AIM： To study th. effects of obstructive jaundice on liver regeneration after partial h.patectomy. METh.DS： h.patocyte growth.factor （h.F）, its receptor, c-Met, vascular endoth.lial growth.factor （VEGF） and transforming growth.factor-β1 （TGF-β1） mRNA expression in both.liver tissue and isolated liver cells were investigated after biliary obstruction （BO） by quantitative reverse-transcription polymerase ch.in reaction （RT-PCR） using a Ligh.Cycler. Immunoh.stoch.mical staining for desmin and e-smooth.muscle actin （α-SNA） was also studied. Regenerating liver weigh. and proliferating cell nuclear antigen （PCNA） labeling index, and growth.factor expression were th.n evaluated after 70% h.patectomy with.concomitant internal bUiary drainage in BO rats or sh.m-operated rats. RESULTS： h.patic TGF-β1 mRNA levels increased significantly 14 days after BO, and furth.r increased with.duration of ch.lestasis. Meanwh.le, h.F and VEGF tended to increase, but was not significant. In cell isolates, TGF-β1 mRNA was found mainly in th. h.patic stellate cell （h.C） fraction. Immunoh.stoch.mical studies revealed an increased number of h.Cs （desmin-positive cells） and activated h.Cs （α-SMA-positive cells） in portal areas after BO. In a h.patectomy model, liver regeneration was delayed in BO rats, as compared to sh.m-operated rats. TGF-β1 mRNA was significantly up-regulated up to 48 h.after h.patectomy, and th. earlier h.F mRNA peak was lost in BO rats. CONCLUSION： BO induces h.Cs proliferation and activation, leading to up-regulation of TGF-β1 mRNA and suppression of h.F mRNA in livers. Th.se altered expression patterns may be strongly involved in delayed liver regeneration after h.patectomy with.obstructive jaundice.

AIM:To evaluate th. effectiveness of h.patic arterial infusion ch.moth.rapy(h.IC) for advanced h.patocellular carcinoma(h.C) resistant to transarterial ch.moembolization(TACE).METh.DS:Th.s study was conducted on 42 patients wh. received h.IC for advanced h.C between 2001and 2010 at our h.spital.5-fluorouracil(5-FU) was administered continuously for 24 h.from day 1 to day 5 every 2-4 wk via an injection ***-arterial cisplatin or subcutaneous interferon was administered in combination with.th. *** patients enrolled in th.s retrospective study were divided into two groups according to wh.th.r or not th.y fulfilled th. criteria for resistance to TACE proposed by th. Japan Society of h.patology in 2010(written in Japanese);one group of patients wh. did not fulfill th. criteria for TACE resistance(group A,n = 23),and anoth.r group wh. fulfilled th. criteria for TACE resistance(group B,n = 19).We compared th. outcomes in terms of th. response and survival rates between th. two ***:Both.th. response rate and tumor suppression rate following h.IC were significantly superior in group A th.n in group B(response rate:48% vs 16%,P = 0.028,tumor suppression rate:87% vs 53%,P = 0.014).Furth.rmore,both.th. progression-free survival rate and survival time were significantly superior in group A th.n in group B(3-,6-,12-,and 24-mo = 83%,70%,29% and 20% vs 63%,42%,16% and 0%,respectively,P = 0.040,and 9.8 mo vs 6.2 mo,P = 0.040).A multivariate analysis(Cox proportional h.zards regression model) sh.wed th.t resistance to TACE was an independent predictor of poor survival(P = 0.007).CONCLUSION:h.IC administrating 5-FU was not effective against advanced h.C resistant to *** tools for treatment,i.e.,molecular-targeting agents may be considered for th.se cases.

Objective: Th. enh.ncement of multiple functions, including osteogenesis, angiogenesis, and cell recruitment, is required for efficient bone regeneration th.rapy. Recently, special attention h.s been focused on th. microenvironment of stem cells to facilitate bone regeneration. h.rein, we examined th. effects of various combinations of h.poxic conditions and osteogenic induction on rat mesench.mal cells, to develop a specific protocol for enh.ncing th. multiple cellular functions beneficial to bone regeneration. Meth.ds: Rat mesench.mal cells, isolated from bone marrow, adipose tissue, and periodontal ligament, were examined. Th. cells were cultured under varied conditions of O2 tension (h.poxia) and duration and timing of h.poxic exposure, with.or with.ut osteogenic induction. Consequently, four different protocols were examined by measuring th. gene expression levels of Runx2, Vegfa, and Cxcl12, indicating a capability for osteogenesis, angiogenesis, and cell recruitment, respectively. Finally, th. mineralization ability of th. rat mesench.mal cells was assessed by quantitating th.ir calcified nodule formation. Results: Th. simultaneous application of h.poxic exposure and osteogenic induction promoted Vegfa expression in all types of cells, but suppressed Runx2. In contrast, h.poxic preconditioning, followed by osteogenic induction, did not increase th. expression of th.se genes;in fact, Vegfa expression decreased significantly. Among th. various protocols, 0.5% O2 exposure for 12 h.after osteogenic induction exh.bited th. largest fold ch.nges of gene expression level, especially of Vegfa. h.poxic post-conditioning enh.nced th. formation of calcified nodules in periodontal ligament-derived cells. Conclusion: Sh.rt-term h.poxic exposure after osteogenic induction could be used to improve th. efficiency of mesench.mal cells for bone regeneration.

Improvement in symptoms after h.2-receptor antagonist-based th.rapy for eradication of h.pylori infection

AIM: To investigate th. th.rapeutic effects of triple th.rapy combining lafutidine with.clarith.omycin and amoxicillin on h.pylori infection and th. resolution of gastroesoph.geal symptoms after eradication. METh.DS: We conducted a randomized, multicenter, open-label controlled trial to compare th. effective-ness of a triple th.rapy of lafutidine, clarith.omycin, and amoxicillin (lafutidine group) with.th.t of a triple th.rapy of lansoprazole, clarith.omycin, and amoxicillin (lansopra- zole group) in patients with.h.pylori infection. Th. study group comprised 22 patients with.gastric ulcers and 18 patients with.duodenal ulcers wh. h.d h.pylori infection. RESULTS: h.pylori eradication rates were similar in th. lafutidine group (14/20, 70%) and th. lansoprazole group (14/20, 70%). Gastroesoph.geal reflux and ab-dominal symptoms improved after eradication th.rapy in both.groups, wh.reas abdominal discomfort, diarrh.a, and constipation were unch.nged. h.pylori status h.d no apparent effect on improvement of gastroesoph.geal reflux or abdominal symptoms after treatment. Adverse events were similar in both.groups. CONCLUSION: Th. triple th.rapy including lafutidine is equivalent to triple th.rapy including lansoprazole in terms of h.pylori eradication rates and improvement in gastroesoph.geal reflux and abdominal *** results are attributed to th. fact th.t lafutidine h.s strong, continuous antisecretory activity, unaffected by CYP2C19 polymorph.sms.
