AIM:To clarify the safety and efficacy of repeat endoscopic submucosal dissection(re-ESD)for locally recurrent gastric cancers after ***:A retrospective evaluation was performed of the therapeutic efficacy,complications and followup results from ESD treatment for early gastric cancers in 521 consecutive patients with 616 lesions at ***`s International Hospital between April 2004 and November *** addition,tumor size,the size of resected specimens and the operation time were compared between re-ESD and initial ESD procedures.A flex knife was used as the primary surgical device and a hook knife was used in cases with severe fibrosis in the submucosal *** variables were analyzed using the non-parametric Mann-Whitney U test and are expressed as medians(range).Categorical variables were analyzed using a Fisher’s exact test and are reported as *** significance was defined as a P-value less than ***:The number of cases in the re-ESD group and the initial ESD group were 5 and 611,*** median time interval from the initial ESD to re-ESD was 14(range,4-44 mo).En bloc resection with free lateral and vertical margins was successfully performed in all re-ESD cases without any *** local or distant recurrence was observed during the median follow-up period of 48(range,11-56 mo).Tumor size was not significantly different between the re-ESD group and the initial ESD group(median 22 mm vs 11mm,P=0.09),although the size of resected specimens was significantly larger in the re-ESD group(median 47 mm vs 34 mm,P<0.05).There was a nonsignificant increase observed in re-ESD operation time compared to initial ESD(median 202 min vs 67 min,respectively,P=0.06).CONCLUSION:Despite the low patient number and short follow-up,the results suggest that re-ESD is a safe and effective endoscopic treatment for recurrent gastric cancer after ESD.

AIM: To investigate the safety and efficacy of endo-scopic band ligation(EBL) for bleeding lesions in the small ***: This is a retrospective study evaluating EBL in six consecutive patients(three males, three fe-males, 46-86 years of age) treated between May 2009 and February 2014: duodenal vascular ectasia; 1, je-junal bleeding diverticulum; 1, ileal Dieulafoy's lesion; 1 and ileal bleeding diverticula; 3. The success of the initial hemostasis was evaluated, and patients were observed for early rebleeding(within 30 d after EBL), and complications such as perforation and abscess for-mation. Follow-up endoscopies were performed in four ***: Initial hemostasis was successfully achieved with EBL in all six patients. Eversion was not sufficient in four diverticular lesions. Early rebleeding occurred three days after EBL in one ileal diverticulum, and arepeat endoscopy revealed dislodgement of the O-band and ulcer formation at the banded site. This rebleeding was managed conservatively. Late rebleeding occurred in this case(13 and 21 mo after initial EBL), and re-EBL was performed. Follow-up endoscopies revealed scar formation and the disappearance of vascular lesions at the banded site in the case with a duodenal bleeding lesion, and unresolved ileal diverticula in three cases. Surgery or transarterial embolization was not required without any complications during the median follow-up period of 45(range, 2-83) ***: EBL is a safe and effective endoscopic treatment for hemostasis of bleeding lesions in the small bowel.

Cell division and differentiation of stem cells are controlled by non- cell-autonomous signals in higher organisms. The plant vascular meristem is a stem-cell tissue comprising procambial cells that produce xylem cells on one side and phloem cells on the other side. Recent studies have revealed that TDIF （tracheary element differentiation inhibitory factor）/CLE41/CLE44 peptide signal controls the procambial cell fate in a non-cell-autonomous manner. TDIF produced in and secreted from phloem cells is perceived by TDR/PXY, a leucine-rich repeat receptor kinase located in the plasma membrane of procambial cells. This signal suppresses xylem cell differentiation of procambial cells and promotes their proliferation. In addition to TDIF, some other CLE peptides play roles in vascular development. Here, we summarize recent advances in CLE signaling governing vascular development.

A ring fan is a propeller fan that applies an axial-flow impeller with a ring-shaped shroud on the blade tip side. In this study, the entire flow field of the ring fan is simulated using computational fluid dynamics (CFD); the accuracy of the CFD is verified through a comparison with the aerodynamic characteristics of a propeller fan of current model. Moreover, the aerodynamic noise generated by the fan is predicted on the basis of the wake characteristics. The aerodynamic characteristic of the ring fan based on CFD can represent qualitatively the variation in the measured value. The main flow domain of the ring fan is formed at the tip side of the blade because blade tip vortex is not formed at that location. Therefore, the relative velocity of the ring fan is increased by the circumferential velocity. The sound pressure levels of the ring fan within the frequency band of less than 200 Hz are larger than that of the propeller fan. In the analysis of the wake characteristics, it revealed that Karman vortex shedding occurred in the main flow domain in the frequency domain lower than 200 Hz ; the aerodynamic noise of the ring fan in the vortex shedding frequency enlarges due to increase in the relative velocity and the velocity fluctuation.

Prognostic factors of T4 gastric cancer patients undergoing potentially curative resection

AIM:To investigate the prognostic factors of T4 gastric cancer patients without distant metastasis who could undergo potentially curative resection. METHODS:We retrospectively analyzed the clinical data of 71 consecutive patients diagnosed with T4 gastric cancer and who underwent curative gastrectomy at our *** clinicopathological factors that could be associated with overall survival were *** cumulative survival was determined by the Kaplan-Meier method,and univariate comparisons between the groups were performed using the log-rank *** analysis was performed using the Cox proportional hazard model and a step-wise procedure. RESULTS:The study patients comprised 53 men (74.6%)and 18 women(25.4%)aged 39-89 years (mean,68.9 years).Nineteen patients(26.8%)had postoperative morbidity:pancreatic fistula developed in 6 patients(8.5%)and was the most frequent complication,followed by anastomosis stricture in 5 patients (7.0%).During the follow-up period,28 patients(39.4%)died because of gastric cancer recurrence,and 3(4.2%) died because of another disease or *** all patients,the estimated overall survival was 34.1%at 5 *** analyses identified the following statistically significant prognostic factors in T4 gastric cancer patients who underwent potentially curative resection: peritoneal washing cytology(P<0.01),number of metastatic lymph nodes(P<0.05),and venous invasion(P <0.05).In multivariate analyses,only peritoneal washing cytology was identified as an independent prognostic factor(HR=3.62,95%CI=1.37-9.57)for longterm survival. CONCLUSION:Positive peritoneal washing cytology was the only independent poor prognostic factor for T4 gastric cancer patients who could be treated with potentially curative resection.
