There has been an increase in spinal surgery for the elderly, with a corresponding potential increase in perioperative complications. In our department, 1833 patients underwent spinal surgery under general anesthesia from April 2001 to October 2012, and 2 of 260 patients aged ≥75 years old had postoperative cerebral infarction. An analysis of the pathogenic mechanism and potential risk factors showed that a history of cerebral infarction was a significant risk factor. Blood pressure rapidly increased on arousal from anesthesia, and particularly on extubation. The change in blood pressure was examined as a potential risk factor for cerebral infarction, but no significant relationship was observed. This result requires further examination in more patients with cerebral infarction after spinal surgery.

Septic arthritis of the shoulder is uncommon in the immunocompetent patient with no previous risk factors for joint infection. We treated an immunocompetent patient who developed septic arthritis of the shoulder after suprascapular nerve block for pain due to rotator cuff tear. An 80-year-old man with no underlying disease visited a nearby orthopedics clinic with complaint of left shoulder joint pain. Left suprascapular nerve block was performed, but the pain gradually aggravated. On the day after the block, he had a fever of 39°C and came to our department. On examination, enlargement and tenderness were present at the injection site. Cellulitis at the site was suspected. He was admitted and administration of a cephem anti-biotic was started. Pain subsequently decreased. Magnetic resonance imaging (MRI) performed 4 days after hospitalization showed massive effusion close to the injection site. The effusion spread into the joint cavity through the tear site of the supraspinatus. Septic arthritis of the shoulder was strongly suspected, open irrigation and debridement were performed 11 days after hospitalization. After surgery, pain immediately improved. In our case the extra-articular infection caused by suprascapular nerve block considered to spread into the shoulder joint cavity through the site of rotator cuff tear, although there have been no reports of such cases. This case suggests the possibility that patients with rotator cuff tear may easily develop septic arthritis because extra-articular infection may spread into the joint cavity through the site of tear.

A Case of Rectus Sheath Hematoma Due to Anticoagulants after Total Knee Arthroplasty

Background: Hematoma in the rectus sheath is not common but if happens it will become major bleeding. Sometimes anticoaglation of deep vein thrombosis (DVT) causes the hematoma in the rectus sheath. Case Report: A 74-year-old female patient after total knee arthroplasty (TKA) due to osteoarthritis. Postoperation, residual DVT was noted in the right soleus vein on ultrasonography of the veins of the lower limbs. Anticoaglation was started 7.5 mg/day Arixtra for treatment of DVT. Anticoaglation was administered throughout rehabilitation, and abdominal pain developed on postoperative day 9. Since respiratory distress developed on day 10, then thoracoabdominal contrast CT was performed. Although no PE was observed, a hematoma was detected in the rectus sheath, and it perforated into the abdominal cavity. The hemoglobin level was reduced by about 4 g/dL, and the patient was admitted to the ICU. Surgical treatment was not performed, anticoagulant treatment was discontinued, and conservative treatment was administered. During management in the ICU, a total of 24 units of red blood cells were transfused. Conclusions: Rectus sheath hematoma should be in mind of surgeons during differential diagnosis of acute abdominal pain especially in patients receiving anticoagulants. Early recognition can be of great importance for patients’ recovery, preventing from severe complications. Management is usually supportive although surgical intervention in some patients should be considered.

The objective of the present study was to determine the optimum sowing time of three quinoa ecotypes (Altipllano, sea level, and valley) for high seed yields in south Kanto, Japan. Pot experiments were conducted in the experimental field at Nihon University during 2011, 2012, 2013, and 2014. In this experiment, the following quinoa varieties were used NL-6, Baer Cajon and Cauquenes (sea-level type), Amarilla de Marangani, Blanca de Junin, CICA-127, ECU-420, ECU-525, Ingapirica, and Narino (valley type), 94R and Isluga (Altiplano type). The quinoa seeds were sown on March 29, June 17 and September 22, 2011;March 27, June 17 and August 28, 2012;March 26, June 15 and 5 September 5, 2013;and March 27, June 17 and August 28, 2014. When the sea-level type and Altiplano type seeds were sowed from March to September, the seeds could be gained in all sowing plots. However, the seed weights of all varieties were the highest in the sowing plots of March. And the seed weights in the sowing plot of March were significantly higher than that in the other sowing plots. The sea-level type and Altiplano type quinoa had almost the same seed growth reaction for day length and day temperature. Thus, to gain a high seed yield of the sea-level and Altiplano type quinoa, March was the optimum sowing time in south Kanto, Japan. When the valley-type seeds were sowed from March to June, the seeds could not be gained, except in 2012. In 2012, the seed weights and seed numbers in sowing plots of March and June were significantly lower than those in the sowing plot of September. Thus, to obtain a high seed yield of the valley type quinoa, the optimum sowing time in south Kanto, Japan was from August to September.

Arbuscular Mycorrhizal Fungal Community Structure in Soybean Roots: Comparison between Kanagawa and Hokkaido, Japan

The objectives of this study were to determine arbuscular mycorrhizal fungi (AMF) community structure in colonized roots of soybean cultivated from Kanagawa and Hokkaido in Japan and to relate the community structure to environmental conditions, which included soil type, preceding crops, and soil chemical properties. The average number of AMF OTU (operational taxonomic unit) colonizing soybean roots collected from Kanagawa and Hokkaido was 11.2 and 5.8, respectively, a significant difference. Moreover, AMF from the family Gigasporasera was not identified in soybean roots collected from Hokkaido, suggesting that AMF in the family Gigasporasea is absent or rare in the soybean fields of sampled in Hokkaido. We postulate that the soil type, preceding crops or soil chemical properties are not the underlying factor differentiating AMF community structure colonizing in soybean roots between Kanagawa and Hokkaido. Instead we conclude that temperature and phosphate absorption coefficient are the determining factors of AMF OTU in this study.

Evapotranspiration Characteristics of a Lowland Dry Evergreen Forest in Central Cambodia Examined Using a Multilayer Model

Development pressure has led to serious deforestation on the Indochina Peninsula. Particularly rapid defor-estation has occurred in easily accessible lowland areas, and it is thus important to accumulate knowledge about these forests immediately. We measured evapotranspiration rates for a lowland dry evergreen forest in Kampong Thom Province, central Cambodia, using the energy balance Bowen ratio (EBBR) method based on meteorological data collected from a 60-m-high observation tower. Daily evapotranspiration was higher during the dry season than during the rainy season of the Asian monsoon climate. The seasonal variation in evapotranspiration generally corresponded to the seasonal difference in the vapor pressure deficit. A multi-layer model was used to simulate the seasonal variation in evapotranspiration. The multilayer model also reproduced the larger evapotranspiration rate in the dry season than in the rainy season. However, observed values substantially exceeded model-calculated results during certain periods at the beginning of the dry season and in the late dry season. Moreover, during the rainy season, the model tended to overestimate evapotranspiration. The differences between these observed and simulated values may have been caused by seasonal characteristics of photosynthesis and transpiration in the lowland dry evergreen forest that were not considered in the model simulation.

Peripheral portal vein-oriented non-dilated bile duct puncture for percutaneous transhepatic biliary drainage

AIM: To evaluate the efficacy of peripheral portal vein(PV)-oriented non-dilated bile duct(BD) puncture for percutaneous transhepatic biliary drainage(PTBD).METHODS: Thirty-five patients with non-dilated BDs underwent PTBD for the management of various biliary disorders, including benign bilioenteric anastomotic stricture(n = 24), BD stricture(n = 5) associated with iatrogenic BD injury, and postoperative biliary leakage(n = 6). Under ultrasonographic guidance, percutaneous transhepatic puncture using a 21-G needle was performed along the running course of the peripheral targeted non-dilated BD(preferably B6 for right-sided approach, and B3 for left-sided approach) or along the accompanying PV when the BD was not well visualized. This technique could provide an appropriate insertion angle of less than 30° between the puncture needle and BD running course. The puncture needle was then advanced slightly beyond the accompanying PV. The needle tip was moved slightly backward while injecting a small amount of contrast agent to obtain the BD image, followed by insertion of a 0.018-inch guide wire(GW). A drainage catheter was then placed usinga two-step GW method. RESULTS: PTBD was successful in 33(94.3%) of the 35 patients with non-dilated intrahepatic BDs. A rightsided approach was performed in 25 cases, while a left-sided approach was performed in 10 cases. In 31 patients, the first PTBD attempt proved successful. Four cases required a second attempt a few days later to place a drainage catheter. PTBD was successful in two cases, but the second attempt also failed in the other two cases, probably due to poor breath-holding ability. Although most patients(n = 26) had been experiencing cholangitis with fever(including septic condition in 8 cases) before PTBD, only 5(14.3%) patients encountered PTBD procedure-related complications, such as transient hemobilia and cholangitis.
