Angle Effect to Dropping of Dew in Closed Suction Catheters

Intratracheal aspiration in a patient on ventilation is generally performed using a catheter. Of late, closed suction systems have been used more frequently than open systems. To remove tracheal secretions through the intratracheal tube, catheter suction is used, and the suction catheter may be of the closed- or open-type. The catheters are cleaned by flushing with sterile 0.9% saline, resulting in dropping of dew. This phenomenon is caused by some factors: influence of flow, clogging of the suction tube, problem of manipulation of flushing and angle between the intubation tube and the connection port. But this dropping has not yet been investigated. In this study, we focused at this angle and we used the test lung which is simulated machine on behalf of patient. We consider two situations adult and child, also two types of catheters Eco-Cath and Ty-care. In child case we consider elbow-type suction catheters and Y-connector type. The angle was set at 0°, 20°, 40°, 60°, and 90°, and we observed the difference between Eco-Cath and Ty-care. In adults difference is significant (p 0.001), In 90°between Tracheal intubation and Tracheotomy (p ≈ 0.26). In child difference is significant (p 0.001), In 90°between Tracheal intubation and Tracheotomy (p ≈ 0.15). In child case we observed dependency of angle on various conditions too. The relation of the angle and dew was clarified.

Inhibitory effect and mechanism of acarbose combined with gymnemic acid on maltose absorption in rat intestine

AIM: To compare the combinative and individual effect of acarbose and gymnemic acid (GA) on maltose absorption and hydrolysis in small intestine to determine whether nutrient control in diabetic care can be improved by combination of them. METHODS: The absorption and hydrolysis of maltose were studied by cyclic perfusion of intestinal loops in situ and motility of the intestine was recorded with the intestinal ring in vitro using Wistar rats. RESULTS: The total inhibitory rate of maltose absorption was improved by the combination of GA (0.1g/L-1.0 g/L) and acarbose (0.1 mmol/L-2.0 mmol/L) throughout their effective duration (P <0.05, U test of Mann-Whitney), although the improvement only could be seen at a low dosage during the first hour. With the combination, inhibitory duration of acarbose on maltose absorption was prolonged to 3h and the inhibitory effect onset of GA was fastened to 15 min. GA suppressed the intestinal mobility with a good correlation (r = 0.98) to the inhibitory effect of GA on maltose absorption and the inhibitory effect of 2 mmol/L (high dose) acarbose on maltose hydrolysis was dual modulated by 1g/L GA in vivo indicating that the combined effects involved the functional alteration of intestinal barriers. CONCLUSION: There are augmented effects of acarbose and GA,which involve pre-cellular and paracellular barriers. Diabetic care can be improved by employing the combination.

Network-Based Predictions and Simulations by Biological State Space Models: Search for Drug Mode of Action

Since time-course microarrav data are short but contain a large number of genes, most of statistical models should be extended so that they can handle such statistically irregular situations. We introduce biological state space models that are established as suitable computational models for constructing gene networks from microarray gene expression data. This chapter elucidates theory and methodology of our biological state space models together with some representative analyses including discovery of drug mode of action. Through the applications we show the whole strategy of biological state space model analysis involving experimental design of time-course data, model building and analysis of the estimated networks.

An optimal design problem of local buffer allocation in the FMS is discussed in order to maximize a reward earned from processed jobs at all workstations. Structural properties of the optimal design problem are analyzed for the model with two job routing policies. Based on these properties, approaches to optimal solutions are given.

Inhibitory effect of voglibose and gymnemic acid on maltose absorption in vivo

AIM: To determine whether diabetic care can be improved by combination of voglibose and gymnemic acid (GA), we compared the combinative and individual effects of voglibose and GA on maltose absorption in small intestine. METHODS: The small intestine 30 cm long from 2 cm caudal ward Treitz's ligament of Wistar rat was used as an in situ loop, which was randomly perfused in recircular mode with maltose (10mmol/L) with or without different dosages of voglibose and/or GA for an hour. To compare the time course, perfusion of 10 mmol/L maltose was repeated four times. Each time continued for 1 hour and separated by 30 minutes rinse. In the first time, lower dosages of GA (0.5g/L) and/or voglibose (2 micromol/L) were contained except control. RESULTS: Absorptive rate of maltose was the lowest in combinative group (P<0.05, ANOVA), for example, the inhibition rate was about 37% during the first hour when 0.5 g/L-GA and 2 micromol/L voglibose with 10 mmol/L maltose were perfused in the loop. The onset time was shortened to 30 minutes and the effective duration was prolonged to 4 hours with the combination; therefore the total amount of maltose absorption during the effective duration was inhibited more significantly than that in the individual administration (P < 0.05, U test of Mann Whitney). The effect of GA on absorptive barriers of the intestine played an important role in the combinative effects. CONCLUSION: There are augmented effects of voglibose and GA. The management of diabetes mellitus can be improved by employing the combination.

Enzyme inhibition assay for pyruvate dehydrogenase complex: Clinical utility for the diagnosis of primary biliary cirrhosis

Primary biliary cirrhosis (PBC) is usually diagnosed by the presence of characteristic histopathological features of the liver and/or antimitochondrial antibodies (AMA) in the serum traditionally detected by immunofluorescence. Recently, new and more accurate serological assays for the detection of AMA, such as enzyme-linked immunosorbent assay (ELISA), immunoblotting, and enzyme inhibition assay, have been developed. Of these, the enzyme inhibition assay for the detection of anti-pyruvate dehydrogenase complex (PDC) antibodies offers certain advantages such as objectivity, rapidity, simplicity, and low cost. Since this assay has almost 100% specificity, it may have particular applicability in screening the at-risk segment of the population in developing countries. Moreover, this assay could be also used for monitoring the disease course in PBC. Almost all sera of PBC-suspected patients can be confirmed for PBC or non-PBC by the combination results of immunoblotting and enzyme inhibition assay without histopathological examination. For the development of a "complete" or "gold standard" diagnostic assay for PBC, similar assays of the enzyme inhibition for anti-2-oxoglutarate dehydrogenase complex (OGDC) and anti-branched chain oxo-acid dehydrogenase complex (BCOADC) antibodies will be needed in future.

Discrete Circular Distributions with Applications to Shared Orthologs of Paired Circular Genomes

For structural comparisons of paired prokaryotic genomes,an important topic in synthetic and evolutionary biology,the locations of shared orthologous genes(henceforth orthologs)are observed as binned *** and other data,e.g.,wind directions recorded at monitoring sites and intensive care unit arrival times on the 24-hour clock,are counted in binned circular arcs,thus modeling them by discrete circular distributions(DCDs)is *** propose a novel method to construct a DCD from a base continuous circular distribution(CCD).The probability mass function is defined to take the normalized values of the probability density function at some pre-fixed equidistant points on the *** families of constructed DCDs which have normalizing constants in closed form are *** studies show that DCDs outperform the corresponding CCDs in modeling grouped(discrete)circular data,and minimum chi-square estimation outperforms maximum likelihood estimation for *** apply the constructed DCDs,invariant wrapped Poisson and wrapped discrete skew Laplace to compare the structures of paired bacterial ***,discrete four-parameter wrapped Cauchy(nonnegative trigonometric sums)distribution models multi-modal shared orthologs in Clostridium(Sulfolobus)better than the others considered,in terms of AIC and Freedman’s goodness-of-fit *** result that different DCDs fit the shared orthologs is consistent with the fact they belong to two ***,these prokaryotes have a common favored site around 70°on the unit circle;this finding is important for building synthetic prokaryotic genomes in synthetic *** DCDs can also be applied to other binned circular data.

Objective:Filaggrin(FLG)is a protein expressed in the epidermis and involved in the maintenance of the epidermal ***,the expression and localization of FLG in the upper airway remain *** present study aimed to determine the significance of FLG and the effect of S100A7 on FLG expression in the upper respiratory ***:Human nasal epithelial cells(HNECs)were cultured and examined for FLG expression and S100A7 effects by real-time polymerase chain reaction and Western *** localization and distribution of FLG were assessed using sinonasal ***:A significant expression of FLG was detected at the mRNA and protein levels in HNECs.A moderate FLG immunoreactivity was observed in the epithelial cells,but no staining was seen in epithelial goblet cells.S100A7 increased the FLG mRNA level in HNECs in a dose-dependent manner and also up-regulated the FLG protein in a dose-dependent ***:This study significantly contributes to a better understanding of the role of FLG in the pathogenesis of airway inflammation from the viewpoint of the epithelial barrier ***-related events in response to S100A7 protein may represent novel therapeutic targets for the treatment of upper airway inflammation.

Cisplatin-induced activation of TGF-βsignaling contributes to drug resistance
