Neuromuscular trunk activation patterns in back pain patients during one-handed lifting

AIM To analyze neuromuscular activity patterns of the trunk in healthy controls(H) and back pain patients(BPP) during one-handed lifting of light to heavy loads. METHODS After assessment of back pain(graded chronic pain scale according to von Korff) all subjects(n = 43) performed a warm-up(treadmill walking). Next, subjects were instructed to lift 3 × a 20 kg weight placed in front of them(with both hand) onto a table(height: 0.75 m). Subsequently, all subjects lifted with one hand(left-side, 3 repetitions) a weight of 1 kg(light), 10 kg(middle) and 20 kg(heavy) in random order from the ground up onto the table left of them. Trunk muscle activity was assessed with a 12-lead EMG(6 ventral/6 dorsal muscles; 4000 Hz). EMG-RMS(%) was averaged over the 3 repetitions and analyzed for the whole one-handed lifting cycle, then normalized to RMS of the two-handed lifting. Additionally, the mean(normalized) EMG-RMS of four trunk areas [right/left ventral area(VR/VL); right/left dorsal area(DR/DL)] was calculated. Data were analyzed descriptively(mean ± SD) followed by student's t-test comparing H and BPP(α = 0.05). With respect to the unequal distribution of subjects in H and BPP, a matched-group analysis was conducted. Seven healthy controls were gender- and age-matched(group H_(matched)) to the 7 BPP. In addition, task failure was calculated and compared between H/H_(matched) vs BPP using χ~2. RESULTS Seven subjects(3m/4f; 32 ± 7 years; 171 ± 7 cm; 65 ± 11 kg) were assigned to BPP(pain grade ≥ 2) and 36(13m/23f; 28 ± 8 years; 174 ± 10 cm; 71 ± 12 kg) to H(pain grade ≤ 1). H and BPP did not differ significantly in anthropometrics(P > 0.05). All subjects were able to lift the light and middle loads, but 57% of BPP and 22% of H were not able to lift the heavy load(all women). χ~2 analysis revealed statistically significant differences in task failure between H vs BPP(P = 0.03). In contrast to Ref.[1]studying MHD processes in a helical channel induced by a vertical rotating electric current layer,in the present report we examine another version with the rotating electric current layer orthogonal to the helical channel axis,which leads to an increase in melt velocity with the growing pitch angle α of the helical channel.

Magnetic resonance imaging(MRI)is a common clinical practice to visualize defects and to distinguish different tissue types and pathologies in the human *** far,MRI data have not been used to model and generate a patient-specific design of multilayered tissue substitutes in the case of interfacial *** orthopedic cases that require highly individual surgical treatment,implant fabrication by additive manufacturing holds great ***-based techniques like 3D plot-ting allow the spatially defined application of several materials,as well as implementation of bioprinting *** the example of a typical multi-zonal osteochondral defect in an osteochondritis dissecans(OCD)patient,this study aimed to close the technological gap between MRI analysis and the additive manufacturing process of an implant based on dif-ferent biomaterial inks.A workflow was developed which covers the processing steps of MRI-based defect identification,segmentation,modeling,implant design adjustment,and implant generation.A model implant was fabricated based on two biomaterial inks with clinically relevant properties that would allow for bioprinting,the direct embedding of a patient’s own cells in the printing *** demonstrated by the geometric compatibility of the designed and fabricated model implant in a stereolithography(SLA)model of lesioned femoral condyles,a novel versatile CAD/CAM workflow was successfully established that opens up new perspectives for the treatment of multi-zonal(osteochondral)defects.

The role of the self-concept in the relationship of menstrual symptom attitudes and negative mood

Background: A relationship between symptom attitudes and negative affect has consistently been found in a range of different symptom domains. Little is known, however, about the role of different aspects of the self in this relationship. We explored the mediating role of interferences of symptom with the self-concept in the association of menstrual symptom attitudes and depressive mood. Methods: Eighty-one women completed an online survey on menstrual symptom attitudes, perceived interferences of symptoms with various self-aspects and negative mood states. We tested our hypothesis in a mediation analysis. Results: We found a complete mediation of the relationship of symptom attitudes and depressive mood by interferences of symptoms with self-aspects. However, interferences with self-aspects did not play a role in the association of anxious mood and symptom report. Conclusion: The self-concept should receive greater attention in research on symptom attitudes and psychological well-being. This would be particularly important in research on medically unexplained symptom report.

Jejunitis and brown bowel syndrome with multifocal carcinogenesis of the small bowel

This is the first report describing a case where prolonged,severe malabsorption from brown bowel syndrome progressed to multifocally spread small bowel adenocarcinoma. This case involves a female patient who was initially diagnosed with chronic jejunitis associated with primary diffuse lymphangiectasia at the age of 26 years. The course of the disease was clinically,endoscopically,and histologically followed for 21 years until her death at the age 47 due to multifocal,metastasizing adenocarcinoma of the small bowel. Multiple lipofuscin deposits(so-called brown bowel syndrome) and severe jejunitis were observed microscopically,and sections of the small bowel showed dense lymphoplasmacytic infiltration of the lamina propria as well as blocked lymphatic vessels. After several decades,multifocal nests of adenocarcinoma cells and extensive,flat,neoplastic mucosal proliferations were found only in the small bowel,along with a loss of the mismatch repair protein MLH1 as a long-term consequence of chronic jejunitis with malabsorption. No evidence was found for hereditary nonpolyposis colon carcinoma syndrome. This article demonstrates for the first time multifocal carcinogenesis in the small bowel in a malabsorption syndrome in an enteritis-dysplasia-carcinoma sequence.

Calcium phosphate cements (CPC) are currently widely used bone replacement materials with excellent bioactivity, but have considerable disadvantages like slow degradation. For critical-sized defects, however, an improved degradation is essential to match the tissue regeneration, especially in younger patients who are still growing. We demonstrate that a combination of CPC with mesoporous bioactive glass (MBG) particles led to an enhanced degradation in vitro and in a critical alveolar cleft defect in rats. Additionally, to support new bone formation the MBG was functionalized with hypoxia conditioned medium (HCM) derived from rat bone marrow stromal cells. HCM-functionalized scaffolds showed an improved cell proliferation and the highest formation of new bone volume. This highly flexible material system together with the drug delivery capacity is adaptable to patient specific needs and has great potential for clinical translation.

Besides obvious benefits,the Three Gorges Dam＇s construction resulted in new pollution scenarios with the potentials to threaten the Three Gorges Reservoir（TGR） *** order to record organic contamination,to find links to ecotoxicological impacts and to serve as reference for ensuing monitoring,several sites in the TGR area were screened applying the triad approach with additional lines-of-evidence as a holistic assessment *** and the benthic fish species Pelteobagrus vachellii were sampled in 2011 and 2012 to determine organic pollution levels,mutagenic potentials and genotoxic *** regional hot-spots near the cities of Chongqing and Kaixian were identified and further investigated in *** polycyclic aromatic hydrocarbons（PAHs） could be detected in sediments in 2011（165-1653 ng/g）,emphasizing their roles as key pollutants of the *** ubiquity was confirmed at Chongqing（150-433 ng/g） and Kaixian（127-590 ng/g） *** were comparable to other major Chinese and German ***,the immense sediment influx suggested a deposition of 216-636 kg PAH/day（0.2-0.6 mg PAH/（m2·day））,indicating an ecotoxicological *** source analysis highlighted primary impacts of combustion sources on the more industrialized upper TGR section,whereas petrogenic sources dominated the mid-low ***,sediment extracts from several sites exhibited significant activities of frameshift promutagens in the Ames fluctuation ***,significant genotoxic impairments in erythrocytes of *** were detected（Chongqing/Kaixian）,demonstrating the relevance of genotoxicity as animportant mode of action in the TGR＇s ***,their derivatives and non-target compounds are considered as main causative agents.
