Changes in presentation and outcomes in cardiac surgery patients aged 70 to 79 years versus patients 80 years or older

Background: To establish which variables between cardiac surgery pts aged 70 - 79 years and 80 years or over are individually different for age group status. methods: group 1, 70 - 79 years, n = 351 and group 2, age 80 or over n = 94. Dem.g.aphics, operative and hospital outcomes were identified and EuroSCORE (ES) scores applied. Logistic regression uni- and mul-tivariate analysis was used to detect pre- and postop-erative variables which m.g.t be independently different between the groups. Results: Difference (p gher in group 2) patients with CHF, NYHA class, atrial fibrillation, aortic valve disease, both ES algorithms. group 2 pts had lower ejection fraction and less coronary artery disease. group 1 had more pts with diabetes. Operative mortality was low (1.7% total) with no difference between the groups. Length of stay in the postoperative intensive care unit and total hospitalization was equal but group 2 pts needed more often prolonged stay. group 2 pts had more postoperative renal failure (p = 0.002) and were more often not discharged home (p = 0.03). Hospital mortality did not reach statistical significance (group 1 18/5.2%, group 2 10/10.7%, p = 0.06). Univariate analysis detected 12 pre- and post-operative variables which identified group status. multivariate analysis using univariate results detected only diabetes (for group 1 status) and additive ES (for group 2 status) to be independently different between the groups. Conclusions: Elderly pts may be operated with low operative and hospital mortality. Only diabetes and additive ES (=comorbidities) were independently different between the groups.

Collatz Sequences and Characteristic Zero-One Strings: Progress on the 3<i>x</i>+ 1 Problem

The unsolved number theory problem known as the 3x + 1 problem involves sequences of positive integers generated more or less at random that seem to always converge to 1. Here the connection between the first integer (n) and the last (m) of a 3x + 1 sequence is analyzed by means of characteristic zero-one strings. This method is used to achieve some progress on the 3x + 1 problem. In particular, the long-standing conjecture that nontrivial cycles do not exist is virtually proved using probability theory.

A Rare Association of giant Cell Arteritis with Recurrent Corneal Ulcer

We report a rare case of association of temporal arteritis with recurrent central corneal ulcer. A 91-year-old male with a remote history of clinically diagnosed giant cell arteritis (gCA) and penetrating keratoplasty (PK) for corneal edema in 1990’s in the left eye presented with irritation and severe dry eye. He progressively developed central corneal ulcer regardless of treatment with aggressive lubrication and patching. The clinical course, blood tests and temporal artery biopsy results yielded the diagnosis of recurrent gCA. The clinical course improved after starting oral steroids. The case report illustrates the importance of ruling out gCA in patients with recurrent corneal ulcers and history of gCA. It also highlights the significance of shared understanding between primary care physicians and specialists to handle common and chronic diseases to make efficient diagnoses and plan treatment regimens. This is the first plausible case of reported gCA association with central corneal ulcer in the United States.

Single Dose Inamrinone in Terminal Warm Cardioplegia in On-Pump Coronary Artery Bypass Patients

Background: Phosphodiesterase inhibitors (PDI) are used in cardiac surgery to improve and stabilize cardiac function after surgery. The aim of this study is to evaluate changes in hemodynamics and early outcomes when PDI (Inocor/inamrinone) is given in terminal warm blood cardioplegia to on-pump CABg only patients and compare results with patients who did not receive the drug. material and methods: From April 2003 through September 2004 241 pts underwent elective on-pump CABg only surgery. 141 pts received Inocor in the terminal warm blood cardioplegia (group 1) and 100 pts did not (group 2). Results: Dem.g.aphic data, preoperative EuroSCORE risk scores and operative details were similar. Of pts preoperatively in sinus rhythm (SR) 80.15% in group 1 and 69.79% in group 2 regained spontaneous SR (p = 0.07) after release of crossclamp. Inotropic support was needed in 5 pts in group 1 and in 12 pts in group 2, p = 0.02. Post cardiopulmonary by-pass (CPB) IABP support was needed for 4 pts in group 2 and none for group 1 pts, p = 0.01. There was no operative mortality in either group and hospital/30 day mortality was similar (3/2.13% vs 3/3.00%, p = 0.69). There were no statistical difference in stay in postoperative intensive care unit (p = 0.15), total hospital stay (p = 0.07), appearance of postoperative atrial fibrillation (p = 0.23) or appearance of postoperative kidney injury (p = 0.27). Post CPB cardiac index improved 16.90% in group1but decreased 1.49% in group 2, p mean arterial pressure decreased 7.46%in group 1 pts and 5.08% in group 2 pts, p = 0.002, but no pts in either group needed medication for too low blood pressure. Systemic vascular resistance decreased 28.40% in group 1 and 9.02% in group 2, p mrinone in terminal warm blood cardioplegia (hot shot) is safe and effective way to improve and stabilize cardiac function after on-pump CABg surgery but does not affect short-term outcomes.

Continuous monitoring of mining Induced Strain in a Road Pavement Using Fiber Bragg grating Sensors

This paper describes the application of fiber Bragg grating （FBg） based sensors for monitoring road pavement strains caused by mining induced ground subsidence as a result of underground longwall coal mining beneath a major highway in New South Wales, Australia. After a lengthy planning period, the risks to the highway pavement were successfully managed by the highway authority and the mining company through a technical committee. The technical committee comprised representatives of the mining company, the highway authority and specialists in the fields of pavement engineering, geotechnical engineering and subsidence. An important component of the management strategy is the installation of a total of 840 strain and temperature sensors in the highway pavement using FBg arrays encapsulated in glass-fiber composite cables. The sensors and associated demodulation equipment provide continuous strain measurements along the pavement, enabling on-going monitoring of the effects of mining subsidence on the pavement and timely implementation of planned mitigation and response measures to ensure the safety and serviceability of the highway throughout the mining period.

The emerging role and targetability of the TCA cycle in cancer metabolism

The tricarboxylic acid （TCA） cycle is a central route for oxidative phosphorylation in cells, and fulfills their bioenergetic, biosynthetic, and redox balance require- ments. Despite early dogma that cancer cells bypass the TCA cycle and primarily utilize aerobic glycolysis, emerging evidence demonstrates that certain cancer cells, especially those with deregulated oncogene and tumor suppressor expression, rely heavily on the TCA cycle for energy production and macromolecule synthesis. As the field progresses, the importance of aberrant TCA cycle function in tumorigenesis and the potentials of applying small molecule inhibitors to perturb the enhanced cycle function for cancer treatment start to evolve. In this review, we summarize current knowledge about the fuels feeding the cycle, effects of oncogenes and tumor suppressors on fuel and cycle usage, common genetic alterations and deregulation of cycle enzymes, and potential therapeutic opportunities for targeting the TCA cycle in cancer cells. With the application of advanced technology and in vivo model organism studies, it is our hope that studies of this previously overlooked biochemical hub will provide fresh insights into cancer metabolism and tumorigenesis, subsequently revealing vulnerabilities for thera- peutic interventions in various cancer types.

Comparison of seldinger and trocar techniques in the percutaneous treatment of hydatid cysts

AIm To comparatively evaluate Seldinger and Trocar techniques in the percutaneous treatment of hydatid disease. mETHODS Trocar and Seldinger techniques were used for 49 and 56 cysts, respectively, among 106 hydatid cysts in 88 patients. The number of males and females were 22 and 66, respectively with a mean age of 44.9 years(range, 15-87). Follow-up studies included cyst diameter, cyst contents, and morphological changes inthe cyst wall, local recurrence, and secondary invasion, using ultrasound, computerized tom.g.aphy and chest X-rays. RESULTS The positive criteria of healing were a decrease in cyst diameter, progressive solidification of the cyst contents, and disappearance of the cyst. Local recurrence was defined as an increase in the cyst diameter and contents, and appearance of daughter cysts in the primary cavity, while secondary dissemination was defined as the appearance of new cysts outside the treated cyst. mean duration of follow-up was 19.23 mo(range, 18-26 mo). Follow-up results demonstrated that no significant differences were present between the Trocar and Seldinger techniques in the percentage of decrease in the cyst volume, rate of early complications, local recurrence and secondary dissemination(P = 0.384, 0.069, 0.215 and 0.533, respectively). CONCLUSION There are no differences between the Seldinger and Trocar techniques that gain entry to the cyst cavity in terms of the efficacy of the treatment and the rates of early and late complications.

In this report,we show that hyperspectral high-resolution photoluminescence mapping is a powerful tool for the selection and optimiz1ation of the laser ablation processes used for the patterning interconnections of subcells on Cu(Inx,ga1-x)Se2(CIgS)*** this way,we show that in-depth monitoring of material degradation in the vicinity of the ablation region and the identification of the underlying mechanisms can be ***,by analyzing the standard P1 patterning line ablated before the CIgS deposition,we reveal an anomalous emission-quenching effect that follows the edge of the molybdenum.g.oove *** further rationalize the origins of this effect by comparing the topography of the P1 edge through a scanning electron microscope(SEm)cross-section,where a reduction of the photoemission cannot be explained by a thickness *** also investigate the laser-induced dam.g. on P1 patterning lines performed after the deposition of *** then document,for the first time,the existence of a short-range dam.g.d area,which is independent of the application of an optical aperture on the laser *** findings pave the way for a better understanding of P1-induced power losses and introduce new insights into the improvement of current strategies for industry-relevant module interconnection schemes.
