Is the clinical head impulse test helpful in cochlear implantation candidacy evaluation?

Objective:Vestibular dysfunction is a known risk of cochlear implantation(cI).However,the utility of the physical exam to screen cI candidates for vestibular dysfunction is not *** objective of this study is to evaluate the preoperative role of the clinical head impulse test(cHIT)in subjects undergoing cI surgery *** Design Setting,and Subjects:We conducted a retrospective review of 64 adult cI candidacy cases between 2017 and 2020 at a tertiary health care ***:All patients underwent audiometric testing and evaluation by the senior *** with an abnormal catch-up saccade contralateral to their worse hearing ear during cHIT were referred for formal vestibular *** included clinical and formal vestibular results,operated ear with regard to audiometric and vestibular results,and postoperative ***:Among all cI candidates,44%(n=28)reported preoperative disequilibrium ***,62%(n=40)of the cHITs were normal,33%(n=21)were abnormal,and 5%(n=3)were *** was one patient who presented with a false positive *** the patients who endorsed disequilibrium,43%had a positive preoperative *** percent of the subjects(n=9)without disequilibrium had an abnormal *** this cohort,bilateral vestibular impairment(71%)was more common than unilateral vestibular impairment(29%).In 3%of the cases(n=2),surgical management was revisited or altered due to cHIT ***:There is a high prevalence of vestibular hypofunction in the cI candidate ***-reported assessments of vestibular function are often not congruent with cHIT *** should consider incorporating cHITs as part of the preoperative physical exam to potentially avoid bilateral vestibular dysfunction in a minority of patients.

Association of initial assessment variables and mortality in severe pediatric traumatic brain injury

Background Predictive scales have been used to prognosticate long-term outcomes of traumatic brain injury(TBI),but gaps remain in predicting mortality using initial trauma resuscitation *** sought to evaluate the association of clinical variables collected during the initial resuscitation of intubated pediatric severe patients with TBI with in-hospital *** Intubated pediatric trauma patientscoma scale(GcS)score≤8)from January 2011 to December 2020 were *** between initial trauma resuscitation variables(temperature,pulse,mean arterial blood pressure,GcS score,hemoglobin,international normalized ratio(INR),platelet count,oxygen saturation,end tidal carbon dioxide,blood glucose and pupillary response)and mortality were evaluated with multivariable logistic *** Among 314 patients,median age was 5.5 years(interquartile range(IQR):2.2-12.8),GcS score was 3(IQR:3-6),Head Abbreviated Injury Score(hAIS)was 4(IQR:3-5),and most had a severe(25-49)Injury Severity Score(ISS)(48.7%,153/314).Overall mortality was 26.8%.GcS score,hAIS,ISS,INR,platelet count,and blood glucose were associated with in-hospital mortality(all pcS score increased,the odds of mortality *** 1-point increase in GcS score was associated with a 35%decrease in odds of *** hAIS,INR,and blood glucose increased,the odds of mortality *** each 1.0 unit increase in INR,the odds of mortality increased by 1427%.conclusions Pediatric patients with severe TBI are at substantial risk for in-hospital *** are needed to examine whether earlier interventions targeting specific parameters of INR and blood glucose impact mortality.

BAcKGROUND cheilectomy of the 1^(st)metatarsophalangeal joint(MTPJ)is one of the most common procedures for the management of hallux ***,there is no consensus regarding outcomes following minimally invasive dorsal cheilectomy(MIDc)for the management of hallux *** To evaluate outcomes following MIDc for the management of hallux *** During November 2023,the PubMed,EMBASE and cochrane Library databases were systematically reviewed to identify clinical studies examining outcomes following MIDc for the management of hallux *** Six studies were *** total,348 patients(370 feet)underwent MIDc for hallux rigidus at a weighted mean follow-up of 37.9±16.5 *** distribution of patients by coughlin and Shurna's classification was recorded in 4 studies as follows:Ⅰ(58 patients,27.1%),Ⅱ(112 patients,52.3%),Ⅲ(44 patients,20.6%).Three studies performed an additional 1^(st)MTPJ arthroscopy and debridement following *** intra-articular bone debris was observed in 100%of patients in 1 *** weighted mean American orthopedic foot and ankle society score improved from a preoperative score of 68.9±3.2 to a postoperative score of *** complication rate was 8.4%,the most common of which was persistent joint pain and ***-two failures(8.7%)were ***-three secondary procedures(8.9%)were performed at a weighted mean time of 8.6±3.2 months following the index *** This systematic review demonstrated improvements in subjective clinical outcomes together with a moderate complication rate following MIDc for the management of hallux rigidus at short-term follow-up.A moderate reoperation rate at short-term follow-up was *** marked heterogeneity between included studies and paucity of high quality comparative studies limits the generation of any robust conclusions.

Multiple myeloma survival in New South Wales, Australia, by treatment era to 2020

Objective: Australia has relatively high multiple myeloma(MM) incidence and mortality rates. Advancements in MM treatment over recent decades have driven improvements in MM survival in high-income countries;however, reporting in Australia is limited. We investigated temporal trends in population-wide MM survival across 3 periods of treatment advancements in New South Wales(NSW), ***: Individuals with an MM diagnosis in the NSW cancer Registry between 1985 and 2015 with vital follow-up to 2020, were categorized into 3 previously defined treatment eras according to their diagnosis date(1985±1995, chemotherapy only;1996±2007, autologous stem cell transplantation;and 2008±2015, novel agents including proteasome inhibitors and immunomodulatory drugs). Both relative survival and cause-specific survival according to Fine and Gray's competing risks cumulative incidence function were calculated by treatment era and age at ***: Overall, 11,591 individuals were included in the study, with a median age of 70 years at diagnosis. Five-year relative survival improved over the 36-year(1985±2020) study period(31.0% in 1985±1995;41.9% in 1996±2007;and 56.1% in 2008±2015). For individuals diagnosed before 70 years of age, the 5-year relative survival nearly doubled, from 36.5% in 1985±1995 to 68.5% in 2008±2015. Improvements for those > 70 years of age were less pronounced between 1985±1995 and 1996±2007;however, significant improvements were observed for those diagnosed in 2008±2015. Similar overall and age-specific patterns were observed for causespecific survival. After adjustment for gender and age at diagnosis, treatment era was strongly associated with both relative and cause-specific survival(P conclusions: Survival of individuals with MM is improving in Australia with treatment advances. charging Ahead with E-Buses: Benefits, costs, and Transition Roadmap

This paper reaches a recommendation for the 10-year e-bus transition roadmap for New York city. The lifecycle model of emission reduction demonstrates the ecological and financial impacts of a complete transition from the current diesel bus fleet to an all-electric bus fleet in New York city by 2033. This study focuses on the NOx pollution, which is the highest among all major cities by Environmental Protection Agency (EPA) and greenhouse gases (GHG) with annual emissions of over five million tons. Our model predicts that switching to an all-electric bus fleet will cut GHG emissions by over 390,000 tons and NOx emissions by over 1300 tons annually, in addition to other pollutants such as VOcs and PM 2.5. yielding an annual economic benefit of over 75.94 million USD. This aligns with the city mayor office’s initiative of achieving total carbon neutrality. We further model an optimized transition roadmap that balances ecological and long-term benefits against the costs of the transition, emphasizing feasibility and alignment with the natural replacement cycle of existing buses, ensuring a steady budgeting pattern to minimize interruptions and resistance. Finally, we advocate for collaboration between government agencies, public transportation authorities, and private sectors, including electric buses and charging facility manufacturers, which is essential for fostering innovation and reducing the costs associated with the transition to e-buses.
