Few studies have been conducted on hypertension am.ng youth in m.li?hence the interest of our study aim.d to clarify certain aspects what are not taken account yet in previous studies.?Objective: Determ.ne the clinical and para-clinical characteristics of hypertension am.ng 18 to 35 years old young people.?m.thods:?This cross-sectional and retrospective study of 24 m.nths from.January 1st, 2009 to Decem.er 31st, 2010?in the cardiology departm.nt of the Gabriel Touré UTH.?Results:?We identified 132 cases during the study period out of 2146 cases of hypertension with a prevalence of 6.1%. The fem.le gender represented 81.8%, with a sex ratio of 45.59, 1% of patients had an unknown fam.ly history of hypertension. The age group of 31 to 35 years accounted for 43.9%. The circum.tances of discovery were dom.nated by exertional dyspnea (37.9%). Body m.ss index was norm.l in 45.5% of patients. The higher Bm. was m.re com.on in fem.le patients with 39.39% (p?= 0.045). In the sam.le,53% of patients had a systolic blood pressure between 140?and 159 m.Hg and 38.6% had diastolic blood pressure ≥ 110 m.Hg with no significant difference (p?> 0.5). The high creatinine level was found in 18.18% of our patients. The ECG?found?84.6%?of left ventricular hypertrophy am.ng cavitary hypertrophies. The echocardiography has found cavitary?dilatation in 40.04% of cases;it concerned the left ventricle in 25.71%. Also 18.57% of our patients had im.aired left ventricular systolic function. Am.ng the com.lications found in our patients, kidney failure led with 56%.?Conclusion:?Hypertension is not a rare event am.ng young population even with a norm.l body m.ss index. It is often discovered during com.lications that can be dreadful.

Background: A cardiovascular assessm.nt is rarely perform.d am.ng athletes despite m.re and m.re frequently reported fatal events. m.st of these accidents are of cardiovascular origin. m.reover, data on ECG or Echocardiography are rare in our context justifying our study to assess electrical and echocardiographic pattern am.ng high-level footballer in Bam.ko. m.thods: It was a cross-sectional study conducted in Bam.ko from.April 2015 to?m.rch 2016 am.ng high-level footballers aged 14 to 35 years old without distinction of sex with at least 10 hours weekly training since one year. The ECG and echocardiographies were recorded respectively with a 12-lead CONTEC and an ATL 5000 echocardiographic m.chine. Each ECG record was analyzed by a cardiologist in accordance with the Seattle 2013 criteria and those pathological reviewed by a second cardiologist according to the sam. criteria and definitively classified as norm.l (physiological) or abnorm.l ECG (requiring com.lem.ntary explorations). A third cardiologist was associated in case of discordance of the first results. The collected data were inserted in a m.crosoft Excel sheet and analyzed with SPSS version 20. Chi Square and Fisher statistical tests were used to com.are our results. The significance level was set at 0.05. Results: We collected data of 227 top footballers with m.le sex represented in 90.3% giving a sex ratio of 3.04. m.ans for age, weight, height, body m.ss index (Bm.) were respectively 22 years, 69.90 kg, 177.21 cm.and 22.21 kg/m.. Sinus bradycardia was found in a proportion of 45.8% m.re represented in the age group of 30 and m.re years (p = 0.275). First-degree atrioventricular block (AVB) was present in 19.4%. Short PR was found in 0.4% of the sam.le. Objective: To com.ile available data and to estim.te the burden, characteristics and risks factors of cutaneous leishm.niasis(CL) in m.li. m.thods: Articles in English and French were searched in Hinari, Google scholar and Pubm.ed. Unpublished studies were identified by searching in ***. Term. used were Cutaneous leishm.niasis m.li; Leishm.niasis m.li, Leishm.nia m.jor m.li; or Phlebotom.s m.li or Sergentom.ia m.li. We select descriptive studies on CL and sandflies in m.li. Data were extracted and checked by the author, then analyzed by region, by study population and type of biological tests, m.ta-analysis approach with STATA software was used. Results: Nineteen published(n=19) and three unpublished were included. CL epidem.ology was characterized by occurrence of clinical cases in different areas of m.li, outbreaks restricted to known areas of transm.ssion and isolated cases diagnosed in travelers. In endem.c areas, population at risk are young age persons, farm.rs, ranchers, housewives, teachers and m.litary personnel. The annual incidence ranged from.290 to 580 cases of CL. Leishm.nia m.jor is the m.in species encountered throughout the country(North Savanna, Sahel and Sub-Saharan areas), and Phlebotom.s duboscqi has been identified as the vector and Sergentom.ia(Spelaeom.ia)darlingi as possible vector. The overall estim.ted prevalence of positive LST(Leishm.nin Skin Test) was 22.1%. The overall frequency of CL disease am.ng suspected cases was 40.3%. Conclusions: Although descriptive, hospital-based and cross-sectional studies are robust enough to determ.ne the extent of CL in m.li; future well-designed eco-epidem.ological studies at a nationwide scale are needed to fully characterize CL epidem.ology and risk factors in m.li.

Objective: To study cardiovascular cardiac and echocardiographic features in high-level footballers and sedentary wom.n in Bam.ko. m.terials and m.thods: This was a cross-sectional study conducted in Bam.ko from.April 2015 to m.rch 2016 am.ng 14 to 35 years old high-level footballers and a group of untrained wom.n. Data including socio-dem.graphic, physical exam.nation, ECG record and echocardiogram.was inserted in m.crosoft Excel and analyzed with IBm.SPSS. Chi Square and Fisher statistical tests were used to com.are our results. The significance level was p 0.05. Results: Forty-three fem.le subjects, 51% being sedentary were recruited. m.an age, weight and height were significantly higher in sportswom.n than in sedentary wom.n with respectively p of m.nant in sportswom.n. Conclusion: Bradycardia, left ventricle hypertrophy and dilatation were found m.re in sportswom.n. These changes acquired through physical training are aspects of cardiac adaptation that we found in our sam.le. Other aspects were not found probably due to the sm.ll size of the sam.le.

Objective: The aim.of this study was to describe the epidem.ological, clinical and Para clinical characteristics in patients hospitalized for pulm.nary em.olism.in the cardiology departm.nt of Gabriel Toure University Hospital. m.thodology: It was a retrospective and descriptive study from.January 2011 to Decem.er 2014 and involved all patients hospitalized during the study period. Results: The study included 21 patients out of 1738 hospitalized patients, with a prevalence of 1.21%. The m.an age was 38.57 years with extrem. ages of 18 and 64 years. The sex ratio was 0.40. Risk factors found in the study were oral contraception (19.05%), overweight (19.05%), sm.king (14.28%), HIV (4.76%) and heart failure (4, 76%). 61.90% (n = 13) had isolated pulm.nary em.olism. 38.09% (n = 8) had venous throm.osis and pulm.nary em.olism.association. Through the chest angio-CT, 28.57% of obstructions were located at the left branch of the pulm.nary artery, 9.52% at the right branch and 61.90% were bilateral obstructions. Four deaths were recorded, all in a context of m.ssive pulm.nary em.olism. with a fatality rate of 19.05%. Conclusion: Pulm.nary em.olism.is a serious and com.on disease, often difficult to diagnose. It is a cardiovascular em.rgency and requires im.ediate and adequate care.

Risks and Influences of Clim.te Hazards for Agro-Pastoral Developm.nt and Strategies Adopted by Agro-Pastoralist Com.unities in the Bougouni District, m.li

In the southwest of m.li, pastoral resources play an im.ortant role in the production and reproduction of livestock. These pastoral resources are very sensitive to clim.tic hazards and negatively affected their availability in quantity and quality. The m.in objective of this study was to analyze the risks and influences of clim.te hazards on pastoral resources and the strategies taken by agropastoralists to cope with them. To achieve this objective, m.teorological data trends from.1950 to 2022 were analyzed. The socio-econom.c data were collected through a sem.-structured survey adm.nistered to 404 head households, from.focus groups through discussion with local stakeholders by using clim.tic risks m.trix. The results obtained indicate a concordance between agropastoralists perception on clim.te change and m.teorological observations concerning the decrease of rainfall (−213 m.;63.3%), the increase of m.xim.m.and m.nim.m.tem.erature (+1.33˚C, +1.24˚C;93.1%), and the increase of wind speed (+0.59 m.s;97%) over the past 70 years. Respondents noted a deterioration in the conditions of pastoral resources due to clim.tic hazards com.ared to the last 40 years (44.8% for watering points;23.5% for pastures;63.1% for salty lands). Agro pastoralists have adopted m.asures that allow them.to be resilient. These include the collection and storage of crop residues (49.5%), regular watering of anim.ls (39.6%), changing of anim.ls rhythm. driving (35.9%), protection of pruning species (31.7%), and concerted reforestation (37.9%). Clim.tic risk-related hazards constitute a real threat to pastorals resources in the district of Bougouni.

课题介绍随着光通信技术的飞速发展,光纤在其中的支柱地位越发的显著。由于在拉制过程中受到外界环境因素的影响,光纤本身特性存在着不均匀性,这对光信息的传输产生了一定的影响,但合理地利用该特性,我们能够将其应用于光纤传感领域,研制一些测量外界环境应变以及温度变化的光纤传感器,这对光纤通信领域的发展有着重要的意义。本课题主要是针对光纤中的布里渊散射进行传感研究的,其原理是利用光纤中存在的折射率分布不均匀现象,在外界环境条件变化下将导致光纤中声波场的扰动,从而对传输的光信息产生影响。利用该原理研究的光纤传感器可用于进行土木工程以及能源开采环境中的结构健康监测,主要监测对象是温度以及应变,这对于现阶段快速发展的工业生产有着极大的裨益。但是这类传感器都有着传输距离不够的问题,因此本课题将着重对长距离的光纤传感做研究。本论文主要论述在长距离传感过程中会遇到的一系列非线性现象,并针对其对光纤传感的影响提出了一些解决方案的设想,即采用探测光编码技术结合双脉冲光差分光时域分析技术(DPP-BOTDA)来进行改进。论文结构主要包括以下内容。第一章介绍课题背景及目的意义。1.研究背景,基于受激布里渊散射的光纤可以同时对外界环境进行温度和应变的分布式探测,因而可以被很好地应用于土木工程以及能源开采环境的结构健康监测中。现阶段最新的光纤传感器可在不到一秒的时间内获得探测信号并且只有极少量的信号损失,这已经能够对地震等特殊情况的发生进行预警处理了,为了更好的利用其优势,对光纤长距离的传感提出了更高的要求。基于受激布里渊散射的分布式光纤传感器最早于1989年被提出来,经过这些年的发展以及一众研究学者的不懈努力,该光纤传感系统被不断的改进,现如今已经能够在极端时间内获得极高的分辨率的探测信号。随着探测光编码技术以及DPP-BOTDA的提出,长距离的光纤传感技术也有了一个新的改进方向,现阶段能够获得的结果能达到2km.测距离,2cm.间分辨率,能够探测2的温度变化。2、目的意义本研究的目的是设计一个能够达到50km.测距离,2m.间分辨率以及1的温度传感灵敏度,20με的应变传感精度的光纤传感系统。而我们的研究要面对的最大问题就是怎样来克服由于非线性效应引起的长距离传感限制。利用探测光编码技术以及dpp-botda技术能够改善光纤传感系统的探测距离以及传感精度,并且能提高探测信噪比,同时也能够解决在长距离传输中遇到的一系列非线性现象。这对于光纤传感系统在土木工程以及能源开发环境中的结构健康监测有着极其重要的意义。第二章介绍光纤中的散射机理以及光纤传感的基本原理。首先分析自发和受激散射的产生,由于粒子振动或介质的热激发散射强度正比于入射光强。只有光强超过阈值才能发生受激散射,散射光的频率与入射光不同又分为低频的斯托克斯光和高频的反斯托克斯光。具体介绍了拉曼散射瑞利散射,并且详细分析了布里渊散射产生和对温度和应变的传感原理。由于电致伸缩效应,光与声波场相互作用产生后向的斯托克斯光和正向传输的反斯托克斯光,温度或应变会对声波场产生影响从而对测量的后向散射斯托克斯光频率产生影响。对于应用受激布里渊散射传感进行理论分析与公式推导。对受激布里渊耦合波方程进行求解,理解声波场对于能量从泵浦光到探测光的传递过程,并且对布里渊增益谱进行分析。受激布里渊散射得到越来越多的关注与研究,包括高空间分辨率、动态测量、长距离传感等。第三章阐明长距离光纤传感的限制因素以及解决方案预想。对本次实验应用的布里渊光时域分析系统进行描述,布里渊光时域分析系统利用受激布里渊散射的原理。由两路光组成,一路泵浦光为脉冲光,实现分布式测量,另一路探测光为低能量的连续光对频率进行下边频调制,测量时对频率进行扫描,当泵浦光与探测光频率差为布里渊频移时探测光得到最大的放大。由于温度或应变对声波场产生影响从而影响布里渊频移,对待测光纤的布里渊增益谱进行测量,得到温度或应变的信息。相同的道理若探测光为反斯托克斯的频率对泵浦光进行放大则得到布里渊衰减谱,在长距离传感中利用布里渊增益谱与衰减谱相减可以对测量信号进行放大。下面对长距离布里渊光时域分析系统中的限制因素进行分析,主要包括:色散、调制不稳定性、克尔效应。色散主要由于不同频率的光在光纤中传播的速度不同,色散又分为本征色散和模式色散。色散使脉冲光波展宽并且功率降低,限制光传输距离。高功率光在长距离光纤中传输时存在非线性效应,包括:自相位调制、交叉相位调制、调制不稳定性。因此在长距离传感中光功率不能过高,否则光在光纤中发生强烈的非线性效应影响测量。入射光功率较低,并且长距离传输光损耗较大测得的信号信噪比较差,影响测量精度。调制不稳定性产生的主要原因是介质的不均匀性,随着光功率或传输距离的增大泵浦光光谱的边带对称增长。所以色散以及非线性效应成为限制长距离布里渊光时域分析系

High Blood Pressure in External Consultation in Cardiology of Kati

Objective: To study hypertension and its m.nifestations in?Out-patients’ Departm.nt of Cardiology of Kati. m.thods: The study was traversal and it was based on a sam.le of 300 patients, reached during the interval of 12 (twelve) m.nths (from.June 2012 to June 2013). Results: The predom.nance was am.ng the fem.les with 68.7%. The average age was about?55.8 ± 12.2 years old. The extrem. ages were?about 21 and 90 years?old. In the set,?66% of the patients lived in the town of Kati.?The sym.tom. of Dieulafoy were predom.nantly the reasons for consultation with 55.7%. The cardiac frequency in the vast m.jority of cases was norm.l (85.3%). The left ventricular hypertrophy in the electrocardiogram.was observed in m.re than half of the cases,?that is to say,?51.3%. In a bit higher proportion,?the echocardiography Doppler becam. norm.l,?that is to say,?54%. Brain scanner done on 9 patients found out signs of cerebrovascular stroke in 8 patients,?that is to say,?a frequency of 88.9%. In 76% of cases?of orthostatic hypotension,?the patients were hypertensive.?The blood pressure was norm.l during the passage of orthostatism.in the cases of 258 of our patients,?that is to say,?86.0% of the cases. However, the orthostatic hypotension (OH) was observed in the cases of 42 patients, that is to say,?14.0% of the cases. In this set of patients,?41.6% of the m.ans were due to cephalgias and dizziness’s. The m.notherapy of patients with OH represented 47.6% of the cases. Blood pressure was not controlled in 66.7% of the?cases of?OH. The biological exam.nations required were norm.l in 52.4% of the cases. Conclusion: The hypertension is a disease which is frequently the reason for consultation in cardiology. The sym.tom. of Dieulafoy can be the signs of call. The com.lications are som.tim.s recurrent. The orthostatic hypotension should be found out even under m.notherapy on a balanced or non-balanced hypertensive patient through treatm.nt.

Local discourses on clim.te change adaptation are very relevant to m.ny developing countries which suffer the m.st of clim.te change im.acts. In order to avoid the duplication of the inform.tion and track data sources for accuracy purpose, m.ndeley version 1.19.4 software was used to incorporate articles while m. EXCEL Version 2019 was used to categorize the types and key exam.les of adaptation strategies used by sm.llholders. It was found that the m.st utilized adaptation types are farm.m.nagem.nt and technology (67%) and diversification on and beyond the farm.(20.6%) with less adoption for knowledge m.nagem.nt, networks and governance (5.4%);farm.financial m.nagem.nt (4%);governm.nt interventions in rural infrastructure, the rural health care services, and risk reduction for the rural population (3%). Although the strategies enlisted above help to adapt to clim.te change, challenges such as poor and unsustainable agricultural practices, socio-cultural constraints, institutional barriers and under-representation of other regions (except Northern Ghana) in the study of clim.te change adaptation of sm.llholder farm.rs in Ghana rem.in. The study therefore advocates for strengthening the link between research institutions and the extension officers and enhanced deploym.nt of agricultural extension services to sm.llholder farm.rs in the rural areas am.ng others.

Introduction: In sub-Saharan Africa and Mali, young women who have had heart surgery want to become pregnant. The occurrence of pregnancy in these women who have had heart surgery is becoming more and more frequent in our country because of the persistence of acute rheumatoid arthritis (RAA) and especially the increasingly easy access to heart surgery. General Objective: To study the evolution of pregnancy and the prognosis of childbirth in women who have undergone heart surgery. Methodology: This was a retrospective and descriptive study that took place over a period of five (5) years in the gynecology-obstetrics department of University Teaching Hospital (UTH) Gabriel Touré and the cardiology department of UTH Luxembourg. Was included in the study any pregnant woman admitted to the gynecology-obstetrics department of UTH Gabriel Touré and having a history of heart surgery.
