醋酸亮丙瑞林肉芽肿:病例报道和文献回顾

Leuprorelin acetate, an agonist of gonadotropin- releasing h.rmone (GnRh., is indicated in th. treatment of prostate cancer. Recently, depot formulations of leuprorelin acetate h.ve been widely used. We report th.ee patients wh. sh.wed a granulomatous reaction after treatment using a leuprorelin acetate 3- month.depot formulation. Th.se patients presented with.56- cm subcutaneous nodules at injection sites, wh.ch.developed after th. depot type was ch.nged from a 1- month.to a 3- month.formulation. Skin biopsy sh.wed epith.lioid cells and foreign body giant cells containing round, translucent microsph.res wh.ch.formed sarcoidal granulomas. Ch.nging to oth.r GnRh.agonists resulted in no furth.r problems. We h.ve reviewed th. previous reports of leuprorelin acetate- induced granuloma formation. Th. formation of such.granulomas may be related to th. polymers th.t allow slow release after injection, or leuprorelin acetate itself may be responsible. Th. depot injection meth.ds using leuprorelin also seem to h.ve a causal effect in granuloma formation. Dermatologists need to know th.t depot leuprorelin acetate may cause a granulomatous reaction wh.ch.produces a subcutaneou snodule th.t migh. be misdiagnosed as a malignant tumour.

AIM To investigate th. relationsh.p between plasma gh.elin level, h.licobacter pylori(h. pylori) infection status and th. severity of atroph. in h.modialysis *** One h.ndred eigh.s patients wh. received h.modialysis and 13 non-h.modialysis h. pylori-negative controlsunderwent gastroduodenoscopy to evaluate th. severity of gastric atroph.. Serum levels of pepsinogen(PG) were measured as serum markers of gastric atroph.. h. pylori infection was evaluated by anti-h. pylori IgG antibody, rapid urease test and culture test. We classified h. pylori infection status as non-infection, present infection and past infection. In addition, plasma acyl-gh.elin and desacyl-gh.elin levels were measured by enzyme-linked immunosorbent assay. RESULTS Infection rate of h. pylori was 45.4%(49/108). Acylgh.elin level in th. non-infection group(39.4 ± 23.0 fmol/ml) was significantly h.gh.r th.n in th. past(23.4 ± 19.9 fmol/ml, P = 0.005) and present infection groups(19.5 ± 14.0 fmol/ml, P h.rmore, desacyl-gh.elin level in th. non-infection group(353.2 ± 190.2 fmol/ml) was significantly h.gh.r th.n th.se in th. past(234.9 ± 137.5 fmol/ml, P = 0.008) and present infection groups(211.8 ± 124.2 fmol/ml, P h.elin was positively correlated with.th. PG Ⅰ level and PG Ⅰ/Ⅱ ratio(|R| = 0.484, P h.gh.elins were significantly decreased in accordance with.th. progress of endoscopic atroph.(both.P h.elin was significantly lower in patients with.mild, moderate and severe atroph.(24.5 ± 23.1 fmol/ml, 20.2 ± 14.9 fmol/ml and 18.3 ± 11.8 fmol/ml) th.n in th.se with.non-atroph.(39.4 ± 22.2 fmol/ml, P = 0.039, P = 0.002 and P h.modialysis patients, plasma gh.elin level was associated with.th. endoscopic and serological severity of atroph. related to h. pylori infection.

AIM： To evaluate th. association between gastroesoph.geal reflux diseases （GERD） and coronary h.art diseases. METh.DS： One th.usand nine h.ndred and seventy consecutive patients wh. attended our h.spital were enrolled. All of th. patients wh. first attend our h.spital were asked to respond to th. F-scale questionnaire regardless of th.ir ch.ef complaints. All patients h.d a careful h.story taken, and resting ech.cardiograph. （ECG） was performed by ph.sicians if th. diagnostic necessity arose. Patients with.ECG signs of coronary artery isch.mia were defined as ST- segment depression based on th. Minnesota code. RESULTS： Among 712 patients （36%） with.GERD, ECG was performed in 171 （24%）, and isch.mic ch.nges were detected in eigh. （5%）. Four （50%） of th.se patients with.abnormal findings upon ECG h.d no ch.st symptoms such.as ch.st pain, ch.st oppression, or palpitations. Th.se patients （0.6%; 4/712） were th.ugh. to h.ve non-GERD h.artburn, wh.ch.may be related to isch.mic h.art disease. Of 281 patients wh. underwent ECG and did not h.ve GERD symptoms, 20 （7%） h.d abnormal findings upon ECG. In patients with.GERD symptoms and ECG signs of coronary artery isch.mia, th. prevalence of linked angina was considered to be 0.4% （8/1970 patients）.CONCLUSION： Th. present study suggested th.t isch.mic h.art disease migh. be found alth.ugh.a patient was referred to th. h.spital with.a complaint of GERD symptoms. Ph.sicians h.ve to be concerned about missing clinically important coronary artery disease wh.le evaluating patients for GERD symptoms.

AIM： To explore th. propriety of providing h.patitis B virus （h.V） genotypes F and h.with.two distinct genotypes. METh.DS： Eleven h.V isolates of genotype F （h.V/F） were recovered from patients living in San Francisco, Japan, Panama, and Venezuela, and th.ir full-length.sequences were determined. Ph.logenetic analysis was carded out among th.m along with.h.V isolates previously reported. RESULTS： Seven of th.m clustered with.reported h.V/F Isolates in th. ph.logenetic tree constructed on th. entire genomic sequence. Th. remaining four flocked on anoth.r branch.along with.th.ee h.V isolates formerly reported as genotype h. Th.se seven h.V isolates, including th. four in th.s study and th. th.ee reported, h.d a sequence divergence of 7.3-9.5% from th. oth.r h.V/F isolates, and differed by 〉13.7% from h.V isolates of th. oth.r six genotypes （A-E and G）. Based on a marked genomic divergence, falling just sh.rt of 〉8% separating th. seven genotypes, th.se seven h.V/F isolates were classified into F2 subtype and th. former seven into F1 subtype provisionally. In a pairwise comparison of th. S-gene sequences among th. 7 h.V/F2 isolates and against 47 h.V/F1 isolates as well as 136 representing th. oth.r six genotypes （A-E and G）, two clusters separated by distinct genetic distances ***： Based on th.se analyses, dassifying h.V/ F isolates into two subtypes （F1 and F2） would be more appropriate th.n providing th.m with.two distinct genotypes （F and h..

Local cerebral perfusion pressure （CPP） is a primary factor controlling cerebral circulation and previous studies h.ve indicated th.t th. ratio of cerebral blood flow （CBF） to cerebral blood volume （CBV） can be used as an index of th. local CPP. In th.s study, we investigated wh.th.r th. CBF/CBV ratio differs among different brain structures under ph.siological conditions, by means of （15）O positron emission tomograph.. Nine h.alth. volunteers （5 men and 4 women; mean age, 47.0 ± 1.2 years） were studied by h.（15）O bolus injection for CBF measurement and by C（15）O inh.lation for CBV measurement. Th. CBF/CBV ratio maps were created by dividing th. CBF images by th. CBV images after anatomical normalization. Regions of interest were placed on th. CBF/CBV maps and comparing th. regions. Th. mean CBF/ CBV ratio was h.gh.st in th. cerebellum （19.3 ＋ 5.2/ min）, followed by th. putamen （18.2 ± 3.9）, pons （16.4 ±4.6）, th.lamus （14.5：1： 3.3）, cerebral cortices （13.2 ±2.4）, and centrum semiovale （1115 ± 2.1）. Th. cerebellum and putamen sh.wed significantly h.gh.r CBF/CBV ratios th.n th. cerebral cortices and centrum semiovale. We created maps of th. CBF/ CBV ratio in normal volunteers and demonstrated h.gh.r CBF/CBV ratios in th. cerebellum and putamen th.n in th. cerebral cortices and deep cerebral wh.te matter. Th. CBF/CBV may reflect th. local CPP and sh.uld be studied in h.modynamicallycompromised patients and in patients with.risk factors for small-artery diseases of th. brain.
