Th. tetrameth.lammonium salts of diplatinum(II) complexes composed of mono-lacunary Keggin-type silico and germanotungstates, [(Ch.sub>3)4N]4[α-SiW11O39{cis-Pt(Nh.sub>3)2}2]·13h.sub>2O (TMA-Si-Pt) and [(Ch.sub>3)4N]4[α-GeW11O39{cis-Pt(Nh.sub>3)2}2]·11h.sub>2O (TMA-Ge-Pt), were synth.sized and crystallized. Single crystals of a tetrameth.lammonium salt of Keggin-type diplatinum(II)-coordinated ph.sph.tungstate [(Ch.sub>3)4N]3[α-PW11O39{cis-Pt(Nh.sub>3)2}2]×10h.sub>2O (TMA-P-Pt) were also obtained. Th. X-ray structural analyses revealed th.t th. two cis-platinum(II) moieties, [cis-Pt(Nh.sub>3)2]2+, were each.coordinated to two oxygen atoms in a mono-vacant site of [XW11O39](12-n)- (Xn+ = Si4+, Ge4+, P5+). FTIR spectra of TMA-Si-Pt and TMA-Ge-Pt also suggested th.t th. two platinum(II) moieties were coordinated to th. vacant site of [SiW11O39]8- and [GeW11O39]8-. Th. 1h.NMR spectra in DMSO-d6 of TMA-Si-Pt and TMA-Ge-Pt sh.wed signals assigned to th. two sets of Nh.sub>3 ligands coordinated to th. platinum sites. Th.se th.ee platinum compounds sh.wed h.drogen evolution from aqueous trieth.nolamine solution under visible ligh. irradiation (λ ≥ 400 nm) in th. presence of eosin Y, α-Keggin mono-aluminum-substituted silicotungstate, and titanium dioxide. Th. catalytic activities were influenced by th. central atoms, and TMA-P-Pt sh.wed th. h.gh.st activities among th. th.ee platinum compounds.

Objective： Th. ideal medication for th. treatment of acid-related diseases, e.g., peptic ulcers, stress- related gastric bleeding, functional dyspepsia, and gastroesoph.geal reflux disease, sh.uld h.ve a rapid onset of action to promote h.mostasis and relieve th. symptoms. Th. aim of our study was to investigate th. inh.bitory effects on gastric acid secretion of a single oral administration of a proton pump inh.bitor, omeprazole 20 mg, and an h.-receptor antagonist, roxatidine 75 mg. Meth.ds： Ten h.ficobacterpylori-negative male subjects participated in th.s randomized, two-way crossover study. Intragastric ph.was monitored continuously for 6 h.after single oral admini- stration of omeprazole 20 mg and roxatidine 75 mg. Each.administration was separated by a 7-d wash.ut period. Results： During th. 6-h.study period, th. average ph.after administration of roxatidine was h.gh.r th.n th.t after administration of omeprazole （median： 4.45 vs. 2.65; P=0.0367）. Also during th. 6-h.study period, a longer duration of maintenance at ph.above 2, 5, and 6 was observed after administration of roxatidine 75 mg th.n after administration of omeprazole 20 mg （median： 90.6% vs. 55.2%, P=-0.0284; 43.7% vs. 10.6%, P=0.0125; 40.3% vs. 3.3%, P=0.0125; respectively）. Conclusions： In h.licobacter pylori-negative h.alth. male subjects, oral administration of roxatidine 75 mg increased th. intragastric ph.more rapidly th.n th.t of omeprazole 20 mg.

Th. paper includes discussions on th. important role of neutron and alph. configurations in proton-rich.nuclei in nuclear astroph.sics in terms of nucleosynth.sis under extremely h.gh.temperature h.drogenburning conditions. Th. νp-process, wh.ch.is supposed to take place at th. very early epoch.of type II supernovae, h.s considerable neutrons and alph.s togeth.r with.protons. Th. alph.-induced reactions on proton-rich.unstable nuclei in th. ligh. mass regions is expected to play a crucial role, but very few of th.m were investigated well yet because of th. experimental difficulties. Specifically, I report our recent experimental effort for th. breakout process from th. pp-ch.in region,~7Be(α, γ)^(11)C(α,p)^(14)N under th. νp-process. Th. neutron-induced reactions on proton-rich.nuclei, wh.ch.is even more a ch.llenging subject, were investigated previously for very few nuclei. One possible experimental meth.d is th. Trojan h.rse Meth.d(Th.). We successfully h.ve applied Th. to th. ^(18)F(n,α)^(14)N reaction study with.an unstable beam of ^(18)F.

h.licobacter pylori (h. pylori) can infect into th. epith.lial cell to cause benign or malignant disorders. Under stressful environment, a spiral form of h. pylori is transformed into a coccoid form. Th. infectivity of th. coccoid form is still controversial. Since spiral forms are transformed into two types of coccoid forms via different mech.nisms, th. infectivity of th. two types of coccoid forms into h.man gastric epith.lial cell was examined. A laboratory and a clinical strain of h. pyloriv were cultured in liquid medium under different conditions to produce th. two types of coccoid forms. Th.se coccoid h. pylorisv were th.n co-cultured with.h.man derived gastric epith.lial cell, MKN-28. Adh.sion and penetration of bacteria into MKN-28 cells were monitored by scanning-, standard transmission- and immunotransmission-electron microscopy (SEM, TEM and ITEM). We observed th.t both.coccoid forms were able to adh.re onto th. surface of MKN-28 cells in agminated formation and also penetrated into th. gastric epith.lial cells besides th. spiral form of h. pyloriv. Coccoid h. pylori is not a passive entity but can actively infect th. h.man gastric epith.lial cell.

Seventy-five gram glucose tolerance test to assess carboh.drate malabsorption and small bowel bacterial overgrowth

AIM： To investigate non-invasively th. incidence of absorption of carboh.drates in diabetic patients during an oral glucose tolerance test （OGTT） and to determine wh.th.r malabsorption may be associated with.insulin secretion and insulin resistance. METh.DS： A standard 75-g OGTT was performed in 82 diabetic patients. Th. patients received 75 g of anh.drous glucose in 225 mL of water after an overnigh. fasting and breath.samples were collected at baseline and up to 120 rain after ingestion. Breath.h.drogen and meth.ne concentrations were measured. Blood glucose and serum insulin concentrations were measured before ingestion and at 30, 60, 90, 120 rain post-ingestion. RESULTS： Wh.n carboh.drate malabsorption was defined as subjects with.an increase of at least 10 ppm （parts per million） in h.drogen or meth.ne excretion with.n a 2-h.period, 28 （34%） h.d carboh.drate malabsorption. According to th. result of increased breath.test, 21 （75%） patients were classified as small bowel bacterial overgrowth.and 7 （25%） as glucose malabsorption. Patients with.carboh.drate malabsorption were older and h.d poor glycemic control as compared with.th.se with.ut carboh.drate malabsorption. Th. h.MA value, th. sum of serum insulin during th. test and th. AinsulinlAglucose ratio were greater in patients with.carboh.drate malabsorption. CONCLUSION： Insulin resistance may be overestimated by using th.se markers if th. patient h.s carboh.drate malabsorption, or th.t carboh.drate malabsorption may be present prior to th. development of insulin resistance. h.nce carboh.drate malabsorption sh.uld be taken into account for estimating insulin resistance and β-cell function.

h.drogen and meth.ne gases are frequently detected in th. stomach

AIM： To investigate th. incidence of bacterial overgrowth.in th. stomach.by using a new endoscopic meth.d in wh.ch.intragastric h.drogen and meth.ne gases are collected and analyzed. METh.DS： Studies were performed in 490 consecutive patients undergoing esoph.gogastroscopy, At endoscopy, we intubated th. stomach.with.ut inflation by air, and 20 mL of intragastric gas was collected th.ough.th. biopsy ch.nnel using a 30 mL syringe, Intragastric h.drogen and meth.ne concentrations were immediately measured by gasch.omatograph., h.pylori infection was also determined by serology. RESULTS： Most of intragastric h.drogen and meth.ne levels were less th.n 15 ppm （parts per million）. Th. median h.drogen and meth.ne values （interquartile range） were 3 （1-8） ppm and 2 （1-5） ppm, respectively. Th. h.gh.h.drogen and meth.ne levels for indication of fermentation were decided if th. patient h.d th. values more th.n 90 percentile range in each.sample. Wh.n a patient h.d a h.gh.level of h.drogen or meth.ne in one or more samples, th. patient was considered to h.ve fermentation. Th. overall incidence of intragastric fermentation was 15.4% （73/473）, Intragastric meth.ne levels were h.gh.r in th. postoperative group th.n in oth.r groups. None of th. mean h.drogen or meth.ne values was related to h.ylori infection. CONCLUSION： h.drogen and meth.ne gases are more frequently detected in th. stomach.th.n expected, regardless of th. presence of abdominal symptoms. Previous gastric surgery influences on th. growth.of meth.neproducing bacteria in th. fasting stomach.

Reduced endogenous insulin secretion in diabetic patients with.low-titer positive antibodies against GAD

Aim: To investigate th. clinical ch.racteristics of diabetic patients with.a low-titer positive for th. anti-glutamic acid decarboxylase 65 antibody (GAD antibody). Meth.ds: Th. subjects were 420 diabetic inpatients. Th. endogenous insulin secretion was estimated on th. basis of th. C-peptide immunoreactivity from a 24 h.urine collection (uCPR). Clinical variables were compared between patients negative for th. GAD antibody (GAD antibody titer h.gh.titer positive GAD antibody (10 U/mL ≤ GAD antibody titer). Results: Th. low and h.gh.titer positive GAD antibodies were found in 25 and 10 patients, respectively. Th. uCPR was significantly lower in both.th. patients with.a low (37 ± 33 ug/24h. and h.gh.titer (39 ± 27 ug/24h. positive GAD antibodies th.n in th.se negative for GAD antibodies (71 ± 52 ug/24h.. Th. uCPR level was significantly lower in th. low-titer positive GAD antibody group (29 ± 22 ug/24h. th.n in th. negative group (67 ± 55 ug/24h. among th. patients not taking insulin secretagogues. Th. difference disappeared in th. subjects taking insulin secreagogues. In th. stepwise multiple regression analysis, a low-titer positive GAD antibody was independently associated with.th. uCPR level. Conclusions: Endogenous insulin secretion is reduced in diabetic patients positive for GAD antibodies, even if th. titer is low. Earlier initiation of insulin th.rapy migh. th.refore protect th. pancreatic β-cell function in diabetic patients with.a low-titer positive GAD antibody.
