f.ndamental movement skills training to promote physical activity in children with and without disability： A pilot study

Background： A positive association between f.ndamental movement skills （f.S） and physical activity （PA） has been shown in previous research of.children with and without disability. This pilot study explored a causal mechanism f.r such relationship, and hypothesized that when f.S prof.ciency is improved, enhanced PA uptake will be f.und in children with and without disability. It was f.rther hypothesized that improving f.S prof.ciency will have a greater impact on children with disability than those without disability. Methods： Participants include typically developing （TD） children without disability and children with cerebral palsy （CP）, who were allocated to f.S training groups （CP-f.S n = 12, TD-f.S n = 13） and control groups （CP-C n = 12, TD-C n = 13）. Training groups practiced f.ve f.S （run, jump, kick, throw, catch） in weekly 45-min sessions f.r 4 weeks. Control groups had their regular physiotherapy （CP） or physical education （TD） sessions. f.S were evaluated using process- and product-oriented measures, and PA was measured using accelerometers, bef.re and af.er training. Results： It was verif.ed that training groups gained improvements in f.S while control groups did not. No signif.cant changes in weekday PA were f.und. Increased weekend moderate to vigorous physical activity （MVPA） was f.und in the CP-f.S group, while decreased weekend sedentary time was f.und in the CP-f.S and TD-f.S groups. The percentages of.participants who exceeded the minimum detectable change （MDC9o） in MVPA and sedentary time were larger in children with CP than in children without disability. Conclusion： The f.ndings suggest that improved f.S prof.ciency could potentially contribute to heightened PA and decreased sedentary time during weekends f.r children. Such ef.ect of.improved f.S prof.ciency on PA appears to be greater in those with physical disability than in those without disability. It is recommended that the f.ndings of.this pilot study should be f.rther examined in f.ture

Selective sphincteroplasty of.the papilla in cases at risk due to atypical anatomy

AIM： To analyze the indications, ef.icacy and saf.ty of.sphincteroplasty in our centre.METHODS： A retrospective study of.sphincteroplasty in 53 cases of.papilla at high risk was perf.rmed in 2004-2006. The procedure consisted of.duodenoscopy with Olympus TJf.145 Videoduodenoscope, approach to the biliary tract using a catheter with a guidewire, and dilatation of.the papilla with a dilatation balloon catheter using a syringe with a manometer f.r control of.the f.lling pressure.RESULTS： The indications included intradiverticular papilla in 26 patients （49%）, stenosis of.a previous sphincterotomy in 19 patients （35.8%）, small size of.the papilla in 4 patients （7.5%）, Billroth R gastrectomy in 3 patients （5.6%）, and coagulopathy in one patient （1.9%）. The ef.icacy was 97.8%, with all the calculi extracted f.om the common bile duct in 84.4% of.the patients, even though 21 of.the patients （39.6%） had calculi with a diameter equal to or greater than 10 ram. Seven patients （13.2%） presented complications： haemorrhage in 1 patient （1.9%） and mild pancreatitis in 6 patients （11.3%）. The mean hospital stay in case of.complications was of.3 ± 0.63 d.CONCLUSION： Sphincteroplasty is highly ef.ective, with a compllcation rate similar to that of.sphincterotomy, f.rthermore, the complications are of.low clinical importance. The use of.the 10 mm balloon makes it possible to extract calculi with a diameter of.over 15 mm and to extract more than 3 calculi without increasing the rate of.complications and reduces the need to resort to lithotripsy or rescue sphincterotomy.
