Anterior cingulated cortex (ACC) is involved in “the state in which patients do not care much about pain despite its presence” which is a goal of psychosomatic treatment. To investigate the absolute concentration of N-acetylaspartate (NAA) in the anterior cingulated cortex (ACC) as predictors of patients that may benefit from cognitive behavioural therapy in the treatment of chronic pain. Proton magnetic resonance spectroscopy (1H-MRS) was performed with a 1.5 T MR system on a voxel in the bilateral ACC in 85 chronic pain patients and 20 age-matched normal control subjects. Eighteen out of 24 (75.0%) patients whose NAA concentration decreased significantly in the ACC, respectively, compared to the mean NAA concentration of the normal control subjects, needed cognitive behavioural therapy. Our results suggest that decreased NAA concentration in the ACC is associated with the necessity of cognitive behavioural therapy. 1H-MRS may serve as a useful non-invasive tool for evaluating chronic pain patients.

Surgical resection for colorectal liver metastases(CRLM)may offer the best opportunity to improve ***,only about 20% of CRLM cases are indicated for resection at the time of diagnosis(initially resectable),and the remaining cases are treated as unresectable(initially unresectable).Thanks to recent remarkable developments in chemotherapy,interventional radiology,and surgical techniques,the resectability of CRLM is ***,some metastases are technically resectable but oncologically questionable for upfront *** pancreatic cancer,such cases are categorized as"borderline resectable",and their definition and treatment strategies are ***,in CRLM,although various poor prognosis factors have been identified in previous reports,no clear definition or treatment strategy for borderline resectable has yet been *** the efficacy of hepatectomy for CRLM was reported in the 1970 s,multidisciplinary treatment for unresectable cases has improved resectability and prognosis,and clarifying the definition and treatment strategy of borderline resectable CRLM should yield further improvement in *** review outlines the present status and the future perspective for borderline resectable CRLM,based on previous studies.

The minimal invasiveness of endoscopic submucosal dissection(ESD) prompted us to apply this technique to large-size early esophageal squamous cell carcinoma and Barrett's adenocarcinoma, despite the limitationsin the study population and surveillance duration. A post-ESD ulceration of greater than three-fourths of esophageal circumference was advocated as an important risk factor for refractory strictures that require several sessions of dilation therapy. Most of the preoperative conditions are asymptomatic, but dilatation treatment for dysphagia associated with the stricture has potential risks of severe complications and a worsening of quality of life. Possible mechanisms of dysphasia were demonstrated based on dysmotility and pathological abnormalities at the site:(1) delayed mucosal healing;(2) severe inflammation and disorganized fibrosis with abundant extracellular matrices in the submucosa; and(3) atrophy in the muscularis proper. However, reports on the administration of anti-scarring agents, preventive dilation therapies, and regenerative medicine demonstrated limited success in stricture prevention, and there were discrepancies in the study designs and protocols of these reports. The development and consequent long-term assessments of new prophylactic technologies on the promotion of wound healing and control of the inflammatory/tumor microenvironment will require collaboration among various research fields because of the limited accuracy of preoperative staging and high-risk of local recurrence.

Adult Congenital Lumbar Kyphosis Requiring Anteroposterior Correction and Fusion: A Case Report with 32-Year Follow-Up

Congenital pure kyphosis due to failure of vertebral body segmentation is a relatively rare entity, and surgical intervention is infrequent compared to that for failure of vertebral body formation [1] [2]. There are very few reports of long-term follow-up of surgical treatment in patients with congenital pure kyphosis, and all the reported cases were diagnosed as failure of formation and had an age at the time of surgery of less than 18 years. It is important for orthopedic surgeons to follow the postoperative course of rare cases over 30 years. Here, we present a surgically treated case with ultra-long term follow-up of a 50-year-old patient with congenital pure kyphosis of the lumbar spine. Imaging of the lumbar spine showed six vertebrae and an unsegmented bar at L3-4 causing a pure kyphosis of 54°. The wedge-shaped block vertebra had 4 pedicles with the neural foramen between the pedicles without concomitant disc space, with compensatory thoracic hypokyphosis and lower lumbar hyperlordosis. One-stage correction and fusion surgery using anterior opening and posterior closing osteotomy was successfully performed. Both clinical and radiographic results were excellent and have been maintained for over 30 years postoperatively. The basic principle in the surgical treatment of adult spinal deformity is to achieve and maintain a good global sagittal balance over time. This case reaffirms the importance of spinopelvic harmony.

A 35-year-old Japanese woman showed the typical fundus appearance of acute hypertensive retinopathy, including multiple cotton-wool spots, retinal hemorrhages, star exudates, and disc edema bilaterally. Her systemic blood pressure was high (246/158 mmHg). Nephrologists diagnosed her with secondary hypertension due to immunoglobulin A nephropathy. After 2 years, all signs of acute retinopathy resolved and only multiple striated or sectorial darkening of the fundus (retinal nerve fiber layer defects [NFLD]) remained. Computerized analysis of the macular thickness measurement by optical coherence tomography alerted the risk of glaucoma. NFLD is a hallmark of early glaucoma that is more reliable than IOP elevations since normal tension glaucoma is popular in Japan. To differentiate this case from true glaucoma, information regarding her past history is critical for her future ophthalmologists.

Gastric cancer(GC) develops as a result of inflammationassociated carcinogenesis due to Helicobacter pylori(H. pylori) infection and subsequent defects in genetic/epigenetic events. Although the indication for eradication therapy has become widespread, clinical studies have revealed its limited effects in decreasing the incidence of GC. Moreover, research on biopsy specimens obtained by conventional endoscopy has demonstrated the feasibility of the restoration of some genetic/epigenetic alterations in the gastric mucosa. Practically, the number of sporadic cases of primary/metachronous GC that emerge after successful eradication has increased, while on-going guidelines recommend eradication therapy for patients with chronic gastritis and those with background mucosa after endoscopic resection for GC. Accordingly, regular surveillance of numerous individuals who have received eradication therapy is recommended despite the lack of biomarkers. Recently, the focus has been on functional reversibility after successful eradication as another cue to elucidate the mechanisms of restoration as well as those of carcinogenesis in the gastric mucosa after H. pylori eradication. We demonstrated that Congo-red chromoendoscopy enabled the identification of the multifocal distribution of functionally irreversible mucosa compared with that of restored mucosa after successful eradication in individuals at extremely high risk for GC. Further research that uses functional imaging may provide new insights into the mechanisms of regeneration and carcinogenesis in the gastric mucosa post-eradication and may allow for the development of useful biomarkers.

Properties of Creeping Streamer Progressed in Dielectric Barrier with Narrow Gap in PFAE Oil

The behaviors of creeping streamer progressed in the narrow gap with the range of 0.1 mm to 0.2 mm between the pressboard mounted a BSE （back side electrode） and the acrylic resin plate were carried out in PFAE （palm fatty acid ester） oil under variable impulse voltage up to -4-140 kVpeak with ＋1.2/50 μs and ±1.2/1,000μs waveforms. The growth of positive and negative streamers depends on the gap space between two solid dielectrics. And the distinctive polarities of impulse voltage affect in the streamer length and flashover voltage between needle and counter electrodes as well. By decreasing the gap space, the flashover voltage largely increased while the mean velocity of positive streamer significantly decreased under the both polarities of impulse voltage. These results have been compared to commercial mineral oil （transformer oil： JIS-C2320）. It is shown that the behaviors of creeping discharge in both oils elegantly reveal the distinctive phenomena.

An estimate of the conversion rates of SO_2 to SO_4^(2-)- and NO_2 to HNO_(3+) NO_3^- for the evaluation air pollution in Beijing

The conversion rates of SO2 to SO42- and NO2 to HNO3+NO3- are estimated from the field-data obtained in Beijing in summer, 1988. The results show that the conversion rate of NO2 is about four times as much as that of SO2; The conversion rates have a diurnal variation in a day. On the average, the rate of SO2 is estimated to be 4.7% h-1 during the daytime and 3.4% h-1 during the nighttime. Similarly, the rate of NO2 is estimated to be 17.2% h-1 and 12% h-1 respectively.

Evaluation of the Bending Loss of the Hollow Optical Fiber for Application of the Carbon Dioxide Laser to Endoscopic Therapy

Since carbon dioxide laser is excellent for incision, hemostasis, coagulation, and vaporization of soft tissues, it has been widely applied in clinical treatments as the laser knife. In these days, flexible thin hollow optical fibers transmitting mid-infrared light have been developed, and the application of carbon dioxide laser to endoscopic therapy has become possible. However, it is expected that the irradiation effect is influenced by the change in the laser power at the tip of the hollow optical fiber due to the change in the transmittance by the bending loss. The purpose of this research is to quantitatively evaluate the change in the output power and therapeutic effect by bending the hollow optical fiber in a gastrointestinal endoscope. The change in the transmittance of the hollow optical fiber due to the insertion of the fiber into the endoscope and bending of the head of the endoscope was measured. Then, the relationship between the irradiated laser power and the incision depth for a porcine stomach was investigated. As the results, the most significant decrease in the transmittance of the hollow optical fiber was caused by the insertion of the fiber into the instrument channel of the endoscope, and bending of the head of the endoscope with the angle of 90° decreased the output laser power and incision depth by 10% and 25%, respectively. Therefore, it was confirmed that the bending loss of the hollow optical fiber due to the bending of the head of the endoscope had no significant influence on the endoscopic therapy using the carbon dioxide laser.

Introduction of cover crops may improve the diversity of arbuscular mycorrhizal fungi (AMF) in roots and soil under crop rotational systems;therefore, it is necessary to determine the potential for AMF communities to improve sustainable food production. We investigated the impact of cover crops, including wheat (Triticum aestivum L.), barley (Hordeum vulgare L.), pea (Pisum sativum L.), and hairy vetch (Vicia villosa Roth.), on the AMF communities in their roots in autumn and spring sowing seasons with PCR-DGGE analysis. Although all four cover crops impacted the AMF community structure in roots, the diversity of AMF communities was unchanged among crop type or sowing season. Redundancy analysis (RDA) demonstrated that AMF communities within crop type were significantly different. However, the AMF community structures were not influenced by growing season, suggesting that growth stage in crops may be more responsive to shaping AMF community structure in crop roots than host crop identity.
