Blount Method in the Management of Supracondylar Fractures of the Humerus in Children

Supracondylar fractures of the humerus are the most common traumatic elbow injury in children. Several techniques are described in their support. Blount’s method is one of the widely used orthopedic means. The objective of our study was to evaluate the results of supracondylar fractures of the humerus treated by this method in our department. This was a descriptive, longitudinal study with the prospective collection over a period of 12 months from May 2018 to April 2019. It focused on children aged 0 to 15 years treated in the Orthopedic Traumatology Department of the CHU of Kati for a supracondylar fracture of the humerus in extension. A minimum follow-up period of 6 months was necessary for inclusion. 12 patients were collected. The sociodemographic, anatomopathologic, therapeutic and evolutionary aspects were studied. The results were evaluated according to Flynn’s criteria. The male sex was predominant with a ratio of 2. The average age was 5.5 years. The average admission time was 12.5 hours. Domestic accidents were the main etiology (66.66%) and Type II fracture was the most frequent with 58.33%. The duration of immobilization was 42 days for 11 patients. Complications were 1 case of secondary displacement and 1 case of 12˚ flexion deficit. The average Baumann angle was 72˚. The result was satisfactory for all our patients. Supracondylar fractures of the humerus are common in children, especially boys. The method of Blount when it is well carried out by respecting the indications gives good results.

It is the need of time that oil palm farmers must perform the spatially planned soil analysis to know about the fertilizer sufficient and deficient zones of *** method is a suitable and fast solution of soil analysis for NPK determination using the digital soil test *** determination procedure with a digital soil test kit was undefined for oil ***,the digital soil test kit determines the passage of light through an opaque medium of soil solution with a specified ***,environmental light may interfere leading to wrong results of NPK ***,this equipment was non-incorporable with the controller of any VRT fertilizer *** this research,these issues were addressed and the NPK measurement procedure was defined for oil palm plantation by modifying the‘soil to water’ratio in sample soil solution with an optimum environmental light range of 18-23 W/m^(2).‘Soil to water’ratios were found for nitrogen,phosphorus and potassium as 0.31 to 5.00,1.00 to 5.00 and 4.50 to 5.00,respectively to fit the requirement of NPK for oil palm in the prescribed range of the *** study of modified digital soil test kit showed that 91.7%N,89.6%P and 93.8%K results of modified digital soil test kit were matched with analytical laboratory ***,the reliability of NPK results using digital soil test kit was enhanced,making the kit incorporable with the controller of variable rate fertilizer applicator through remote monitoring based data acquisition *** outcome of this research can be used in the development of an IoT network data fusion for dynamic assessment of the NPK variation in the soil and nutrient management in oil palm plantations.

Magnetohydrodynamic Effect on Free Convection of Three Dimensional Laminar Flow in Porous Annulus

A numerical study has been carried out to investigate heat transfer by free convection under the effect of MHD (magnetohydrodynamic) for steady state three-dimensional laminar flow in horizontal and vertical cylindrical annulus filled with saturated porous media (sand silica) with fins attached to the inner cylinder. A single electric coil placed around the inner cylinder to generate a magnetic field. The governing equations which used are continuity, momentum (using Darcy"s law) and energy equations which are transformed to dimensionless equations. The finite difference approach is used to obtain all the computational results using Fortran 90 program. The parameters affected on the system are Rayleigh number ranging within (102 ≤ Ra* ≤ 104), and MHD (Mn) (0 ≤ Mn ≤ 100) and radius ratio Rr (0.225, 0.338 and 0.435). The results obtained are presented graphically in the form of streamline and isotherm contour plots and the results show that heat transfer decrease with the increase of magnetohydrodynamic. It was found that the average Nusselt number increase with Ra* and decrease with Hf, Mn and Rr. A correlation for the average Nusselt number in terms of Ra* and Mn, has been developed for the inner cylinder.

The Comoros is an archipelago in the Indian Ocean located in the Mozambique Channel between the African continent and Madagascar. Geographically Comoros is composed of four islands: Grande Comores, Anjouan, Mohéli and Mayotte (under French administration). Apart from Mayotte, the others three independent islands commonly known as the Union of the Comoros are suffering from energy stress since their independence year 1975 until nowadays. The energy supplied and distributed by the national electricity company, SONELEC produced by diesel engines, is not stable with so much load shedding all the time. This instability energy leads to a lack of a phone network in some telecommunications towers connected to grid. The majority of telecommunications towers are located in rural areas not connected to grid and running on diesel generators, which once again leads to a problem with the telecommunications network when diesel engines fail. The two competing companies which operate in the field of telecommunications in the Comoros, namely Comores Télécom, a national public company and Telma, the private one, are still unable to ensure the provision of the telecommunications network on a regular basis. This is why we propose in the present work, a sizing of hybrid system composed essentially of a diesel generator, a wind turbine and a photovoltaic solar system with storage in batteries for supplying telecommunications towers in order to permanently ensure the provision of the telecommunications network for the well-being of the population. Our future energy must be based on non-polluting energies with significant resources. Impact of the Armed Conflict of 2015-2016 in Aden on Health Services and the Availability of Medicines

To assess the impact of the armed conflict of 2015-2016 on the availability of medicines and health service in Aden Governorate whilst the rebels over-run the city. A cross sectional survey was done during the period, September-November 2016. The survey included 63 medicinal preparations as marker medicines in 142 public health facilities including private pharmacies. Direct questions and a retrospective study of the United Nations and other organizations that were working in area reporting were studied. I.V. fluids had the highest availability in the market whilst vaccines had the lowest average availability at 60%. Medicine prices were increased by 71%. The function of health facilities was almost on the verge of collapse. The four districts containing the main hospital and the medical stores were inaccessible and completely controlled by the rebels. Two out of six hospitals remain functional. Dengue and malaria rampantly spread and approximately 65,250 civilians were affected. The situation was further aggravated by the absence of basic services such as electricity, water and food.

Background:Laryngopharyngeal reflux disease(LPRD)is a condition with nonspecific symptoms and most of times patients present late with advanced disease which may pre-dispose to *** magnitude and clinical characteristics of this condition are not well known among patients attending Otorhinolaryngology services in *** and methods:This was a hospital based descriptive cross sectional study,conducted in the wards and clinics of Otorhinolaryngology department of Muhimbili National *** with symptoms of Laryngopharyngeal reflux disease were included in the *** was collected using questionnaires and clinical examination forms,were processed and analysed by using *** presented in frequency tables,cross tabulations and ***:This study recruited 256 participants among them males were 131(51.2%).The mean age was(41.38±13.94)*** of Laryngopharyngeal reflux disease was 18.4%without gender *** commonest symptoms were globus sensation,hoarseness of voice and excessive urge to clear the throat with 95.7%,88.1%and 83.0%respectively while the most observed signs were thick endolaryngeal mucus,Vocal cord oedema and partial ventricular obliteration with 90.9%,88.6%and 72.7%*** down less than two hours after meal and spices foods consumption were the leading risk *** and Diabetes Mellitus type 2 were the most prevalent co morbid conditions associated with Laryngopharyngeal reflux ***:The prevalence of Laryngopharyngeal reflux disease is high among patients attending Otorhinolaryngology services at Muhimbili national *** patients with Laryngopharyngeal reflux disease related symptoms should get thorough evaluation for early diagnosis and treatment.

Spontaneous Pneumothorax Complicating Miliary Tuberculosis: About a Case at the “Centre Medical Principal De La Gendarmerie Nationale Du Mali”

Introduction: Spontaneous pneumothorax is a rare but serious complication of tuberculosis. Miliary tuberculosis (MT) is a severe form of tuberculosis secondary to hematogenous spread of Mycobacterium tuberculosis. Objective: To report a case of MT complicated by pneumothorax. Methodology: This was a 25-year-old patient, farmer, followed up at the Pneumo-phtisiology department of the CHU du Point G for MT whose condition was improving after the introduction of anti-tuberculosis chemotherapy. He consulted again after experiencing chest pain. Clinical and imaging revealed a Spontaneous pneumothorax complicating MT. The treatment combined anti-tuberculosis chemotherapy, chest drainage and respiratory physiotherapy. Outcome was favorable with improvement of clinical and radiological signs. Conclusion: Pneumothorax complicating MT requires a reflective diagnostic approach and rapid management to improve its prognosis.

Shiga toxin producing Escherichia coli(STEC)outbreak is a public health concern as it can potentially cause a variety of clinical manifestations including diarrhea,hemorrhagic colitis and hemolytic uremic syndrome(HUS).However *** are generally innocuous commensal organisms,and there is a need to discriminate pathogenic from non-pathogenic isolates rapidly and *** this study,we have used standard culture based methods and advanced molecular approaches to characterize *** in food in a local outbreak *** show that the application of DNA based detection methods including real-time PCR and DNA microarray along with a traditional culture method can identify the organism implicated in an outbreak at the strain level for pathogenic potential.
