Objective To investigate the role of oxidative stress, inflammation, hypercoagulability and neuroendocrine activation in the transition of hypertensive heart disease to heart failure with preserved ejection fraction （HFPEF）. methods We performed echocardiography for 112 patients （≥ 60 years old） with normal EF （18 controls and 94 with hypertension）, and determined protein carbonylation （PC）, and tetrahydrobiopterin （BH4）, C-reactive protein （CRP）, interleukin-6 （IL-6）, tumor necrosis factor-α （TNF-α）, fibrinogen, plasminogen activator inhibitor type-I （PAI-I）, von Willebrand factor, chromogranin A （cGA） and B-type natriuretic peptide （BNP） levels from their blood samples. Results We found that 40% （38/94） of the patients with hypertension （HT） had no diastolic dysfunction （HTDD-）, and 60% （56/94） had diastolic dysfunction （HTDD＋）. Compared to the controls, both patient groups had increased PC and BH4, TNF-α, PAI-I and BNP levels, while the HTDD＋ group had elevated cGA and CRP levels. Decreased atrial and longitudinal left ventficular （LV） systolic and diastolic myocardial deformation （strain and strain rate） was demonstrated in both patient groups versus the control. Patients whose LV diastolic function deteriorated during the follow-up had elevated PC and IL-6 level compared to their own baseline values, and to the respective values of patients whose LV diastolic function remained unchanged. Oxidative stress, inflammation, BNP and PAI-I levels inversely correlated with LV systolic, diastolic and atrial function. Conclusions In patients with HT and normal EF, the most common HFPEF precursor condition, oxidative stress and inflammation may be responsible for LV systolic, diastolic and atrial dysfunction, which are important determinants of the transition of liT to HFPEF.

Clinopyroxene-enriched upper mantle xenoliths classified as wehrlites are common(~20% of all xenoliths) in the central part of the Nograd-G(o| ")m(o|")r Volcanic Field(NGVF),situated in the northern margin of the Pannonian Basin in northern Hungary and southern *** this study,we thoroughly investigated 12 wehrlite xenoliths,two from each wehrlite-bearing occurrence,to determine the conditions of their *** textural features,including clinopyroxene-rich patches in an olivine-rich lithology,orthopyroxene remnants in the cores of newlyformed clinopyroxenes and vermicular spinel forms all suggest that wehrlites were formed as a result of intensive interaction between a metasomatic agent and the peridotite wall *** on the major and trace element geochemistry of the rock-forming minerals,significant enrichment in basaltic(Fe,mn,Ti) and high field strength elements(Nb,Ta,Hf,Zr) was observed,compared to compositions of common lherzolite *** presence of orthopyroxene remnants and geochemical trends in rock-forming minerals suggest that the metasomatic process ceased before complete wehrlitization was *** composition of the metasomatic agent is interpreted to be a mafic silicate melt,which was further confirmed by numerical modelling of trace elements using the plate *** model results also show that the melt/rock ratio played a key role in the degree of petrographic and geochemical *** lack of equilibrium and the conclusions drawn by using variable lherzolitic precursors in the model both suggest that wehrlitization was the last event that occurred shortly before xenolith entrainment in the host mafic *** suggest that the wehrlitization and the Plio-Pleistocene basaltic volcanism are related to the same magmatic event.

AIm: To assess the prevalence and stability of different antiphospholipid antibodies(APLAs) and their association with disease phenotype and progression in inflammatory bowel diseases(IBD) ***: About 458 consecutive patients [Crohn's disease(CD): 271 and ulcerative colitis(UC): 187] were enrolled into a follow-up cohort study in a tertiary IBD referral center in Hungary. Detailed clinical phenotypes were determined at enrollment by reviewing the patients' medical charts. Disease activity, medical treatment and data about evolvement of complications or surgical interventions were determined prospectively during the follow-up. Disease course(development f complicated disease phenotype and need for surgery),occurrence of thrombotic events, actual state of diseaseactivity according to clinical, laboratory and endoscopic scores and accurate treatment regime were recorded during the follow-up,(median, 57.4 and 61.6 mo for CD and UC). Sera of IBD patients and 103 healthy controls(HC) were tested on individual anti-β2-Glycoprotein-I(anti-β2-GPI IgA/m/G), anti-cardiolipin(ACA IgA/m/G)and anti-phosphatidylserine/prothrombin(anti-PS/PT IgA/m/G) antibodies and also anti-Saccharomyces cerevisiae antibodies(ASCA IgA/G) by enzyme-linked immunosorbent assay(ELISA). In a subgroup of CD(n = 198) and UC patients(n = 103), obtaining consecutive samples over various arbitrary timepoints during the disease course, we evaluated the intraindividual stability of the APLA status. Additionally,we provide an overview of studies, performed so far, in which significance of APLAs in IBD were ***: Patients with CD had significantly higher prevalence of both ACA(23.4%) and anti-PS/PT(20.4%) antibodies than UC(4.8%, p < 0.0001 and10.2%, p = 0.004) and HC(2.9%, p < 0.0001 and15.5%, p = NS). A comperative clinical study of mucotomy and KTP laser treatment of the inferior turbinate in allergic and non-allergic subjects

mucotomy is one of the most frequently applied surgical techniques for the management of inferior turbinate hyperplasia. mucotomy guarantees patent airway, however, it might lead to the emergence of sicca syndrome. In contrast, KTP (potassium titanyl phosphate) laser treatment spares the medial part of the inferior turbinate mucosa, contributing to maintenance of physiological nasal function. A retrospective comparative clinical study was performed to reveal the advantages and side-effects of both surgical methods in medium- and long-term in allergic rhinitis and non-allergic patients. Furthermore, we wished to determine the exact indications of the up-to-date laser treatment. Ninety-one of the 117 patients who underwent bilateral turbinate surgery during an 8-year period (2000-2007) responded to our questionnaire focusing on subjective postoperative changes. Patients were separated into 6 groups, based on the type of operation they underwent, the length of the follow-up and whether they suffered from allergies. The major complaint, nasal obstruction, improved in all 6 groups, which reached significance (p ? 0.05) in 4 groups. The most pronounced improvement was observed in the group of non-allergic patients with medium-term follow-up who underwent mucotomy. Nevertheless, a serious side- effect: crusting also increased significantly (p ? 0.05) in the latter group, while it was absent in allergic patients with medium-term follow-up, who underwent mucotomy. These results lead us to propose the following protocol for the treatment of inferior turbinate hyperplasia: 1) after unsuccessful conservative treatment, laser treatment is suggested for non-allergic patients;2) following unsuccessful conservative and even repeated laser treatment in the allergic group, mucotomy or turbinoplasty should be attempted.

Nervous System and Gastrointestinal Effects of the Insecticide Esfenvalerate on the Rat:An Ex Vivo Study

Esfenvalerate belongs to the pyrethroid group of insecticides which display significant selective toxicity against insects compared to mammalian species, nevertheless, they may pose health risks, especially in case of accidental exposure. The aim of the present study was to model the effect of acute, relatively high-dose exposure of the esfenvalerate-containing formulation Sumi-Alpha?. Eventual functional alterations in the central nervous system and in the gastrointestinal tract were studied on in vitro tissue preparations at different delays after intragastric administration to rats. Neuronal effects were characterized by field potential recording in cortical and hippocampal brain slices, while gastrointestinal effects were examined by analyzing the motility and excitability of isolated ileum segments. On the brain slices originating from esfenvalerate-treated animals, changes in excitability of both inhibitory and excitatory type could be observed. Voltage thresholds necessary to evoke responses in neocortex slices were elevated, and population spike amplitudes were lower in hippocampal slices. However, epileptiform potentials with pronounced late components were also observed. A decreased long-term potentiation (LTP) could be seen in both brain areas after esfenvalerate treatment. Seizure susceptibility of the slices was not significantly altered, but tended to be somewhat higher in slices originating from treated rats. In ileum segments, both spontaneous and acetyl-choline (ACh)-elicited contractions were modified by treatment. Esfenvalerate raised the amplitude of contractions in the low ACh concentration range. However, the solvent xylene also considerably contributed to the detected changes. We can conclude that a relatively high, single oral dose of Sumi-Alpha? exerted mild and temporary effects on the elementary brain functions and intestine functions of the rat.

BACKGROUND Despite the improvement in the endoscopic hemostasis of non-variceal upper gastrointestinal bleeding(NVUGIB),rebleeding remains a major *** To assess the role of prophylactic transcatheter arterial embolization(PTAE)added to successful hemostatic treatment among NVUGIB *** We searched three databases from inception through October 19th,*** controlled trials(RCTs)and observational cohort studies were *** compared patients with NVUGIB receiving PTAE to those who did not get *** outcomes were rebleeding,mortality,reintervention,need for surgery and transfusion,length of hospital(LOH),and intensive care unit(ICU)*** the quantitative synthesis,odds ratios(ORs)and weighted mean differences(WmDs)were calculated with the random-effects model and interpreted with 95%confidence intervals(CIs).RESULTS We included a total of 3 RCTs and 9 observational studies with a total of 1329 patients,with 486 in the intervention *** was associated with lower odds of rebleeding(OR=0.48,95%CI:0.29–0.78).There was no difference in the 30-d mortality rates(OR=0.82,95%CI:0.39–1.72)between the PTAE and control *** who underwent PTAE treatment had a lower chance for reintervention(OR=0.48,95%CI:0.31–0.76)or rescue surgery(OR=0.35,95%CI:0.14–0.92).The LOH and ICU stay was shorter in the PTAE group,but the difference was non-significant[WmD=-3.77,95%CI:(-8.00)–0.45;WmD=-1.33,95%CI:(-2.84)–0.18,respectively].CONCLUSION PTAE is associated with lower odds of rebleeding and any reintervention in ***,further RCTs are needed to have a higher level of evidence.

What have we learned about non-involved psoriatic skin from large-scale expression studies?

Psoriasis is a chronic inflammatory skin disorder; its genetic background has been widely studied in recent decades. Recognition of novel factors contributing to the pathogenesis of this disorder was facilitated by potent molecular biology tools developed during the 1990 s. Large-scale gene expression studies, including differential display and microarray, have been used in experimental dermatology to a great extent; moreover, skin was one of the first organs analyzed using these methods. We performed our first comprehensive gene expression analysis in 2000. With the help of differential display and microarray, we have discovered several novel factors contributing to the inherited susceptibility for psoriasis, including the EDA+ fibronectin splice variant and PRINS. The long non-coding PRINS RNA is expressed at higher levels in non-involved skin compared to healthy and involved psoriatic epidermis and might be a factor contributing cellular stress responses and, specifically, to the development of psoriatic symptoms. This review summarizes the most important results of our large-scale gene expression studies.

Examination of Whey Degreasing by modified membrane Filtration

The largest amount of dairy by-products, especially the whey, comes from the manufacture of cheese. The whey proteins are used in several different industry technologies. The forage production is used for animal feeding in the forms of various flours mixed in feeds, and the food industry uses whey proteins as human nutrition, such as different dry soups, infant formulas and supplements. The fat components of whey may inhibit the efficient processing and might impair the use of whey in these technologies. Thus, the aim of the experiment was to investigate a cheap and economical separation of the lipid fraction of whey. This separation method was made by microfiltration, which is an inexpensive, effective and energy efficient method for this task. During the measurements, 0.2 μm and 0.45 μm microfiltration membranes were used in a laboratory tubular membrane filtration module, and the membrane separation method was combined and modified by using astatic mixer and/or air insufflation. The same pore size membranes were used in a vibrating membrane filtration equipment （VSEP）, too. The two different membrane filtration devices allowed the comparison of the effect of vibration and the effect of the static mixer and/or air insufflation. The flux values above 0.2 mPa transmembrane pressures strongly decreased on using the tubular membrane. Therefore, it can be determined that the use of the lower transmembrane pressures gave better flux combined with air insufflation and the use of static mixer. The flux values increased three times higher with using vibration during the microfiltration process than that without vibration. Comparing these methods, it can be concluded that the separation made on tubular membrane （0.2 μm） combined with statics mixer gave sufficient result according to the degreasing, retentions and flux values of the other components.

AIm： To determine the prevalence of a new set ot anti-glycan and anti-outer membrane protein （anti- OmP） antibodies in a Hungarian cohort of adult Celiac disease （CD） patients. mETHODS： 190 consecutive CD patients [m/F： 71/119, age：39.9 （SD：14.1） years], 100 healthy, and 48 gastrointestinal controls were tested for glycan anti-Saccharomyces cerevisiae （gASCA）, anti-laminaribioside （ALCA）, anti-chitobioside, anti-mannobioside, anti-OmP antibodies and major NOD2/CARD15 mutations. Thirty out of 82 CD patients enrolled at the time of diagnosis were re-evaluated for the same antibodies after longstanding gluten-free diet （GFD）. RESULTS： 65.9% of the CD patients were positive for at least one of the tested antibodies at the time of the diagnosis. Except anti-OmP and ALCA, antimicrobial antibodies were exclusively seen in untreated CD; however, the overall sensitivity was low. Any glycan positivity （LR＋： 3.13; 95% CI： 2.08-4.73） was associated with an increased likelihood ratio for diagnosing CD. Significant correlation was found between the levels of anti-glycan and anti-endomysial or anti-transglutaminase antibodies. Anti-glycan positivity was lost after longstanding GFD. Anti-glycan antibody titers were associated with symptoms at presentation, but not the presence of NOD2/CARD15 mutations. Patients with severe malabsorption more frequently had multiple antibodies at diagnosis （P = 0.019）. CONCLUSION： The presence of anti-glycan antibodies in CD seems to be secondary to the impaired small bowel mucosa which can lead to increased antigen presentation. Furthermore, anti-glycan positivity may be considered an additional marker of CD and dietary adherence.
