Background The levels of long-term elevated serum or intracellular free fatty acid （FFA） induce insulin resistance associated with central obesity. The insulin-mimetic protein visfatin is preferentially produced by visceral adipose tissues and has been implicated in obesity and insulin resistance. To identify that FFA is capable of inducing insulin resistance and to clarify the role of FFA on visfatin, we examined the effect of monounsaturated FFA oleate （C 18： 1） and saturated FFA palmitate （C 16：0） on glucose transport and visfatin gene expression in cultured 3T3-L1 adipocytes or preadipocytes.Methods FFA-free DMEM/F12, 0.125 mmol/L, 0.5 mmol/1 and 1.0 mmol/L oleate or palmitate was added to cultured 3T3-L1 adipocytes or preadipocytes and incubated overnight. Glucose transport was assessed as 3H- 2-deoxy-glucose uptake. Total rNA was extracted and subjected to rT-PCr for the measurement of visfatin mrNA levels. Statistical comparisons between control group and other groups were performed with the two-tailed paired t test, and one-way ANOVA was used to compare the mean values among the groups.results Insulin increased specific membrane glucose transport in 3T3-L1 preadipocytes. Upregulation was evident from 15 minutes to 1 hour exposure to insulin. However, after 6-hour exposure to insulin, there was a downregulation in the response to insulin. Dose response studies demonstrated that 2-deoxy glucose transport was increased by 336% at 50 nmol/L insulin （P〈0.01）, and reached a maximal effect at 100 nmol/L insulin （P〈0.01）. Oleate and palmitate treatment did not influence basal glucose transport （without insulin stimulation）, whereas insulin-stimulated glucose transport was inhibited after overnight oleate and palmitate treatment in preadipocytes and adipocytes. In 3T3-L1 preadipocytes, insulin resistance could be achieved at 0.125 mmol/L oleate or palmitate （P〈0.05, respectively）, and the inhibition was dose dependent. Background:routine tuberculosis(TB)notifications are geographically heterogeneous,but their utility in predicting the location of undiagnosed TB cases is *** aimed to identify small-scale geographic areas with high TB notification rates based on routinely collected data and to evaluate whether these areas have a correspondingly high rate of undiagnosed prevalent ***:We used routinely collected data to identify geographic areas with high TB notification rates and evaluated the extent to which these areas correlated with the location of undiagnosed cases during a subsequent community-wide active case finding intervention in Kampala,*** first enrolled all adults who lived within 35 contiguous zones and were diagnosed through routine care at four local TB Diagnosis and Treatment *** calculated average monthly TB notification rates in each zone and defined geographic areas of“high risk”as zones that constituted the 20%of the population with highest notification *** compared the observed proportion of TB notifications among residents of these high-risk zones to the expected proportion,using simulated estimates based on population size and random variation *** then evaluated the extent to which these high-risk zones identified areas with high burdens of undiagnosed TB during a subsequent community-based active case finding campaign using a chi-square ***:We enrolled 45 adults diagnosed with TB through routine practices and who lived within the study area(estimated population of 49527).Eighteen zones reported no TB cases in the 9-month period;among the remaining zones,monthly TB notification rates ranged from 3.9 to 39.4 per 100000 *** five zones with the highest notification rates constituted 62%(95%CI:47–75%)of TB cases and 22%of the population–significantly higher than would be expected if population size and random chance were the only determinants of zone-to-zone variation(48%,95%simulation interval:40–59%).These f

Delayed post-traumatic small bowel stricture is rare and differentiation from other more common causes of small intestinal stricture is difficult. Presentation may be delayed up to years after the injury. Very often the findings mimic those of other causes of long segment strictures such as tuberculosis, Crohn’s disease, lymphoma etc. This study analyzes two cases of post-traumatic small bowel stricture, with different presentations and different therapeutic challenges. The literature on this condition is not well recognized and will be reviewed. resection and reconstruction are diagnostic and therapeutic in patients who present with post-traumatic small bowel stricture, as inflammatory and neoplastic causes cannot be ruled out without histological diagnosis.

Thermodynamic dissociation constants pKa of 2,2-bis(hydroxymethyl)-2,2’,2”-nitrilotriethanol have been determined at 12 temperatures from (278.15 to 328.15) K including the body temperature 310.15 K by the electromotive-force measurements (emf) of hydrogen-silver chloride cells without liquid junction of the type: Pt(s), H2(g), 101.325 kPa|BIS-TrIS (m) + BIS-TrIS·HCl (m)| AgCl(s), Ag(s), where m denotes molality. The pKa values for the dissociation process of BIS-TrIS·H++ H2O = H3O+ + BIS-TrIS given as a function of T in Kelvin (K) by the equation pKa = 921.66 (K/T) + 14.0007-1.86197 ln(T/K). At 298.15 and 310.15 K, the values of pKa for BIS-TrIS were found to be 6.4828 ± 0.0005 and 6.2906 ± 0.0006 respectively. Thus buffer solutions composed of BIS-TrIS and its hydrochloride would be useful as secondary pH buffer standards and for control of acidity in the pH range 6 to 8. At 298.15 K the thermodynamic functions G°, H°, S° and Cp° for the dissociation process of BIS-TrIS·H+ are G°=37,005 J·mol-1, H° = 28,273 J·mol-1, S°= 29.3 J·K-1·mol-1 and Cp° = 36 J·K-1·mol-1. These results are compared with the dissociation of protonated bases structurally related to BIS-TrIS·H+.

Gender differences in mental health service utilization among respondents reporting depression in a national health survey

This study examined whether people who self-reported depression sought mental health treatment in the year after being interviewed, and how gender affected utilization. Depression data were obtained from the Canadian Community Health Survey (2000-01), and linked to medical records in Ontario (n = 24,677). Overall, women had higher rates of mental health service utilization, but there were no gender differences in rates of specialist care. The gender difference in mental health contact was greater for those without depression, as opposed to those with depression. Among those without depression, women were significantly more likely than men to use mental health services;however, rates were similar for women and men with depression. This finding suggests that men may be more likely than women to delay seeing a doctor for minor mental health concerns, but will seek help once a problem reaches a threshold.

The lipid oxidation product, the toxic 4-hydroxynonenal, was measured for fried chicken in commercial samples from two fast food restaurants and one commercial establishment. The fried chicken samples were breasts, thighs, chicken nuggets and popcorn chicken. Fried chicken samples were separated to breast skin and meat, and thigh skin and meat. Chicken nuggets and popcorn chicken were not separated from the coating materials and the meat was analyzed together. Samples were analyzed for total fat, fatty acid distribution, reactivity with thiobarbituric acid (TBAr), measuring the secondary lipid peroxidation products such as aldehydes, ketones and related carbonyl compounds. Samples were analyzed for HNE, a toxic aldehyde using high performance liquid chromatography (HPLC). Fatty acid distribution showed in breast and thigh meats, a higher level of palmitic acid, compared to the breast and thigh skins. TBAr values were significantly higher in all breast meats compared to skins. TBAr values were significantly higher in thigh meat than in skin samples. HNE concentrations (μg HNE/g fat) were significantly very low in breast skins compared to breast meat. HNE concentration was generally higher in thigh meat than skin but not in every sample. In chicken nuggets, both the TBAr value and HNE concentration were much higher from one establishment than from the two others. Chicken nuggets TBAr and HNE concentrations resembled one of the chicken nugget samples. The average toxic HNE concentration for 100 g fried chicken breast (skins + meat) was 12.55 μg and for thighs (skin + meat) was 26.76 μg. The average total HNE concentration was 2.1 times higher in the fried chicken thighs than in the breasts. It is clear that HNE is produced during the heating process of oils and incorporated into the fried meat and skin samples during the frying process. 研究运动对链脲佐菌素所致糖尿病大鼠A SP的影响。选用雄性SD大鼠,随机分为4组(每组10只),即:正常对照组,正常运动组,糖尿病模型组,糖尿病模型+运动组。运动组进行10周的游泳训练。结果表明:1)糖尿病模型组与正常对照组大鼠相比,血浆A SP浓度、血糖、TG、和LDL明显升高,血清FFA有升高趋势,血清胰岛素和IS I明显降低;2)糖尿病模型+运动组较其非运动组血浆A SP浓度、血糖、TG、TC、FFA和LDL明显降低,血清胰岛素和HDL含量明显升高。提示:糖尿病机体血浆A SP浓度升高,伴随着其活性下降,使得FFA和富含TG的脂蛋白消除障碍,长期中等负荷运动可以降低血浆中A SP浓度,增加其活性,进而降低FFA和TG水平,升高HDL,改善糖尿病机体的脂质代谢,预防其慢性并发症的发生。
