Cytoreductive radical prostatectomy after chemohormonal therapy in patients with primary metastatic prostate cancer

Objective:Cytoreductive radical prostatectomy(cRP)has been proposed as local treatment option in metastatic hormone-sensitive prostate cancer(mHSPC)to prevent local complications and potentially improve oncological *** this study,we examined the feasibility of a multimodal concept with primary chemohormonal therapy followed by cRP and analyzed prostate size reduction under systemic treatment,postoperative complication rates,as well as early postoperative ***:In this retrospective study,38 patients with mHSPC underwent cRP after primary chemohormonal therapy(3-monthly luteinising hormone-releasing hormone-analogue+six cycles 3-weekly docetaxel 75 mg/m2)at two centers between September 2015 and December ***:Overall,10(26%)patients had high volume and 28(74%)patients had low volume disease at diagnosis,according to CHAARTED *** prostate-specific antigen(PSA)decreased from 65 ng/mL(interquartile range[IQR]35.0-124.5 ng/mL)pre-chemotherapy to 1 ng/mL(IQR 0.3-1.7 ng/mL)*** gland volume was significantly reduced by a median of 50%(IQR 29%-56%)under chemohormonal therapy(p=0.003).Postoperative histopathology showed seminal vesicle invasion in 33(87%)patients and negative surgical margins in 17(45%)*** complications(Grade 3 according to Clavien-Dindo)were observed in 4(11%)patients within 30 *** was reached in 87%of patients after 1 month and in 92%of patients after 6 *** time to castration-resistance from begin of chemohormonal therapy was 41.1 months and from cRP was 35.9 *** PSA-nadir≤1 ng/mL versus>1 ng/mL was a significant predictor of time to castration-resistance after cRP(median not reached versus 5.3 months;p<0.0001).Conclusion:We observed a reduction of prostate volume under chemohormonal therapy going along with a low postoperative complication and high early continence ***,the oncologic benefit from cRP is still under evaluation.

Experimental and Modeling Study of Forest Fire Effect on Soil Thermal Conductivity

An understanding of soil thermal conductivity after a wildfire or controlled burn is important to land management and post-fire recovery efforts. Although soil thermal conductivity has been well studied for non-fire heated soils, comprehensive data that evaluate the long-term effect of extreme heating from a fire on the soil thermal conductivity are limited. The purpose of this study was to evaluate the long-term impact of fire on the effective thermal conductivity of soils by directly comparing fire-heated and no-fire control soils through a series of laboratory studies. The thermal conductivity was measured for ten soil samples from two sites within the Manitou Experimental Forest, Colorado, USA, for a range of water contents from saturation to the residual degree of saturation. The thermal conductivity measured was compared with independent estimates made using three empirical models from literature, including the Campbell et al.(1994), Cté and Konrad(2005), and Massman et al.(2008) models. Results demonstrate that for the test soils studied, the thermal conductivity of the fire-heated soils was slightly lower than that of the control soils for all observed water *** results show that the Campbell et al.(1994) model gave the best agreement over the full range of water contents when proper fitting parameters were employed. Further studies are needed to evaluate the significance of including the influence of fire burn on the thermal properties of soils in modeling studies.

Background Previous studies have shown evidence of health-related risk behaviors among women who have sex with women （WSW）, such as sex with men, multiple bisexual partners, and drug use. Women who have sex with women have also been known to avoid routine physical examinations and conceal their same-sex history from physicians, which can affect their ability to receive an accurate diagnosis and treatment. No previous research has targeted women who have sex with women in China. We sought to describe women who have sex with women in China and explore risk factors for their reproductive tract infections （RTI）/sexually transmitted infections （STI）. Methods Participants were recruited through outreach in venues and online for a cross-sectional study. Data were collected using interviews and laboratory tests. Results We recruited 224 women who have sex with women. In the year preceding their participation in the study, 92% （206/224） of women reported sexual relations with women. The RTI rates were： gonorrhea （15.8%）, chlamydia （3.5%）, syphilis （0.5%）, bacterial vaginosis （14.4%）, hepatitis B virus （HBV）（0.9%）, hepatitis C virus （HCV）（0.5%）, and candidiasis （6.9%）. No HIV or herpes simplex virus （HSV） positive cases were detected. Factors associated with gonorrhea infection were non-Beijing local residency （odds ratio （OR）=2.1, 95% confidence interval （C/）： 1.2-3.8） and genital-genital contact （OR=3.1, 95% CI： 1.3-7.2）; factors associated with curable STI （excluding bacterial vaginosis, candidiasis, HBV and HCV） were non-Beijing local residency （OR=1.9; 95% CI： 1.2-3.0） and bleeding during or after sex （OR=18.1; 95% CI： 5.2-62.6）; and the factor associated with RTI （including all the infections tested） was bleeding during or after sex （OR=37.8, 95% CI： 11.2-127.4）. Conclusions Behaviors that may cause RTI/STI exist among Chinese women who have sex with women. Researchers should consider these behaviors when planninq correspondin.q prevention and interventions.

Sperm counts and sperm sex ratio in male infertility patients

In recent years, investigators have noted a trend toward a declining proportion of male births in many industrialized nations. While men bear the sex-determining chromosome, the role of the female partner as it pertains to fertilization or miscarriage may also alter the gender ratio. We attempted to determine a man＇s secondary sex ratio （F1 generation） by directly examining the sex chromosomes of his sperm. We examined our male infertility clinic database for all men who had undergone a semen fluorescence in situ hybridization （FISH）. Patient demographic and semen parameters were recorded. Chi-squared analysis was used to compare gender ratios （Y chromosomes/total chromosomes）. Multivariable logistic regression was used to predict the odds of possessing a Y-bearing sperm after accounting for demographic and semen parameters. A total of 185 men underwent sperm FISH. For the entire cohort, the proportion of Y chromosome-bearing sperm was 51.5%. Men with less than five million motile sperm had a significantly lower proportion of Y chromosome-bearing sperm （50.8%） compared to men with higher sperm counts （51.6%; P= 0.02）. After multivariable adjustment, a higher sperm concentration, total motile sperm count and semen volume significantly increased the odds of having a Y chromosome-bearing sperm （P〈0.01）. As a man＇s sperm production declines, so does the proportion of Y chromosome-bearing sperm. Thus, a man＇s reproductive potential may predict his ability to sire male offspring.

Transient occult cardiotoxicity in children receiving continuous beta-agonist therapy

Background:Continuous beta-agonist therapy,typically in the form of inhaled albuterol,is the first line therapy for the treatment of acute and severe bronchospasm in *** this treatment is commonly used,concerns about cardiotoxicity have been *** aimed to investigate the cardiotoxic effects of continuous beta-agonist therapy in ***:We conducted a retrospective review of children admitted to the intensive care unit(ICU)between May 2008 and April 2009,who were treated with continuous beta-agonist therapy(intravenous and nebulized).Results:Twenty of the 36 children treated with continuous albuterol had repeated serum troponin-T and lactate levels *** patients(55%)were also treated with continuous intravenous *** levels of troponin-T levels were found in 25%of children,and elevated lactate levels were found in 60%.However,all returned to normal levels within 48 hours of ICU admission,despite continued beta-agonist *** children experienced arrhythmias during *** was no association between intravenous terbutaline use and elevated troponin-T[odds ratio(OR),1.3;95%CI,0.2-10.3]or with elevated serum lactate(OR,0.6;95%CI,0.1-3.7).There was also no association between elevated troponin-T or lactate and ICU or hospital length of ***:In this small study,a significant proportion of children had elevated serum troponin-T and lactate levels while receiving inhaled continuous beta-agonist therapy,irrespective of intravenous ***,these abnormal values all returned to normal within 48 hours of ICU admission and were not associated with increased duration of hospitalization.

Anti-diabetic properties and bioactive compounds of Teucrium polium L.

Diabetes mellitus is a common metabolic disease with considerable morbidity and mortality. Untreated or improperly-treated diabetes can be associated with several long-term complications that necessitate an effective way to manage diabetes. Due to the side effects of synthetic glucose-lowering agents, alternative therapeutic modalities such as medicinal plants have attracted notable attention. Teucrium polium L. is a medicinal herb with antioxidant, antinociceptive, anti-inflammatory, hypolipidemic, hepatoprotective, and hypoglycemic properties. In vitro and in vivo studies have been conducted to characterize the anti-diabetic properties of Teucrium polium L. and its bioactive compounds. We conducted a literature study using Scopus, Pub Med, and Google Scholar including the keywords "diabetes" and "Teucrium polium". We also scanned all the references cited by the retrieved articles. According to this review, Teucrium polium administration displayed anti-diabetic effects by targeting different mechanisms and pathways, such as enhancement of insulin secretion and insulin level, improvement of oxidative damage, regeneration of pancreatic β-cells, and promotion of glucose uptake in muscle tissues by increasing GLUT-4 translocation as well as inhibiting α-amylase activity. Although Teucrium polium has been widely regarded as a traditional method, the pharmacological studies on anti-diabetic effects are not sufficient, most studies are either in-vivo or in-vitro. The preclinical and clinical studies are further required to confirm the efficacy of Teucrium polium.
