Air Pollution in Ulaanbaatar City, Mongolia

Air pollution has a major health impact on people living in UB (Ulaanbaatar) city. The excessive high particulate matter concentrations, especially in winter and in the ger areas, lead to serious illnesses and deaths. In order to get information to improve air quality in UB city, we investigate the current situation of location of ger areas and emission data of ger stoves and three thermal power plants. We reported the burning experimental result of the ger stoves and the present situation of the health hazard in a previous report. Now, in this report, we calculate the several diffusion simulations of air pollution in 2010. Our study has been carried out in collaboration with our Mongolian partner"s TCDC (Tolgoit community development center, Songinokhairkhan district), and we want to make a contribution towards a cleaner and greener UB city.

Objective: This multicenter clinical study was to assess the clinical usability of an oral moisture-checking device in detecting the dry mouth patients and evaluating the optimal measurement site. Materials and Methods: The study group comprised 250 patients with dry mouth and 241 healthy volunteer subjects at 13 medical centers. This device was used to measure the moisture degrees of the lingual mucosa and the buccal mucosa. Subjective oral dryness, objective oral dryness, and saliva flow rates were also compared between the two groups. For statistical analysis, receiver-operating characteristic analysis was performed to calculate the area under the curve (AUC). Results: The moisture degree of the lingual mucosa was significantly lower in the dry mouth group (27.2 ± 4.9) than that in the healthy group (29.5 ± 3.1, AUC = 0.653). When a lingual mucosa moisture degree of 31.0 or higher was defined as normal, less than 27.0 as dry mouth, and 27.0 to less than 31.0 as borderline zone of dry mouth, both the sensitivity and the specificity for the diagnosis of dry mouth were close to 80%. Conclusion: These results suggest that the oral moisture-checking device is a usable screening device for dry mouth.

目的分析20株临床分离于脑脊液、血液、尿液、痰液和支气管灌洗液的隐球菌菌株的生物学特性。方法分别以油菊籽、咖啡酸玉米琼脂、刀豆氨酸-甜菜碱-溴麝香草酚蓝培养基、尿素酶试验、37℃生长试验I、D32生化鉴定板和荚膜形成试验等初步鉴定后,再经大亚基核糖体DNA(LSUrDNA)的D1/D2区域序列分析进一步鉴定菌种。同时进行血清分型和随机扩增DNA多态性研究。结果20株菌皆具有以下特征:①形成荚膜,②37℃3天内生长试验皆为阳性,③咖啡酸玉米琼脂和油菊籽培养基皆可以产生褐色菌落,④尿素酶试验96 h皆呈阳性,⑤CGB培养基未见蓝色菌落生长,⑥ID32鉴定板可以鉴定到属,种的鉴定结果各不相同,⑦大亚基核糖体DNA(LSUrDNA)的D1/D2区域序列分析证实皆为新生隐球菌的新生变种,⑧血清型仅见A和AD二型,以A型为多数(83.33%),⑨20株临床分离株随机扩增DNA多态性分析有一定的生态多样性。结论20株临床分离株皆为新生隐球菌的新生变种,血清型多数为A型,少数AD型,但不同菌株间存在一定的生物多态性。

目的了解波氏假阿利什菌和尖端赛多孢子菌的基因学特征,研究DNA分型与菌种来源的关系。方法采用随机扩增多态性DNA分析(RAPD)方法。结果3种引物可将来自5个国家的13株波氏假阿利什菌和18株尖端赛多孢子菌分为31个基因型。多引物聚类分析所得树状图显示,除来自哥伦比亚土壤的3株波氏假阿利什菌外,其他受试菌株无地域性群集分布特点。但受试菌株中的多数波氏假阿利什菌和尖端赛多孢子菌株分别聚集成一群。结论波氏假阿利什菌和尖端赛多孢子菌存在较大株间差异,致病菌没有明显的地域性分布趋势,RAPD分型聚类分析结果与形态学分类之间具有一定一致性。

目的研究新疆地区汉族和维吾尔族患者来源的50株白念珠菌的基因型及其对两性霉素B、5-氟胞嘧啶、米卡芬净、伊曲康唑、氟康唑和咪康唑的体外敏感性。方法采用PCR法扩增白念珠菌rDNA 25S的Ⅰ类内含子包含区,根据扩增产物的大小判断基因型(A型为450 bp,B型为840 bp,C型为450 bp和840 bp)。采用CLSI M27-A液基微量稀释法测定50株白念珠菌对上述6种抗真菌药的体外敏感性。结果 50株菌分为3种基因型:A型30株,B型和C型各10株。所有菌株对两性霉素B、5-氟胞嘧啶、米卡芬净和咪康唑的MIC值较低,MIC范围依次为0.25～0.5μg/mL,0.125～0.5μg/mL,≤0.03μg/mL,0.25～8μg/mL;对伊曲康唑和氟康唑的MIC值较高,MIC范围分别为0.25～8μg/mL,0.5～64μg/mL。B型和C型对5-氟胞嘧啶的MIC值均为0.125μg/mL,对伊曲康唑和氟康唑的耐药率分别为84%、70%。不同族别来源的菌株基因型比较无显著差异(P>0.05),不同基因型菌株的抗真菌药物敏感性比较也无显著差异(P>0.05)。结论新疆地区白念珠菌分A,B,C三种基因型。汉族和维吾尔族患者来源的白念珠菌基因型及其对抗真菌药的敏感性以及基因型与种族间均无相关性。B型和C型对5-氟胞嘧啶高度敏感,所有菌株对伊曲康唑和氟康唑的耐药率较高。
