Reactive oxygen species and nitric oxide scavenging nanoparticles alleviating rheumatoid arthritis through adjusting the seeds and growing soils

Normalizing inflamed soils including reactive oxygen species(ROS),nitric oxide(NO),cellfree DNA,and regulating infammation-related seeds such as macrophages,neutrophils,fibroblasts,represent a promising strategy to maintain synovial tissue homeostasis for rheumatoid arthritis(RA)***,ROS scavenging amphiphilic block copolymer PEgylated bilirubin and NO-scavenging PEgylated o-phenylenediamine were fabricated to self-assemble into a dually responsive nanoparticle loaded with JAK inhibitor notopterol(Not@BR/oPDA-PEg,NBOP NPs).The simultaneous ROS and NO depletion combined with JAK-STAT pathway inhibition could not only promote m2 polarization to reduce further ROS and NO generation,but also decrease cytokines and chemokines to prevent immune cell ***,NBOP NPs responded to high level ROS and NO,and disintegrated to release notopterol in inflamed joints as the hydrophobic heads BR and oPDA were transformed into hydrophilic *** released notopterol could inhibit the JAK-STAT pathway of infammatory cells to reduce the secretion of pro-inflammatory cytokines and *** strategy represented an effective way to regulate RA soils and seeds through breaking the positive feedback loop of inflammation aggravation,achieving an excellent anti-RA efficacy in a collagen-induced arthritis rat *** together,our work offered a reference to adjust RA soils and seeds for enhanced RA treatment.

Interprofessional Communication and Relationships in the management of “Difficult to Treat” Depression: Perceptions of the Role of general Practitioners

Background: Team based care is an essential ingredient of chronic disease management including chronic mental illness. Effective health care teams include members who have defined, yet intersecting roles, where mutual respect characterises professional interaction and the patient’s well-being is central. The aim was to explore the perception of psychologists, psychiatry registrars and psychiatrists with respect to gPs’ role in managing difficult-to-treat-depression (DTTD). methods: A previously developed semi-structured interview schedule comprising six questions was used. Thirty-two health professionals participated. Data were analysed using the Framework method. Findings: Four main themes emerged: 1) The team approach was important, particularly to ensure information accuracy and/or when responding to patient needs and pressures;2) Referrals, usually generated by gPs can be a vehicle for other health professionals to provide advice to the gP;3) Availability and accessibility often depended on health professionals work location and knowing how to navigate the system;4) Limited availability of government funding impacts on patients’ accessibility to health professionals. Discussion: Interprofessional relationships were described as paramount. Appropriate and timely referrals are integral to patient management, regardless of challenges. Ongoing challenges include program funding, workforce numbers and costs to patients. Improvement to mental health care access was noted, even for patients among relatively disadvantaged groups and those receiving medicare Benefits Schedule-subsidised services. Conclusion: Despite adequate gP/specialist communication, the delivery of optimal team based care to patients with difficult-to-treat depression is compromised by lack of access to specialised services and inadequate funding.

Difficult-to-Treat-Depression and gPs’ Role: Perceptions of Psychiatry Registrars

Introduction: For patients, gPs are the most accessible medical resource in the community and are the gatekeepers to other community resources including psychiatrists. Qualifying as a psychiatrist in Australia involves completing a five-year training program that includes rotations in hospitals and community settings. The aims of this research were to 1) explore psychiatry registrars’ perceptions of difficult-to-treat-depression (DTTD) and 2) what they thought about the gPs’ role in this regard. methods: A semi-structured interview schedule comprising six questions was used;10 psychiatry registrars (6 females, 4 males) participated in a one-and-half-hour focus group. All were in their final year of training and undertaking a training post in a public hospital in melbourne, Australia. Data were analysed using the Framework method. Findings: Similar to gPs and gP trainees, psychiatry registrars’ perceptions and understanding of DTTD varied. While acknowledging limited experience in diagnosis and management, issues important to them included the utility of labels such as DTTD;patients distressed because of another diagnosis, substance abuse and/or life problems, the importance of accurate histories and notes, cost and limited availability of services particularly in the private sector, prescribing regimens, referring to allied health professionals, and suggesting/prescribing non pharmacological and/or complementary treatment. Also what was of concern was communication, both between health professionals and between health professionals and patients. Consensus was that treating depression in general practice is one of the hardest things for gPs to manage but there was value in using mental health plans. Discussion and Conclusion: While this cohort was small in number with limited experience, this study is the first to contribute to the literature that provides some insight into psychiatry registrars’ experiences and perceptions of DTTD. Introduction: Since the Australian government introduced the “Better Access to mental Health Services” program in 2006, psychiatrists, psychologists and general practitioners (gPs) have become increasingly involved in service provision for people seeking help with mental health problems. The aim of this research was to a) explore psychologists’ perceptions of difficult to treat depression (DTTD) and b) explore what they thought about the gPs’ role in managing these patient given that most patients are referred to psychologist by gPs. methods: A previously developed semi-structured interview schedule comprising six questions was used. Seven psychologists participated in a focus group held in melbourne. Data were analysed using the framework method. Findings, including Discussion: While psychologists understood the term DTTD it was suggested that using different terms may limit understanding between health professionals. Rather than diagnosing, psychologists were more likely to conduct further assessment contextually to confirm.g.s’ diagnosis. Communication with gPs was important, particularly when managing “long-term” and suicidal patients. management included cognitive and behavioural interventions and referring to other mental health services, psychiatrists and/or other allied health professionals. Referral to psychiatrists could be difficult because of limited availability and for some patients, prohibitive costs. Although psychologists discussed non-pharmacological and/or complementary treatment options with patients, they were more likely to rely on gPs to discuss/prescribe these options. Conclusion: While generalisability may be limited, this study is the first to document some understanding of psychologists perceptions of DTTD and the importance of gPs and other health professionals’ role in managing this patient cohort.

Incidence of Subclinical Hypothyroidism in Cardiac Surgery Patients. Comparison of Presentation Characteristics, Hospital and medium-Term Outcomes with Euthyroid Patients

Subclinical hypothyroidism (SCHT) is common, with an occurrence of up to 10% of the adult population and defined biochemically only by elevated TSH and normal T4. SCHT affects negatively on lipid and carbohydrate metabolism increasing the risk of ischemic heart disease, affects negatively on cardiac performance and have a close correlation with renal function. The aim of this study is to compare presentation characteristics and outcomes between euthyroid pts and pts with SCHT who underwent cardiac surgery. methods: 474 pts from June 2003 through September 2004 had TSH and T4 measured. 365 pts were euthyroid (group 1), 41 pts had SCHT (group 2). groups were compared by dem.g.aphics and EuroSCORE (ES) risk profiles. Operative and hospital outcomes were compared as was follow-up mortality up to 96 months. Results: There were more females in group 2, p = 0.04, more pts with CHF and number of NYHA III-IV pts (p more pts in group 2 had elevated s-crea (p mparing the groups by EuroSCORE (ES) showed higher risk scores in group 2 pts (Additive ES 6.8 vs 8.5 and Logistic ES 12.3% vs 18.1%, p = 0.01 and 0.03). Hospital mortality was higher in group 2 (12.2% vs 4.1%, p = 0.04) and the number of pts needing extended care was higher in group 2 (p = 0.01). Follow up mortality was doubled in group 2 pts up to 96 months compared to group 1 (p mpared with euthyroid pts. Hospital and follow-up mortality are increased in SCHT pts.

Complications of Learning Curve in Supratentorial Intraventricular Region. The Concept of Fast Daily Fresh Cadaver Practice. (Technical Note for Training method)

Background: The difficulty of learning curve in different microsurgical regions is one of the most challenging factors that affect neurosurgeon’s life. Two of the delicate areas are the suprasellar intraventricular and pontocerebellar region. High level of microsurgery or endoscopic surgery requires virtuosity, which can not be learnt only by assisting. Even among the well skilled neurosurgeons who work in big centers we can often hear “in spite of centralization there are not enough operations available to keep the high level of our skills”. The situation of surgeons working at mid centers is more difficult especially on duties. methods: Huge number of scientific articles demonstrate the importance of different types of cadaver practice, but always did not pay enough attention to the daily fast fresh cadaver exercises. Focusing on lifelong every day practice we examined the difficulties of learning curve in intraventricular and pontocerebellar region and try to give some useful advice for practicing on fresh cadavers on a daily basis. We offer a simple endovascular fresh cadaver model for practicing. Results: We have performed more than 800 quick (60 - 90 minutes long) routine practice sessions in these two regions during last 18 years. We found that implantation a modelized pathology and removing it by microscopical or endoscopic way without causing injuries to sensitive structures seems to be the most practical way of fast everyday training. Nerves and vascular bypass sutures are also excellent practical method. Conclusions: We have focused on daily fast fresh cadaver practice as a novel training method like used in professional sport and art. After 18 years experience we can state that, for mastering the microsurgery of pontocerebellar and intraventricular region, which is one of the most challenging regions minimum 100 fresh cadaver practice sessions need before performing the operation. Difficult-to-Treat-Depression—Perceptions of gPs and gP Trainees

Introduction: gPs are often the first contact point for mental health concerns. Training to qualify as a gP involves completing a three-to-four year program. Among other things, the training program may include a six-hour mental health training workshop. The study’s purpose was to compare gPs’ and Trainees’ perceptions of difficult-to-treat-depression (DTTD). methods: Using a semi-structured interview schedule comprising six questions, 10 gPs and six Trainees participated in focus groups or telephone interview. To understand participants’ perspectives, data were analyzed using the Framework method. Findings: Trainees were less clear than gPs about the meaning of DTTD and other diagnostic terms. gPs’ diagnosis included querying diagnosis and treatment options. Trainees’ experience was limited but they raised barriers including non-response to various treatment trials. Both groups identified management difficulties including: doctor shopping, suicide risk, patients not being well informed;with management difficulties exacerbated by limited access/referring to health professionals, cost and/or unavailability of bulk billing. While some gPs and Trainees had heard of an illness management model or chronic illness model, few used a model. most reported limited referring to psychiatrists, mainly because of cost and/or limited availability. gPs were more likely to refer to other health professionals and use pharmacological and complementary therapies. Both groups discussed the impact of external factors including cultural factors, patient compliance, treatment failure, and the importance of the relationship between the patient and the professional. Trainees were more likely to stress the importance local clinical guidelines. Discussion and Conclusion: Despite the small size and limited nature, this research provides insight into some of the similarities and differences of gPs’ and Trainees’ experiences and understanding of DTTD. As part of a collaborative study between the 3rd Institute of Oceanography, Xiamen, and the U-K. Universities of East Anglia and St. Andrews, a 4-day experiment was conducted on a ridge- and -runnel beach of north Norfolk on the North Sea coast- Detailed surveys were made of every low water of an area 10m by 30m and electrom.g.etic current meters were used to measure wave- induced currents over the ridge and in the runnel- The locations of bedform fields were noted, fluorescent sand was used to follow the sand movement and, at the end of the experiment, lacquer peels were taken of the top 0-2 m of a vertical section through the ridge- and- runnel-During the 4 days 0-75 m3 of sand per metre width of beach accreted in the runnel and a similar volume was eroded from the upper foreshore. The dominant shoreward transport, identified qualitatively by the movement of the fluorescent sand, suggests the ridge- and- runnel system m.g.ated shorewards up the general beach slope of 1 by 2 - 3 m. Linking Diffractive and geometrical Optics Surface Scattering at a Fundamental Level

Optical surface scattering analyses based on diffractive optics (DO) are typically applied to one surface;however, there is a need for simulating surface scattering losses for devices having many surface interactions such as light pipes. Light pipes are often simulated with geometric optics (gO) using ray tracing, where surface scattering is driven by the surface slope distribution. In the DO case, surface scattering analyses depend on the spatial frequency distribution and amplitude as well as wavelength, with the sinusoidal grating as a fundamental basis. A better understanding of the link, or transition, between DO and gO scattering domains would be helpful for efficiently incorporating scattering loss analyses into ray trace simulations. A formula for the root-mean-square (rms) scattered angle width of a sinusoidal reflection grating that depends only on the surface rms slope is derived from the nonparaxial scalar diffraction theory, thereby linking it to gO. The scatter angle’s mean and rms width are evaluated over a range of grating amplitudes and periods using scalar theory and full vector simulations from the COmSOL® wave optic module for a sinusoidal reflection grating. The conditions under which the diffraction-based solution closely approximates the gO solution, as predicted by the rms slope, are identified. Close agreement is shown between the DO and gO solutions for the same surface rms slope scattering loss due to angular filtering near the critical angle of a total internal reflection (TIR) glass-to-air interface.

A multi-Channel CmOS Analog Front-End Interface IC with 157.8 d B Current Detection Dynamic Range

A high dynamic range and low-noise CmOS(complementary metal-oxide-semiconductor transistor)front-end interface integrated circuit(IC) with multi-channel detection is presented in this paper. Two different current detection channels, composed of a trans-impedance amplifier(TIA) and an integrator-differentiator TIA, are used to boost the current detection range. A capacitance-coupled instrument amplifier(CCIA) is also included to realize high precision voltage detection. A fourth-order sigma-delta modulator using a second-order loop filter and a second-order noise shaping integral quantizer is adopted to realize effective number of bits above 16 bits. The presented interface IC is implemented in 0.18-μm CmOS process with supply voltage of 3.3 V, and a proto-type electrochemical sensor platform with miniaturized sensor array is developed to verify the functionality of the interface IC. measurement results indicate that the designed interface IC achieves 157.8 d B current detection dynamic range, and the measured input-referred current noise and voltage noise floor are 1.04 p A and 58.4 n V within 10 k Hz integration bandwidths,respectively.
