Background The utility of routine extensive molecular profiling of pediatric tumors is a matter of debate due to the high number of genetic alterations of unknown significance or low evidence and the lack of standardized and personalized decision support *** drug assignment(DDA)is a novel computational method to prioritize treatment options by aggregating numerous evidence-based associations between multiple drivers,targets,and targeted *** has been validated to improve personalized treatment decisions based on the outcome data of adult patients treated in the SHIVA01 clinical *** aim of this study was to evaluate the utility of DDA in pediatric *** Between 2017 and 2020,103 high-risk pediatric cancer patients(<21 years)were involved in our precision oncology program,and samples from 100 patients were eligible for further *** or blood samples were analyzed by whole-exome(WES)or targeted panel sequencing and other molecular diagnostic modalities and processed by a software system using the DDA algorithm for therapeutic decision ***,a molecular tumor board(MTB)evaluated the results to provide therapy *** Of the 100 cases with comprehensive molecular diagnostic data,88 yielded WES and 12 panel sequencing *** identified matching off-label targeted treatment options(actionability)in 72/100 cases(72%),while 57/100(57%)showed potential drug *** reached 88%(29/33)by 2020 due to the continuous updates of the evidence *** approved the clinical use of a DDA-top-listed treatment in 56 of 72 actionable cases(78%).The approved therapies had significantly higher aggregated evidence levels(AELs)than dismissed *** of WES results for targeted panels missed important mutations affecting therapy *** DDA is a promising approach to overcome challenges associated with the interpretation of extensive molecular profiling in the routine car

Surfactant food emulsifiers are among the most extensively used food *** all authorized food additives,emulsifiers have been evaluated by risk assessors,who consider them as ***,there are growing concerns among scientists about their possible harmful effects on intestinal barriers and *** is also suggested that emulsifier consumption might be one of the main causes of the rising incidence of a number of diseases,such as allergic diseases,celiac disease,type I diabetes and Crohn’s ***,it has recently been suggested that emulsifier consumption might contribute to the development of metabolic syndrome and can promote colitis-associated colorectal cancer *** paper provides an overview of the current scientific knowledge on possible effects of surfactant emulsifiers on intestinal barriers and also of regulatory risk assessment *** main objective is to reveal the reasons for the discrepancies between the opinions of risk assessors and the scientific *** would like to draw the attention of the academic world to the need of specific in vivo and/or clinical studies for each emulsifier in order to help risk assessors make adequate evaluations and rule out with certainty that authorized food emulsifiers pose a safety concern to consumers’health.

本文基于对移位与区分原则的分析,探究洛特曼文学符号学中的意义生成与更新机制,揭示其符号学术语体系内在的关联性与一致性。作者将移位与区分置于有序化、符号域、约定性等概念与术语的有机联系中加以综合考察,从语义、结构、语境等多重视角阐释这一问题在洛特曼文学符号学研究中的独特价值。本文认为,移位作为意义区分的一种主要符号学策略,参与文本内与文本间符号交流的动态过程,对意义的不确定性、开放性、多元性等问题的研究具有重要意义。

This review is part two of three, which will present an update on the classification of gastrointestinal submucosal tumors. Part one treats of the diagnosis and part three of the therapeutic methods regarding gastrointestinal submucosal tumors. In the past there has been some confusion as to the classification of gastrointestinal submucosal tumors. Changes in classifications have emerged due to recent advances in mainly immunohistochemistry and electron microscopy. The aim of this paper is to update the reader on the current classification. Literature searches were performed to find information related to classification of gastrointestinal submucosal tumors. Based on these searches the twelve most frequent submucosal tumor types were chosen for description of their classification. The factors that indicate whether tumors are benign or malignant are mainly size and number of mitotic counts. Gastrointestinal stromal tumors are defined mainly by their CD117 positivity. In the future, there should be no more confusion between gastrointestinal stromal tumors and other types of submucosal tumors.

Low bone mineral density is an established, frequent, but often neglected complication in patients with inflammatory bowel disease (IBD). Data regarding the diagnosis, therapy and follow-up of low bone mass in IBD has been partially extrapolated from postmenopausal osteoporosis; however, the pathophysiology of bone loss is altered in young patients with IBD. Fracture, a disabling complication, is the most important clinical outcome of low bone mass. Estimation of fracture risk in IBD is difficult. Numerous risk factors have to be considered, and these factors should be weighed properly to help in the identification of the appropriate patients for screening. In this editorial, the authors aim to highlight the most important clinical aspects of the epidemiology, prevention, diagnosis and treatment of IBD-related bone loss.

AIM: To assess the relationship between severity of gastroesophageal reflux disease and apnea-hypopnea index (AHI) as an indicator of the severity of obstructive sleep apnea. METHODS: Data of 57 patients with proven obstructive sleep apnea and gastroesophageal reflux disease were analyzed. Patients were divided into two groups according to severity of the sleep apnea: 'mild-moderate' (A)-AHI 5≥5-30, n=27, 'severe' (B)-AHI >30, n=30. All patients underwent apnea monitoring during the night, upper panendoscopy and were asked about typical reflux symptoms. RESULTS: All examined patients in both groups showed a significant overweight and there was a positive correlation between body mass index and the degree of sleep apnea (P = 0.0002). The occurence of erosive reflux disease was significantly higher in 'severe' group (P = 0.0001). Using a logistic regression analysis a positive correlation was found between the endoscopic severity of reflux disease and the AHI (P = 0.016). Forty-nine point five percent of the patients experienced the typical symptoms of reflux disease at least three times a week and there was no significant difference between the two groups. CONCLUSION: A positive correlation can be found between the severity of gastroesophageal reflux disease and obstructive sleep apnea.

AIM: To investigate the relationship between certain biochemical parameters of lipid metabolism in the serum and steatosis in the ***: The grade of steatosis (0-3) and histological activity index (HAI, 0-18) in liver biopsy specimens were correlated with serum alanine aminotransferase (ALT), total cholesterol and triglyceride levels in 142 patients with chronic hepatitis C (CH-C), and 28 patients with non-alcoholic fatty liver disease (NAFLD) without hepatitis C virus (HCV) infection. The serum parameters were further correlated with 1 797 age and sex matched control patients without any liver ***: Steatosis was detected in 90 out of 142 specimens (63%) with CH-C. The ALT levels correlated with the grade of steatosis, both in patients with CH-C and NAFLD (P＜0.01). Inserting the score values of steatosis as part of the HAI, correlation with the ALT level (P＜0.00001) was found. The triglyceride and cholesterol levels were significantly lower in patients with CH-C (with and without steatosis), compared to the NAFLD group and to the virus-free control ***: Our study confirms the importance of liver steatosis in CH-C which correlates with lower lipid levels in the sera. Inclusion of the score of steatosis into HAI, in case of CH-C might reflect the alterations in the liver tissue more precisely, while correlating with the ALT enzyme elevation.

Soil health depletion due to intensive tillage operations is a global issue in the agricultural *** tillage(CT)which involves non-inversion tillage and leaving~30%of the soil surface covered with crop residues,is a strategy designed to enhance soil ***,no comprehensive study to investigate the long-term effect of CT on soil biological activity and the soil nutrient supply has yet been widely carried *** and chemical soil properties were assessed at depths 0-5,10-15,and 20-25 cm depths after 18 years of CT and conventional tillage practice(PT).Various stages in the vegetative growth of maize were investigated in 2021 in *** findings indicated that tillage intensity,soil depth,and growth stages all significantly influenced soil enzyme activities and the concentration of soil *** soil disturbance resulted in a significantly larger concentration of soil carbon parameters(total organic carbon and labile carbon)in CT plots,where the activity ofβ-glucosidase and dehydrogenase(DHA)in the upper soil layer increased significantly(0.7-2.6 and 2.6-4.7 times,respectively)compared to *** high amount of organic matter and the greater resistance to erosion observed in CT also contributed to the higher concentration of available nutrients(NH_(4),NO_(3),Ca,K)and total P in the surface soil *** activity was highest in the mid-stage of vegetative growth and was positively correlated to the total P *** alterations in soil water content were clearly negatively corelated with the change in DHA and phosphatase ***,due to the more balanced environmental conditions,the decomposition of organic substances was more balanced and slower in CT than in *** implied that the mobilization of nutrients in the soil was more balanced as well,and that the nutrients were released *** enhancement of the soil nutrient-supplying capacity achieved by means of long-term conservation tillage provid
