Lower extremity injury and deformity can result from a number of etiologies. Regardless of the underlying cause, the decision to pursue amputation or reconstruction of a lower limb is challenging for both patients and *** decision is largely dependent on the patient's premorbid health and function, functional goals and preferences,and characteristics and viability of the affected limb. The role of adaptive devices following surgery should never be underestimated. Advances in prostheses and orthoses have provided patients with a wider range of options to consider when deciding between limb reconstruction and amputation. The primary goals of any adaptive device are to improve function, prevent recurrence or ulceration of the defect, and allow for use of conventional footwear and/or *** a lower extremity amputation is indicated, selection of the correct level is of critical importance in order to optimize healing potential and function. Each distinct level has certain inherent prosthetic and orthotic considerations. Likewise,the application of an adaptive device following reconstruction of the lower extremity also has demonstrable benefits and must be tailored to the specific defect and procedure performed. Knowledge of available prosthetic and orthotic options is of considerable importance for the reconstructive surgeon tasked with limb salvage or resurfacing an amputated extremity. This article reviews considerations of various types of lower extremity amputation and reconstruction, and provides a framework for the role of adaptive devices following surgery.

Acid extraction methods have been used in the last half century to selectively extract the cO_(2)produced from different carbonate minerals in mixed ***,these methods are often time-consuming and labor *** application to clumped isotope(Δ47)analysis has not been *** propose here an acid extraction method with phosphoric acid for bulk stable and clumped isotope analysis that treats mixtures of calcite and dolomite the same regardless of the proportional ***_(2)evolved from calcite is extracted by allowing a reaction with phosphoric acid to proceed for 10 min at 50℃.We then extract cO_(2)evolved from dolomite by rapid ramping the acid temperature from 50 to 90℃and allowing the reaction to *** experimental results show that our method yields accurate calcite and dolomiteΔ_(47)values from mixed samples under different proportional *** method also displays equal or higher accuracy for calciteδ^(13)c and dolomiteδ^(13)c andδ^(18)O values from mixtures when compared to previous *** approach exhibits higher sample throughput than previous methods,is adequate for clumped isotopic analysis and simplifies the reaction progression from over 24 h to less than 2 h,while maintaining relatively high isotopic obtaining *** yet poorly resolves calciteδ18O values,as found with previous methods.

Recently,single nucleotide polymorphisms,in the vicinity of the interferon lambda 3(IFNL3)gene have been identified as the strongest predictor of spontaneous and treatment induced clearance of hepatitis c virus(HcV)*** then,increasing evidence has implicated the innate immune response in mediating the IFNL3 genotype *** cells(Dcs)are key to the host immune response in HcV infection and their vital role in the IFNL3 genotype effect is *** have identified subclasses of Dcs,particularly myeloid Dc2s and potentially plasmacytoid Dcs as the major producers of IFNL3 in the setting of HcV *** the complexities of dendritic cell biology and the conflicting current available data,this review aims to summarize what is currently known regarding the role of dendritic cells in HcV infection and to placeit into context of what is know about lambda interferons and dendritic cells in general.

Back pain is a common condition with a high social impact and represents a global health *** disc disease(IVDD)is one of the major causes of back pain;no therapeutics are currently available to reverse this *** impact of bone mineral density(BMD)on IVDD has been controversial,with some studies suggesting osteoporosis as causative for IVDD and others suggesting it as protective for *** studies to evaluate the influence of genetic components of BMD in IVDD could highlight opportunities for drug development and *** taking a holistic 3D approach,we established an aging zebrafish model for spontaneous *** BMD in aging,detected by automated computational analysis,is caused by bone deformities at the ***,aged zebrafish spines showed changes in bone morphology,microstructure,mineral heterogeneity,and increased fragility that resembled *** of the discs recapitulated IVDD symptoms found in humans:the intervertebral ligament(equivalent to the annulus fibrosus)showed disorganized collagen fibers and herniation,while the disc center(nucleus pulposus equivalent)showed dehydration and cellular *** manipulated BMD in young zebrafish by mutating sp7 and cathepsin K,leading to low and high BMD,***,we detected IVDD in both groups,demonstrating that low BMD does not protect against IVDD,and we found a strong correlation between high BMD and *** learning was applied to high-resolution synchrotron\icJ image data to analyze osteocyte 3D lacunar distribution and morphology,revealing a role of sp7 in controlling the osteocyte lacunar 3D *** findings suggest potential avenues through which bone quality can be targeted to identify beneficial therapeutics for IVDD.

Microglia shape the synaptic environment in health and disease,but synapses do not exist in a ***,pre-and postsynaptic terminals are surrounded by extracellular matrix(EcM),which together with glia comprise the four elements of the contemporary tetrapartite synapse *** research in this area is still just beginning,accumulating evidence points toward a novel role for microglia in regulating the EcM during normal brain homeostasis,and such processes may,in turn,become dysfunctional in *** it relates to synapses,microglia are reported to modify the perisynaptic matrix,which is the diffuse matrix that surrounds dendritic and axonal terminals,as well as perineuronal nets(PNNs),specialized reticular formations of compact EcM that enwrap neuronal subsets and stabilize proximal *** interconnected relationship between synapses and the EcM in which they are embedded suggests that alterations in one structure necessarily affect the dynamics of the other,and microglia may need to sculpt the matrix to modify the synapses ***,we provide an overview of the microglial regulation of synapses,perisynaptic matrix,and PNNs,propose candidate mechanisms by which these structures may be modified,and present the implications of such modifications in normal brain homeostasis and in disease.

URTI （upper respiratory tract infections） are caused by acute, chronic or recurrent infections including pharyngitis andtonsillitis. The etiologic agents involved are bacteria, virus, and fungi. The standard treatment for bacterial infections is the use ofantibiotics. The antibiotic resistance, side effects of drugs and URTI complications indicate the importance of new therapies. PDT（photodynamic therapy） is a treatment option with a fast onset of action, low side effects, and easy administration of PS（photosensitizer） based on this study. The main aim of this study was to develop a new modality for the treatment of infectiouspharyngotonsillitis by photodynamic therapy. In addition, this study aims to evaluate components of the therapy as the PS, curcumin,in two formulations and irradiation conditions in the effectiveness of treatment. The procedure consists of two steps： oraladministration of cFs （curcumin formulations） containing a PS and illumination （450 nm and 0-60 J/cm2）. Bacterial isolates wereobtained from patients in a routine clinical pratice. The new methodology has been developed from the principle ofPI （photodynamicinactivation）. The strain identification number of colony forming units determined the efficacy of PI using each component, lightingand PS. The use of curcumin formulations in PI presented microbial reduction of 5 log.
