Prediction of performance and evaluation of flexible pavement rehabilitation strategies

Five test sections with different additives and strategies were established to rehabilitate a State-maintained highway more effectively in Rhode Island （RI）： control, calcium chloride, asphalt emulsion, Portland cement and geogrid. Resilient moduli of subgrade soils and subbase materials before and after full depth rehabilitation were employed as input pa- rameters to predict the performance of pavement structures using AASHTOWare Pave- ment ME Design （Pavement ME） software in terms of rutting, cracking and roughness. It was attempted to use Level i input （which includes traffic full spectrum data, climate data and structural layer properties） for Pavement ME. Traffic data was obtained from a Weigh- in-Motion （WIM} instrument and Providence station was used for collecting climatic data. Volumetric properties, dynamic modulus and creep compliance were used as input pa- rameters for 19 mm （0.75 in.} warm mix asphalt （WMA） base and 12.S mm （0.5 in.） WMA surface layer. The results indicated that all test sections observed AC top-down （longitu- dinal） cracking except Portland cement section which passed for all criteria. The order in terms of performance （best to worst） for all test sections by Pavement ME was Portland cement, calcium chloride, control, geogrid, and asphalt emulsion. It was also observed that all test sections passed for both bottom up and top down fatigue cracking by increasing thickness of either of the two top asphalt layers. Test sections with five different base/ subbase materials were evaluated in last two years through visual condition survey and measurements of deflection and roughness to confirm the prediction, but there was no serious distress and roughness. Thus these experiments allowed selecting the best reha- bilitation/reconstruction techniques for the particular and/or similar highway, and a framework was formulated to select an optimal technique and/or strategy for future rehabilitation/reconstruction projects. The role of B-type natriuretic peptide in the evaluation of congestive heart failure patients in emergency department

Aim: Congestive heart failure (CHF) is a severe cardiovascular disorder seen in the Emergency Department (ED). B-type Natriuretic Peptide (BNP) is usually ordered to evaluate the CHF severity. However, it is difficult to interpret serum BNP level when different clinical entities existed. The aim of this study is to illustrate the correlation between serum BNP level and relevant clinical variables and further determine the role of serum BNP in different CHF patients. Methods: Univariate comparisons between 26 clinical variables and serum BNP level were analyzed. In order to avoid confounding factors, potential independent clinical variables were analyzed together using multivariate regression. Results: 529 CHF patients were reviewed and divided into different groups by 26 clinical variables. Serum BNP levels were found statistically significant different by univariate compareson between groups divided by 8 clinical variables that included obesity, diastolic/systolic heart failure (HF), serum blood urea nitrogen (BUN) level, serum creatinine (Cr) level, serum sodium (Na) level, patients taking loop diuretics, history of cerebrovascular accident (CVA), and history of dementia. Among all 8 clinical variables, obesity, serum BUN, Cr level, and diastolic/systolic HF had weak-to-moderate correlation effects with serum BNP level by correlation coefficient analysis. However, only obesity and dia- stolic/systolic HF were two moderately stronger clini- cal variables that can affect the serum BNP levels by multivariate regression. Analyzing CHF patients separately by obesity and diastolic/systolic HF subsets showed longer hospitalization in diastolic HF patients with relatively higher serum Cr level. In addition, poor correlation was found between serum BNP level and length of hospitalization (LOH) as well. Use of a New Algorithm with an Internally Cooled Electrode for Radiofrequency Ablation of Small Hepatocellular Carcinomas

Purpose: To investigate the efficacy of a new algorithm to increase the volume of tissue ablation via gradual ramp-up of power using an internally cooled electrode for ablating hepatomas 3 cm or less. Materials and Methods: 44 patients with 62 hepatomas were treated from March 4, 2004 to May 24, 2009. Ablation with a gradual ramp-up of power was performed using a single needle with an internally cooled electrode. Evaluation for tumor response was performed with 4-phase CT at 24 hours and 3 months. All immediate and follow-up complications were recorded. Results: Complete tumor ablation was achieved in 86%. The ablation volumes were 16 cm3 +/- 12 cm3 for tumors 3 +/- 12 cm3 for tumors 2 - 3 cm. Out of 68 total ablation sessions, there were 2 major complications (pleural effusions) requiring intervention (thoracentesis). Conclusion: Compared with existing techniques using a constant full-power setting, ablation of small hepatomas using an algorithm of gradual ramp-up of power provides comparable rate of complete tumor ablation, adequate ablation volumes, and a low rate of complications that require treatment.

Diagnostic Performance and Inter-Observer Agreement of 4-Dimensional Computed Tomography Parathyroid Scans in Patients with Primary and Secondary Hyperparathyroidism

Background: 4D-CT has been used to localize the parathyroid adenomas and hyperplasia since 2006 as a second line study after TC-99 m MIBI and ultrasonography. However, multiple studies have shown that 4D-CT is a robust imaging method with high diagnostic accuracy, becoming increasingly popular among surgeons and radiologists. Purpose: To assess the diagnostic performance of 4D-CT scans to identify the pathologic gland(s), using pathology and intraoperative findings as gold standards. Methods: We analyzed patients with primary and secondary hyperparathyroidism who had intraoperative reports, pathology, parathyroid hormone levels, and preoperative 4D-CT. Histology, surgical findings, and decreased parathyroid hormone levels were used as gold standards. The sensitivity, specificity, positive predictive value (PPV), negative predictive value (NPV), accuracy and 95% confidence interval were calculated. Fleiss’ kappa was used to assess the inter-observer agreement. Results: Sixty-seven patients were included. Sixty-two patients had a single adenoma, and five patients had a multiple gland disease (adenomas or hyperplasia). A total of 72 glands were proven to have parathyroid adenomas or hyperplasia. The sensitivity, specificity, PPV, NPV and accuracy are 85%, 97%, 96%, 87% and 91% for lateralization and 76%, 96%, 85%, 92% and 90% for quadrant localization, respectively in single-gland disease. The sensitivity, specificity, PPV, NPV and accuracy are 88%, 100%, 100%, 50% and 90% for lateralization and 71%, 100%, 100%, 60% and 80% for quadrant localization respectively in multiple-gland disease. Fleiss’ kappa value is 5.6 (moderate inter-observer agreement). Conclusion: 4D-CT is a robust method in the localization of hyperfunctioning parathyroid glands with high accuracy and at least moderate inter-observer agreement.

Sour cherry(Prunus cerasus L.)is a valuable fruit crop in the Rosaceae family and a hybrid between progenitors closely related to extant Prunus fruticosa(ground cherry)and Prunus avium(sweet cherry).Here we report a chromosome-scale genome assembly for sour cherry cultivar Montmorency,the predominant cultivar grown in the *** also generated a draft assembly of *** to use alongside a published *** sequence for syntelog-based subgenome assignments for‘Montmorency’and provide compelling evidence *** is also an *** hierarchal k-mer clustering and phylogenomics,we show‘Montmorency’is trigenomic,containing two distinct subgenomes inherited from a ***-like ancestor(A and A’)and two copies of the same subgenome inherited from a ***-like ancestor(BB).The genome composition of‘Montmorency’is AA’BB and little-to-no recombination has occurred between progenitor subgenomes(A/A’and B).In Prunus,two known classes of genes are important to breeding strategies:the self-incompatibility loci(S-alleles),which determine compatible crosses,successful fertilization,and fruit set,and the Dormancy Associated MADS-box genes(DAMs),which strongly affect dormancy transitions and flowering *** S-alleles and DAMs in‘Montmorency’and *** were manually annotated and support subgenome ***,the hybridization event‘Montmorency’is descended from was estimated to have occurred less than 1.61 million years ago,making sour cherry a relatively recent ***‘Montmorency’genome highlights the evolutionary complexity of the genus Prunus and will inform future breeding strategies for sour cherry,comparative genomics in the Rosaceae,and questions regarding neopolyploidy.

AIM: To show a new paradigm of simultaneously testing whether breast cancer therapies impact other causes of death. METHODS: MA.14 allocated 667 postmenopausal women to 5 years of tamoxifen 20 mg/daily ± 2 years of octreotide 90 mg, given by depot intramuscular injections monthly. Event-free survival was the primary endpoint of MA.14; at median 7.9 years, the tamoxifen+octreotide and tamoxifen arms had similar event-free survival(P = 0.62). Overall survival was a secondary endpoint, and the two trial arms also had similar overall survival(P = 0.86). We used the median 9.8 years follow-up to examine by intention-to-treat, the multivariate time-to-breast cancer-specific(Br Ca) and other cause(OC) mortality with log-normal survival analysis adjusted by treatment and stratification factors. We tested whether baseline factors including Insulin-like growth factor 1(IGF1), IGF binding protein-3, C-peptide, body mass index, and 25-OH vitamin D were associated with(1) all cause mortality, and if so; and(2) cause-specific mortality. We also fit step-wise forward cause-specific adjusted ***: The analyses were performed on 329 patients allocated tamoxifen and 329 allocated tamoxifen+octreotide. The median age of MA.14 patients was 60.1 years: 447(82%) < 70 years and 120(18%) ≥ 70 years. There were 170 deaths: 106(62.3%) BrC a; 55(32.4%) OC, of which 24 were other malignancies, 31 other causes of death; 9(5.3%) patients with unknown cause of death were excluded from competing risk assessments. BrC a and OC deaths were not significantly different by treatment arm(P = 0.40): tamoxifen patients experienced 50 BrC a and 32 OC deaths, while tamoxifen + octreotide patients experienced 56 Br Ca and 23 OC deaths. Proportionately more deaths(P = 0.004) were from BrC a for patients< 70 years, where 70% of deaths were due to Br Ca, compared to 54% for those ≥ 70 years of age.
