Post-colonoscopy colorectal cancer rate in the era of highdefinition colonoscopy

AIM To investigate the post-colonoscopy colorectal cancer(PCCRC) rate for high-definition(HD) colonoscopy compared with that for standard-definition colonoscopy reported *** Using medical records at Sano Hospital(SH) and Dokkyo Medical University Koshigaya Hospital(DMUKH), we retrospectively obtained data on consecutive patients diagnosed as having CRC between January 2010 andDecember 2015. The definition of PCCRC was diagnosis of CRC between 7 and 36 mo after initial high-definition colonoscopy that had detected no cancer, and patients were divided into a PCCRC group and a non-PCCRC group. The primary outcome was the rate of PCCRC for HD colonoscopy. The secondary outcomes were factors associated with PCCRC and possible reason for occurrence of early and advanced *** Among 892 CRC patients, 11 were diagnosed as having PCCRC and 881 had non-PCCRC. The PCCRC rate was 1.7%(8/471) at SH and 0.7%(3/421) at DMUKH. In comparison with the non-PCCRC group, the PCCRC group had a significantly higher preponderance of smaller tumors(39 mm vs 19 mm, P = 0.002), a shallower invasion depth(T1 rate, 25.4% vs 63.6%, P = 0.01), a non-polypoid macroscopic appearance(39.0% vs 85.7%, P = 0.02) and an earlier stage(59.7% vs 90.9%, P = 0.03). Possible reasons for PCCRC were "missed or new" in 9 patients(82%), "incomplete resection" in 1(9%), and "inadequate examination'" in 1(9%). Among 9 "missed or new" PCCRC, the leading cause was non-polypoid shape for early PCCRC and blinded location for advanced *** The PCCRC rate for HD colonoscopy was 0.7%-1.7%, being lower than that for standard-definition colonoscopy(1.8%-9.0%) reported previously employing the same methodology.

Many studies have argued the importance of the flower-pollinator size matching for flower trait diversification. However, non-pollinator agents may also influence flower traits. In this study, we investigated the altitudinal variation of flower size (spur length, stamen length, petal blade length, corolla diameter, and sepal length) and flower visitors of Aquilegia buergeriana var. buergeriana (long-spurred columbine) in central Japan. Although this species was primarily visited by Bombus consobrinus, which has an unvarying mouthpart length, flower sizes varied greatly among populations with no correlation with altitude. These results suggest that non-pollinator and non-meteorological agents, directly and/or indirectly, impose selection pressure on flower size in A. buergeriana.

Geographic Variation in Flower Size and Flower-Visitor Composition of Two Bumblebee-Pollinated, Spring-Flowering Herbs, Lamium albumL. var. barbatum(Lamiaceae) and Meehania urticifolia(Lamiaceae)

Geographic variation of flower size is potentially affected by various biotic and abiotic factors such as pollinator size, abundance of nectar robbers, and temperature. To elucidate the effect of geographic variation of flower-visitor fauna on flower size, we investigated two spring-flowering, bumblebee-pollinated herbs, Lamium album L. var. barbatum (seven populations) and Meehania urticifolia (five populations), in central Japan. Flower-size variations were not correlated with altitude (an index of abiotic environmental change) in either species, but in both species they were related to the local species composition of flower visitors. In L. album, flowers were bigger in populations visited by long-tongued Bombus consobrinus and B. diversus queens, whereas they were smaller in populations visited by short-tongued bees such as Apis mellifera. Flower size did not vary much among populations of M. urticifolia visited by long-tongued bumblebees, but it was uniquely small in a population whose flowers were predominantly visited by the nectar-robbing bee Megachile japonica. In conclusion, both the size and nectar-robbing behavior of bees affect local flower-size adaptation in these two long-tubed, spring-flowering herbs.

Pharmacological Evidence for the Involvement of Calcium Entry through TRPV1 Channels in Nifedipine-Induced [Ca2+]i Elevation in Gingival Fibroblasts

Background: Among anti-hypertension drugs, calcium (Ca2+) antagonists cause gingival overgrowth as a side effect. We previously discovered that this side effect was due to elevation of the calcium concentration in the cytosol ([Ca2+]i). Ca2+ entry through non-selective cation channels (NSCCs) and Ca2+ release from intracellular Ca2+ stores are involved in this [Ca2+]i elevation. Furthermore, we discovered that calcium-sensing receptors (CaSRs) participate in nifedipine-induced [Ca2+]i elevation. Transient receptor potential (TRP) channels have been identified as NSCCs. In the present study, we undertook experiments to determine if TRPV1 channels are present in gingival fibroblasts and to ascertain if nifedipine-activated NSCCs are TRPV1 channels. Methods Normal human gingival fibroblast Gin-1 cells were used. The [Ca2+]i was measured using a video-imaging analysis system with the Ca2+-sensitive fluorescent dye fura-2/AM. Results: The NSCC inhibitor SKF96365 significantly inhibited nifedipine-induced [Ca2+]i elevation. TRPV1 channel agonists such as capsaicin, olvanil and resiniferatoxin concentration-dependently elevated the [Ca2+]i. The TRPV1 channel activator anandamide concentration-dependently increased the [Ca2+]i. The TRPV1 channel antagonists capsazepine, AMG9810, iodoresiniferatoxin, ruthenium red, and SB366791 significantly inhibited nifedipine-induced [Ca2+]i elevation. Conclusion: These results suggest that Ca2+ entry through TRPV1 channels is involved in the nifedipine-induced [Ca2+]i elevation seen in gingival fibroblasts. We describe here a modified version of our ‘calcium trigger theory’.

Elevational Floral Size Variation in Prunella vulgaris

Floral size is potentially influenced by local pollinators’ body size. As pollinator fauna and size often change with elevation, correlation between elevational variation of pollinator size and floral size is expected. We investigated the variation of floral size in Prunella vulgaris L. (Laminaceae) and the variation of their pollinator fauna along an elevational gradient. We measured the floral size of four traits: corolla length (CL), corolla tube length (CTL), corolla width (CW), and calyx length (CAL), in 23 populations, and found that CL and CTL were negatively correlated with elevation, and CW and CAL were not. Six bumblebee species visited the flower, and the visiting bee fauna differed among populations;the smallest and the largest bumblebee species visited the high elevational range (above 1800 m a. s. l.) populations, and the largest and the second largest bumblebee species visited the middle elevational range (1400 - 1800 m a. s. l.). Although abiotic factors can potentially affect floral size, the fact that we do not find an elevational decrease in CW and CAL suggests that the elevational change in P. vulgaris’s CL and CTL reflects the local pollinator size.

We synthesized dammarane-type triterpene derivatives and evaluated their ability to inhibit HIV-1 and HCV proteases to understand their structure-activity *** of the mono- and disuccinyl derivatives(5a-5f) were powerful inhibitors of HIV-1 protease(IC1000 lM).These findings indicated that the mono-succinyl dammarane type derivatives(5a- 5d) selectively inhibited HIV-1 protease and that the di-succinyl(5e,5f) as well as 2,3-seco-2,3-dioic acid(3b) derivatives preferably inhibited both viral proteases.

Production of a High-Mach-Number Plasma Flow for an Advanced Plasma Space Thruster

A higher specific impulse and a larger thrust are required for a mannedinterplanetary space thruster. Prior to a realization of a fusion-plasma thruster, amagneto-plasma-dynamic arcjet (MPDA) powered by a fission reactor is one of the promising candidatesfor a manned Mars space thruster. The MPDA plasma is accelerated axially by a self-induced j x Bforce. Thrust performance of the MPDA is expected to increase by applying a magnetic nozzle insteadof a solid nozzle. In order to get a much higher thruster performance, two methods have beeninvestigated in the HITOP device, Tohoku University. One is to use a magnetic Laval nozzle in thevicinity of the MPDA muzzle for converting the high ion thermal energy to the axial flow energy. Theother is to heat ions by use of an ICRF antenna in the divergent magnetic nozzle. It is found thatby use of a small-sized Laval-type magnetic nozzle, the subsonic flow near the muzzle is convertedto be supersonic through the magnetic Laval nozzle. A fast-flowing plasma is successfully heated byuse of an ICRF antenna in the magnetic beach configuration.
